    Estudo do abandono do tratamento odontológico em um serviço público de Porto Alegre: o Centro de Saúde Murialdo

    This study aimed at describing and analyzing patient’s abandoning odontological treatment at the “Centro de Satide Murialdo” [Murialdo Healthcare Center| set up in Porto Alegre, RS, Brazil. Twenty-nine patients, or 30% out of a total of 100, who had used the available service in search of odontological attention in the second semester of 1998 and had quit treatment before clinical discharge, were interviewed at home. It was found out that the majority of patients are females, were seen during morning shift, and dwell, at an average of 15.6 years, in the area ascribed to Murialdo Healthcare Center. Dwellings are selfowned, residents are natives of Porto Alegre and have two children on average. Their average age is 30.2 years old, educational level is incomplete elementary school and the average family income is 3.2 minimum salaries. They looked for odontotological treatment due to pain or dental cavity. Patients refer that they were very well received when they enrolled at Murialdo Healthcare Center’s odontological program. Concerning information received, they remember being told that they were allowed to be absent twice for justified reasons only and that they should go every week in order to complete treatment. At the appointment meeting which is peculiar to the service, they were warned against missing dates set up and told how treatment worked. They were well received on the first appointment, were given explanation on what was wrong with their mouths and on what to do so problems would not return. Their main problem was not solved during the first consultation and this factor did not influence treatment discontinuity. They report that treatments lasted two months on average and that it was not necessary to get away from important activities in order to be treated. Of those patients who pointed out a cause for treatment interruption, the need to be absent from work was referred to as their main reason. Patients are users of “Sistema Unico de Satide and point out pain as a problem in buccal health. The following are quoted as causes for quitting: not being able to be absent from work, length of time for treatment, and external causes to the service, such as illness, pregnancy and change of address. Up to abandonment, treatment offered was considered good. Data are analyzed and discussed in an attempt to establish a reflection on publicsector service evaluation and to enhance patients’ worth as social actors and sources of information. Therefore, it is a first evaluation approach regarding Murialdo Healthcare Center from the perspective of patients who abandoned odontological treatment with a view to contributing to health rationality in improving the “Sistema Unico de Satide” in the city of Porto Alegre, State of Rio Grande do Sul, Brazil.Este estudo objetivou descrever e analisar o abandono do tratamento odontológico no Centro de Saúde Murialdo, em Porto Alegre, RS. Foram entrevistados em seus domicílios vinte e nove pacientes, 30% de um total de 100 que consultaram o serviço em busca de atenção odontológica, no segundo semestre de 1998, e que abandonaram o tratamento antes da alta clínica. Citam-se como principais causas do abandono a impossibilidade de faltar ao trabalho, o tempo de duração do tratamento demasiado longo e as causas externas ao serviço, como doenças, gravidez e mudança de domicílio.     The present work is a case study of 8 bipolar patients under lithium therapy, being all of them members of the lithium group of the Hospital de Clínicas de Porto Alegre. It describes the dental management of bipolar patients under lithium therapy, emphasizing dental caries and the clinical method of intervention. The work was divided into three phases, existing a ninety-day interval between each ofthem. In the first part ofthe research, dental examinations were carried out and the patients were interviewed with the special purpose of describing each one of them regarding his buccal health. They were also inquired about their experience in living with the bipolar disease, about the medication taken and its side effects. The patients received dental treatment according to the clinical method as they also attended health educational sessions destined to increase their autonomy up to the dental dismissal. In the second and third phases, the exams that had been carried out in the first phase were repeated in order to analyze the commitment of each bipolar patient to the proposed dental treatment. The questions about the relationship between the bipolar patient and the treatment of this disease were also repeated. It was concluded that the results of the dental exams were related to the degree of stabilization of the bipolar disease; the patients detaining the best results in the dental treatment were those who had the bipolar disease stabilized. It was concluded that, having in mind the occurrence frequency of bipolar disease, it would be plausible that a denfist who works in the Public Service sooner or later would attend bipolar patients; bipolar patients are a peculiar population group that need a specific dental program; the planning and scheduling of these specific odontological programs for bipolars should take into account the disruptions in the dental therapy, caused by the disease and it is also necessary to renegotiate with the bipolar, his responsibilities and his motivation for buccal self-care, which will result in more frequent dental visits.O presente trabalho é um estudo de casos com 8 pacientes bipolares, em litioterapia do grupo do lítio, do Hospital de Clínicas de Porto Alegre. Descreve o manejo odontológico do paciente bipolar em litioterapia, com ênfase para a cárie dentária, no método clínico de intervenção. O trabalho dividiu-se em três fases, com intervalos de 90 dias. Na primeira fase da pesquisa realizou-se exames odontológicos e entrevista semiestruturada para descrever o paciente quanto a sua saúde bucal e quanto a sua convivência com a doença bipolar, com a medicação e efeitos adversos. Os pacientes receberam tratamento odontológico no método clínico e sessões de educação em saúde objetivando o aumento de sua autonomia em relação à saúde bucal até o momento da alta odontológica. Na segunda fase da pesquisa foram repetidos os exames da primeira fase, analisada a adesão do paciente bipolar ao tratamento odontológico e o paciente foi novamente entrevistado quanto a sua relação com a doença e o tratamento desta. Na terceira fase da pesquisa realizou-se os exames odontológicos e as mesmas questões da segunda fase. Observou-se que os resultados dos exames odontológicos relacionaram-se com o grau de estabilização da doença bipolar. Os pacientes com os quais se obteve os melhores resultados no tratamento odontológico foram os que estavam com a doença bipolar estabilizada. Concluiu-se que dada a frequência da doença bipolar é lícito esperar que em algum momento o cirurgião-dentista do setor público atenda pacientes bipolares; os pacientes bipolares constituem grupo populacional específico necessitando de programa odontológico específico; os programas odontológicos específicos para os bipolares devem considerar no planejamento e no agendamento as interrupções na terapia odontológica vinculadas às características da doença bipolar, é necessário repactuar as responsabilidades e a motivação para o autocuidado bucal, o que implica em consultas odontológicas mais frequentes

This Paper examines demographic aspects, current concepts,introduces new concepts concerning Gerodontology And shows how the Gerodontology is starting as a discipline at Universidade Federal do Rio Grande do Sul Dental School.O número de pessoas idosas está aumentando no mundo. Este aumento na população idosa é mais rápido nos países em desenvolvimento, incluindo o Brasil. As necessidades de saúde bucal e a preservação de dentes entre as pessoas idosas têm aumentado o interesse na Odontogeriátria.
