    Eye Bleeding Due to Leech Infestation in an Ischemic Heart- Disease Patient

    A 49-year-old Iranian male with diagnosis of ischemic heart disease (IHD) and arterial hypertension (AH) was admitted at Emergency Ward of Vase’ee Hospital of Sabzevar, Iran. As well, the patient had red eye and left eye bleeding. On eye examination, leech infestation was found to be the cause of the eye bleeding. In the endemic regions in a patient presenting red eye and eye bleeding, leech infestation came up to be an important differential diagnosis. Therefore, to prevent leech infestation in endemic regions, local people are advised to be informed more effectively about the necessity to use safe, clean, and filtered drinking water, and a perfect scrutiny for leeches or other sources of infestation that should be undertaken before bathing

    Expanding the clinical phenotype of IARS2-related mitochondrial disease.

    BACKGROUND: IARS2 encodes a mitochondrial isoleucyl-tRNA synthetase, a highly conserved nuclear-encoded enzyme required for the charging of tRNAs with their cognate amino acid for translation. Recently, pathogenic IARS2 variants have been identified in a number of patients presenting broad clinical phenotypes with autosomal recessive inheritance. These phenotypes range from Leigh and West syndrome to a new syndrome abbreviated CAGSSS that is characterised by cataracts, growth hormone deficiency, sensory neuropathy, sensorineural hearing loss, and skeletal dysplasia, as well as cataract with no additional anomalies. METHODS: Genomic DNA from Iranian probands from two families with consanguineous parental background and overlapping CAGSSS features were subjected to exome sequencing and bioinformatics analysis. RESULTS: Exome sequencing and data analysis revealed a novel homozygous missense variant (c.2625C > T, p.Pro909Ser, NM_018060.3) within a 14.3 Mb run of homozygosity in proband 1 and a novel homozygous missense variant (c.2282A > G, p.His761Arg) residing in an ~ 8 Mb region of homozygosity in a proband of the second family. Patient-derived fibroblasts from proband 1 showed normal respiratory chain enzyme activity, as well as unchanged oxidative phosphorylation protein subunits and IARS2 levels. Homology modelling of the known and novel amino acid residue substitutions in IARS2 provided insight into the possible consequence of these variants on function and structure of the protein. CONCLUSIONS: This study further expands the phenotypic spectrum of IARS2 pathogenic variants to include two patients (patients 2 and 3) with cataract and skeletal dysplasia and no other features of CAGSSS to the possible presentation of the defects in IARS2. Additionally, this study suggests that adult patients with CAGSSS may manifest central adrenal insufficiency and type II esophageal achalasia and proposes that a variable sensorineural hearing loss onset, proportionate short stature, polyneuropathy, and mild dysmorphic features are possible, as seen in patient 1. Our findings support that even though biallelic IARS2 pathogenic variants can result in a distinctive, clinically recognisable phenotype in humans, it can also show a wide range of clinical presentation from severe pediatric neurological disorders of Leigh and West syndrome to both non-syndromic cataract and cataract accompanied by skeletal dysplasia

    On behalf of all the members of the Organizing Committee, I would like to welcome you to the 5th International Conference on Signal Processing and Telecommunication Systems, ICSPCS’2011. The ICSPCS conferences evolved from the series of successful symposia and workshops, i.e. International Symposia on DSP and Communication Systems (DSPCS), and Workshops on the Internet, Telecommunications and Signal Processing (WITSP). As in the past, the Organizing Committee has succeeded this year in achieving an official technical co-sponsorship of the IEEE Communications Society for conference. As a result the ICSPCS’2011 conference has been listed in the IEEE database of the conferences and the proceedings will be published in IEEE Xplore. This year response to the call for papers was very good, with 223 full paper submissions. All submitted papers have been put through a rigorous peer review process, and each paper received at least two but often three or more peer reviews. Based on those reviews, 83 papers have been accepted, and finally 81 included in the program – 48 for oral and 33 for poster presentations. To complement the program, the Organizing Committee invited a keynote speaker, Dr. Thaddeus Owens Walker III of United States Naval Academy. His address: Securing Wireless in the Cyberdomain will be a great addition to the already excellent program. At this point, I wish to thank the authors who put so many efforts, first in preparing their manuscripts for submission, and then for incorporating reviewers’ comments in the camera-ready papers. I extend my sincere thanks to the 68 members of the Technical Program Committee for organizing reviews for the submitted papers, and 222 reviewers for thorough reviews of the manuscripts.     Effect of food allergen exculsion on symptoms and quality of life in irritable bowel syndrome

    Background and Objective: Irritable bowel syndrome (IBS) is one of the most common gastrointestinal disorders characterized by abdominal pain or discomfort and altered bowel habits. Although IBS etiology is poorly understood, some psychological disturbances and food allergies have been proposed. This study was done to determine the effect of food allergen exculsion on symptoms and quality of life in IBS. Materials and Methods: In this descriptive and anolytical study was done on 100 IBS patients in Tabriz, Iran during 2011. Food allergies of specific origin and effects of non allergic regimens on quality of life, pain severity and disease symptoms were evaluted during one month period. Data were analyzed using SPSS-16, student’s t-test, Chi-Square and Fisher’s exact tests. Results: Patients age were 31.5±7.2 between 15-43 years. Specific antibodies against allergic foods were detectable in 31 (31%) cases. Penaut showed to induce the highest food allergy in 5 (16.1%) patients. The median disease duration was 4.5±2.6 years in allergic patients while 3.5±2.9 in non allergic group with no significant difference. The changes is quality of life was significant in non allergic regimens after one month in allergic group (P<0.05) while disease symptoms such as mucus defecation and flatulence had no significant changes. Conclusion: This study showed that one month non allergic regimens reduce abdominal pain and improve quality of life in IBS patients
