    Thermal deformation of concentrators in an axisymmetric temperature field

A governing differential equation is derived using Mushtari-Donnel-Vlasov simplifications, and a solution is presented which makes it possible to determine the principal deformation characteristics

    Dependent coordinates in path integral measure factorization

    The transformation of the path integral measure under the reduction procedure in the dynamical systems with a symmetry is considered. The investigation is carried out in the case of the Wiener--type path integrals that are used for description of the diffusion on a smooth compact Riemannian manifold with the given free isometric action of the compact semisimple unimodular Lie group. The transformation of the path integral, which factorizes the path integral measure, is based on the application of the optimal nonlinear filtering equation from the stochastic theory. The integral relation between the kernels of the original and reduced semigroup are obtained.Comment: LaTeX2e, 28 page

    In the face of COVID-19 pandemic, the postgraduate education has been changed due to distance learning. Using Microsoft Teams program, the interns and students of the Department of Neurology and Neurosurgery of Odesa National Medical University continue to study neurology. Analysis of case studies after watching a neurocinema, training in making the diagnosis and administration of treatment are in complement to the forced lacking of clinical practice. Neurocinema provides the opportunity for addressing a broad range of diseases, for instance, stroke, traumatic brain injury, traumatic spinal cord injury, multiple sclerosis, amyotrophic lateral sclerosis, epilepsy, Parkinson’s disease, Alzheimer’s disease etc. A modern approach with the watchful guidance of teachers and the possibility of a discussion in real-time increases the activity and interest of interns and students.В умовах пандемії COVID-19 отримання післядипломної освіти змінилося завдяки дистанційному навчанню. Використовуючи програму відеоконференц-звʼязку Microsoft Teams, на кафедрі неврології та нейрохірургії Одеського національного медичного університету інтерни-невропатологи й студенти активно продовжують вивчення неврології. Розбір клінічних випадків після перегляду фільмів, що присвячені неврологічним хворим, тренування у виставленні функціонального та клінічного діагнозів й призначенні лікування доповнюють вимушену нестачу практики з пацієнтами. Нейрокінематограф дозволяє широко охопити такі хвороби, як інсульт, черепно-мозкова травма, хребетно-спинномозкова травма, розсіяний склероз, боковий аміотрофічний склероз, епілепсія, хвороба Паркінсона, хвороба Альцгеймера та ін. Такий сучасний підхід з пильним керівництвом викладачів і можливістю дискусії у режимі реального часу збільшує активність, зацікавленість лікарів-інтернів та студентів

    The authors thank Ph. D. M. K. Alibaeva, Ph. D. I. A. Gudkova and Ph. D. I. V. Korolkova for participation in the research.The review summarizes experimental data on the phase formation, structure and properties of new complex oxide compounds group - triple molybdates containing tetrahedral molybdate ion, two different singly charged cation, together with tri- or divalent cation. The several structural families of these compounds were distinguished and it shown that many of them are of interest as luminescent, laser, ion-conducting or nonlinear optical materials.The work is executed at partial support of the Russian Foundation for basic research (projects No. 08-03-00384, 13-03-01020 and 14-03-00298)

    Observance of Human Rights in the Use of AI as Technology

    [Purpose] The purpose of article is devoted to the study of the specifics of legal regulation, use and development of Artificial Intelligence (AI) and related issues of respect for fundamental human rights in modern Ukraine and the EU. [Methodology] The normative bases of legal regulation of relations in the field of AI are considered. One of the methods, used in this article, is establishment of such general rules for the use of AI that could not violate fundamental human rights. [Practical Implications] The practical significance lies in establishment that the use of AI is a multi-vector phenomenon that affects all spheres of human life and can violate various groups of rights [Findings] It was concluded that in order to regulate the use of AI, states need to pay attention to and take into account the Recommendations on AI developed by the European Commission and find a compromise approach to the use of AI, which provides for respect for fundamental human rights.[Purpose] The purpose of article is devoted to the study of the specifics of legal regulation, use and development of Artificial Intelligence (AI) and related issues of respect for fundamental human rights in modern Ukraine and the EU.     Systemic vasculitides: сlinical cases

