Additional Burden of Diseases Associated with Cadmium Exposure: A Case Study of Cadmium Contaminated Rice Fields in Mae Sot District, Tak Province, Thailand
The cadmium (Cd) contaminated rice fields in Mae Sot District, Tak Province, Thailand has been one of the major environmental problems in Thailand for the last 10 years. We used disability adjusted life years (DALYs) to estimate the burden of disease attributable to Cd in terms of additional DALYs of Mae Sot residents. Cd exposure data included Cd and β2–microglobulin (β2-MG) in urine (as an internal exposure dose) and estimated cadmium daily intake (as an external exposure dose). Compared to the general Thai population, Mae Sot residents gained 10%–86% DALYs from nephrosis/nephritis, heart diseases, osteoporosis and cancer depending on their Cd exposure type and exposure level. The results for urinary Cd and dietary Cd intake varied according to the studies used for risk estimation. The ceiling effect was observed in results using dietary Cd intake because of the high Cd content in rice grown in the Mae Sot area. The results from β2-MG were more robust with additional DALYs ranging from 36%–86% for heart failure, cerebral infraction, and nephrosis/nephritis. Additional DALYs is a useful approach for assessing the magnitude of environmental Cd exposure. The Mae Sot population lost more healthy life compared to populations living in a non- or less Cd polluted area. This method should be applicable to various types of environmental contamination problems if exposure assessment information is available
Modeling Dynamic Responses to COVID-19 Epidemics: A Case Study in Thailand
Quantifying the effects of control measures during the emergence and recurrence of SARS-CoV-2 poses a challenge to understanding the dynamic responses in terms of effectiveness and the population’s reaction. This study aims to estimate and compare the non-pharmaceutical interventions applied in the first and second outbreaks of COVID-19 in Thailand. We formulated a dynamic model of transmission and control. For each outbreak, the time interval was divided into subintervals characterized by epidemic events. We used daily case report data to estimate the transmission rates, the quarantine rate, and its efficiency by the maximum likelihood method. The duration-specific control reproduction numbers were calculated. The model predicts that the reproduction number dropped by about 91% after the nationwide lockdown in the first wave. In the second wave, after a high number of cases had been reported, the reproduction number decreased to about 80% in the next phase, but the spread continued. The estimated value was below the threshold in the last phase. For both waves, successful control was mainly induced by decreased transmission rate, while the explicit quarantine measure showed less effectiveness. The relatively weak control measure estimated by the model may have implications for economic impact and the adaptation of people
Effects of participatory action-oriented training (PAOT) intervention for hypertension management among intercity van drivers
Objectives
This study aims to evaluated the effectiveness of participatory action-oriented training (PAOT) intervention for hypertension management among intercity van drivers.
Material and Methods
This quasi-experimental study applied concept and process of participatory action-oriented training and self-management to guide the development of the intervention addressing improvement in hypertension management behaviors. A total of 104 intercity van drivers with uncontrolled hypertension in Thailand were recruited to participate in this program. The intervention group (N = 52) received PAOT program, while the control group (N = 52) received conventional program. Data on hypertension management behaviors, and blood pressure were measured at baseline, 1 month and 3 months after intervention.
Results
At 3 months after intervention, hypertension management behavior, and systolic blood pressure were significantly different between 2 groups (p < 0.05).
Conclusions
This PAOT was found to be feasible and could potentially improve hypertension management, and blood pressure level of intercity van drivers. The program should be applied in further studies with other workplaces in both formal and informal sectors with different characteristics and other health issues
