Screening for asthma in Cantonese-speaking immigrant children
BACKGROUND: Asthma prevalence among Chinese immigrant children is poorly understood and attempts to screen these children have produced varied outcomes. We sought to learn how to improve screening for asthma in Chinese immigrant children. METHODS: Children (n = 152) were administered the Brief Pediatric Asthma Screen in either Cantonese or English, they then viewed and reacted to a video showing people wheezing and subsequently took a pulmonary function test. RESULTS: The diagnosed asthma prevalence for our study population was 27.0%, with another 5.3% having possible undiagnosed asthma. Very few children had spirometry findings below normal. In multivariate analysis, being native born (p = 0.002) and having a family history of asthma (p = 0.003) were statistically associated with diagnosis of asthma. After viewing the video, 35.6% of respondents indicated that the images differed from their conception of wheezing. Of four translations of the word "wheeze" no single word was chosen by a majority. CONCLUSION: Our findings suggest that asthma diagnoses are higher for Chinese children who were born in the US suggesting that desegregation of data might reveal at risk subpopulations. Care needs to be taken when diagnosing asthma for Cantonese speakers because of the centrality of the word wheeze and the challenges of translation
Osteochondral transplantation using autografts from the upper tibio-fibular joint for the treatment of knee cartilage lesions
Purpose Treatment of large cartilage lesions of the knee
in weight-bearing areas is still a controversy and challenging topic. Autologous osteochondral mosaicplasty has
proven to be a valid option for treatment but donor site
morbidity with most frequently used autografts remains a
source of concern. This study aims to assess clinical results
and safety profile of autologous osteochondral graft from
the upper tibio-fibular joint applied to reconstruct symptomatic osteochondral lesions of the knee.
Methods Thirty-one patients (22 men and 9 women) with
grade 4 cartilage lesions in the knee were operated by
mosaicplasty technique using autologous osteochondral
graft from the upper tibio-fibular joint, between 1998 and
2006. Clinical assessment included visual analog scale
(VAS) for pain and Lysholm score. All patients were
evaluated by MRI pre- and post-operatively regarding joint
congruency as good, fair (inferior to 1 mm incongruence),
and poor (incongruence higher than 1 mm registered in any
frame). Donor zone status was evaluated according to
specific protocol considering upper tibio-fibular joint
instability, pain, neurological complications, lateral collateral ligament insufficiency, or ankle complaints.
Results Mean age at surgery was 30.1 years (SD 12.2). In
respect to lesion sites, 22 were located in weight-bearing
area of medial femoral condyle, 7 in lateral femoral condyle, 1 in trochlea, and 1 in patella. Mean follow-up was
110.1 months (SD 23.2). Mean area of lesion was 3.3 cm
2
(SD 1.7), and a variable number of cylinders were used,
mean 2.5 (SD 1.3). Mean VAS score improved from 47.1
(SD 10.1) to 20.0 (SD 11.5); p = 0.00. Similarly, mean
Lysholm score increased from 45.7 (SD 4.5) to 85.3
(SD 7.0); p = 0.00. The level of patient satisfaction was
evaluated, and 28 patients declared to be satisfied/very
satisfied and would do surgery again, while 3 declared as
unsatisfied with the procedure and would not submit to
surgery again. These three patients had lower clinical scores
and kept complaints related to the original problem but
unrelated to donor zone. MRI score significantly improved
at 18–24 months comparing with pre-operative (p = 0.004).
No radiographic or clinical complications related to donor
zone with implication in activity were registered.
Conclusions This work corroborates that mosaicplasty
technique using autologous osteochondral graft from the
upper tibio-fibular joint is effective to treat osteochondral
defects in the knee joint. No relevant complications related
to donor zone were registered
The Effects of Dietary Mobile Apps on Nutritional Outcomes in Adults with Chronic Diseases : A Systematic Review and Meta-Analysis
Abstract Background Dietary interventions are effective prevention and treatment strategies for chronic diseases; however, they require extensive commitment, time, and resources. Dietary mobile applications (apps) have gained popularity and are thus being incorporated into dietary management. Objective The aim of this review is to assess the effects of the use of dietary mobile apps on nutritional outcomes in adults with chronic diseases. Methods A systematic review was conducted following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines using MEDLINE, PubMed, Embase, and CINAHL databases. The protocol was registered on PROSPERO. Intervention studies evaluating the nutritional outcomes of dietary apps, published in English between January 1, 2007 and November 15, 2017 were included. The methodological quality of included articles was assessed via the Academy of Nutrition and Dietetics\u27 Quality Criteria Checklist: Primary Research. Heterogeneity was confirmed using the I2 index and a random-effects meta-analysis was performed for randomized controlled trials. Estimates of the pooled mean difference were calculated for app usage compared to no app usage. Main outcomes measure Nutritional outcomes, categorized as food-/nutrition-related, anthropometric measurements, pertinent clinical/biochemical data, and nutrition-focused physical findings, were extracted from the included intervention studies. Results Upon completion of the searches, 18,649 articles were identified, and data were extracted from 22 articles. Pooled estimates showed a significantly greater decrease in weight ( “2.45 kg, 95% CI “3.33 to “1.58 kg; P Conclusions The findings of this systematic review and meta-analysis indicate that dietary mobile apps are effective self-monitoring tools, and that their use results in positive effects on measured nutritional outcomes in chronic diseases, especially weight loss

