    Heterogeneity in Schooling Rates of Return

    This paper relaxes the assumption of homogeneous rates of return to schooling by employing nonparametric kernel regression. This approach allows us to examine the differences in rates of return to education both across and within groups. Similar to previous studies we find that on average blacks have higher returns to education than whites, natives have higher returns than immigrants and younger workers have higher returns than older workers. Contrary to previous studies we find that the average gap of the rate of return between white and black workers is larger than previously thought and the gap is smaller between immigrants and natives. We also uncover significant heterogeneity, the extent of which differs both across and within groups. The estimated densities of returns vary across groups and time periods and are often skewed. For example, during the period 1950-1990, at least 5% of whites have negative returns. Finally, we uncover the characteristics common amongst those with the smallest and largest returns to education.     Adverse drug reaction reporting among health care workers at Mulago National Referral and Teaching hospital in Uganda

    Background: Adverse Drug Reactions (ADRs) are an important contributor to patient morbidity and hospitalisation in Uganda. Under-reporting of ADRs may increase medicine-induced morbidity and mortality among patients. This study determined the extent of ADR reporting, and associated factors, among healthcare workers in Uganda.Methods: A quantitative, cross-sectional, study was conducted. Pretested, semi-structured questionnaires were administered to 289 randomly sampled healthcare workers over a three-month period in Mulago National Referral Hospital, Uganda. The primary outcome was the proportion of healthcare workers who had ever reported an ADR. Data was double-entered in Epidata version 3.0, cleaned and exported to STATA version 10.1 for analysis.Results: The overall response rate was 77.2% (n=223). The majority of the respondents were females (139, 62.3%). The median age of all respondents was 32.6 years (min-23; max-65). Only about 16.6% (n=37) of healthcare workers had ever reported an ADR. Very few (n= 84, 37.7%) healthcare workers knew the tools used in ADR reporting. Less than a quarter (n=41, 18.4%) of the healthcare workers knew where to report ADRs. Lack of training was reported as the major (56.5%, 126) deterrent to reporting ADRs by healthcare workers.Conclusion: Adverse drug reactions are under-reported in Uganda, and healthcare workers have insufficient knowledge of existing pharmacovigilance systems, including ADR reporting systems. To address these challenges, there is need to sensitize and train healthcare workers in patient-centred aspects of medicine surveillance, so as to provide appropriate care while optimising patient safety.Keywords: Adverse drug reaction, ADR, adverse effects, reporting, healthcare worker, pharmacovigilance, Ugand

    Growth of climate change commitments from HFC banks and emissions

    Chlorofluorocarbons (CFCs) are the primary cause of ozone depletion, and they also contribute to global climate change. With the global phaseout of CFCs and the coming phaseout of hydrochlorofluorocarbons (HCFCs), the substitute hydrofluorocarbons (HFCs) are increasingly used. While CFCs were originally used mainly in applications such as spray cans and were released within a year after production, concern about the ozone layer led to reductions in rapid-release applications, and the relative importance of slower-release applications grew. HFCs are now mainly used in refrigerators and air conditioners (AC) and are released over years to a decade after production. Their containment in such equipment represents banks, which are building up as production grows. A key finding of our work is that the increases of HFC banks represent a substantial unseen commitment to further radiative forcing of climate change also after production of the chemicals ceases. We show that earlier phaseouts of HFCs would provide greater benefits for climate protection than previously recognized, due to the avoided buildup of the banks. If, for example, HFC production were to be phased out in 2020 instead of 2050, not only could about 91–146 GtCO2-eq of cumulative emission be avoided from 2020 to 2050, but an additional bank of about 39–64 GtCO2-eq could also be avoided in 2050. Choices of later phaseout dates lead to larger commitments to climate change unless growing banks of HFCs from millions of dispersed locations are collected and destroyed

    Proactive and reactive cognitive control and dorsolateral prefrontal cortex dysfunction in first episode schizophrenia.

    Cognitive control deficits have been consistently documented in patients with schizophrenia. Recent work in cognitive neuroscience has hypothesized a distinction between two theoretically separable modes of cognitive control-reactive and proactive. However, it remains unclear the extent to which these processes are uniquely associated with dysfunctional neural recruitment in individuals with schizophrenia. This functional magnetic resonance imaging (fMRI) study utilized the color word Stroop task and AX Continuous Performance Task (AX-CPT) to tap reactive and proactive control processes, respectively, in a sample of 54 healthy controls and 43 patients with first episode schizophrenia. Healthy controls demonstrated robust dorsolateral prefrontal, anterior cingulate, and parietal cortex activity on both tasks. In contrast, patients with schizophrenia did not show any significant activation during proactive control, while showing activation similar to control subjects during reactive control. Critically, an interaction analysis showed that the degree to which prefrontal activity was reduced in patients versus controls depended on the type of control process engaged. Controls showed increased dorsolateral prefrontal cortex (DLPFC) and parietal activity in the proactive compared to the reactive control task, whereas patients with schizophrenia did not demonstrate this increase. Additionally, patients' DLPFC activity and performance during proactive control was associated with disorganization symptoms, while no reactive control measures showed this association. Proactive control processes and concomitant dysfunctional recruitment of DLPFC represent robust features of schizophrenia that are also directly associated with symptoms of disorganization

    CIDI-Lung-Seg: A Single-Click Annotation Tool for Automatic Delineation of Lungs from CT Scans

