A rare case of spontaneous simultaneous bilateral tubal ectopic pregnancy
Ectopic pregnancy is still an important cause of maternal mortality. The incidence of ectopic pregnancies is reported to be rising. Spontaneous bilateral tubal ectopic pregnancy in the absence of preceding induction of ovulation is extremely unusual occurrence and is the rarest form of ectopic pregnancy. The estimated incidence is 1 in 725 to 1580 of all ectopic corresponding to one per 200000 live births. The preoperative diagnosis of bilateral ectopic pregnancy remains a challenge. Serum β-hCG, TVS, color Doppler help making diagnosis of ectopic pregnancy but laparoscopy is gold standard diagnostic modality. The diagnosis of bilateral tubal pregnancy is usually made intra-operatively. This underscores the importance of identifying and closely examining both tubes at the time of surgery, even in the presence of significant adhesive disease. Salpingostomy, salpingotomy, salpingectomy, segmental resection, fimbrial expression are the operative modalities described for management of ectopic pregnancy
Recommendation Based On Comparative Analysis of Apriori and BW-Mine Algorithm
With The Growth of WWW recommending appropriate and relevant page to the user is a challenging task. In many web Applications, user would like to get recommendation based on their interest of surfing. Web Mining is used to extract relevant information for the user from logs, web content, hyperlinks etc. In this paper we will be using logs to recommend frequent access patterns to the users .This paper aims at using the logs of user ,cleaning logs , identifying users , identifying session , completing sessions from website structure and then using and comparing different recommendation algorithm like Apriori Algorithms and BW-Mine to recommend frequent items to the user. We will also be comparing different recommendations Algorithm with the help of example. The fundamental of finding access patterns with Apriori is that any set that occurs frequently must have its frequent subset. The fundamental of finding access pattern with BW-Mine, it constructs the WB-table, VI-List, and HI-Counter for finding frequent patterns
PRELIMINARY ANTIMICROBIAL STUDY OF KANTAKARI (SOLANUM XANTHOCARPUM SCHRAD & WENDL) BY DITCH PLATE TECHNIQUE
Lower Urinary tract Infection (Lower UTI) is the second most common infection in human population. Escherichia coli is one of the microbial strain responsible for lower UTI in most of the cases. Standard antibiotics can control the infection but there is recurrence of episodes in many cases. Constant research is carried out in this field to discover new antimicrobial agents. It is difficult for the microorganisms to acquire resistance to herbs and the polycompounds in them. They may make potential and promising antimicrobial agents references from classical text of Ayurveda suggest Kantakari (Solanum xanthocarpum Schrad & Wendl) to be effective in urinary disorders. The antimicrobial activity of Kantakari was studied invitro in its traditional forms that is Swaras (juice), Kalka (paste) and Chuna (powder) against the strains of Escherichia coli, Klebsiella pneumoniae, Staphylococcus aureus, Pseudomonas aeuriginosa, and Candida albicans. The study was carried out by using ditch plate technique and zones of inhibition were measured and statistically analyzed. It was observed that juice form and paste were more effective than powder form. Escherichia coli strain was found to be sensitive to the juice, paste and powder samples of Kantakari. Klebsiella pneumoniae showed intermediate resistance as compared to other organisms. In conclusion of this study it was found that Kantakari has good antimicrobial potential and must be studied further
An approximate theoretical analysis and experimental verification of turbulent entrance region flow of drag reducing fluids
Entry lengths for pipe flows of moderately drag reducing fluids are determined using momentum integral technique. It is shown theoretically that the entry lengths for drag reducing fluids could be significantly larger than the Newtonian fluids flowing turbulently under otherwise identical conditions. The experimental data from the literature bear out the theoretical calculations
Atypical leiomyoma: unusual locations and findings coupled with uncommon presentation
Extrauterine leiomyomas are a rare phenomenon, and they present a great diagnostic challenge. Retroperitoneal leiomyomas remain asymptomatic for a variable period and usually present with wide variety of signs and symptoms. A 51 years old female was multiparous and had undergone a total laparoscopic hysterectomy along with a bilateral salpingo-oophorectomy about four years ago. She visited the outpatient department of a tertiary care hospital complaining of something coming out per vaginum and expressed difficulty in passing urine and also complained of stress urinary incontinence. Bimanual pelvic examination, ultrasonography (USG) and Magnetic resonance imaging (MRI) revealed a large oval shaped well defined solid mass completely filling the vaginal canal while compressing urinary bladder. Tumor marker levels were normal. Laparotomy with adhesiolysis and complete excision of retroperitoneal mass was performed. Frozen section followed by histopathology and immunohistochemistry confirmed that the mass was a leiomyoma. After surgical intervention and removal of mass, the symptoms dissipated and patient had a complete recovery. Despite hysterectomy the manifestation of leiomyoma appears to be apparent. Surgical excision of the mass appeared to be the correct move forward after the leiomyoma was diagnosed with the help of the MRI and the USG

Use of intrauterine balloon tamponade in successful conservative management of life threatening menorrhagia in a patient with idiopathic thrombocytopenic purpura and severe anemia
A 45-year-old woman was brought into the emergency ward of a tertiary care hospital. The patient gave a history of excess bleeding and passage of clots per vaginum since the past 15 days. The patient had a history of severe headaches, body ache, giddiness and vomiting. She was diagnosed with idiopathic thrombocytopenic purpura (ITP) and severe anemia six months before this incidence. The authors performed a diagnostic pelvic and transvaginal ultrasound and reported the presence of uterine fibroids. They determined that these fibroids had aggravated the bleeding which was caused by ITP. The diagnosis of ITP coupled with severe anemia prevented the authors from using the traditional approach of a hysterectomy in such a case. She was administered tranexamic acid, testosterone enanthate, blood, intravenous immunoglobulins, steroids and platelets transfusion. Uterine artery embolization (UAE) was tried for arresting menorrhagia, but was unsuccessful. Finally, a Bakri® balloon, normally used for management of post-partum hemorrhage (PPH), was inserted and retained for 48 hrs.. The bleeding gradually ceased and significant improvements in hematocrit and platelet count were observed. However, the patient then suffered from a deep vein thrombosis (DVT) in the lower extremity, which was treated with low molecular weight heparin (LMWH), a thrombectomy and an Inferior vena cava Filter. Patient was advised a hysterectomy to curb her symptoms, however, she was too exhausted of surgical procedures and refused. Therefore, the authors decided to employ the use of Selective Estrogen Receptor Molecule Therapy (SERM). 3-months post-intervention, patient is thriving and stable
