MEDUSA - New Model of Internet Topology Using k-shell Decomposition
The k-shell decomposition of a random graph provides a different and more
insightful separation of the roles of the different nodes in such a graph than
does the usual analysis in terms of node degrees. We develop this approach in
order to analyze the Internet's structure at a coarse level, that of the
"Autonomous Systems" or ASes, the subnetworks out of which the Internet is
assembled. We employ new data from DIMES (see http://www.netdimes.org), a
distributed agent-based mapping effort which at present has attracted over 3800
volunteers running more than 7300 DIMES clients in over 85 countries. We
combine this data with the AS graph information available from the RouteViews
project at Univ. Oregon, and have obtained an Internet map with far more detail
than any previous effort.
The data suggests a new picture of the AS-graph structure, which
distinguishes a relatively large, redundantly connected core of nearly 100 ASes
and two components that flow data in and out from this core. One component is
fractally interconnected through peer links; the second makes direct
connections to the core only. The model which results has superficial
similarities with and important differences from the "Jellyfish" structure
proposed by Tauro et al., so we call it a "Medusa." We plan to use this picture
as a framework for measuring and extrapolating changes in the Internet's
physical structure. Our k-shell analysis may also be relevant for estimating
the function of nodes in the "scale-free" graphs extracted from other

PLASMA CONCENTRATIONS OF METHADONE DURING POSTOPERATIVE PATIENT-CONTROLLED EXTRADURAL ANALGESIA
Plasma concentrations of methadone were measured by gas chromatography in 16 patients receiving extradural methadone by continuous infusion for relief of postoperative pain. Venous blood samples were taken after a loading dose of extradural methadone 2 mg and during infusion of 0.46 mg h−1 plus patient-controlled increments of 0.2-1 mg. Mean (SD) plasma concentration of methadone was 9.8 (2.1) ng ml−1 at 15 min; this did not change significantly during the first 2 h, after which it increased gradually to 32.2 (4.6) ng ml−1 (P < 0.001) at the end of 24 h. The mean quantity of extradural methadone required to produce effective analgesia was 10.3 (1.8) mg during the first 12 h after operation and 6 (1.0) mg for the subsequent 12 h. The mean amount of methadone for effective analgesia on the second day was 7.6 (1.1) mg. No adverse effects were detected during the 2-3 days of methadone therapy. Plasma concentration of methadone increased significantly during patient-controlled infusion of extradural methadone in the first 24 h after operation, suggesting rapid vascular uptake. Runtime Analysis of Probabilistic Crowding and Restricted Tournament Selection for Bimodal Optimisation
Many real optimisation problems lead to multimodal domains and so require the identifi-
cation of multiple optima. Niching methods have been developed to maintain the population
diversity, to investigate many peaks in parallel and to reduce the effect of genetic drift. Using
rigorous runtime analysis, we analyse for the first time two well known niching methods: probabilistic
crowding and restricted tournament selection (RTS). We incorporate both methods
into a (µ+1) EA on the bimodal function Twomax where the goal is to find two optima at
opposite ends of the search space. In probabilistic crowding, the offspring compete with their
parents and the survivor is chosen proportionally to its fitness. On Twomax probabilistic
crowding fails to find any reasonable solution quality even in exponential time. In RTS the
offspring compete against the closest individual amongst w (window size) individuals. We
prove that RTS fails if w is too small, leading to exponential times with high probability.
