    Effectiveness of maintenance immunosuppression therapies in a matched-pair analysis cohort of 16 years of renal transplant in the Brazilian national health system

    Maintenance of patients with renal transplant typically involves two or more drugs to prevent rejection and prolong graft survival. The calcineurin inhibitors (CNI) are the most commonly recommended medicines in combinations with others. While immunosuppressive treatment regimens are well established, there is insufficient long-term effectiveness data to help guide future management decisions. The study analyzes the effectiveness of treatment regimens containing CNI after renal transplantation during 16 years follow-up with real-world data from the Brazilian National Health System (SUS). This was a retrospective study with SUS patients after renal transplantion. Patients were propensity score-matched (1:1) by sex, age, type and year of transplantation. Kaplan-Meier was used to estimate the cumulative probabilities of survival. A Cox proportional hazard model was used to evaluate factors associated with progression to graft loss. 2,318 patients were included. Multivariable analysis, adjusted for diabetes mellitus and race / color, showed a greater risk of graft loss for patients using tacrolimus plus mycophenolate compared to patients treated with cyclosporine plus azathioprine. In conclusion, this Brazilian real-world study, with a long follow-up period using the matched analysis for relevant clinical features, and the representativeness of the sample, demonstrated improved long-term effectiveness for therapeutic regimens containing cyclosporine plus azathioprine. Consequently, we recommend that protocols and clinical guidelines for renal transplantation should consider cyclosporine plus azathioprine regimen as a potential first line option along with others

    Ten-year kidney transplant survival of cyclosporine- or tacrolimus-treated patients in Brazil

    Background: Cyclosporine and tacrolimus are well established immunosuppressants; however little know about long term survival rates. Aim: Compare 10-year graft survival and associated factors among kidney transplant patients within the Brazilian Public Health system (SUS) prescribed either medicine. Methods: Analyze a national cohort of kidney transplant recipients within SUS. Graft loss defined by death or dialysis for more than three months. Kaplan-Meier method used to estimate cumulative probabilities of survival. Cox proportional hazards model used to evaluate factors associated with progression to graft loss. Results: 13,811 patients were included, 5,887 used cyclosporine and 7,924 tacrolimus. A higher risk of graft loss was associated with tacrolimus, a deceased donor, additional years of age, median period of dialysis greater than 47 months, diagnosis of diabetes as the primary cause of chronic kidney disease and transplantation between 2005 and 2009. Conclusions: Among other factors, tacrolimus-based regimens were associated with worse graft survival

    Desde a Antiguidade, papagaios, periquitos e afins (Psittacidae) fascinaram os europeus por seu vivo colorido e uma notĂĄvel capacidade de interação com seres humanos. A descoberta do Novo Mundo nada faria alĂ©m de acrescentar novos elementos ao trĂĄfico de animais exĂłticos hĂĄ muito estabelecido pelos europeus com a África e o Oriente. Sem possuir grandes mamĂ­feros, a AmĂ©rica tropical participaria desse comĂ©rcio com o que tinha de mais atrativo, essencialmente felinos, primatas e aves - em particular os papagaios, os quais eram embarcados em bom nĂșmero. Contudo, a julgar pelos documentos do Brasil colĂŽnia, esses volĂĄteis podiam inspirar muito pouca simpatia, pois nenhum outro animal - exceto as formigas - foi tantas vezes mencionado como praga para a agricultura. AlĂ©m disso, alguns psitĂĄcidas mostravam-se tĂŁo loquazes que inspiravam a sĂ©ria desconfiança de serem animais demonĂ­acos ou possessos, pois sĂł trĂȘs classes de entidades - anjos, homens e demĂŽnios - possuĂ­am o dom da palavra. Nos dias de hoje, vĂĄrios representantes dos Psittacidae ainda constituem uma ameaça para a agricultura, enquanto os indivĂ­duos muito faladores continuam despertando a suspeita de estarem possuĂ­dos pelo demĂŽnio. Transcendendo a mera curiosidade, essa crença exemplifica o quĂŁo intrincadas podem ser as relaçÔes do homem com o chamado “mundo natural”, revelando um universo mais amplo e multifacetado do que se poderia supor a princĂ­pio. Nesse sentido, a existĂȘncia de aves capazes de falar torna essa relação ainda mais complexa e evidencia que as dificuldades de estabelecer o limite entre o animal e o humano se estendem alĂ©m dos primatas e envolvem as mais inusitadas espĂ©cies zoolĂłgicas.Since ancient times, parrots and their allies (Psittacidae) have fascinated Europeans by their striking colors and notable ability to interact with human beings. The discovery of the New World added new species to the international exotic animal trade, which for many centuries had brought beasts to Europe from Africa and the Orient. Lacking large mammals, tropical America participated in this trade with its most appealing species, essentially felines, primates and birds - especially parrots - which were shipped in large numbers. It should be noted, however, that at times these birds were not well liked. In fact, according to documents from colonial Brazil, only the ants rank higher than parrots as the animals most often mentioned as agricultural pests. On the other hand, some of these birds were so chatty that people suspected them to be demonic or possessed animals, since only three classes of beings - angels, men and demons - have the ability to speak. Nowadays, several Psittacidae still constitute a threat to agriculture, and the suspicion that extremely talkative birds were demon possessed has also survived. More than a joke or a mere curiosity, this belief exemplifies how intricate man’s relationships with the “natural world” may be. In this sense, the existence of birds that are able to speak adds a further twist to these relationships, demonstrating that the problem of establishing a boundary between the animal and the human does not only involve primates, but also includes some unusual zoological species

