    Information in crisis : health & technology-related information behaviors of parents in emergency departments

    This research examined the information behavior of parents (including legal guardians) during an emergent health situation with a child. Although many studies examine health-related information behaviors, very few explore health consumers' information practices during moments of health crisis. This study explored parents' information needs, source use, and source preferences during a visit to the emergency department. An online questionnaire was administered using iPads given to 897 parents with children at one of 32 participating general emergency departments (ED) across Canada. Before taking their child to the ED, only 38.8% (n=348) of parents looked for information; those who did search looked most frequently for information about specific symptoms or severity of illness. Prior to visiting the ED the sources most frequently accessed were the internet and direct contact with healthcare professionals. At the emergency department, parents' information needs centred on immediate concerns, including explanations of their child's illness, treatments, and care instructions. Household income, education level, and parents' age were factors that affected information seeking in emergent health situations. Overwhelmingly, speaking to a healthcare professional in person is the typical and preferred way to obtain health information when facing a health crisis involving a child. The results have implications for how and when healthcare information is shared; the findings add to the limited research on parents' information behavior, particularly their roles as information proxies for their children

    Deaf writers’ application of ASL knowledge to English

    Language transfer theory elucidates how first language (L1) knowledge and grammatical features are applied in second language (L2) writing. Deaf and hard of hearing (d/hh) students who use or are developing American Sign Language (ASL) as their L1 may demonstrate use of ASL linguistic features in their writing of English. In this study, we investigated the extent to which 29 d/hh students in grades 6-8 (mean age = 13.2) with diverse ASL exposure incorporated ASL features in their English writing. We also investigated the impact of one year of Strategic and Interactive Writing Instruction (SIWI) to increase students’ metalinguistic knowledge and linguistic competence, and subsequently reduce ASL features in writing. Results indicate that ASL transfer is found in the writings of students with varied L1 experiences, and that SIWI can lead to significant reductions of ASL features in writing. The findings suggest that bilingual literacy programs where there is an emphasis on implicit language competence and metalinguistic knowledge can support d/hh students in the development of written English

    Identification of Dimethyldioctadecylammonium Ion (m/z 550.6) and Related Species (m/z 522.6, 494.6) as a Source of Contamination in Mass Spectrometry

    Chemical contamination can be one of the more common problems encountered when performing trace-level analysis regardless of the analytical technique. Minimizing or eliminating background interferences can be a difficult task, so knowledge of the chemical composition of these contaminants can prove invaluable when it comes to identifying the source. Once the source is identified, proper steps may be taken to reduce or eliminate it. In this study, we report the identity of some commonly seen contaminants (m/z 550.6, 522.6, and 494.6) in electrospray ionization (ESI) mass spectrometry (MS). Through MS, tandem MS, accurate-mass, and high-resolution measurements we have identified these background contaminants as being quaternary ammonium species that contain long-chain hydrocarbon groups, where m/z 550.6 is a dimethyldioctadecylammonium ion (C18, C18) and m/z 522.6 and 494.6 are similar in nature but have shorter alkyl-chain groups. The lipophilic nature of these compounds and the fact that they have molecular weights similar to lysophospholipids make them a frequent contaminant in lipidomic studies. The likely sources of these compounds are commonly used personal and household products

    Destabilization of The Ornithine Decarboxylase mRNA Transcript by the RNA-Binding Protein Tristetraprolin

    Ornithine decarboxylase (ODC) is the first and usually rate-limiting enzyme in the polyamine biosynthetic pathway. In a normal physiological state, ODC is tightly regulated. However, during neoplastic transformation, ODC expression becomes upregulated. The studies described here show that the ODC mRNA transcript is destabilized by the RNA-binding protein tristetraprolin (TTP). We show that TTP is able to bind to the ODC mRNA transcript in both non-transformed RIE-1 cells and transformed Ras12V cells. Moreover, using mouse embryonic fibroblast cell lines that are devoid of a functional TTP protein, we demonstrate that in the absence of TTP both ODC mRNA stability and ODC enzyme activity increase when compared to wild-type cells. Finally, we show that the ODC 3′ untranslated region contains cis acting destabilizing elements that are affected by, but not solely dependent on, TTP expression. Together, these data support the hypothesis that TTP plays a role in the post-transcriptional regulation of the ODC mRNA transcript

    Comprehensive analysis of T cell leukemia signals reveals heterogeneity in the PI3 kinase-Akt pathway and limitations of PI3 kinase inhibitors as monotherapy.

