    Role of immunoturbidimetric plasma fibrin D-dimer test in patients with coronary artery disease as well as ischemic heart disease in emergency medicine

    The aim of the present was to assess the value of the ELISA D-dimer (hemostatic marker) assay in patients with coronary artery disease as well as ischemic heart disease presenting to the emergency department with chest pain syndrome. Methods: We measured levels of D-dimers (µg/ml by immunoturbidimetric assay) in 120 patients with angiographically proved CAD, consecutive outpatients with chest pain, arterial fibrillation, acute coronary syndromes and 240 age and sex matched healthy controls. Demographic characteristics were assessed by a standardized questionnaire, and a complete lipid profile was performed for all subjects. In addition to this inflammatory marker C- reactive protein was also measured. Result: The distribution of D-dimer levels skewed to the right, and plasma mean levels were higher in cases than in control (mean: 2.51±3.60 vs .41±.59 µg/ml; p<0.001). In contrast, correlation of D-dimer was found with C-reactive protein (p<0.001) and is higher in cases than controls. Conclusion: Plasma D-dimer levels are strongly and independently associated with the presence of CAD in patients with stable angina. These results support the concept of a contribution of intravascular fibrin to atherothrombogenesis

    Assessment of water quality in disease burdened industrializing town of mid hills of North India

    Background: Anthropogenic activities associated with rapid urbanization and industrialization have deteriorated the water quality across the world. Rampant industrialization and poor water, sanitation and hygiene in an industrializing town of northern India necessitated the assessment of drinking water in the region. Methods: The study of physico-chemical parameter of the water and water quality index of the most commonly used ten drinking water sources in 2017-2018 was undertaken. Grab water samples were collected during monsoon, the rainy season (June, July and August) and post monsoon (September, October and November) during the years 2017 and 2018 by following the standard procedures. Weighted arithmetic index method was used for the water quality index (WQI) analysis. Results: Pooled analysis inferred water pH, temperature, total dissolved solids and carbonated oxygen demand to be within, whereas, electrical conductivity (91.00-431.50 µS/cm), turbidity (1.00-4.30 mg/l), dissolved oxygen (6.53-7.23 mg/l) and biological oxygen demand (6.12-7.62 mg/l), exceeding the Bureau of Indian standards permissible limits. Calcium, nitrate, chlorides and zinc were within limits, magnesium concentrations (9.16-29.35 mg/l) were below whereas lead (0.06-0.62 mg/l), chromium (0.01-0.12 mg/l), cadmium (0.00-0.25 mg/l) and mercury (0.00-0.08 mg/l) were above the standards. WQI was above 50 in all the drinking water sources. Conclusions: Water was of poor quality and unsuitable for drinking purposes, indicating alarming water pollution. WQI of the various drinking water sources of the region deteriorated more in the monsoon season as compared to the post monsoon season

    Postthrombotic syndrome (PTS) is a late outcome of deep vein thrombosis characterized by cramping pain, swelling, hyperpigmentation, eczema, lipodermatosclerosis, and ulceration in the leg due to increased venous outflow resistance and reflux venous flow. Newer surgical and endovascular interventions have a promising result in the management of postthrombotic syndrome. Early surgical or endovascular interventions in appropriately selected patients may decrease the incidence of recurrent ulceration and skin changes and provide a better quality of life. Duplex and IVUS (intravenous ultrasound) along with venography serve as cornerstone investigative tools for assessment of reflux and obstruction. Venous obstruction, if present, should be addressed earlier than reflux. It requires endovenous stenting, endophlebectomy, or open bypass procedures. Venous stripping, foam sclerotherapy, radiofrequency, or laser ablation are used to abolish superficial venous reflux. Valvuloplasty procedures are useful for incompetent but intact deep venous valves, while transposition or axillary vein autotransplantation is done for completely destroyed valves

    An Automatic Rice Plant Disease Detection Model Built With Unstructured Data Using IMDT Tiling and CNN Cognitive Object Detection

    Nowadays agriculture and processes are getting more intelligent mechanisms to improve the yield and reduce manual work. Smart agriculture provides numerous modern ideologies to farmers. But still, farmers face one important issue crop disease. Many researchers provide plenty of ways to recover and tackle the situation to come out of this problem. Therefore, they proceed with image processing to identify diseases from rice plant images. Farmers mainly face problems to take proper images for classification. Because of various reasons like various environmental factors, farmers ignorance, field size, capturing angle, device limitations, etc. are affecting the quality of the disease detection system, and these factors degrade overall performance. For this problem, introducing the Intelligent multi-dimensional tiling (IMDT) technique with an advanced convolution neural network with cognitive object detection (CNN-COD). IMDT technique developed with an intelligent expert system that adjusts input image size, capturing angles and other factors automatically. This advanced tiling technique supports to do the cropping and fluttering of input images for resizing. And CNN-COD model was used to calculate rice leaf width size and rescaled at the time of image segmentation with the Residual network (ResNet) model. Created dynamic tiled images are uniformly and scaled dimensional objects. These input values are used to train the CNN-COD rice plant disease, prediction model. Our proposed models were appraised on more than 4960 images which contain 8 various types of rice crop diseases. The experimental result portrayed out the CNN-COD model receives significant improvement in objection detection and image classification for the rice plant disease detection system. Mean average precision (MAP) values compared the CNN-COD model with the YOLOv4 model it got improved by 3.7% with the tiled input dataset

