    Lessons Learned from Initial Piloting of Farmer-Orientated Value Chain Training in the Pacific

    There have been numerous initiatives in the Pacific region over the past decade to mainstream the concept of value chains into the agricultural development arena. Since 2012, the ACP/EU Technical Centre for Agricultural and Rural Cooperation (CTA) has been at the forefront of value chain development in the Pacific region, supporting a number of value chain studies and the publication of the ‘Agricultural Value Chain Guide for the Pacific Islands’. This CTA guide was intended to provide a simplified approach to value chain analysis and therefore make that approach useful to ‘farmers, traders and policy makers’. From 2014-2017, the Pacific Island Farmers Organisation Network (PIFON) implemented a programme to pilot farmer-orientated value chain training through its farmer organisation members. The CTA guide was used and the training sessions were supported through several externally funded projects, including the IFAD/SDC1 funded MTCP II project and the EU/SPC PAPP project. PIFON believes that farmer organisations have an important role to play in disseminating key information to their members and in helping to ‘extend the reach of government and aid agencies’. These VC training sessions targeted chain actors involved in the: 1. Value chain for spices in Vanuatu (through the Farm Support Association and Venui Vanilla) 2. Value chain for ginger in Fiji (through Fiji Crop and Livestock Council) 3. Value chain for papaya in Tonga (through Growers Federation of Tonga) 4. Value chain for Taro in Fiji (through Tei Tei Taveuni and the Tutu Rural Training Centre) 5. Value chain for papaya in Fiji (through Nature’s Way Cooperative) CTA is currently implementing a project entitled ‘Promoting Nutritious Food Systems in the Pacific Islands’ (2016-2020), in partnership with the International Fund for Agricultural Development (IFAD) and the Pacific Islands Private Sector Organisation (PIPSO). Under this project, CTA has commissioned PIFON to document the lessons learned about the value chain training, through production of a video and this publication. This report explores a range of impacts of the initial piloting of value chain training in the Pacific. These included: • The beginnings of a change in the mind-set of the actors in the value chain –particularly farmers • The incorporation of the value chain ‘way of thinking’ into normal extension activities of farmer organisations • Improvements in relationships and better collaboration • Increased supply of produce • Value chain training materials being translated/adapted/adopted into training programme

    Baseline patient and tumor characteristics differentially affected type of death in the MA.17 placebo-controlled letrozole trial where cardiovascular death was not separately identified. The MA.27 trial allowed competing risks analysis of breast cancer (BC), cardiovascular, and other type (OT) of death. MA.27 was a phase III adjuvant breast cancer trial of exemestane versus anastrozole. Effects of baseline patient and tumor characteristics were tested for whether factors were associated with (1) all cause mortality and (2) cause-specific mortality. We also fit step-wise forward cause-specific-adjusted models. 7576 women (median age 64 years; 5417 (72 %) < 70 years and 2159 (28 %) ≥ 70 years) were enrolled and followed for median 4.1 years. The 432 deaths comprised 187 (43 %) BC, 66 (15 %) cardiovascular, and 179 (41 %) OT. Five baseline factors were differentially associated with type of death. Older patients had greater BC (p = 0.03), cardiovascular (p < 0.001), and other types (p < 0.001) of mortality. Patients with pre-existing cardiovascular history had worse cardiovascular mortality (p < 0.001); those with worse ECOG performance status had worse OT mortality (p < 0.001). Patients with T1 tumors (p < 0.001) and progesterone receptor positive had less BC mortality (p < 0.001). Fewer BC deaths occurred with node-negative disease (p < 0.001), estrogen receptor-positive tumors (p = 0.001), and without adjuvant chemotherapy (p = 0.005); worse cardiovascular mortality (p = 0.01), with trastuzumab; worse OT mortality, for non-whites (p = 0.03) and without adjuvant radiotherapy (p = 0.003). Overall, 57 % of deaths in MA.27 AI-treated patients were non-breast cancer related.     “I’ve been doing this for years”: the COVID-19 pandemic and family caregiver isolation and loneliness

