The Sun's Journey Through the Local Interstellar Medium: The PaleoLISM and Paleoheliosphere
Over the recent past, the galactic environment of the Sun has differed
substantially from today. Sometime within the past ~130,000 years, and possibly
as recent as ~56,000 years ago, the Sun entered the tenuous tepid partially
ionized interstellar material now flowing past the Sun. Prior to that, the Sun
was in the low density interior of the Local Bubble. As the Sun entered the
local ISM flow, we passed briefly through an interface region of some type. The
low column densities of the cloud now surrounding the solar system indicate
that heliosphere boundary conditions will vary from opacity considerations
alone as the Sun moves through the cloud. These variations in the interstellar
material surrounding the Sun affected the paleoheliosphere.Comment: To be published in Astrophysics and Space Sciences Transactions

Destruction of Interstellar Dust in Evolving Supernova Remnant Shock Waves
Supernova generated shock waves are responsible for most of the destruction
of dust grains in the interstellar medium (ISM). Calculations of the dust
destruction timescale have so far been carried out using plane parallel steady
shocks, however that approximation breaks down when the destruction timescale
becomes longer than that for the evolution of the supernova remnant (SNR)
shock. In this paper we present new calculations of grain destruction in
evolving, radiative SNRs. To facilitate comparison with the previous study by
Jones et al. (1996), we adopt the same dust properties as in that paper. We
find that the efficiencies of grain destruction are most divergent from those
for a steady shock when the thermal history of a shocked gas parcel in the SNR
differs significantly from that behind a steady shock. This occurs in shocks
with velocities >~ 200 km/s for which the remnant is just beginning to go
radiative. Assuming SNRs evolve in a warm phase dominated ISM, we find dust
destruction timescales are increased by a factor of ~2 compared to those of
Jones et al. (1996), who assumed a hot gas dominated ISM. Recent estimates of
supernova rates and ISM mass lead to another factor of ~3 increase in the
destruction timescales, resulting in a silicate grain destruction timescale of
~2-3 Gyr. These increases, while not able resolve the problem of the discrepant
timescales for silicate grain destruction and creation, are an important step

Elimination of mother- to - child transmission of HIV in South Africa : Rapid scale- up using quality improvement
PKBackground: South Africa (SA) is committed to achieving the goal of eliminating mother-to-child transmission (MTCT) of HIV by 2015. To achieve this, universal coverage of quality antenatal, labour, delivery and postnatal services for all women has to be attained. Over the past decade, the prevention of mother-to-child transmission (PMTCT) programme has been scaled up to reach all healthcare facilities in the country. However, challenges persist in achieving 100% coverage and access to the programme.
Objectives: We describe the process undertaken by the National Department of Health (NDoH), in collaboration with partners, to develop, implement and monitor a data-driven intervention to improve facility, district, provincial and national PMTCT-related performance.
Methods: Between 2011 and 2013, the NDoH developed and implemented an intervention using data-driven participatory processes to understand facility-level bottlenecks to optimise PMTCT implementation and to scale up priority PMTCT actions nationally.
Results: There was remarkable improvement across all key indicators in the PMTCT cascade over the 3 years 2011 - 2013. Simple monitoring tools such as a visual dashboard and data for action reports were successfully used to improve the performance of the PMTCT programme across SA. MTCT has shown a significant downward trend.
Conclusions: It is feasible to implement district-level, data-driven quality improvement processes at a national scale to improve the performance of the PMTCT programme at the local level
Boundary Conditions of the Heliosphere
Radiative transfer equilibrium models of nearby interstellar matter (ISM)
yield the boundary conditions of the heliosphere when constrained with
observations of ISM inside and outside of the heliosphere. Filtration factors
for interstellar neutrals crossing the heliosheath region, from charge exchange
with interstellar plasma, are given for H, He, N, O, Ar, and Ne. The best
models predict n(HI)~0.2 /cc, n(e)~0.1 /cc, however if the isotropic 2 kHz
emission observed by Voyager (Kurth & Gurnett 2003) is formed in the
surrounding ISM, an alternate model with lower electron densities is indicated.
Observations of nearby ISM, the radiative transfer models, and historical 10Be
records provide information on past variations in the galactic environment of
the Sun
