    The role of interventional radiology in the treatment of lower limb vascular injuries after orthopaedic surgery

    Purpose: This paper focuses on the role of interventional radiology embolisations in a series of patients presenting with iatrogenic vascular injuries of the lower limbs following orthopaedic interventions. Material and methods: Fourteen patients (mean age: 64 years, range 23-90 years) were retrospectively analysed. Clinical presentation consisted of palpable pulsatile mass, pain, reduced lower limb motion, or visible haematoma; 11 patients had also anaemia (haemoglobin < 7 g/dl). Results: The time between orthopaedic surgery and embolisation ranged between 0 and 67 days (mean: 15 days). Injured arterial vessels were as follows: inferior gluteal artery (2), superficial external pudendal artery (2), deep femoral artery (1), lateral circumflex femoral artery (3), medial circumflex femoral artery (2), articular branch of descending genicular artery (1), perforating femoral arteries (3), posterior tibial recurrent artery (1), and anterior tibial artery (1). The typologies of vascular lesion were: pseudoaneurysm 57%, bleeding with extraluminal contrast agent blush of the terminal arterial segment 36%, and laceration and bleeding with extraluminal contrast agent blush of the arterial main trunk 7%. Embolising agents adopted were microcoils 57%, glue 14%, microplug 7%, particles 14%, and covered stent 7%. In all cases clinical and procedural technical successes were obtained (100%). Conclusions: For the management of vascular injuries occurring after different orthopaedic interventions of the lower limbs, endovascular embolisations have proven to be safe and effective; orthopaedic surgeons should be aware of the support that interventional radiology could provide in the case of iatrogenic vascular complications

A partir de revisão de literatura e de análise de relatórios econômicos, contextualiza-se o peso do setor agrícola no PIB de cada país e analisa-se o desempenho de ambos na atual dinâmica da globalização, aprofundando a discussão com as questões relativas ao embate centro-periferia. Por fim, verifica-se que Brasil e Paraguai são dependentes do cenário mundial, regido pelos países que controlam o capitalismo internacional, portanto na condição de periféricos e sem perspectivas de mudança

    Decoding Lua: formal semantics for the developer and the semanticist

    We provide formal semantics for a large subset of the Lua programming language, in its version 5.2. We validate our model by mechanizing it and testing it against the test suite of the reference interpreter of Lua, con rming that our model accurately represents the language. In addition, we set us an ambitious goal: to target both a PL semanticist —not necessarily versed in Lua—, and a Lua developer —not necessarily versed in semantic frameworks. To the former, we present the peculiarities of the language, and how we model them in a traditional small-step operational semantics, embedded within Felleisen-Hieb’s reduction semantics with evaluation contexts. e mechanization is, naturally, performed in PLT Redex, the de facto tool for mechanizing reduction semantics. To the reader unfamiliar with such concepts, we provide, to our best possible within the space limitations, a gentle introduction of the model. It is our hope that developers of the di erent Lua implementations and dialects understand the model and consider it both for testing their work and for experimenting with new language features.Fil: Soldevila Raffa, Mallku Ernesto.     Therapeutic Ultrasound as a Potential Male Dog Contraceptive: Determination of the Most Effective Application Protocol

    Contents: Ultrasound is one of the most promising forms of non-invasive contraception and has been studied in several animal models. The objective of the current investigation was to determine the most practical and effective application protocol for dog sterilization. A total of 100 dogs were divided into five equal groups. Group A received 5-min applications three times performed at 48-hr intervals and covering the entire testicular area at frequency of 1 MHz; Group B received 5-min applications three times performed at 48-hr intervals over the dorso-cranial area of the testis at frequency of 3 MHz; Group C received three sequential 5-min applications (at 5-min intervals between applications) covering the entire testicular area at frequency of 1 MHz; Group D received 15-min applications two times performed at 48-hr intervals and covering the entire testicular area at frequency of 1 MHz. The experimental groups' ultrasound had an intensity of 1.5W/cm2. The Control Group had the same procedure as Group A, but with the transducer switched-off. Dogs were surgically castrated 40 days following the treatment for histological examination. Azoospermia, testicular volume reduction and apparently irreversible testicular damage were achieved by Group A. No effects were noticed in the other groups. Testosterone levels remained within physiological range with all application protocols. A regimen of three applications of ultrasound at 1 MHz, and 1.5 W/cm2, lasting 5 min with an interval of 48 h was effective as permanent sterilization in the dog without hormonal impact