    T cell acute lymphoblastic leukemia (T-ALL) is an aggressive hematologic cancer. Poly-chemotherapy with cytotoxic and genotoxic drugs causes substantial toxicity and more specific therapies targeting the underlying molecular lesions are highly desired. Perturbed Ras signaling is prevalent in T-ALL and occurs via oncogenic RAS mutations or through overexpression of the Ras activator RasGRP1 in ~65% of T-ALL patients. Effective small molecule inhibitors for either target do not currently exist. Genetic and biochemical evidence link phosphoinositide 3-kinase (PI3K) signals to T-ALL, PI3Ks are activated by Ras-dependent and Ras-independent mechanisms, and potent PI3K inhibitors exist. Here we performed comprehensive analyses of PI3K-Akt signaling in T-ALL with a focus on class I PI3K. We developed a multiplex, multiparameter flow cytometry platform with pan- and isoform-specific PI3K inhibitors. We find that pan-PI3K and PI3K Îł-specific inhibitors effectively block basal and cytokine-induced PI3K-Akt signals. Despite such inhibition, GDC0941 (pan-PI3K) or AS-605240 (PI3KÎł-specific) as single agents did not efficiently induce death in T-ALL cell lines. Combination of GDC0941 with AS-605240, maximally targeting all p110 isoforms, exhibited potent synergistic activity for clonal T-ALL lines in vitro, which motivated us to perform preclinical trials in mice. In contrast to clonal T-ALL lines, we used a T-ALL cancer model that recapitulates the multi-step pathogenesis and inter- and intra-tumoral genetic heterogeneity, a hallmark of advanced human cancers. We found that the combination of GDC0941 with AS-605240 fails in such trials. Our results reveal that PI3K inhibitors are a promising avenue for molecular therapy in T-ALL, but predict the requirement for methods that can resolve biochemical signals in heterogeneous cell populations so that combination therapy can be designed in a rational manner

    Accountability by Design: Moving Primary Care Reform Ahead in Alberta

    Health-care reform is perennially popular in Alberta, but reality doesn’t match the rhetoric. Government has invested more than $700 million in Primary Care Networks — with little beyond anecdotal evidence of the value achieved with this investment. As the province redirects primary care to Family Care Clinics, the authors assert that simply tinkering with one part of the system is not the answer: health care must change on a system-wide basis. Drawing on the experiences of frontline staff and a rich body of literature, the authors present their vision for integrated team-based primary care, designed to be accountable to meet the needs of populations. This will require governance that makes primary care the hub of the system, and brings together government and health-services leadership to support the integration of primary and specialty care. There are shared accountabilities for achieving primary care that exhibits the attributes of high performing primary care systems, and these exist at multiple levels, from individuals seeking primary care, up to and including government. The authors make these accountabilities explicit, and outline strategies to secure their achievement that include system redesign, service delivery redesign and payment reform. All of this demands whole-system reform focused on primary care, and it won’t be easy. There are plenty of vested interests at stake, and a truly transformative vision requires buy-in at every level. However, Alberta’s rapidly growing and aging population makes it more urgent than ever to realize such a vision. This paper offers guidelines to spark the fresh thinking required

    Oyster toadfish (Opsanus tau) boatwhistle call detection and patterns within a large-scale oyster restoration site

    During May 2015, passive acoustic recorders were deployed at eight subtidal oyster reefs within Harris Creek Oyster Sanctuary in Chesapeake Bay, Maryland USA. These sites were selected to represent both restored and unrestored habitats having a range of oyster densities. Throughout the survey, the soundscape within Harris Creek was dominated by the boatwhistle calls of the oyster toadfish, Opsanus tau. A novel, multi-kernel spectral correlation approach was developed to automatically detect these boatwhistle calls using their two lowest harmonic bands. The results provided quantitative information on how call rate and call frequency varied in space and time. Toadfish boatwhistle fundamental frequency ranged from 140 Hz to 260 Hz and was well correlated (r = 0.94) with changes in water temperature, with the fundamental frequency increasing by similar to 11 Hz for every 1 degrees C increase in temperature. The boatwhistle call rate increased from just a few calls per minute at the start of monitoring on May 7 th to similar to 100 calls/min on May 10 th and remained elevated throughout the survey. As male toadfish are known to generate boatwhistles to attract mates, this rapid increase in call rate was interpreted to mark the onset of spring spawning behavior. Call rate was not modulated by water temperature, but showed a consistent diurnal pattern, with a sharp decrease in rate just before sunrise and a peak just after sunset. There was a significant difference in call rate between restored and unrestored reefs, with restored sites having nearly twice the call rate as unrestored sites. This work highlights the benefits of using automated detection techniques that provide quantitative information on species-specific call characteristics and patterns. This type of non-invasive acoustic monitoring provides longterm, semi-continuous information on animal behavior and abundance, and operates effectively in settings that are otherwise difficult to sample

