    Measuring Quality of Service in Retail Outlets Using Fuzzy Numbers

    The measurement of quality of service in any service providing organisation has been a very complex process to deal with. Most of the attributes here are intangible; largely depend upon customers’ expectations and perceptions. The quality of service may be poor, average, good, and so on depending upon the performance of the service provider. The linguistic terms like poor, average and good could be vague (fuzzy) unless various attributes of a particular service are understood precisely. In order to avoid the ambiguity of the concept associated with customers’ expectations and perceptions of service quality fuzzy numbers are widely used in many complex management issues. In this paper we focus on quantifying the quality of service in retail outlets based on some specific characteristics using fuzzy numbers.     Development of Railqual:A Service Quality Scale for Measuring Indian Railway Passenger

    The main objective of this study is to present a framework developed for assisting Railways to monitor and control the quality of services provided to passengers. The study evaluated the passenger Rail Service quality of Indian Railways by developing RAILQUAL instrument on the basis of SERVQUAL and Rail Transport quality. Three new transport dimensions (comfort, security and convenience) are added to the original five SERVQUAL dimensions (i.e. assurance, empathy, reliability, responsiveness and tangibles). The instrument is tested for reliability and validity. Empirical study was conducted at Secunderabad Railway station of South Central Railway, India, using a purposive sample of 100 respondents. Valid responses from the questionnaire are statistically analyzed by using factor analysis. This study identified the attributes to evaluate the quality of Railway Passenger Services and develops a comprehensive instrument “RAILQUAL”, which can be used by the Railways for collecting feedback from passengers. This study would help the Railways to monitor, control and improve the service. It can also be used as a tool for comparing the performance of various Railway Zones and Divisions across the country.Keywords: Railqual; Service Quality; Indian Railway

    Omecamtiv mecarbil in chronic heart failure with reduced ejection fraction, GALACTIC‐HF: baseline characteristics and comparison with contemporary clinical trials

    Aims: The safety and efficacy of the novel selective cardiac myosin activator, omecamtiv mecarbil, in patients with heart failure with reduced ejection fraction (HFrEF) is tested in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTIC‐HF) trial. Here we describe the baseline characteristics of participants in GALACTIC‐HF and how these compare with other contemporary trials. Methods and Results: Adults with established HFrEF, New York Heart Association functional class (NYHA) ≄ II, EF ≀35%, elevated natriuretic peptides and either current hospitalization for HF or history of hospitalization/ emergency department visit for HF within a year were randomized to either placebo or omecamtiv mecarbil (pharmacokinetic‐guided dosing: 25, 37.5 or 50 mg bid). 8256 patients [male (79%), non‐white (22%), mean age 65 years] were enrolled with a mean EF 27%, ischemic etiology in 54%, NYHA II 53% and III/IV 47%, and median NT‐proBNP 1971 pg/mL. HF therapies at baseline were among the most effectively employed in contemporary HF trials. GALACTIC‐HF randomized patients representative of recent HF registries and trials with substantial numbers of patients also having characteristics understudied in previous trials including more from North America (n = 1386), enrolled as inpatients (n = 2084), systolic blood pressure < 100 mmHg (n = 1127), estimated glomerular filtration rate < 30 mL/min/1.73 m2 (n = 528), and treated with sacubitril‐valsartan at baseline (n = 1594). Conclusions: GALACTIC‐HF enrolled a well‐treated, high‐risk population from both inpatient and outpatient settings, which will provide a definitive evaluation of the efficacy and safety of this novel therapy, as well as informing its potential future implementation

    The aim of the present work is to formulate fast dispersible tablets of lactic acid bacillus & Zinc sulphate. In this study, different formulations of fast dispersible tablets were prepared using various excipients & super disintegrants. Different concentrations of crospovidone, cross carmellose sodium & sodium starch glycolate were used as super disintegrants in the formulation of fast dissolution tablet. The powder mass was evaluated for flow properties. All these formulations were prepared by wet granulation method. The tablets were evaluated for hardness, thickness, weight variation, friability, wetting time, disintegration time, and water absorption ratio. Tablets containing crospovidone showed shorter disintegration time i.e. less than 30 sec, comparaed to cross carmellose sodium & sodium starch glycolate hence better patient compliance & effective therapy

    Urinary stones and intervention quality of life (USIQoL): development and validation of a new core universal patient-reported outcome measure for urinary calculi

    Background Urolithiasis has a significant impact on patients’ health-related quality of life (HRQoL). Objective To develop a core patient-reported outcome measure (PROM) using modern psychometric methods to quantify the impact of urolithiasis and different treatments. Design, setting, and participants Adult patients with urinary calculi, attending urology departments, covering all index categories and treatment spectrum, participated during different development phases. The pilot instrument was created from potential items (phases 1 and 2) within the conceptual framework. The instrument was pretested (phase 3) and then underwent psychometric evaluation in two parts (phases 4 and 5). Outcome measurements and statistical analysis The validity and reliability of the new PROM were assessed using Rasch measurement theory (RUMM 2030 statistical software) and traditional analyses. Results and limitations In total, 683 patients (median age 51 yr, range 18–92 yr) participated during different phases. The initial 60-item draft (five scales) was completed by 212 patients (phase 4). A revised 25-item draft was produced after removal of unstable items. In the second field test, the revised version was evaluated by 369 patients. This led to the final Urinary Stones and Intervention Quality of Life (USIQoL; 15 items) with summated logit scores. The PROM includes three scales: pain with physical health (six items), psychosocial health (seven items) and work performance (two items). Lower scores indicate better outcomes. Results demonstrate that USIQoL is reliable (r ≄ 0.8) and internally consistent (α ≄ 0.7), and has good construct validity (good hypothesised correlations, r > 0.3) and satisfactory sensitivity to change (p < 0.01). All scales demonstrated unidimensionality with good item fit and person separation indices. A limitation is that USIQoL was developed in the English language within the UK population. Conclusions USIQoL is a short, unidimensional, valid, and reliable PROM for assessing the HRQoL impact of urinary calculi and treatments. It is expected to serve as a core PROM across the entire spectrum of urolithiasis. Patient summary Kidney stones are a common condition for which various treatment options are available. The condition and treatments have a significant impact on a patient’s quality of life. This can be measured objectively using a valid and reliable patient-reported outcome measure (PROM) developed using modern methods. We have developed a PROM that provides helpful and accurate measurement useful for all stakeholders
