    Fitness for work after vocational rehabilitation organized by the State Social Security Institute of Iceland

Material and methods: New disability claimants who had been unable to work because of illness for a few months at least were referred by SSSI physicians to a multidisciplinary team for assessment of rehabilitation potentials and an advice on the appropriate type of rehabilitation. The study group included all the 109 individuals who were referred to the team in the year 2000. Data on marital status, number of children and level of education was compared with information about the Icelandic population obtained in a national survey. The outcome of the rehabilitation was assessed in a telephone survey, carried out by the Social Science Research Institute, University of Iceland, 1-2 years after the assessment and by information obtained from the disability register at SSSI. The effectiveness of the rehabilitation programme was evaluated by comparing the study group with a comparable group that had started to receive rehabilitation pension before the SSSI could offer vocational rehabilitation. Their progress was assessed a year and half after they had contacted the SSSI, the same length of time as the study group had been in the in the rehabilitation programme. Results: In the study group there were about twice as many women as men. The mean age was 35 years (range 18-57 years). The main medical reasons for referral to the team were musculosceletal and psychiatric disorders. Those evaluated were more likely to be unmarried or divorced, had more children and a lower educational level than the general Icelandic population. After evaluation 40 individuals were referred to vocational rehabilitation for approximately 2 months in a rehabilitation clinic; 19 were referred to a 6 week personal computer training at a vocational rehabilitation centre and 15 to a longer (usually 18 months) rehabilitation program in the same centre. In all, 46 individuals received other treatment or education. Almost three quarters (72%) of the participants in the telephone survey said that their fitness for work had increased after rehabilitation, but only 47% had returned to work. At the time of the reserach, 23% were students and it is likely that a part of them will return to work when their studies are completed. Between one and two years after the evaluation by the multidisciplinary team 44 out of 109 (40.4%) in the study group received disability pension and a equal number received no social insurance benefits at all. In the comparison group 97 out of 119 (81.5%) received disability pension and 21 (17.7%) received no social insurance benefits at all. Conclusions: This study shows that vocational rehabilitation organized by the SSSI is effective and can prevent disability. The results of this study are similar to the results of two Swedish studies on the same topic.Tilgangur: Markmiðið með rannsókninni var að meta árangur starfsendurhæfingar á vegum Tryggingastofnunar ríkisins (TR) og hvort hún geti komið í veg fyrir örorku. Efniviður og aðferðir: Læknar TR vísuðu einstaklingum sem verið höfðu óvinnufærir í nokkra mánuði vegna sjúkdóms til þverfaglegs teymis, til mats á möguleikum á endurhæfingu og væri mælt með henni, hvernig henni yrði þá best háttað. Aflað var upplýsinga úr skýrslum endurhæfingarmatsteymisins um sjúkdómsástand, kyn, aldur, hjúskaparstöðu, fjölda barna á framfæri og menntunarstig þeirra einstaklinga sem metnir voru á árinu 2000. Upplýsingarnar um hjúskaparstöðu, fjölda barna á framfæri og menntunarstig voru bornar saman við upplýsingar um þjóðina frá sama ári (svör við Þjóðmálakönnun Félagsvísindastofnunar Háskóla Íslands um hjúskaparstöðu og barnafjölda og upplýsingar frá Hagstofu Íslands um menntunarstig). Upplýsingar um starfshæfni og vinnu þátttakenda eftir endurhæfingu voru fengnar með símakönnun sem Félagsvísindastofnun framkvæmdi. Fengnar voru upplýsingar úr gagnasafni TR um hvort þeir sem metnir höfðu verið af endurhæfingarmatsteymi á árinu 2000 voru á bótum frá TR einu til tveimur árum