Simulated changes in extreme rainfall over southern Africa
A general circulation model simulation is used to investigate possible changes in rainfall over southern Africa resulting from a doubling of atmospheric carbon dioxide. Simulated increases in rainfall intensity are found to be a spatially coherent and an apparently less regionally dependent signal of climatic change than changes in annual means or number of rainâdays. Accordingly, increases in both the frequency and intensity of extreme daily rainfall events are simulated throughout most of the subcontinent. Simulated increases in the intensity of the lowest frequency floods are shown to be particularly severe, suggesting that greenhouseârelated climatic change may be most detectable through an increase in extreme flood events rather than changes in longâterm means. Similar results are evident when changes in the frequency and intensity of prolonged rainfall events, measured over a period of five consecutive days, are analysed. All results are qualitatively similar to those for the Australian region, except that the model's sensitivity to sharp changes in topography over southern Africa is highlighted

Droughts over southern Africa in a doubled-CO2 climate
The southern African region is susceptible to climatic extremes and particularly to extended dry periods. Possible changes in the probability of dry years under doubledâCO2 conditions are examined using output from the CSIRO nineâlevel general circulation model. Changes in annual mean rainfall are not expected to be significant. However, the model simulates an increase in the probability of dry years in the tropics, to the southâwest of the subcontinent, as well as over the western and eastern parts of South Africa and southern Mozambique, where large percentage increases in the most intense dry spells are indicated. A decrease in the frequency of dry years is simulated over much of the interior of the subcontinent south of 10°S. In regions where the frequency of dry years decreases, the most severe events occur less often. The CSIRO nineâlevel model indicates a shift in the frequency distribution of daily rainfall events under doubledâCO2 conditions. A small change in the frequency distribution of daily rainfall events may have further implications for the frequency of midâsummer droughts during the peak summer rainfall period of DecemberâFebruary. La sĂ©curitĂ© du patient est primordiale et l'utilisation de check-lists dans le domaine mĂ©dical se dĂ©veloppe. L'objectif de ce travail est de proposer une liste de critĂšres Ă vĂ©rifier lors de la prise en charge en cabinet de montagne des patients traumatisĂ©s. Le but n'est pas de dicter une conduite Ă tenir aux professionnels mais de proposer un outil d'aide Ă la pratique permettant de constater un oubli et de le corriger avant qu'il ne devienne problĂ©matique. Nous avons d'abord rĂ©alisĂ© une enquĂȘte de pratique permettant d'Ă©tablir une prĂ©-liste de critĂšres. Une validation de chaque critĂšre Ă©tait ensuite nĂ©cessaire pour l'inclusion finale : soit par transposition de recommandations de domaines proches de la mĂ©decine de montagne, soit par avis d'experts. On obtient ainsi une liste de vingt-trois points essentiels de la prise en charge d'un patient traumatisĂ© sans considĂ©rer une zone douloureuse en particulier. Il revient Ă chaque mĂ©decin de dĂ©cliner cette check-list gĂ©nĂ©rale en check-list spĂ©cifique d'un type de traumatisme, selon les donnĂ©es actualisĂ©es de la science. Pour ne pas rendre chronophage la vĂ©rification, nous proposons d'intĂ©grer la plupart des critĂšres dans le logiciel mĂ©dical. Cette check-list permet une amĂ©lioration des pratiques en Ă©vitant les oublis les plus frĂ©quents. Elle vise Ă amĂ©liorer la sĂ©curitĂ© des patients, avec des soins de qualitĂ© fondĂ©s sur des preuves. La principale limite Ă son utilisation est sa rĂ©actualisation nĂ©cessaire. C'est un premier travail sur le sujet qui introduit l'intĂ©rĂȘt de l'utilisation de check-lists dans ce domaine particulier qu'est la mĂ©decine de montagne. Une validation secondaire est nĂ©cessaire.Patients' safety is paramount and to contribute to this the use of check-lists is currently developing in the medical field. The purpose of this work is to propose a list of criteria which need to be checked in mountain practices during the medical care of injured patients. The objective is not to dictate a procedure to professionals but to offer an aid to practice enabling to spot an oversight and correct it before it becomes an issue. We first realized a practice survey which enabled us to