    Mother\u27s feelings of distress and related factors resulting from the crying of her one-month-old infants

    The purpose of this study was to clarify the nature of mother\u27s distress and its related factors resulting from the crying of her one-month-old infant. Subjects and Methods The subjects were mothers who delivered children in hospitals/maternity clinics in the Hokuriku district and who gave consent to our survey at the time of health examinations for one-month-old infants. The contents of the questionnaire were: characteristics of the infant\u27s crying, states of mother\u27s sleep, feeding and receiving support related to her distress about her infant and its associated factors. Scores were obtained using a 4-point Likert scale. Results Effective responses were obtained from 630 mothers, who consisted of 298 primiparas (47.3%) and 332 multiparas (52.7%). About 50% of the mothers experienced distress because they felt at a loss when their infant cried and when the infant did not stop crying even when being held or lulled. New mothers who had no experience with infants were more likely to indicate distress. The distress was significantly associated with factors such as the way the infant cried and if the infant failed to fall asleep in a timely way. It also was associated with factors in the mother\u27s life such as feeling burdened with childcare and lack of confidence in childcare. Conclusion In order to support mothers having a one-month-old infant, it is important to pay attention to the characteristics of infant\u27s crying, states of mother\u27s fatigue, mother\u27s expression of depression, and to know how mother comprehends childcare and her infant\u27s crying. For screening to be effective it must include both infant and mother factors. 目 的  生後1 ヶ月児の泣きに対する母親の困難感とその感情に関連する要因を明らかにすることを目的とし た。 対象と方法  北陸地方の病産院にて出産し,1 ヶ月健診時に調査の同意が得られた母親を対象に,自己記入式質問 紙調査を実施した。調査内容は,児の泣きに対する母親の困難感と,その関連要因として,児の泣きの 性質や母親の睡眠・授乳状況,サポート状況などの質問項目を設定し,各々4段階リカート尺度で点数 化した。 結 果  有効回答は,初産婦298名(47.3%),経産婦332名(52.7%),合計630名であった。全体の約半数の母親が, 児が泣くと戸惑ったり,抱いたり,あやしても泣きやまない困難な状況を経験していた。困難感を示し た母親は,小さな子どもと接したことのない初産婦に多く,子どもの泣き方が特徴的であったり,なか なか寝入らないなど,子ども側の要因と母親の生活状況,育児に対する負担感や自信感等の母親側の要 因が困難感に関連していた。 結 論  生後1 ヶ月時の母児の支援には,児側の要因と母親側の要因の双方に着目し,児の泣きの特徴や,母 親の疲労状態,育児に対する気持ち等に注意を向け,母親が児の泣きをどのようにとらえているのかを 知ることが重要であり,これらのスクリーニングの必要性が示唆された

    Recognition of the "team medical-care" in medical field.

    チーム医療に対して高い関心が寄せられていることから,チーム医療として認識される具体的な事柄や各職種に対する認識,その利点や職種間の認識の違いなどを明らかにする目的で,岡山県内の病院の看護師,診療放射線技師,臨床検査技師,計652名に対してアンケート調査を行った。有効回答者数470名で,その内訳は看護師207名,診療放射線技師91名,臨床検査技師172名であった。結果はチーム医療と認識される具体的な事柄については,異業種間カンファレンスがチーム医療を促進する事柄として認識されているが,整備・実施されていないことがうかがえた。各職種に対する認識については,職種別・経験年数別にみても,医師,看護師,理学・作業療法士はチーム医療のメンバーとして認識されている割合が高かった。また,経験年数別では,10~14年の人までは統計上ややばらつきがみられるが,経験年数15年以上の人はあらゆる職種をチーム医療のメンバーであると考え,その重要性を認識していることがわかった。チーム医療の利点については,どの職種も共通して「患者中心の医療」と考えている割合が高かった。Recently, the "team medical-care" has been received increasing attention in medical field. Therefore we have to develop a clear understanding of "team medical-care". The purpose of this study is to investigate things regarded as "team medical-care" by co-medical, recognition of other category of license, the advantages of "team medical-care" and recognition against the "team medical-care" by different category of license. Total 652 nurses, clinical radiological technologists and clinical laboratory technologists who are working at 8 hospitals in Okayama prefecture were examined by using questionnaire and 470 members (72.1%) responded to it. They were nurses 207 (44.0%), clinical radiological technologists 91 (19.4%), clinical laboratory technologists 172 (36.6%). Majority of respondents recognized that the medical conference consisted of different category of license promoted the "team medical-care", and ninety percent of participants prompted more frequent medical conference. In both by category of license and years of experience, they regarded doctor, nurse, physical therapist and occupational therapist as the menber of the "team medical-care" at high rate. The respondences which have over 15 years of experience regarded all category of license as the menber of the "team medical-care", while the respondences which have less than 10 to 14 years of experience showed disperision of the recongnition. The advantages of "team medical-care" were highly perceived as "patient-centered medical care" by all category of license