    Association of Entrepreneurship Ecosystem with Economic Growth in Selected Countries: An Empirical Exploration

    This study was created the entrepreneurship ecosystem index (EEI) as an effective tool for measurement of entrepreneurship ecosystem for 34 selected economies during 2000-2017. It was considered EEI as an integrated index of 12 different indicators (i.e. financing for entrepreneurs, governmental support and policies, taxes and bureaucracy, governmental programs, basic school entrepreneurial education and training, post school entrepreneurial education and training, R&amp;D transfer, commercial and professional infrastructure, internal market dynamics, internal market openness, physical and services infrastructure, and cultural and social norms) of entrepreneurship activities. Composite Z-score technique was used to create EEI for undertaken economies. Thereupon, it assesses the association of estimated EEI with socio-economic, science &amp; technology (S&amp;T) and IPRs related factors using correlation coefficient techniques. Estimated values of EEI show that India has 8th position in entrepreneurship ecosystem among the 34 economies. Also, there was found a high diversity in entrepreneurship ecosystem in efficiency, factor and innovation driven economies due to variation in 12 indicators of entrepreneurship ecosystem and socio-economic activities. Accordingly, it investigates the causal relationship between EEI and per capita GDP using linear, non-linear and log-linear regression models at country-wise panel data. Empirical results imply that per capita GDP is significantly associated with entrepreneurship ecosystem, socio-economic, S&amp;T and IPRs related indicators. Entrepreneurship ecosystem have a causal relationship with per capita GDP. Entrepreneurship ecosystem is significantly associated with socio-economic development and S&amp;T and IPRs related indicators. It brings several practical policy proposals to create effective entrepreneurship ecosystem, and to sustain economic and social development in India.     Recruiting for Ideas: How Firms Exploit the Prior Inventions of New Hires

    When firms recruit inventors, they acquire not only the use of their skills but also enhanced access to their stock of ideas. But do hiring firms actually increase their use of the new recruits’ prior inventions? Our estimates suggest they do, quite significantly in fact, by approximately 202% on average. However, this does not necessarily reflect widespread “learning-by-hiring.” In fact, we estimate that a recruit’s exploitation of her own prior ideas accounts for almost half of the above effect. Furthermore, although one might expect the recruit’s role to diminish rapidly as her tacit knowledge diffuses across her new firm, our estimates indicate that her importance is surprisingly persistent over time. We base these findings on an empirical strategy that exploits the variation over time in hiring firms’ citations to the recruits’ pre-move patents. Specifically, we employ a difference-in-differences approach to compare pre-move versus post-move citation rates for the recruits’ prior patents and the corresponding matched-pair control patents. Our methodology has three benefits compared to previous studies that also examine the link between labor mobility and knowledge flow: 1) it does not suffer from the upward bias inherent in the conventional cross-sectional comparison, 2) it generates results that are robust to a more stringently matched control sample, and 3) it enables a temporal examination of knowledge flow patterns.

    Study of prevalence of thyroid dysfunction in type 2 diabetes mellitus

    Background: Type 2 diabetes mellitus (T2DM) is associated with increased incidence of thyroid dysfunction (TD). The coexistence of thyroid dysfunction in T2DM patient is an important barrier in achieving treatment goal. The study regarding prevalence of TD in T2DM has not been done in patients of Eastern Uttar Pradesh. The present study was aimed to know the prevalence of thyroid dysfunction in T2DM in patients of Eastern Uttar Pradesh.Methods: This is an observational cross sectional prevalence study of thyroid dysfunction in 250 diagnosed T2DM patients deleat diagnosed based on ADA criteria attending the department of medicine Baba Raghav Das Medical College, Gorakhpur, Uttar Pradesh, Study was India, done over a period of 1 year. Thyroid dysfunction was classified on the basis of ATA criteria. All subjects underwent a detailed history, clinical examination and relevant biochemical tests.Results: The prevalence of thyroid dysfunction in T2DM patients was found to be 20.4% in this study.Conclusions: The prevalence of thyroid dysfunction was found to be higher in type 2 diabetes mellitus patients in comparison to general population as mentioned in different literature
