    Improved Relation Networks for End-to-End Speaker Verification and Identification

    Speaker identification systems in a real-world scenario are tasked to identify a speaker amongst a set of enrolled speakers given just a few samples for each enrolled speaker. This paper demonstrates the effectiveness of meta-learning and relation networks for this use case. We propose improved relation networks for speaker verification and few-shot (unseen) speaker identification. The use of relation networks facilitates joint training of the frontend speaker encoder and the backend model. Inspired by the use of prototypical networks in speaker verification and to increase the discriminability of the speaker embeddings, we train the model to classify samples in the current episode amongst all speakers present in the training set. Furthermore, we propose a new training regime for faster model convergence by extracting more information from a given meta-learning episode with negligible extra computation. We evaluate the proposed techniques on VoxCeleb, SITW and VCTK datasets on the tasks of speaker verification and unseen speaker identification.     Sustainable Community-Based Solutions: Source to Tap and Back Again

    This article reflects the thoughts and conclusions of a session sponsored by the Women in Water Programme Steering Committee of the International Water Association (IWA) at the Second IWA Development Congress and Exhibition held in Kuala Lumpur, Malaysia in November 2011. The session, held on 24 November, examined the role women can play in community-based water and sanitation solutions in urbanizing areas. Such solutions range from watershed protection and integrated water resources management (IWRM), to capacity development and stakeholder mobilization. The cases presented reflect the experience and results of projects undertaken by the authors in Asia. The experience of each author brings a broad range of approaches to tackling the challenges faced by water managers in densely populated urban areas. These provide the basis for innovative solutions to expand coverage, strengthen service provision and create new businesses. The article explores ways for communities to work together more effectively to increase knowledge and understanding of the wide variety of interests that exist in a water basin. In particular, community-based solutions can promote protection of the sources of water, and sustainable water and sanitation management. Source to tap and back again speaks to the imperative that all people understand the relationship between upstream and downstream users, and that both women and men be involved in resource management.     The disruptive impact of COVID-19 on the utilization of cancer chemotherapy-related healthcare assistance at the principal nationwide referral hospital In Kenya

    Background: Lockdown measures to reduce the outbreak of COVID-19 negatively impacted the administration of cancer chemotherapy globally; however, data from Kenya are limited. Researchers strove to address this information gap and assess chemotherapy trends before and during the pandemic at Kenyatta National Hospital (KNH), the most wide-ranging referral infirmary in Kenya, along with associated factors to provide future guidance. Methods: Time series analyses and patient interviews were undertaken at the KNH Cancer Treatment Centre. Patient data were analyzed descriptively and inferentially. The average quarterly scores of chemotherapy-related patients from January 2019 to December 2020 were computed for the time series analysis. Results: A total of 241 participants were recruited. Of the participants, 164 (68%) were female, and the mean age was 55. Breast cancer was the most typical cancer type. Independent risk factors for missed chemotherapy sessions were a considerable increase in travel costs, rescheduled appointments alongside difficulties in securing an appointment, comorbidities, and marital status. There was a decline in chemotherapy utilization before COVID-19, with a sharp drop at the pandemic's peak. Conclusion: COVID-19 and associated measures did appreciably affect the treatment of cancer patients with chemotherapy in this developing country, with several factors contributing to this. Efforts should be geared toward continuity of services during future pandemics in developing countries to improve patient outcomes

    The impact of the COVID-19 pandemic on the education of medical, dental and non-medical healthcare professionals in Bangladesh : findings and connotation

    Lockdown measures in response to the COVID-19 pandemic had an appreciable impact on the education of all medical, dental, and non-medical healthcare professional (HCP) students. These included the closure of universities necessitating a rapid move to e-learning and new approaches to practical’s. However initially, there was a lack of knowledge and expertise regarding e-learning approaches and the affordability of internet bundles and equipment. We initially con-ducted two pilot studies to assess such current challenges, replaced by a two-stage approach including a full investigation involving 32 private and public universities during the early stages of the pandemic followed by a later study assessing the current environment brought about by the forced changes. Top challenges at the start of the pandemic included a lack of familiarity with e-learning approaches, cost of the internet, lack of IT equipment and the quality of the classes. Universities offered support to staff and students to a varying degree to address identified challenges. Since then, e-learning approaches have widened the possibilities for teaching and learning at convenient times. However, challenges remain. In conclusion, there were considerable challenges at the start of them pandemic. Several key issues have been addressed with hybrid learning here to stay. Remaining challenges include a lack of ICT equipment. However, new innovations will continue

    Insulin Resistance Is Cheerfully Hitched with Hypertension

    Cardiovascular diseases and type 2 diabetes mellitus (T2DM) have risen steadily worldwide, particularly in low-income and developing countries. In the last hundred years, deaths caused by cardiovascular diseases increased rapidly to 35–40%, becoming the most common cause of mortality worldwide. Cardiovascular disease is the leading cause of morbidity and mortality in type 2 diabetes mellitus (T2DM), which is aggravated by hypertension. Hypertension and diabetes are closely interlinked since they have similar risk factors such as endothelial dysfunction, vascular inflammation, arterial remodeling, atherosclerosis, dyslipidemia, and obesity. Patients with high blood pressure often show insulin resistance and have a higher risk of developing diabetes than normotensive individuals. It has been observed that over the last 30 years, the prevalence of insulin resistance (IR) has increased significantly. Accordingly, hypertension and insulin resistance are strongly related to an increased risk of impaired glucose tolerance, diabetes, cardiovascular diseases (CVD), and endocrine disorders. Common mechanisms, for instance, upregulation of the renin–angiotensin–aldosterone system, oxidative stress, inflammation, and activation of the immune system, possibly have a role in the association between diabetes and hypertension. Altogether these abnormalities significantly increase the risk of developing type 2 diabetes