Inclusive differential cross sections and

\antilambda particles are measured at HERA in proton-induced reactions on C,
Al, Ti, and W targets. The incident beam energy is 920 GeV, corresponding to
GeV in the proton-nucleon system. The ratios of differential
cross sections \rklpa and \rllpa are measured to be and , respectively, for \xf . No significant dependence upon the
target material is observed. Within errors, the slopes of the transverse
momentum distributions also show no significant
dependence upon the target material. The dependence of the extrapolated total
cross sections on the atomic mass of the target material is
discussed, and the deduced cross sections per nucleon are

Magnetic hyperthermia controlled drug release in the GI tract : solving the problem of detection
Drug delivery to the gastrointestinal (GI) tract is highly challenging due to the harsh environments any drug- delivery vehicle must experience before it releases it’s drug payload. Effective targeted drug delivery systems often rely on external stimuli to effect release, therefore knowing the exact location of the capsule and when to apply an external stimulus is paramount. We present a drug delivery system for the GI tract based on coating standard gelatin drug capsules with a model eicosane- superparamagnetic iron oxide nanoparticle composite coating, which is activated using magnetic hyperthermia as an on-demand release mechanism to heat and melt the coating. We also show that the capsules can be readily detected via rapid X-ray computed tomography (CT) and magnetic resonance imaging (MRI), vital for progressing such a system towards clinical applications. This also offers the opportunity to image the dispersion of the drug payload post release. These imaging techniques also influenced capsule content and design and the delivered dosage form. The ability to easily change design demonstrates the versatility of this system, a vital advantage for modern, patient-specific medicine
Knotted vs. Unknotted Proteins: Evidence of Knot-Promoting Loops
Knotted proteins, because of their ability to fold reversibly in the same topologically entangled conformation, are the object of an increasing number of experimental and theoretical studies. The aim of the present investigation is to assess, on the basis of presently available structural data, the extent to which knotted proteins are isolated instances in sequence or structure space, and to use comparative schemes to understand whether specific protein segments can be associated to the occurrence of a knot in the native state. A significant sequence homology is found among a sizeable group of knotted and unknotted proteins. In this family, knotted members occupy a primary sub-branch of the phylogenetic tree and differ from unknotted ones only by additional loop segments. These "knot-promoting" loops, whose virtual bridging eliminates the knot, are found in various types of knotted proteins. Valuable insight into how knots form, or are encoded, in proteins could be obtained by targeting these regions in future computational studies or excision experiments