    A singular case of massive urethrorrhagia solved by transarterial embolization

    Abstract Background Urethrorrhagia is frequent in pelvic trauma, rarely due to traumatic injuries of internal pudendal artery branches. Case presentation Our aim is to underline the role of transarterial embolization in selected patients, as in this case in which a young man manifested urethral hemorrhage after high-energy motorcycle crash not associated with injuries of the inferior urinary tract. Multi-detector computed tomography (MDCT) showed pubic symphysis diastasis and perineal hematoma with pseudoaneurysm into the penis bulb. The first approach was conservative with perineal external compression and intravenous injection of tranexamic acid. Afterward, due to the decline of clinical conditions, we decided to perform a selective angiography, confirming the vascular injury of distal branches of both internal pudendal arteries with contrast agent extravasation into urethral bulb; endovascular embolization was performed with detachable micro-coils. The principal results were seen quickly, indeed urethrorrhagia arrested and hemoglobin values normalized. Erectile function was preserved at 6-months follow-up. Conclusions Endovascular embolization proved to be a minimally invasive therapeutic approach, clinically effective, with a low rate of complications and high probability to preserve erectile function

    Successful endovascular embolization of a giant splenic artery pseudoaneurysm secondary to a huge pancreatic pseudocyst with concomitant spleen invasion

    Pseudoaneurysms of the pancreatic and peripancreatic arteries is a well-known complication of chronic or necrotizing pancreatitis due to proteolytic enzymatic digestion of the arterial wall. A major part of peripancreatic pseudoaneurysms involve the splenic artery, but any peripancreatic artery may be involved and bleed. They are potentially life threatening for patients, due to spontaneous intraperitoneal rupture, rupture and fistulization into the surrounding organs, or fistulization into the pancreatic duct. Small ones are usually asymptomatic and are often diagnosed incidentally, while giant (> 5 cm) aneurysms and pseudoaneurysms are symptomatic and may be detected as a pulsatile mass in the upper-left quadrant or epigastrium. Imaging plays a key role in the identification of splenic artery aneurysms and pseudoaneurysms, while angiography still represents the gold standard for the diagnosis, although nowadays it plays a prominent role in treatment. Treatment of splenic artery pseudoaneurysms is mandatory because of the high probability of rupture, with a mortality rate of up to 90%. The gold standard treatment is represented by surgery, with a mortality rate between 16% and 50%. In recent years the endovascular approach has proven to be an effective alternative treatment for splenic artery pseudoaneurysms, and it is currently the method of choice.     Controle Sanitário de Filmes Flexíveis de PVC Comercializados no Estado do Rio de Janeiro

    n total, 37 samples of flexible PVC films were analyzed for specific migration of di-(2-ethylhexyl) phthalate (DEHP) and di-(2-ethylhexyl) adipate (DEHA) from packaging to fatty foods, using simulant. The Resolution n. 17, of March 17, 2008, published by ANVISA, establishes specific migration limits for DEHP in 1.5 mg kg−1 of simulant and for DEHA in 18 mg kg−1 of simulant. The migration test was performed through contact between a 1-dm2 PVC film cutout and 100 mL of food simulant, ethanol 95% (v/v), for 48 h at 20ºC. The migrations of DEHP and DEHA were determined by gas chromatography with a flame ionization detector and a fused silica column internally coated with a stationary phase consisting of 5% phenylmethylsilicone. The results from the samples showed specific migration of DEHP ranging between not detectable (&lt; 0.35 mg kg−1) and 304 mg kg−1 of food simulant and between not detectable (&lt; 2.23 mg kg−1) and 231 mg kg−1 of food simulant for DEHA. Among the tested samples, 95% had results above the limit established by legislation.Foram analisadas trinta e sete amostras de filme flexível de PVC quanto à migração específica dos plastificantes ftalato de di-(2-etil-hexila) – DEHP (Nº CAS 117-81-7) e adipato de di-(2-etil-hexila) – DEHA (Nº CAS 103-23-1) da embalagem para alimentos gordurosos, utilizando-se simulante. A Resolução nº 17, de 17 de março de 2008, publicada pela ANVISA, estabelece limite de migração específica para DEHP de 1,5 mg kg-1 do simulante e para o DEHA em 18 mg kg-1 do simulante. O teste de migração foi realizado por meio do contato entre 1 dm2 do filme de PVC e 100 mL de simulante de alimento, solução de etanol a 95% (v/v), por 48 h a 20ºC. As migrações dos plastificantes DEHP e DEHA foram determinadas por cromatografia a gás com detecção por ionização em chama e coluna de sílica fundida recoberta internamente com fase estacionária constituída de 5% fenilmetilsilicone. As amostras apresentaram resultados para a migração específica de DEHP entre não detectável (&lt; 0,35 mg kg-1) e 304 mg kg-1 de simulante de alimentos e entre não detectável (&lt; 2,23 mg kg-1) e 231 mg kg-1 de simulante de alimentos para o DEHA. Dentre as amostras ensaiadas, 95% apresentaram resultado insatisfatório para pelo menos um dos plastificantes