eftir matið. Til samanburðar var skoðuð bótastaða einstaklinga sem fengið höfðu endurhæfingarlífeyri áður en starfsendurhæfing á vegum TR hófst. Niðurstöður: Af þeim 109 sem metnir voru af endurhæfingarmatsteymi á árinu 2000 voru konur í talsverðum meirihluta (um helmingi fleiri en karlar). Meðalaldur matsþega var 35 ár. Læknisfræðilegar forsendur tilvísana til matsteymis voru fyrst og fremst stoðkerfisraskanir og geðraskanir. Matsþegar voru í meiri mæli ógiftir eða fráskildir, höfðu fleiri börn á framfæri og lægra menntunarstig en Íslendingar almennt. Fjörutíu matsþegum var vísað í atvinnulega endurhæfingu á Reykjalundi, 19 á tölvunámskeið hjá Hringsjá (starfsþjálfun fatlaðra) og 15 í fullt starfsnám hjá Hringsjá, en 46 fóru í aðra meðferð eða nám. Tæplega þrír fjórðu þátttakenda í símakönnun Félagsvísindastofnunar töldu starfshæfni sína hafa aukist, en innan við helmingur þátttakenda taldi sig þó vinnufæran (43,6%) og hafði stundað einhverja vinnu (46,9%). Auk þess var nær fjórðungur (22,8%) þátttakenda í námi og er líklegt að hluti þeirra skili sér út á vinnumarkaðinn að því loknu. Einu til tæplega tveimur árum eftir að mati lauk fengu 44 af 109 (40,4%) örorkubætur (örorkulífeyri eða örorkustyrk) frá TR og jafnmargir fengu engar bætur frá TR, en í samanburðarhópi fólks sem fengið hafði endurhæfingarlífeyri áður en starfsendurhæfing á vegum TR hófst fengu 97 af 119 (81,5%) örorkubætur, en 21 (17,7%) engar bætur frá TR hálfu öðru ári eftir upphaf lífeyrisgreiðslu. Ályktanir: Árangur af starfsendurhæfingu á vegum TR er svipaður og af starfsendurhæfingu á vegum Tryggingastofnana í Svíþjóð. Material and method: The Institute of Social Sciences carried out a telephone survey in October 2001, where it was attempted to contact the 109 individuals evaluated for rehabilitation potential by a multidisciplinary team in the year 2000. The data was analysed using descriptive statistics. Results: 83 (76.1%) replied. After the evaluation 40 individuals were referred to vocational rehabilitation for approximately 2 months in a rehabilitation clinic; 19 were referred to a 6 week personal computer training at a vocational rehabilitation centre and 15 to a longer (usually 18 months) rehabilitation program in the same centre. Approximately 80% were content with the vocational rehabilitation offered. 54% of those evaluated by the multidisciplinary team said that it had been useful for them to meet the members of the team and 59% said that the team had informed them on resources they had not been aware of. Approximately half of the participants said that their self-confidence and self-reliance had increased. Conclusion: Approximately 80% of the participants were content with the vocational rehabilitation offered and approximately 50% had gained more self-confidence and self-reliance.Tilgangur: Að kanna viðhorf matsþega til endurhæfingarmats og starfsendurhæfingarúrræða á vegum Tryggingastofnunar ríkisins (TR) og áhrif þessa ferlis á sjálfstraust og sjálfsbjargarviðleitni þeirra. Efniviður og aðferðir: Félagsvísindastofnun Háskóla Íslands kannaði viðhorf matsþega fyrir TR í október 2001. Reynt var að finna þá 109 einstaklinga sem metnir höfðu verið af matsteymi á árinu 2000 og tekið viðtal við þá í síma. Við úrvinnslu var notuð lýsandi tölfræði. Niðurstöður: Svör fengust frá 83 (76,1%) af matsþegum. Fjörutíu (48,2%) hafði að tillögu matsteymis verið vísað í atvinnulega endurhæfingu á Reykjalundi, 19 (22,9%) á tölvunámskeið í Hringsjá og 15 (18,1%) í fullt starfsnám í Hringsjá. Um fjórir af hverjum fimm reyndust ánægðir með þá starfsendurhæfingu sem TR hafði boðið þeim uppá á Reykjalundi eða í Hringsjá. Rúmlega helmingur þeirra sem metnir voru af matsteyminu töldu það hafa verið gagnlegt fyrir sig að hitta teymið og 59% matsþega töldu teymið hafa vísað á úrræði sem þeir vissu ekki um fyrir. Um helmingur þátttakenda sagði sjálfstraust sitt og sjálfsbjargarviðleitni hafa aukist frá því mat teymis fór fram.     Dreifing tannátu og glerungseyðingar eftir búsetu meðal 1., 7. og 10. bekkinga á Íslandi : niðurstöður úr MUNNÍS 2005

