Strategies for Prevention of Varicella Pneumonia: A Cost-Effectiveness Analysis
ABSTRACT Objective: The objective of this study was to compare the cost-effectiveness of 3 strategies of serologic enzyme-linked immunosorbent assay (ELISA) testing and post-exposure varicella zoster immune globulin (VZIG) prophylaxis for the prevention of maternal varicella pneumonia during pregnancy in patients with negative or uncertain histories of varicella infection. Methods: A decision tree was constructed to compare the following strategies: 1) routine serologic testing for varicella immunity followed by targeted post-exposure VZIG prophylaxis, 2) post-exposure serologic testing followed by targeted VZIG prophylaxis, and 3) untargeted post-exposure VZIG administration. The probabilities for the model were obtained from the medical literature and supplemented by expert opinion. The costs were obtained by a review of inpatient hospitalizations for varicella pneumonia. All costs were converted to 1995 dollars. Results: Routine serologic testing followed by targeted post-exposure VZIG prophylaxis was the most costly strategy ($37.22/person), with no demonstrable increase in benefit compared with the other 2 strategies. The disutility of this strategy compared with the others was stable across a wide range of values for the probabilities and costs utilized in the sensitivity analysis. We were unable to differentiate between the cost-effectiveness of the other 2 strategies. Conclusions: Routine serologic testing for varicella immunity in patients with negative or uncertain histories of varicella infection should not be performed. The remaining options of screening and prophylaxis appear to be reasonable alternatives for dealing with varicella exposures. (C
Impact of changing US cigarette smoking patterns on incident cancer: Risks of 20 smoking-related cancers among the women and men of the NIH-AARP cohort
Background: Historically, US women started smoking at a later age than men and had lower relative risks for smoking-related cancers. However, more recent birth cohorts of women and men have similar smoking histories and have now reached the high-risk age for cancer. The impact of these changes on cancer incidence has not been systematically examined. Methods: Relative risks (RR), 95% confidence intervals (CI) and attributable fractions were calculated for cigarette smoking and incidence of 20 smoking-related cancers in 186 057 women and 266 074 men of the National Institutes of Health-AARP cohort, aged 50 to 71 years in 1995 and followed for 11 years. Results: In the cohort, which included participants born between 1924 and 1945, most women and men started smoking as teenagers. RRs for current vs never smoking were similar in women and men for the following cancers: lung squamous-cell (RR women: 121.4, 95% CI: 57.3–257.4; RR men:114.6, 95% CI: 61.2–214.4), lung adenocarcinoma (RR women: 11.7, 95% CI: 9.8–14.0; RR men: 15.6, 95% CI: 12.5–19.6), laryngeal (RR women: 37.0, 95% CI: 14.9–92.3; RR men: 13.8, 95% CI: 9.3–20.2), oral cavity-pharyngeal (RR women:4.4, 95% CI: 3.3–6.0; RR men: 3.8, 95% CI: 3.0–4.7), oesophageal squamous cell (RR women: 7.3, 95% CI: 3.5–15.5; RR men: 6.2, 95% CI: 2.8–13.7), bladder (RR women: 4.7, 95% CI: 3.7–5.8; RR men: 4.0, 95% CI: 3.5–4.5), colon (RR women: 1.3, 95% CI: 1.2–1.5; RR men: 1.3, 95% CI: 1.1–1.4), and at other sites, with similar attributable fractions. Conclusions: RRs for current smoking and incidence of many smoking-related cancers are now similar in US women and men, likely reflecting converging smoking patterns
Real-Time Detection and Rapid Multiwavelength Follow-up Observations of a Highly Subluminous Type II-P Supernova from the Palomar Transient Factory Survey
The Palomar Transient Factory (PTF) is an optical wide-field variability
survey carried out using a camera with a 7.8 square degree field of view
mounted on the 48-in Oschin Schmidt telescope at Palomar Observatory. One of
the key goals of this survey is to conduct high-cadence monitoring of the sky
in order to detect optical transient sources shortly after they occur. Here, we
describe the real-time capabilities of the PTF and our related rapid
multiwavelength follow-up programs, extending from the radio to the gamma-ray
bands. We present as a case study observations of the optical transient
PTF10vdl (SN 2010id), revealed to be a very young core-collapse (Type II-P)
supernova having a remarkably low luminosity. Our results demonstrate that the
PTF now provides for optical transients the real-time discovery and
rapid-response follow-up capabilities previously reserved only for high-energy

PTF11eon/SN2011dh: Discovery of a Type IIb Supernova From a Compact Progenitor in the Nearby Galaxy M51
On May 31, 2011 UT a supernova (SN) exploded in the nearby galaxy M51 (the
Whirlpool Galaxy). We discovered this event using small telescopes equipped
with CCD cameras, as well as by the Palomar Transient Factory (PTF) survey, and
rapidly confirmed it to be a Type II supernova. Our early light curve and
spectroscopy indicates that PTF11eon resulted from the explosion of a
relatively compact progenitor star as evidenced by the rapid shock-breakout
cooling seen in the light curve, the relatively low temperature in early-time
spectra and the prompt appearance of low-ionization spectral features. The
spectra of PTF11eon are dominated by H lines out to day 10 after explosion, but
initial signs of He appear to be present. Assuming that He lines continue to
develop in the near future, this SN is likely a member of the cIIb (compact
IIb; Chevalier and Soderberg 2010) class, with progenitor radius larger than
that of SN 2008ax and smaller than the eIIb (extended IIb) SN 1993J progenitor.
