Micro CT analysis of the subarticular bone structure in the area of the talar trochlea
Purpose: Certain regions of the talar trochlea are recognized as exhibiting varying cartilage thickness and degrees of subchondral bone mineralization. These changes have been attributed to the long-term loading history. For the current study, we accepted the hypothesis that stress-induced alterations of the joint surface include not only varying degrees of subchondral lamellar mineralization, but also structural changes of the subarticular cancellous bone. Methods: In order to examine the structure of the subarticular cancellous bone, ten formalin-fixed talar trochleae were analyzed using micro CT. Sixteen measurement zones were defined and then evaluated in five layers each of 1-mm thickness, enabling assessment of the cancellous architecture extending 5mm below the trochlear surface using numerical and structural parameters. Results: As with mineralization patterns in the subchondral lamella, large variation was observed regarding bone volume, trabecular quantity, thickness, and spacing, as well as for structure model index and degree of anisotropy, depending on localization. In addition, like previous reports examining mineralization of the subchondral lamella, two distinct groups could be identified as "bicentricâ or "monocentricâ. Zur subartikulÀren Knochenarchitektur der humanen Trochlea tali - eine mikrocomputertomographische Analyse
Hintergrund: Bekannt sind Areale auf der Trochlea tali mit unterschiedlicher Knorpelschichtdicke als auch mit verschiedenem Mineralisationsgrad des subchondralen Knochens, welche als Korrelat einer langwÀhrenden loading history interpretiert werden. Als Hypothese dieser Arbeit wurde angenommen, dass sich entsprechende Anpassungen an die Beanspruchung einer GelenksflÀche nicht nur im unterschiedlichen Mineralisierungsgrad der subchondralen Lamelle nachweisen lassen, sondern auch in der Feinarchitektur der subartikulÀren Spongiosa zeigen.
Methoden: Zur detaillierten Betrachtung der subartikulĂ€ren Spongiosastruktur wurden zehn formalinfixierten Trochleae tali mittels Mikro-CT analysiert. Hierzu wurden insgesamt 16 Messfelder definiert, welche in fĂŒnf Schichten von je 1 mm Dicke ausgewertet wurden, wodurch ein Spongiosabereich bis 5 mm unterhalb der TrochleaoberflĂ€che anhand numerischer und struktureller Parameter beurteilt werden konnte.
Resultate: Analog zu den unterschiedlichen Mineralisierungsmustern in der subchondralen Lamelle konnten grosse Unterschiede in Bezug auf das Knochenvolumen, der Anzahl der Trabekel, der trabekulĂ€ren Dicke sowie der AbstĂ€nde der einzelnen Trabekel in der subartikulĂ€ren Spongiosa in AbhĂ€ngigkeit von der Lokalisation nachgewiesen werden. Ebenso konnten in Anlehnung an frĂŒhere Untersuchungen zur Mineralisation der subchondralen Lamelle zwei Gruppen als "bizentrisch" bzw. "monozentrisch" beschrieben werden.
Konklusion: Die Ergebnisse zeigen, dass sich eine strukturelle gewebliche Anpassung im Sinne einer loading history auch innerhalb der subartikulÀren Spongiosa nachweisen lÀsst
Somatic mosaicism and common genetic variation contribute to the risk of very-early-onset inflammatory bowel disease
Very-early-onset inflammatory bowel disease (VEO-IBD) is a heterogeneous phenotype associated with a spectrum of rare Mendelian disorders. Here, we perform whole-exome-sequencing and genome-wide genotyping in 145 patients (median age-at-diagnosis of 3.5 years), in whom no Mendelian disorders were clinically suspected. In five patients we detect a primary immunodeficiency or enteropathy, with clinical consequences (XIAP, CYBA, SH2D1A, PCSK1). We also present a case study of a VEO-IBD patient with a mosaic de novo, pathogenic allele in CYBB. The mutation is present in ~70% of phagocytes and sufficient to result in defective bacterial handling but not life-threatening infections. Finally, we show that VEO-IBD patients have, on average, higher IBD polygenic risk scores than population controls (99 patients and 18,780 controls; Pâ<â4âĂâ10-10), and replicate this finding in an independent cohort of VEO-IBD cases and controls (117 patients and 2,603 controls; Pâ<â5âĂâ10-10).