    Accurate and fast extraction of lung volumes from computed tomography (CT) scans remains in a great demand in the clinical environment because the available methods fail to provide a generic solution due to wide anatomical variations of lungs and existence of pathologies. Manual annotation, current gold standard, is time consuming and often subject to human bias. On the other hand, current state-of-the-art fully automated lung segmentation methods fail to make their way into the clinical practice due to their inability to efficiently incorporate human input for handling misclassifications and praxis. This paper presents a lung annotation tool for CT images that is interactive, efficient, and robust. The proposed annotation tool produces an "as accurate as possible" initial annotation based on the fuzzy-connectedness image segmentation, followed by efficient manual fixation of the initial extraction if deemed necessary by the practitioner. To provide maximum flexibility to the users, our annotation tool is supported in three major operating systems (Windows, Linux, and the Mac OS X).     Deep Neural Networks Rival the Representation of Primate IT Cortex for Core Visual Object Recognition

    The primate visual system achieves remarkable visual object recognition performance even in brief presentations and under changes to object exemplar, geometric transformations, and background variation (a.k.a. core visual object recognition). This remarkable performance is mediated by the representation formed in inferior temporal (IT) cortex. In parallel, recent advances in machine learning have led to ever higher performing models of object recognition using artificial deep neural networks (DNNs). It remains unclear, however, whether the representational performance of DNNs rivals that of the brain. To accurately produce such a comparison, a major difficulty has been a unifying metric that accounts for experimental limitations such as the amount of noise, the number of neural recording sites, and the number trials, and computational limitations such as the complexity of the decoding classifier and the number of classifier training examples. In this work we perform a direct comparison that corrects for these experimental limitations and computational considerations. As part of our methodology, we propose an extension of "kernel analysis" that measures the generalization accuracy as a function of representational complexity. Our evaluations show that, unlike previous bio-inspired models, the latest DNNs rival the representational performance of IT cortex on this visual object recognition task. Furthermore, we show that models that perform well on measures of representational performance also perform well on measures of representational similarity to IT and on measures of predicting individual IT multi-unit responses. Whether these DNNs rely on computational mechanisms similar to the primate visual system is yet to be determined, but, unlike all previous bio-inspired models, that possibility cannot be ruled out merely on representational performance grounds.Comment: 35 pages, 12 figures, extends and expands upon arXiv:1301.353

    Non-Salmonella Bacteremia Among Seropositive Hiv Patients Attending Three Tertiary Hospital In Nasarawa State, Nigeria.

    Background: Bacterial blood stream infections constitute a significant public-health problem and it is an important cause of morbidity and mortality in HIV infected patients. Little is known in developing countries regarding salmonella bacteraemia among HIV patients. The purpose of this study was to determine the bacterial pathogens causing blood stream infection among febrile adults attending three medical centers in Nasarawa State. Methods: A prospective cross-sectional study involving 346 consecutive, febrile adult patients admitted at three medical centers in Nasarawa State, was conducted. Demographic and other data were collected using standardized questionnaires. Blood culture was done followed by susceptibility testing using disc diffusion method. HIV testing was also performed as per Nigeria national algorithm and total white blood cell counts and CD4+ counts determined. Results: Of 346 febrile adult patients 33 (9.5%) had blood stream infections. The common isolates were Salmonella spp 13(39.4%), Escherichia coIi 8 (24.2%), Streptococcus pneumonia 5(15.2%), Staphylococcus aureus 4(12.1%), Citrobacter spp 1(3%), Streptococcus pyogenes 1(3%) and Kiebsiella pneumonia 1(3%). A total of 156 (45.1%) patients were HIV infected; of whom 12/1 56 (7.6%) were infected by non-typhoid Salmonella spp compared to 1/190 (0.5%) of non-HIV infected patients (RRR 11.2, p=0.029) infected with Salmonella typhi. HIV infected patients with bacteremia had significantly lower CD4+ count than those without bacteremia (median 28 vs. 88 cells/mI, p=0.01). Patients with salmonella bacteremia had significantly lower median of WBC than those with non-salmonella as well as those without bacteremia (median, 3.6 vs. 17.5 vs. 9.8x109, p0.0001). All Salmonella spp were sensitive to ceftriaxone and imipenem, while being 84%, 69.2%, 38% and 8% resistant to chloramphenicol, ampicillin, sulphamethaxazole/trimethoprim and ciprofloxacin respectively. Predictors of mortality were HIV infection (OR 2.3, p=0.005), Glasgow coma score of less than 15 (OR 3.4, p=0.0001) and night sweats (OR 2.4, p=0.014). Conclusion: Non-typhoid Salmonella spp that are highly resistant to common antibiotics are predominant cause of bacterial blood stream infection among HIV patients attending Three tertiary medical centers in Nasarawa State. Continuous surveillance and intervention strategies should be put in place to monitor and manage cases of bloodstream infections in HIV-positive patients in  Nasarawa state.

    Long Range Hops and the Pair Annihilation Reaction A+A->0: Renormalization Group and Simulation

    A simple example of a non-equilibrium system for which fluctuations are important is a system of particles which diffuse and may annihilate in pairs on contact. The renormalization group can be used to calculate the time dependence of the density of particles, and provides both an exact value for the exponent governing the decay of particles and an epsilon-expansion for the amplitude of this power law. When the diffusion is anomalous, as when the particles perform Levy flights, the critical dimension depends continuously on the control parameter for the Levy distribution. The epsilon-expansion can then become an expansion in a small parameter.
