    Secular trends in breastfeeding in Brazil

    The objective of this study was to document the secular trend in breastfeeding in Brazil. Data bases from seven national surveys conducted from 1975 to 2008 were reanalyzed. To obtain compatible data from the different surveys, children in the same age group and the same indicators were analyzed, using the same statistical techniques. The median duration of breastfeeding increased from 2.5 to 11.3 months, and the prevalence of exclusive breastfeeding in infants under six months of age increased from 3.1% to 41.0% in the period. The results indicate important challenges in accelerating the rhythm at which this practice in Brazil moves towards meeting international recommendations.Com o objetivo de documentar a tendência secular da amamentação no Brasil, foram reanalisadas as bases de dados de sete pesquisas nacionais realizadas de 1975 a 2008. Para obter dados comparáveis entre os diferentes inquéritos, foram analisadas as mesmas faixas etárias e indicadores, e utilizadas as mesmas técnicas estatísticas. A duração mediana da amamentação aumentou de 2,5 para 11,3 meses e a prevalência da amamentação exclusiva em menores de seis meses passou de 3,1% para 41,0% no período. Os resultados apontam importantes desafios no sentido de acelerar o ritmo de crescimento dessa prática no País, rumo às recomendações internacionais.Con el objetivo de documentar la tendencia secular de la lactancia en Brasil, se hicieron re-análisis de las bases de datos de siete investigaciones nacionales efectuadas de 1975 a 2008. Para obtener datos comparables entre las diferentes pesquisas, se analizaron los mismos grupos etarios e indicadores y se utilizaron las mismas técnicas estadísticas. La duración mediana de la lactancia aumentó de 2,5 a 11,3 meses y la prevalencia de la lactancia exclusiva en menores de seis meses pasó de 3,1% a 41% en el período. Los resultados señalan importantes desafíos en el sentido de acelerar el ritmo de crecimiento de esta práctica en el País, de acuerdo a las recomendaciones internacionales

    The consumption of unhealthy foods by Brazilian children is influenced by their mother’s educational level

    Objective: To evaluate the association between the consumption of unhealthy foods in children under one year and the education level of the mothers, data obtained from participants of the II Survey on the prevalence of breastfeeding in the Brazilian capitals and the Federal District in 2008 was analyzed. Methods: During the second stage of the campaign for multi-vaccination, a questionnaire on food consumption in the last 24 hours was given to mothers or guardians of children under one year old. We analyzed the consumption of unhealthy foods according to age group, maternal education, region of residence and breastfeeding status. The state capitals and the Federal District were grouped according to the five macro-regions of the country (North, Northeast, Southeast, South and West). Processed juice, soda, coffee, cookies/salted snacks and sugar and/or honey were defined as unhealthy foods. Prevalence ratios (RP) for the association between the consumption of unhealthy foods and maternal education were estimated using Poisson regression models. Results: The study included 34,366 children. The consumption of sweet foods started early and was predominant until the age of six months; after this age, the consumption of biscuits and/or snacks became more prevalent. The consumption of these foods also differs in relation to the macro-region of residence. Consumption of unhealthy foods was higher among mothers with lower education levels. Conclusions: The consumption of unhealthy foods by Brazilian children under one year old was high, indicating a need for developing effective strategies to combat the consumption of unhealthy foods in Brazilian children as a way of preventing obesity and other future disorders

    Impaired lung function in individuals chronically exposed to biomass combustion

    Background: The use of biomass for cooking and heating is considered an important factor associated with respiratory diseases. However, few studies evaluate the amount of particulate matter less than 2.5 mu in diameter (PM2.5), symptoms and lung function in the same population. Objectives: To evaluate the respiratory effects of biomass combustion and compare the results with those of individuals from the same community in Brazil using liquefied petroleum gas (Gas). Methods: 1402 individuals in 260 residences were divided into three groups according to exposure (Gas, Indoor-Biomass, Outside-Biomass). Respiratory symptoms were assessed using questionnaires. Reflectance of paper filters was used to assess particulate matter exposure. In 48 residences the amount of PM2.5 was also quantified. Pulmonary function tests were performed in 120 individuals. Results: Reflectance index correlated directly with PM2.5 (r=0.92) and was used to estimate exposure (ePM2.5). There was a significant increase in ePM2.5 in Indoor-Biomass and Outside-Biomass, compared to Gas. There was a significantly increased odds ratio (OR) for cough, wheezing and dyspnea in adults exposed to Indoor-Biomass (OR=2.93, 2.33, 2.59, respectively) and Outside-Biomass (OR=1.78, 1.78, 1.80, respectively) compared to Gas. Pulmonary function tests revealed both Non-Smoker-Biomass and Smoker-Gas individuals to have decreased %predicted-forced expiratory volume in the first second (FEV1) and FEV1/forced vital capacity (FVC) as compared to Non-Smoker-Gas. Pulmonary function tests data was inversely correlated with duration and ePM2.5. The prevalence of airway obstruction was 20% in both Non-Smoker-Biomass and Smoker-Gas subjects. Conclusion: Chronic exposure to biomass combustion is associated with increased prevalence of respiratory symptoms, reduced lung function and development of chronic obstructive pulmonary disease.