    The evolving SARS-CoV-2 epidemic in Africa: Insights from rapidly expanding genomic surveillance

    INTRODUCTION Investment in Africa over the past year with regard to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) sequencing has led to a massive increase in the number of sequences, which, to date, exceeds 100,000 sequences generated to track the pandemic on the continent. These sequences have profoundly affected how public health officials in Africa have navigated the COVID-19 pandemic. RATIONALE We demonstrate how the first 100,000 SARS-CoV-2 sequences from Africa have helped monitor the epidemic on the continent, how genomic surveillance expanded over the course of the pandemic, and how we adapted our sequencing methods to deal with an evolving virus. Finally, we also examine how viral lineages have spread across the continent in a phylogeographic framework to gain insights into the underlying temporal and spatial transmission dynamics for several variants of concern (VOCs). RESULTS Our results indicate that the number of countries in Africa that can sequence the virus within their own borders is growing and that this is coupled with a shorter turnaround time from the time of sampling to sequence submission. Ongoing evolution necessitated the continual updating of primer sets, and, as a result, eight primer sets were designed in tandem with viral evolution and used to ensure effective sequencing of the virus. The pandemic unfolded through multiple waves of infection that were each driven by distinct genetic lineages, with B.1-like ancestral strains associated with the first pandemic wave of infections in 2020. Successive waves on the continent were fueled by different VOCs, with Alpha and Beta cocirculating in distinct spatial patterns during the second wave and Delta and Omicron affecting the whole continent during the third and fourth waves, respectively. Phylogeographic reconstruction points toward distinct differences in viral importation and exportation patterns associated with the Alpha, Beta, Delta, and Omicron variants and subvariants, when considering both Africa versus the rest of the world and viral dissemination within the continent. Our epidemiological and phylogenetic inferences therefore underscore the heterogeneous nature of the pandemic on the continent and highlight key insights and challenges, for instance, recognizing the limitations of low testing proportions. We also highlight the early warning capacity that genomic surveillance in Africa has had for the rest of the world with the detection of new lineages and variants, the most recent being the characterization of various Omicron subvariants. CONCLUSION Sustained investment for diagnostics and genomic surveillance in Africa is needed as the virus continues to evolve. This is important not only to help combat SARS-CoV-2 on the continent but also because it can be used as a platform to help address the many emerging and reemerging infectious disease threats in Africa. In particular, capacity building for local sequencing within countries or within the continent should be prioritized because this is generally associated with shorter turnaround times, providing the most benefit to local public health authorities tasked with pandemic response and mitigation and allowing for the fastest reaction to localized outbreaks. These investments are crucial for pandemic preparedness and response and will serve the health of the continent well into the 21st century

    A ColĂŽnia do Sacramento, no atual Uruguai, na primeira metade do sĂ©culo XVIII, constituiu uma cidade de prĂłdigo comĂ©rcio na regiĂŁo platina. Inseridos tanto nas rotas comerciais e sociais portuguesas quanto nas castelhanas, os habitantes de Sacramento materializavam uma fronteira mĂșltipla, onde coexistiam espanhĂłis, portugueses e diferentes grupos indĂ­genas. O presente estudo analisa os vĂ­nculos sociais e comerciais existentes entre os habitantes de Sacramento e os de Buenos Aires. No interior do espaço platino as redes sociais estabelecidas atravĂ©s do rio da Prata, ligando Sacramento e Buenos Aires, eram vias de acĂșmulo de prestĂ­gio, poder e riqueza em uma sociedade de antigo regime.<br>The Sacramento Colony, currently Uruguayan territory, in the first half of the XVIII century, was a city with great commerce on the River Plate Region. Placed in both Portuguese and Spanish social and commercial routes, the Sacramento habitants formed a multiple frontier where Spanish, Portuguese and different indigenous groups coexisted. The present study analyses the social and commercial links that existed between the Sacramento and Buenos Aires inhabitants. In the River Plate region, the social networks developed connecting Sacramento and Buenos Ayers were gateways to social status, power and wealth in an old regime society

    France-Brésil : vingt ans de coopération

    Peu d'ouvrages sont consacrés à la coopération scientifique, technique et technologique. Pourtant son rÎle est considérable : elle contribue à la fois au progrÚs de la connaissance et au développement des nations. La coopération franco-brésilienne constitue un cas exemplaire et significatif. Plusieurs milliers de chercheurs, ingénieurs et techniciens, souvent parmi les plus renommés, en ont été les acteurs. Ils ont connu ainsi une aventure exceptionnelle qui a marqué profondément leur vie. Afin d'illustrer cette coopération entre la France et le Brésil, les deux coordonnateurs de cet ouvrage (un diplomate et un universitaire) ont sollicité le témoignage de quinze chercheurs et ont rassemblé les contributions d'une trentaine d'institutions et organismes des deux pays. De l'archéologie préhistorique à la physique nucléaire, de l'ethnologie et l'histoire aux biotechnologies et à la géologie, de l'astronomie et des activités aéronautiques et spatiales à l'informatique, à la chimie des nouveaux matériaux et aux services postaux, de la géographie, de l'économie et des sciences politiques aux sciences de la vie, à la recherche et à l'exploitation de la mer et des ressources hydrauliques ou forestiÚres..., les multiples facettes d'une recherche en coopération se trouvent ainsi décrites et livrées au public. Cet ouvrage aspire à faire davantage comprendre les enjeux que représente la coopération entre deux pays. Il est publié simultanément en France et au Brésil