    Systemic vasculitides (SV) are a group of diseases characterized by vessel wall inflammation that leads to ischemic changes in the organs and tissues, which the respective vessels supply blood to; the spectrum of the clinical manifestations of SV depends on the type, size, location of the affected vessels, and on the activity of systemic inflammation. The etiology of the majority of primary vasculitides is unknown. It is assumed that many microorganisms may initiate inflammation of vessels of various sizes, but only some forms of SV (and in some patients) can be clearly related to certain causative and/or trigger factors (drug hypersensitivity, hepatitis B virus, hepatitis C virus, etc.). The paper gives clinical examples of the timely diagnosis and treatment of 2 SV cases in young men working on the railway of the town of Ruzaevka

    Literary and Tourist Project “OmskLiteraTur” as a Way to Preserve Cultural Tradition

    Статья поступила в редакцию 27.03.2023 г.В статье прослежено соотношение тенденции сохранения устойчивой культурной традиции с процессом популяризации литературного наследия Омского региона посредством создания сайта, подготовки новых экскурсий, систематизации литературного контента. Проект «ОмскЛитераТур» позволил обратить внимание на проблему сохранения и продвижения культурного наследия. В ходе исследования было проведено входное тестирование на знание омского литературного наследия, был составлен реестр литературных экскурсионных объектов, разработаны сценарии литературных видеоэкскурсий, взяты интервью и т. д. Основное внимание в исследовании-проекте было уделено молодежи как перспективной аудитории для трансляции культурных традиций. На основе изучения литературного творчества Омского региона сделаны выводы о взаимосвязи трансляции и сохранения культурных ценностей как элементе сохранения культурного наследия в целом.The article traces the tendency of correlation between the preservation of a stable cultural tradition and the process of popularization of the literary heritage of he Omsk region through creation of a website, creation of new excursions, and systematization of literary content. The analysis of the conducted research allowed us to make a conclusion about the importance of information promotion. The project “OmskLiteraTur” allowed to draw attention to the problem of preservation and promotion of cultural heritage. In the course of the study, an entrance test was conducted for knowledge of the Omsk literary heritage, a register of literary excursion objects was compiled, scenarios of literary video tours were developed, interviews were taken, etc. The main emphasis in the research project was placed on young people as participants of the project, as a promising audience for broadcasting cultural traditions of a literary nature.     Опыт применения чрескожного обхода правого желудочка в раннем периоде после имплантации системы левожелудочкового обхода (клиническое наблюдение и обзор литературы)

