    Evaluation of sexual functional status and consistency of scales in patients with hypogonadotropic hypogonadism before and after testosterone replacement therapy: a single-center experience

    ABSTRACT Objective: This study aimed to investigate the frequency of sexual dysfunction and the effect of short-term testosterone replacement therapy on sexual functions in congenital hypogonadism patients. Furthermore, we sought to reveal the consistency of the self-report scales used for the diagnosis of sexual dysfunction and the relationship between biochemical parameters. Materials and methods: The study was conducted on 47 young male patients aged above 18 years who were diagnosed with hypogonadotropic hypogonadism. Short (IIEF-5) and long (IIEF-15) forms of the International Index of Erectile Function and Arizona Sexual Experiences Scale (ASEX) were applied before treatment under the supervision of a physician. The patients’ blood pressure, height, and weight were measured, and their luteinizing hormone (LH), FSH, and total testosterone levels were recorded. Patients who started their treatments were called for a follow-up checkup after 6 months. Their blood pressure, height, and weight were measured by reapplying the ASEX, IIEF-5, and IIEF-15. In addition, their LH, FSH, and total testosterone levels in the biochemical tests were rerecorded. Results: In this study, the sexual dysfunction status of patients diagnosed with hypogonadotropic hypogonadism before and after treatment was evaluated using the ASEX, IIEF-15, and IIEF-5 scales. A decrease in sexual dysfunction was observed in all three scales after treatment compared with that before treatment. The IIEF-5 and IIEF-15 scales were found to be uncorrelated in terms of the pretreatment values but were correlated in terms of the post-treatment values. Although a correlation was observed between ASEX and IIEF-5 before treatment, no correlation was detected between ASEX and IIEF-15. After the treatment, ASEX was found to be correlated with both IIEF-5 and IIEF-15. The results of the scales indicated the correlation in all categories, except the pretreatment results of the IIEF-15 scale. Conclusion: The results of the current study demonstrated a significant improvement in the sexual function of hypogonadism patients undergoing short-term testosterone therapy. The ASEX, IIEF-5, and IIEF-15 scales used in the diagnosis and follow-up of sexual dysfunction were useful for evaluating sexual functions in hypogonadotropic hypogonadism patients

    Effect of pathologic fractures on survival in multiple myeloma patients: a case control study

    <p>Abstract</p> <p>Background</p> <p>Multiple Myeloma (MM) is a B cell neoplasm characterized by the clonal proliferation of plasma cells. Skeletal complications are found in up to 80% of myeloma patients at presentation and are major cause of morbidity.</p> <p>Methods</p> <p>49 patients were enrolled with MM admitted to Black Sea Technical University Hospital between 2002–2005. Pathologic fractures (PFs) were determined and the patients with or without PF were followed up minumum 3 years for survival analysis.</p> <p>Results</p> <p>PF was observed in 24 patients (49%) and not observed in 25 patients (51%). The risk of death was increased in the patients with PF compared with patients who had no fractures. While overall survival was 17.6 months in the patients with PFs, it was 57.3 months in the patients with no PFs.</p> <p>Conclusion</p> <p>These findings suggest that PFs may induce reduced survival and increased mortality in the MM patients, however, larger sample size is essential to draw clearer conclusions added to these data.</p

    Characterization of greater middle eastern genetic variation for enhanced disease gene discovery

    The Greater Middle East (GME) has been a central hub of human migration and population admixture. The tradition of consanguinity, variably practiced in the Persian Gulf region, North Africa, and Central Asia1-3, has resulted in an elevated burden of recessive disease4. Here we generated a whole-exome GME variome from 1,111 unrelated subjects. We detected substantial diversity and admixture in continental and subregional populations, corresponding to several ancient founder populations with little evidence of bottlenecks. Measured consanguinity rates were an order of magnitude above those in other sampled populations, and the GME population exhibited an increased burden of runs of homozygosity (ROHs) but showed no evidence for reduced burden of deleterious variation due to classically theorized ‘genetic purging’. Applying this database to unsolved recessive conditions in the GME population reduced the number of potential disease-causing variants by four- to sevenfold. These results show variegated genetic architecture in GME populations and support future human genetic discoveries in Mendelian and population genetics

    Artificial Neural Networks (ANN) is a problem solving technique imitating the basic working principles of the human brain. The formwork labour cost constitutes an important part within the costs of the reinforced concrete frame buildings. This study suggests a method based on artificial neural networks developed for estimating the required manhours for the formwork activity of such buildings. The introduced method has been verified in the study with reference to the test conducted involving two case studies. In all cases, the model produced results reasonably close to actual field measurements. The model is a simple and quick tool for the estimators and planners to aid them in their work. Statant gelžbetoninius karkasinius pastatus, nemažą sąnaudų dalį sudaro klojinių ruošimas. Šiame tyrime siūlomas dirbtiniais neuroniniais tinklais pagrįstas metodas, kurio paskirtis – apskaičiuoti, kiek žmogaus darbo valandų reikės ruošti klojinius tokiuose pastatuose. Pristatomas metodas tyrimo metu patikrintas remiantis bandymu, susijusiu su dviem atvejo tyrimais. Visais atvejais modelio pateikti rezultatai buvo gana artimi faktiniams matavimams. The molecular geometry, vibrational frequencies, molecular electrostatic potential (MEP), thermodynamic properties, the dipole moments, HOMO-LUMO energy has been calculated by using the Density Functional Theory (DFT) method with 6-311G(d,p) and 6-311++G(d,p) basis sets. H-1 and C-13 NMR chemical shifts show good agreement with experimental values. According to calculated results, the 6-311G(d,p) and 6-311++G(d,p) basis sets have showed similar results. Thermo gravimetric analysis (TGA and DTA) carried out to obtain information about its thermal stability. The molecular structure and spectroscopic properties of the ligand were obtained with FT-IR, H-1 and C-13 NMR, UV-vis investigations as experimentally and compared with theoretical results obtained from DFT/B3LYP/6-311 + +G(d,p) basis set. In addition to molecular calculations of the title compound, molecular electrostatic potential (MEP), dipole moments, atomic charges, HOMO-LUMO, NLO and NBO analysis were computed. The calculated results show that the optimized geometry can well reproduce the crystal structure parameters, and the theoretical vibrational frequencies, H-1 and C-13 NMR chemical shifts show good agreement with experimental values.     WOS: 000462800500002Two novel tridentate SNS pincer type ligands, 2,6-bis[[(2-methoxyphenyl)thio]methyl]pyridine (L1) and 2,6-bis[[(2-chlorophenyl)thio]methyl]pyridine (L2), each possessing two sulfur and one nitrogen donor functionalities (SNS), based on 2,6-bis(thioether)pyridine ligands were prepared and metallised with and metal salts. Two new unanticipated complexes were obtained from the L2 ligand, the dimeric bidentate Cu(I) complex and tridentate Fe(II) complex while two new tridentate pincer-type complexes M(-L1)] () were formed from the L1 ligand. It was observed that the structure of this Cu(I) complex has a tetrahedral geometry using single crystal X-ray diffraction analysis. In addition, catalytic properties of metal complexes towards the formation of cyclic carbonates from and epoxides were investigated. The less sterically hindered Fe(II) complex with the L1 ligand (2) showed the best catalytic activity. Several parameters including temperature, time, epoxide identity and pressure were investigated to find the optimum catalytic reaction conditions.