However, if w is chosen large enough, it finds both optima for Twomax in time O(µn log n)
with high probability. Our theoretical results are accompanied by experimental studies that
match the theoretical results and also shed light on parameters not covered by the theoretical
results
Prior Cancer Is Associated with Lower Atherosclerotic Cardiovascular Disease Risk at First Acute Myocardial Infarction
BACKGROUND: Patients with cancer are at increased risk of acute myocardial infarction (AMI). It is unclear if the Atherosclerotic Cardiovascular Disease (ASCVD) risk score at incident AMI is reflective of this higher risk in patients with prior cancer than those without. METHODS: We linked nationwide AMI and cancer registries from 2008 to 2019. A total of 18,200 eligible patients with ASCVD risk score calculated at incident AMI were identified (1086 prior cancer; 17,114 no cancer). RESULTS: At incident AMI, age-standardized mean ASCVD risk was lower in the prior cancer group (18.6%) than no cancer group (20.9%) (p < 0.001). Prior to incident AMI, smoking, hypertension, hyperlipidemia and diabetes mellitus were better controlled in the prior cancer group. However post-AMI, prior cancer was associated with lower guideline-directed medical therapy usage and higher all-cause mortality (adjusted hazard ratio 1.85, 95% confidence interval 1.66-2.07). CONCLUSIONS: AMI occurred despite better control of cardiovascular risk factors and lower age-standardized estimated mean 10-year ASCVD risk among patients with prior cancer than no cancer. Fabrication of Coaxial Si1−xGex Heterostructure Nanowires by O2 Flow-Induced Bifurcate Reactions
We report on bifurcate reactions on the surface of well-aligned Si1−xGex nanowires that enable fabrication of two different coaxial heterostructure nanowires. The Si1−xGex nanowires were grown in a chemical vapor transport process using SiCl4 gas and Ge powder as a source. After the growth of nanowires, SiCl4 flow was terminated while O2 gas flow was introduced under vacuum. On the surface of nanowires was deposited Ge by the vapor from the Ge powder or oxidized into SiO2 by the O2 gas. The transition from deposition to oxidation occurred abruptly at 2 torr of O2 pressure without any intermediate region and enables selectively fabricated Ge/Si1−xGex or SiO2/Si1−xGex coaxial heterostructure nanowires. The rate of deposition and oxidation was dominated by interfacial reaction and diffusion of oxygen through the oxide layer, respectively
Fractal Boundaries of Complex Networks
We introduce the concept of boundaries of a complex network as the set of
nodes at distance larger than the mean distance from a given node in the
network. We study the statistical properties of the boundaries nodes of complex
networks. We find that for both Erd\"{o}s-R\'{e}nyi and scale-free model
networks, as well as for several real networks, the boundaries have fractal
properties. In particular, the number of boundaries nodes {\it B} follows a
power-law probability density function which scales as . The clusters
formed by the boundary nodes are fractals with a fractal dimension . We present analytical and numerical evidence supporting these
results for a broad class of networks. Our findings imply potential
applications for epidemic spreading
Epidermal Growth Factor–PEG Functionalized PAMAM-Pentaethylenehexamine Dendron for Targeted Gene Delivery Produced by Click Chemistry
Aim of this study was the site-specific conjugation of an epidermal growth factor (EGF)-polyethylene glycol (PEG) chain by click chemistry onto a poly(amido amine) (PAMAM) dendron, as a key step toward defined multifunctional carriers for targeted gene delivery. For this purpose, at first propargyl amine cored PAMAM dendrons with ester ends were synthesized. The chain terminal ester groups were then modified by oligoamines with different secondary amino densities. The oligoamine-modified PAMAM dendrons were well biocompatible, as demonstrated in cytotoxicity assays. Among the different oligoamine-modified dendrons, PAMAM-pentaethylenehexamine (PEHA) dendron polyplexes displayed the best gene transfer ability. Conjugation of PAMAM-PEHA dendron with PEG spacer was conducted via click reaction, which was performed before amidation with PEHA. The resultant PEG-PAMAM-PEHA copolymer was then coupled with EGF ligand. pDNA transfections in HuH-7 hepatocellular carcinoma cells showed a 10-fold higher efficiency with the polyplexes containing conjugated EGF as compared to the ligand-free ones, demonstrating the concept of ligand targeting. Overall gene transfer efficiencies, however, were moderate, suggesting that additional measures for overcoming subsequent intracellular bottlenecks in delivery have to be taken
Development and Implementation of a Registry of Patients Attending Multidisciplinary Pain Treatment Clinics: The Quebec Pain Registry
The Quebec Pain Registry (QPR) is a large research database of patients suffering from various chronic pain (CP) syndromes who were referred to one of five tertiary care centres in the province of Quebec (Canada). Patients were monitored using common demographics, identical clinical descriptors, and uniform validated outcomes. This paper describes the development, implementation, and research potential of the QPR. Between 2008 and 2013, 6902 patients were enrolled in the QPR, and data were collected prior to their first visit at the pain clinic and six months later. More than 90% of them (mean age ± SD: 52.76 ± 4.60, females: 59.1%) consented that their QPR data be used for research purposes. The results suggest that, compared to patients with serious chronic medical disorders, CP patients referred to tertiary care clinics are more severely impaired in multiple domains including emotional and physical functioning. The QPR is also a powerful and comprehensive tool for conducting research in a “real-world” context with 27 observational studies and satellite research projects which have been completed or are underway. It contains data on the clinical evolution of thousands of patients and provides the opportunity of answering important research questions on various aspects of CP (or specific pain syndromes) and its management