    Effects of feeding mode on the performance, life span and greenhouse gas emissions of a vertical flow macrophyte assisted vermifilter

    This study was conducted to investigate the impact of intermittent feeding on performance, clogging, and gaseous emission on macrophyte assisted vermifiltration (MAVF) based treatment system. Synthetic slaughterhouse wastewater was applied to two different integrated vertical flow based MAVFs. Triplicates were used throughout the study. Eisenia fetida earthworms were added to MAVFs, and Carex muskingmenis plants were planted. Wastewater was applied to the reactors on 1) intermittent (8 h/day) (IMAVF) and 2) continuous (24 h/day) (CMAVF) basis. The average chemical oxygen demand, total nitrogen, and total phosphorous removals achieved by the IMAVF were 80.2 ± 1.6%, 53.9 ± 1.3% and 66.5 ± 1% respectively, and 68.3 ± 1.3%, 61.2 ± 1.4%, and 60.5 ± 1.4% by the CMAVF, respectively. The diffusion of air to the bedding of IMAVFs during no-flow conditions facilitated higher organics oxidation, adsorption of phosphorous, nitrification, and ammonification. At the end of the study, hydraulic conductivity of IMAVF and CMAVF were found to be 0.036 cm/s and 0.037 cm/s, respectively. CO2, CH4 and N2O emissions from IMAVF were 245.5 ± 38.0 mg C/m2, 5.0 ± 4.6 mg C/m2 and 2513.5 ± 2629.9 μg N/m2 respectively, while CO2, CH4 and N2O emissions from CMAVF were 123.3 ± 14.5 mg C/m2, 74.8±45.2 mg C/m2 and 328.4 ± 93.4 μg N/m2, respectively. Intermittent application of influent could be considered for improving the performance and lifespan of MAVFs, causing lower environmental footprints

    Analysis of Wastewater Samples to Explore Community Substance Use in the United States: Pilot Correlative and Machine Learning Study

    Background: Substance use disorder and associated deaths have increased in the United States, but methods for detecting and monitoring substance use using rapid and unbiased techniques are lacking. Wastewater-based surveillance is a cost-effective method for monitoring community drug use. However, the examination of the results often focuses on descriptive analysis. Objective: The objective of this study was to explore community substance use in the United States by analyzing wastewater samples. Geographic differences and commonalities of substance use were explored. Methods: Wastewater was sampled across the United States (n=12). Selected drugs with misuse potential, prescriptions, and over-the-counter drugs and their metabolites were tested across geographic locations for 7 days. Methods used included wastewater assessment of substances and metabolites paired with machine learning, specifically discriminant analysis and cluster analysis, to explore similarities and differences in wastewater measures. Results: Geographic variations in the wastewater drug or metabolite levels were found. Results revealed a higher use of methamphetamine (z=–2.27, P=.02) and opioids-to-methadone ratios (oxycodone-to-methadone: z=–1.95, P=.05; hydrocodone-to-methadone: z=–1.95, P=.05) in states west of the Mississippi River compared to the east. Discriminant analysis suggested temazepam and methadone were significant predictors of geographical locations. Precision, sensitivity, specificity, and F1-scores were 0.88, 1, 0.80, and 0.93, respectively. Finally, cluster analysis revealed similarities in substance use among communities. Conclusions: These findings suggest that wastewater-based surveillance has the potential to become an effective form of surveillance for substance use. Further, advanced analytical techniques may help uncover geographical patterns and detect communities with similar needs for resources to address substance use disorders. Using automated analytics, these advanced surveillance techniques may help communities develop timely, tailored treatment and prevention efforts