establish a pre-list of criteria. A validation of each of them has then been necessary before their final integration to the list: validation came either from transposition of recommendations acknowledged in medical fields close to mountain medicine or from expert opinion. Thanks to this process we obtain a list of twenty-three essential points for the medical care of injured patients without considering a specific painful area. lt is up to every physician to adapt this general check-list and create a specific one tailored to a given trauma according to state-of-the-art scientific knowledge. To avoid making the checking task time-consuming we propose to integrate most of the criteria into the medical software. This check-list enables to improve practices by preventing the most frequent oversights from happening. The check-list aims at improving patients' safety thanks to proof-based quality care. The main limit to its use is the need for regular updates. This work is a first step on this topic. lt shows the interest of using check-lists in the specific field of mountain medicine. Strengthening public health in South Africa: building a stronger evidence base for improving the health of the nation
An assessment of the relative burden attributable to selected risk factors provides an important evidence base for prioritising risk factors that should be targeted for public health interventions. Selecting interventions should be based on a robust and transparent process of scientific evaluations of their effectiveness, as well as assessment of their cost effectiveness, local applicability and appropriateness, and likely effects on health inequalities. Establishing such an evidence base is an ongoing process that is still at an early stage in South Africa. A recent review of disease control priorities for developing countries (DCPP) examined the global evidence regarding the effectiveness of interventions for major health burdens. Despite acknowledging the lack of intervention trials in developing countries, this DCPP review provides a unique resource for identifying interventions that might be useful in South Africa
Structural analysis, molecular modelling and preliminary competition bnding studies of AM-DAN as a NMDA Receptor PCP-Site Fluorescent Ligand
Excitotoxicity related to the dysfunction of the N-methyl-d-aspartate receptor (NMDAR)
has been indicated to play an integral role in the pathophysiology of multiple disease states, including
neurodegenerative disorders such as Parkinsonâs disease. There is a notable gap in the market
for novel NMDAR antagonists, however current methods to analyse potential antagonists rely
on indirect measurements of calcium flux and hazardous radioligand binding assays. Recently, a
fluorescent NMDAR ligand, N-adamantan-1-yl-dimethylamino-1-naphthalenesulfonic acid, known
as AM-DAN was developed by our group. Additional studies on this ligand is necessary to evaluate
its potential as a biological tool in NMDAR research. Therefore, this study was aimed at conducting
structural analyses, fluorescence experiments, high-accuracy NMDAR molecular modelling and
NMDAR phencyclidine (PCP) site competition binding studies using AM-DAN. Results revealed that
AM-DAN has appropriate structural properties, significant fluorescent ability in various solvents and
is able to bind selectively and compete for the PCP-binding site of the NMDAR. Therefore, AM-DAN
holds promise as a novel fluorescent ligand to measure the affinity of prospective drugs binding at
the NMDAR PCP-site and may circumvent the use of radioligands
The Hong Kong Statement on Practice Research 2017 : Contexts and Challenges of the Far East
This statement on social work practice research highlights the contributions of scholars, practitioners, and conference participants in the Fourth International Conference on Practice Research (ICPR) in 2017, hosted by The Hong Kong Polytechnic University in May 2017. It focuses on the contexts and challenges of carrying out practice research in the Far East and beyond as well as raises pertinent questions about the development of practice research. It begins with a brief description of the context of social work practice research in the Far East. The second part explores the organizational and community contexts and challenges of practice research with special attention to the perspectives of practitioners. It concludes with reviewing some of the continuing challenges that will guide the program planning for the Fifth ICPR in 2020 in Melbourne, Australia, located at the crossroads between East and West.Peer reviewe