Efniviður og aðferðir: Gögn úr rannsókn á munnheilsu Íslendinga – MUNNÍS voru notuð. MUNNÍS var framkvæmd að mestu á vorönn árið 2005 og náði til 2.251 barns í 1., 7. og 10. bekk víðsvegar um landið. Alls voru börn úr 31 skóla skoðuð. Úrtakið var slembið og lagskipt klasaúrtak 20% allra barna á landinu í 1., 7. og 10. bekk. Jafnt hlutfall barna af höfuðborgarsvæði og landsbyggð var í úrtakinu. Niðurstöðum var skipt í þrennt eftir búsetu: 1) höfuðborgarsvæði, 2) til sjávar og 3) til sveita. Tannátustuðull var reiknaður út fyrir s.k. bestu greiningu, þ.e. frá niðurstöðum sjónrænnar og röntgengreiningar fyrir byrjandi tannskemmdir D1MFT og lengra komnar tannskemmdir D3MFT fyrir 12 og 15 ára, en einungis sjónræna skoðun fyrir 6 ára börn. Ef glerungseyðing fannst á a.m.k. einni tönn var sá einstaklingur talinn með glerungseyðingu. Niðurstöðutölur voru vigtaðar samkvæmt úrtaksaðferð. Niðurstöður: Hjá 10. bekk mældist marktækur munur á tannskemmdum í fullorðinstönnum eftir búsetu á höfuðborgarsvæði eða til sjávar og sveita þar sem tannátustuðull var hæstur í sjávarbyggðum. Hjá 7. bekk var hærra hlutfall barna með tannátustuðul D3MFT=0 til sveita (p=0,008). Í hinum árgöngunum var einnig munur á tannátustuðli milli svæða en hann reyndist ekki tölfræðilega marktækur. Engin glerungseyðing fannst á fullorðinstönnum meðal 6 ára barna. Hlutfall unglinga með glerungseyðingu var marktækt lægra til sveita (3,9%) en annars staðar hjá 7. bekkingum. Í 10. bekk var ekki jafn mikill munur eftir búsetu en hlutfall unglinga með glerungseyðingu í 10. bekk var þó hærra á höfuðborgarsvæði en á landsbyggðinni að meðaltali. Unglingar á höfuðborgarsvæðinu voru með hæst hlutfall glerungseyðingar bæði í 7. og 10. bekk (18,8% og 33,8%). Ályktanir: Minni munur var á niðurstöðum eftir búsetu en í fyrri rannsóknum hvað tannátu snertir en umtalsverður munur var á glerungseyðingu eftir búsetu

    Space-time recapture dynamics of PIT-tagged Northeast Atlantic mackerel (Scomber scombrus) reveal size-dependent migratory behaviour