    Improved methods for detection of β-galactosidase (lacZ) activity in hard tissue

    The ß-galactosidase gene (lacZ) of Escherichia coli is widely used as a reporter gene. The expression of lacZ can be detected by enzyme-based histochemical staining using chromogenic substrates such as 5-bromo-4-chloro-3-indolyl-ß-D: -galactoside (X-gal). Because the enzymatic activity of lacZ is vulnerable to high temperatures and acid treatment for demineralization, detection of lacZ on paraffinized sections is difficult, especially for hard tissues, which require demineralization before sectioning in paraffin. To circumvent this problem, whole-mount X-gal staining before sectioning is performed. However, detection of lacZ activity in the center of larger portions of hard whole adult tissues is challenging. In this study, focusing on fixation procedures, we determined the conditions conducive to improved detection of lacZ activity in deeper areas of whole tissues. We used an annexin a5 (Anxa5)-lacZ reporter mouse model in which the Anxa5 expression in hard tissue is indicated by lacZ activity. We found that lacZ activity could be detected throughout the periodontal ligament of adult mice when fixed in 100% acetone, whereas it was not detected in the periodontal ligament around the root apex fixed in glutaraldehyde and paraformaldehyde. This staining could not be detected in wild-type mice. Acetone maintains the lacZ activity within 48 h of fixation at both 4°C and at room temperature. In conclusion, acetone is the optimal fixative to improve permeability for staining of lacZ activity in large volumes of adult hard tissues

    Extreme Suppression of Lateral Floret Development by a Single Amino Acid Change in the VRS1 Transcription Factor

    Increasing grain yield is an endless challenge for cereal crop breeding. In barley (Hordeum vulgare), grain number is controlled mainly by Six-rowed spike 1 (Vrs1), which encodes a homeodomain leucine zipper class I transcription factor. However, little is known about the genetic basis of grain size. Here, we show that extreme suppression of lateral florets contributes to enlarged grains in deficiens barley. Through a combination of fine-mapping and resequencing of deficiens mutants, we have identified that a single amino acid substitution at a putative phosphorylation site in VRS1 is responsible for the deficiens phenotype. deficiens mutant alleles confer an increase in grain size, a reduction in plant height, and a significant increase in thousand grain weight in contemporary cultivated germplasm. Haplotype analysis revealed that barley carrying the deficiens allele (Vrs1.t1) originated from two-rowed types carrying the Vrs1.b2 allele, predominantly found in germplasm from northern Africa. In situ hybridization of histone H4, a marker for cell cycle or proliferation, showed weaker expression in the lateral spikelets compared with central spikelets in deficiens. Transcriptome analysis revealed that a number of histone superfamily genes were up-regulated in the deficiens mutant, suggesting that enhanced cell proliferation in the central spikelet may contribute to larger grains. Our data suggest that grain yield can be improved by suppressing the development of specific organs that are not positively involved in sink/source relationships

    亜急性期脳卒中患者および健常者における歩行観察時のミラーニューロンシステムの活性化

    The observation of walking improves gait ability in chronic stroke survivors. It has also been suggested that activation of the mirror neuron system contributes to this effect. However, activation of the mirror neuron system during gait observation has not yet been assessed in sub-acute stroke patients. The objective of this study was to clarify the activation of mirror neuron system during gait observation in sub-acute stroke patients and healthy persons. In this study, we sequentially enrolled five sub-acute stroke patients who had undergone gait training and nine healthy persons. We used fMRI to detect neuronal activation during gait observation. During the observation period in the stroke group, neural activity in the left inferior parietal lobule, right and left inferior frontal gyrus was significantly higher than during the rest period. In the healthy group, neural activity in the left inferior parietal lobule, left inferior frontal gyrus, left middle frontal gyrus, left superior temporal lobule and right and left middle temporal gyrus was significantly higher than during the rest period. The results indicate that the mirror neuron system was activated during gait observation in sub-acute stroke patients who had undergone gait training and also in healthy persons. Our findings suggest that gait observation treatment may provide a promising therapeutic strategy in sub-acute stroke patients who have experienced gait training

    Sequential therapies after atezolizumab plus bevacizumab or lenvatinib first-line treatments in hepatocellular carcinoma patients

    Introduction: The aim of this retrospective proof-of-concept study was to compare different second-line treatments for patients with hepatocellular carcinoma and progressive disease (PD) after first-line lenvatinib or atezolizumab plus bevacizumab.Materials and methods: A total of 1381 patients had PD at first-line therapy. 917 patients received lenvatinib as first-line treatment, and 464 patients atezolizumab plus bevacizumab as first-line.Results: 49.6% of PD patients received a second-line therapy without any statistical difference in overall survival (OS) between lenvatinib (20.6 months) and atezolizumab plus bev-acizumab first-line (15.7 months; p = 0.12; hazard ratio [HR] = 0.80). After lenvatinib first-line, there wasn't any statistical difference between second-line therapy subgroups (p = 0.27; sorafenib HR: 1; immunotherapy HR: 0.69; other therapies HR: 0.85). Patients who under-went trans-arterial chemo-embolization (TACE) had a significative longer OS than patients who received sorafenib (24.7 versus 15.8 months, p < 0.01; HR = 0.64). After atezolizumab plus bevacizumab first-line, there was a statistical difference between second-line therapy subgroups (p < 0.01; sorafenib HR: 1; lenvatinib HR: 0.50; cabozantinib HR: 1.29; other therapies HR: 0.54). Patients who received lenvatinib (17.0 months) and those who under-went TACE (15.9 months) had a significative longer OS than patients treated with sorafenib (14.2 months; respectively, p = 0.01; HR = 0.45, and p < 0.05; HR = 0.46).Conclusion: Approximately half of patients receiving first-line lenvatinib or atezolizumab plus bevacizumab access second-line treatment. Our data suggest that in patients progressed to atezolizumab plus bevacizumab, the systemic therapy able to achieve the longest survival is lenvatinib, while in patients progressed to lenvatinib, the systemic therapy able to achieve the longest survival is immunotherapy