    Implantable left ventricular assist device (LVAD) is a state-of-the-art treatment for adults and children with end-stage heart failure. The early and late period after LVAD implantation can be severely complicated. Right ventricular failure (RVF) still remains a common complication after LVAD implantation. RVF is the cause of reduced post-implant survival. We suggest that an additional temporary or permanent right ventricular assist device (RVAD) is an effective treatment for LVAD-associated RVF. In this clinical case report, we describe the medical history of a pediatric patient (14 years old) with severe heart failure (PediMACS Level 1) against a background of dilated cardiomyopathy. The patient required peripheral venoarterial extracorporeal membrane oxygenation (VA-ECMO) prior to urgent LVAD (HM3) implantation. In the early post-LVAD implantation (1 POD) period, the patient presented with hemodynamic and echocardiographic events of acute RVF that was resistant to drug therapy (inotropic/vasopressor support, iNO) and required mechanical circulatory support (MCS) with a preoperatively implanted VA-ECMO. In the LVAD-associated RVF scenario, VA-ECMO as a means of total cardiac bypass is a non-physiological MCS method and, therefore, undesirable. In this clinical situation, our solution was to use a paracorporeal centrifugal blood pump for temporary right heart support. A RVAD was assembled using percutaneous cannulation in two sites and a modification of the pre-existing VA-ECMO circuit. For RVAD, we used an ECMO cannula previously installed through the femoral vein (26 F) and added a reverse venous cannula (22 F) through the right internal jugular vein into the pulmonary trunk. To facilitate the passage of the return cannula into the pulmonary artery, we used a contralateral sheath (6 F, 40 cm) and an Amplatz Super Stiff guidewire under radiological control. The oxygenator was removed from the circuit on day 2 of RVAD. Central hemodynamics (reduction in right atrial pressure (RAP) to 10 mm Hg, increase in pulmonary capillary wedge pressure (PCWP) to 14 mm Hg), as well as right ventricular (RV) and left ventricular (LV) volume characteristics all improved. These observations allowed us to optimize the performance of the implantable LVAD (increase in flow rate to 4.2 l/min or 2.1 l/min/m2). The duration of paracorporeal RVAD after LVAD implantation was 7 days with an average flow rate of 2.3 ± 0.2 l/min. Postoperative treatment in the intensive care unit (ICU) lasted for 15 days. The patient was discharged from the hospital on postoperative day 34.Система имплантируемого левожелудочкового обхода (LVAD) - это современный метод лечения взрослых и детей с сердечной недостаточностью в терминальной стадии. Ранние и поздние периоды после имплантации LVAD могут серьезно осложняться. Дисфункция правого желудочка (Д11Ж) по-прежнему остается одним из самых частых осложнений у пациентов с LVAD и причиной низкой постимплантационной выживаемости. Мы предполагаем, что дополнительное временное или постоянное вспомогательное устройство для правого желудочка (RVAD) является эффективным методом лечения LVAD-ассоциированного ДПЖ. В данном клиническом случае мы описываем историю болезни педиатрического пациента (14 лет) с тяжелой сердечной недостаточностью (Pedimacs Level 1) на фоне дилатационной кардиомиопатии, которому потребовалась периферическая ВА ЭКМО перед срочной имплантацией LVAD (HM3). В раннем постимплантационном (1 POD) периоде LVAD у этого пациента наблюдались гемодинамические и эхокардиографические явления остро развившейся ДПЖ, резистентной к медикаментозной терапии (инотропная/ вазопрессорная поддержка, iNO) и потребовавшей механической поддержки кровообращения с помощью предоперационно имплантированного ВА ЭКМО. В сценарии LVAD-ассоциированной ДПЖ ВА ЭКМО как средство полного шунтирования сердца является нефизиологическим методом МПК, и следовательно, нежелательным. В этой клинической ситуации нашим решением было использование паракорпорального центробежного насоса для временной поддержки правых отделов сердца. Вспомогательное устройство для правого желудочка (RVAD) было собрано с использованием метода чрескожной канюляции в двух местах и модификации ранее существовавшего контура ВА ЭКМО. Для RVAD мы использовали канюлю ЭКМО, установленную ранее через бедренную вену (26 F), и добавили возвратную венозную канюлю (22 F) через правую внутреннюю яремную вену в ствол легочной артерии. Для облегчения прохождения возвратной канюли в легочную артерию мы использовали контралатеральный интродьюсер (6 F, 40 см) и проводник типа Amplatz Super Stiff под рентгенологическим контролем. Оксигенатор был удален из контура на вторые сутки RVAD. На фоне ПЖО мы отметили улучшение показателей центральной гемодинамики (снижение ДПП до 10 мм рт. ст., увеличение ЗДЛА до 14 мм рт. ст.) и объемных характеристик ПЖ и ЛЖ, что позволило оптимизировать функционирование иЛЖО (увеличение производительности до 4,2 л/мин, или 2,1 л/ мин/м2). Продолжительность паракорпорального ПЖО после имплантации ЛЖО составила 7 суток при усредненной производительности 2,3 ± 0,2 л/мин. Период послеоперационного лечения в условиях ОРИТ составил 15 суток. Пациент выписан из стационара на 34-е послеоперационные сутки