    Based on GIS-mapping and semi-parametric modelling of recaptures from PIT-tag experiments in the North Sea nursery area (September 2011), the Celtic Seas spawning area (May-June 2014-2021) and the Icelandic Waters feeding area (August 2015-2019), we argue that the distribution of Northeast Atlantic (NEA) mackerel is influenced by a size-dependent migratory behaviour. The time-space recapture dynamics revealed that larger mackerel tended to migrate a longer distance between spawning and feeding areas, either through a western route from the Celtic Seas into the Icelandic Waters and the Greenland Sea or by following the main route northwards through the Faroe-Shetland Channel into the Norwegian Sea. This long-distance travel resulted in turn in delayed arrival in the North Sea wintering area. During the return spawning migration into the Celtic Seas, larger individuals remained in the front, likely heading to spawning grounds farther south than smaller conspecifics. Migration patterns also evolved with time at liberty as the mackerel grew older and larger, while possibly covering a progressively wider area over its annual migration cycle as suggested from the tagging data. However, the study also showed large inter-annual variability in the recapture patterns which likely reflect changes in environmental condition (prey availability and ocean current), NEA mackerel population demographics, and the spatial fishery dynamics.publishedVersio

    Differential gene expression during early development in recently evolved and sympatric Arctic charr morphs

    Phenotypic differences between closely related taxa or populations can arise through genetic variation or be environmentally induced, leading to altered transcription of genes during development. Comparative developmental studies of closely related species or variable populations within species can help to elucidate the molecular mechanisms related to evolutionary divergence and speciation. Studies of Arctic charr (Salvelinus alpinus) and related salmonids have revealed considerable phenotypic variation among populations and in Arctic charr many cases of extensive variation within lakes (resource polymorphism) have been recorded. One example is the four Arctic charr morphs in the ∼10,000 year old Lake Thingvallavatn, which differ in numerous morphological and life history traits. We set out to investigate the molecular and developmental roots of this polymorphism by studying gene expression in embryos of three of the morphs reared in a common garden set-up. We performed RNA-sequencing, de-novo transcriptome assembly and compared gene expression among morphs during an important timeframe in early development, i.e., preceding the formation of key trophic structures. Expectedly, developmental time was the predominant explanatory variable. As the data were affected by some form of RNA-degradation even though all samples passed quality control testing, an estimate of 3′-bias was the second most common explanatory variable. Importantly, morph, both as an independent variable and as interaction with developmental time, affected the expression of numerous transcripts. Transcripts with morph effect, separated the three morphs at the expression level, with the two benthic morphs being more similar. However, Gene Ontology analyses did not reveal clear functional enrichment of transcripts between groups. Verification via qPCR confirmed differential expression of several genes between the morphs, including regulatory genes such as AT-Rich Interaction Domain 4A (arid4a) and translin (tsn). The data are consistent with a scenario where genetic divergence has contributed to differential expression of multiple genes and systems during early development of these sympatric Arctic charr morphs.he project was funded by The Icelandic Center for Research (grant number: 100204011) to Sigurður S.     Extensive genetic differentiation between recently evolved sympatric Arctic charr morphs

    Publisher's version (útgefin grein)The availability of diverse ecological niches can promote adaptation of trophic specializations and related traits, as has been repeatedly observed in evolutionary radiations of freshwater fish. The role of genetics, environment, and history in ecologically driven divergence and adaptation, can be studied on adaptive radiations or populations showing ecological polymorphism. Salmonids, especially the Salvelinus genus, are renowned for both phenotypic diversity and polymorphism. Arctic charr (Salvelinus alpinus) invaded Icelandic streams during the glacial retreat (about 10,000 years ago) and exhibits many instances of sympatric polymorphism. Particularly, well studied are the four morphs in Lake Þingvallavatn in Iceland. The small benthic (SB), large benthic (LB), planktivorous (PL), and piscivorous (PI) charr differ in many regards, including size, form, and life history traits. To investigate relatedness and genomic differentiation between morphs, we identified variable sites from RNA‐sequencing data from three of those morphs and verified 22 variants in population samples. The data reveal genetic differences between the morphs, with the two benthic morphs being more similar and the PL‐charr more genetically different. The markers with high differentiation map to all linkage groups, suggesting ancient and pervasive genetic separation of these three morphs. Furthermore, GO analyses suggest differences in collagen metabolism, odontogenesis, and sensory systems between PL‐charr and the benthic morphs. Genotyping in population samples from all four morphs confirms the genetic separation and indicates that the PI‐charr are less genetically distinct than the other three morphs. The genetic separation of the other three morphs indicates certain degree of reproductive isolation.     Transcriptional dynamics of a conserved gene expression network associated with craniofacial divergence in Arctic charr