Our data imply that the object identified in pre-explosion Hubble Space
Telescope images at the SN location is possibly a companion to the progenitor
or a blended source, and not the progenitor star itself, as its radius (~10^13
cm) would be highly inconsistent with constraints from our post-explosion
photometric and spectroscopic data
Strong Ultraviolet Pulse From a Newborn Type Ia Supernova
Type Ia supernovae are destructive explosions of carbon oxygen white dwarfs.
Although they are used empirically to measure cosmological distances, the
nature of their progenitors remains mysterious, One of the leading progenitor
models, called the single degenerate channel, hypothesizes that a white dwarf
accretes matter from a companion star and the resulting increase in its central
pressure and temperature ignites thermonuclear explosion. Here we report
observations of strong but declining ultraviolet emission from a Type Ia
supernova within four days of its explosion. This emission is consistent with
theoretical expectations of collision between material ejected by the supernova
and a companion star, and therefore provides evidence that some Type Ia
supernovae arise from the single degenerate channel.Comment: Accepted for publication on the 21 May 2015 issue of Natur
Double Blind Study Investigating the Effect of Different Voice Prostheses on Ease of Swallowing and Residue Post Laryngectomy.
Voice prostheses have been examined for their effect on voice production but there is little datum on their effect on swallow function. This study investigated the difference between six commonly available voice prostheses in terms of swallowing. Laryngectomy patients had up to six voice prostheses placed in a random order over two visits. Swallowing was evaluated for each prosthesis using FEES (Fibreoptic Endoscopic Evaluation of Swallowing). After each prosthesis trial, patients self-evaluated their experience of swallowing. Three independent experts indicated which prosthesis they considered best for swallowing for each patient and judged residue on the voice prosthesis and in the upper esophagus. Raters were blinded to participant details, voice prosthesis type and scores of other raters. On patient self-evaluation, scores were equally distributed across all prostheses for swallowing. Experts most frequently chose the Blom Singer Low pressure and Blom Singer Classic Indwelling voice prostheses as best for swallowing but consensus was poor for most patients. Experts found that the Blom Singer Classic Indwelling and the Provox Vega had least residue on the voice prosthesis on thin liquid (p ≤ 0.001) and soft (p = 0.001), respectively. Experts also found that the Blom Singer Low Pressure had least residue in the upper esophagus on soft consistency (p ≤ 0.001). While self-evaluation by patients did not identify a consistently preferred prosthesis for swallow, many patients expressed personal preferences, suggesting benefits to involving patients in the choice of prosthesis. Some voice prostheses may be associated with lower levels of residue on the prosthesis and upper esophagus with certain consistencies

Managing talent across advanced and emerging economies: HR issues and challenges in a Sino-German strategic collaboration
The human resource (HR) practices involved in global talent management continue to advance and evolve. A majority of talent management commentary is from multinational corporation (MNC) perspectives. However, the less commented small-to-medium-sized enterprise (SME) also confronts challenges grounded in economic (i.e. resources, finance), organisational (i.e. size, scope and structure) and consequent behavioural rationales (i.e. mindsets and stances). This paper establishes and examines a number of propositions which consider how these factors impact on an advanced economy SME’s talent management in emerging economy collaborations. An interpretive qualitative methodology is employed using interviews conducted within two cases – SME and an MNC comparator case. The SME case is used as the driving force of the paper and its theoretical focus and findings. The MNC is used to develop issues as a comparator case. The findings show SME economic and organisational drivers producing behavioural dynamics in relation to mimesis of planned actions yet informal serendipitous responses in reality; a predilection for the proximate and familiar; design configurations of short-term expatriate visits and inpatriates; cumulating in ongoing inpatriate acculturisation and re-acculturation oscillation. Consequently, the implication is that the SME needs a HR practices encompassing resignation to the situation, flexibility and resilience in order to survive and progress