    BackgroundFamily caregivers are family members or friends of care recipients who assist with activities of daily living, medication management, transportation, and help with finances among other activities. As a result of their caregiving, family caregivers are often considered a population at risk of experiencing increased stress, isolation, and loneliness. During the COVID-19 pandemic in the US, social isolation and decrease in social activities were a top concern among older adults and their family caregivers. Using secondary analysis of survey data as part of a multi-site implementation trial of a caregiver skills training program, we describe differences in caregiver experiences of loneliness before and during the COVID-19 pandemic.MethodsHealth and wellbeing surveys of family caregivers were collected on 422 family caregivers of veterans before and during COVID-19. Logistic regression modeling examined whether the loneliness differed between caregiver groups pre vs during COVID-19, using the UCLA 3-item loneliness measure. Rapid directed qualitative content analysis of open-ended survey questions was used to explore the context of how survey responses were affected by the COVID-19 pandemic.ResultsThere were no significant differences in loneliness between caregivers pre vs during COVID-19. In open-ended responses regarding effects of COVID-19, caregivers described experiencing loneliness and social isolation; why they were unaffected by the pandemic; and how caregiving equipped them with coping strategies to manage negative pandemic-related effects.ConclusionLoneliness did not differ significantly between pre vs during COVID-19 caregivers. Future research could assess what specific characteristics are associated with caregivers who have resiliency, and identify caregivers who are more susceptible to experiencing loneliness. Understanding caregiver loneliness could assist other healthcare systems in developing and implementing caregiver support interventions

    Rapid feedback on hospital onset SARS-CoV-2 infections combining epidemiological and sequencing data.

    BACKGROUND: Rapid identification and investigation of healthcare-associated infections (HCAIs) is important for suppression of SARS-CoV-2, but the infection source for hospital onset COVID-19 infections (HOCIs) cannot always be readily identified based only on epidemiological data. Viral sequencing data provides additional information regarding potential transmission clusters, but the low mutation rate of SARS-CoV-2 can make interpretation using standard phylogenetic methods difficult. METHODS: We developed a novel statistical method and sequence reporting tool (SRT) that combines epidemiological and sequence data in order to provide a rapid assessment of the probability of HCAI among HOCI cases (defined as first positive test >48 hr following admission) and to identify infections that could plausibly constitute outbreak events. The method is designed for prospective use, but was validated using retrospective datasets from hospitals in Glasgow and Sheffield collected February-May 2020. RESULTS: We analysed data from 326 HOCIs. Among HOCIs with time from admission ≥8 days, the SRT algorithm identified close sequence matches from the same ward for 160/244 (65.6%) and in the remainder 68/84 (81.0%) had at least one similar sequence elsewhere in the hospital, resulting in high estimated probabilities of within-ward and within-hospital transmission. For HOCIs with time from admission 3-7 days, the SRT probability of healthcare acquisition was >0.5 in 33/82 (40.2%). CONCLUSIONS: The methodology developed can provide rapid feedback on HOCIs that could be useful for infection prevention and control teams, and warrants further prospective evaluation. The integration of epidemiological and sequence data is important given the low mutation rate of SARS-CoV-2 and its variable incubation period.     Non-Sexual Transmission of Trichomonas vaginalis in Adolescent Girls Attending School in Ndola, Zambia

    OBJECTIVES: To identify risk factors for trichomoniasis among young women in Ndola, Zambia. METHOD: The study was a cross-sectional study among adolescent girls aged 13-16 years in Ndola, Zambia. Study participants were recruited from schools in selected administrative areas that represented the different socio-economic strata in town. Consenting participants were interviewed about their socio-demographic characteristics; sexual behaviour; and hygiene practices. Self-administered vaginal swabs were tested for Trichomonas vaginalis. HSV-2 antibodies were determined on serum to validate the self-reported sexual activity. RESULTS: A total of 460 girls participated in the study. The overall prevalence of trichomoniasis was 27.1%, 33.9% among girls who reported that they had ever had sex and 24.7% among virgins. In multivariate analysis the only statistically significant risk factor for trichomoniasis was inconsistent use of soap. For the virgins, none of the risk factors was significantly associated with trichomoniasis, but the association with use of soap (not always versus always) and type of toilet used (pit latrine/bush versus flush toilet) was of borderline significance. CONCLUSION: We found a high prevalence of trichomoniasis in girls in Ndola who reported that they had never had sex. We postulate that the high prevalence of trichomoniasis in virgins in Ndola is due to non-sexual transmission of trichomoniasis via shared bathing water and inconsistent use of soap