    Background Understanding the molecular basis of craniofacial variation can provide insights into key developmental mechanisms of adaptive changes and their role in trophic divergence and speciation. Arctic charr (Salvelinus alpinus) is a polymorphic fish species, and, in Lake Thingvallavatn in Iceland, four sympatric morphs have evolved distinct craniofacial structures. We conducted a gene expression study on candidates from a conserved gene coexpression network, focusing on the development of craniofacial elements in embryos of two contrasting Arctic charr morphotypes (benthic and limnetic). Results Four Arctic charr morphs were studied: one limnetic and two benthic morphs from Lake Thingvallavatn and a limnetic reference aquaculture morph. The presence of morphological differences at developmental stages before the onset of feeding was verified by morphometric analysis. Following up on our previous findings that Mmp2 and Sparc were differentially expressed between morphotypes, we identified a network of genes with conserved coexpression across diverse vertebrate species. A comparative expression study of candidates from this network in developing heads of the four Arctic charr morphs verified the coexpression relationship of these genes and revealed distinct transcriptional dynamics strongly correlated with contrasting craniofacial morphologies (benthic versus limnetic). A literature review and Gene Ontology analysis indicated that a significant proportion of the network genes play a role in extracellular matrix organization and skeletogenesis, and motif enrichment analysis of conserved noncoding regions of network candidates predicted a handful of transcription factors, including Ap1 and Ets2, as potential regulators of the gene network. The expression of Ets2 itself was also found to associate with network gene expression. Genes linked to glucocorticoid signalling were also studied, as both Mmp2 and Sparc are responsive to this pathway. Among those, several transcriptional targets and upstream regulators showed differential expression between the contrasting morphotypes. Interestingly, although selected network genes showed overlapping expression patterns in situ and no morph differences, Timp2 expression patterns differed between morphs. Conclusion Our comparative study of transcriptional dynamics in divergent craniofacial morphologies of Arctic charr revealed a conserved network of coexpressed genes sharing functional roles in structural morphogenesis.     Differential expression of the aryl hydrocarbon receptor pathway associates with craniofacial polymorphism in sympatric Arctic charr

    Background The developmental basis of craniofacial morphology hinges on interactions of numerous signalling systems. Extensive craniofacial variation in the polymorphic Arctic charr, a member of the salmonid family, from Lake Thingvallavatn (Iceland), offers opportunities to find and study such signalling pathways and their key regulators, thereby shedding light on the developmental pathways, and the genetics of trophic divergence. Results To identify genes involved in the craniofacial differences between benthic and limnetic Arctic charr, we used transcriptome data from different morphs, spanning early development, together with data on craniofacial expression patterns and skeletogenesis in model vertebrate species. Out of 20 genes identified, 7 showed lower gene expression in benthic than in limnetic charr morphs. We had previously identified a conserved gene network involved in extracellular matrix (ECM) organization and skeletogenesis, showing higher expression in developing craniofacial elements of benthic than in limnetic Arctic charr morphs. The present study adds a second set of genes constituting an expanded gene network with strong, benthic–limnetic differential expression. To identify putative upstream regulators, we performed knowledge-based motif enrichment analyses on the regulatory sequences of the identified genes which yielded potential binding sites for a set of known transcription factors (TFs). Of the 8 TFs that we examined using qPCR, two (Ahr2b and Ap2) were found to be differentially expressed between benthic and limnetic charr. Expression analysis of several known AhR targets indicated higher activity of the AhR pathway during craniofacial development in benthic charr morphotypes.
