    Short-Term Outcome of Discharged Low-Risk Chest Pain without Provoke Ischemia Study

    Introduction: Chest pain is a common problem in patients referring to emergency units. The present study was undertaken to evaluate the short-term outcome of patients presenting with a low risk chest pain and discharging without provoke ischemia study during emergency department admission. Methods: In the present prospective cohort study, patients with low-risk chest pain, referring to the emergency department of Imam Hossein Hospital, Tehran, during the first half of 2012, were evaluated. All the patients underwent electrocardiogram (ECG) and cardiac enzyme tests, including cardiac isoenzymes creatine kinase MB and troponin I. One week after referring to the emergency department, the patients underwent an exercise test and were followed for a month. Data were analyzed with chi-squared test at a significant level of P<0.05. Results: A total of 252 patients were included. The mean and standard deviation of patient ages was 56Ā±7.7 years (47.5% male). The results of exercise tests for 47 (26.3%) subjects were positive [32 (28.8%) patients in the 41-60 year age group and 15 (22.7%) over 60 years of age].The angiography examination results of 5 patients (2.8%) were abnormal. There were no significant relationships between the age and gender and the results of exercise test and angiography (P>0.05). During the one-month follow-up no cases of mortality, cardiac problems, or referring again to the hospital were recorded.Ā Conclusion: Based on the results of the present study, prevalence of cardiac etiology in patients with low risk chest pain was 2.8% and one-month follow-up did not reveal any complications or serious problems in such cases

    Study on the fouling behavior of HDPE/PE-g-MA/EVA blend membrane fabricated via thermally induced phase separation method

    In this study, neat HDPE and HDPE/PE-g-MA/EVA blend membranes were fabricated via thermally induced phase separation (TIPS) method and their fouling behaviors were examined using filtration of BSA protein. Membranes were characterized using FESEM, AFM, ATR-FTIR analyses and porosity measurement. Fouling behavior of membranes was analyzed using the resistance-in-series (RIS), classic and combined pore blocking models. The results of RIS model revealed that the magnitude of inherent, reversible and irreversible resistances decreased from 0.611 Ɨ1013m-1 and ,1.578 Ɨ1013m-1 and 0.525 Ɨ1013m-1 for the neat membrane to 0.237 Ɨ1013m-1, 0.789 Ɨ1013m-1 and 0.154Ɨ1013m-1 for the blend membrane, respectively. None of the classical Hermiaā€™s models were able to accurately predict fouling during the entire filtration run. The results obtained from the combined pore blocking model indicated that the combined cake formation-intermediate blocking model provided good prediction of fouling mechanism for both the membranes. However, comparison between fitted parameters showed that much greater fouling occurred for pure HDPE membrane. The key reasons for such different fouling behaviors were mainly attributed to the difference in hydrophobicity as well as the distribution of pore size on the surface of the pure and blend membranes

    Normal Values of Nerve Conduction Studies in Children Aged 7 Days to 14 Years Referred to Electrodiagnosis Clinic of Iranian Childrenā€™s Medical Center

    Background: The normal values of nerve conduction studies (NCS) are different in children compared to adults. Moreover, racial and geographical factors can affect these values. Objectives: The present study aimed to investigate the normal NCS values in children of different ages. Methods: The present cross-sectional study included children referred to the Electrodiagnosis Clinic of the Childrenā€™s Medical Center in Iran, who had normal NCS results based on the references and had no exclusion criteria. The patients were divided into 8 age groups (7 days to one month, 1 - 3 months, 3 - 6 months, 6 - 12 months, 1 - 2 years, 2 - 4 years, 4 - 6 years, and 6 - 14 years), and the NCS was performed using a Nihon Kohden electromyogram. The NCS values in the normal range were included in the analysis. Results: The normal ranges of amplitude and conduction velocity of 4 motor nerves (median, ulnar, deep peroneal, and tibial) and 2 sensory nerves (median and medial plantar), as well as the F-wave latency values of 2 nerves (ulnar and tibial), were established as the reference values for the pediatric patients. Conclusions: The NCS parameters of Iranian children were slightly different from the normal references reported by studies in other countries.     Representing a Model Using Data Mining Approach for Maximizing Profit with Considering Product Assortment and Space Allocation Decisions

    The choice of which products to stock among numerous competing products and how much space to allocate to those products are central decisions for retailers. This study aimed to apply data mining approach so that, we got needed information from large datasets of sale transactions to find the relations between products and to make product assortments. Thus, we represented a model for product assortment and space allocation. Research population was transactional data of a store, the sample included transactional data of one-month period in the time series. Data were collected in October and November, 2015 from Shaghayegh store. 525 transactions with regard to 79 different products were analyzed. Based on the result 10 product assortments formed although some products were allocated to more than 1 product category. By solving profit equation and finding volume increase indices we allocated spaces for each product assortment

    Background: Pentavalent antimonials are the first line drugs for the treatment of leishmaniasis. Unresponsiveness of Leishmania spp. to antimonial drugs is a serious problem in some endemic areas. Investigations on molecular mechanisms involved in drug resistance are essential for monitoring and managing of the disease. CalcineuĀ­rin is an essential protein phosphatase for number of signal transduction pathways in eukaryotic cells and it has a mediated role in apoptosis. This study aimed to determine of biomarker(s) in GlucantimeĀ® resiatance strain of L. infanĀ­tum.Methods: We used cDNA ampliļ¬ed fragment length polymorphism (cDNA-AFLP) and real time-RT PCR assays to compare gene expression profiles at the mRNA levels in resistant and susceptible L. infantum field isolates.Results: The cDNA-AFLP results showed downlegulation of calcineurin in resisĀ­tant isolate in comparison with susceptible one. Significant downregulation of calcineuĀ­rin (0.42 fold) (P<0.05) was found in resistant isolate compared to susceptiĀ­ble one by Real time-RT PCR.Conclusion: This is the first report of calcineurin implication in GlucantimeĀ® drug resistance of field (natural) isolate of L. infantum. Downregulation of calcineurin could protect parasites from antimonial-induced apoptosis

    Childhood Guillainā€“Barre syndrome in the SARSā€CoVā€2 era: Is there any causative relation?

    Abstract We reported an association between SARSā€CoVā€2 infection and Guillainā€“Barre syndrome (GBS). From 37 patients with GBS, previous SARSā€CoVā€2 clinical clues, including fever, cough, and diarrhea, were recorded in 18 patients. Among them, SARSā€CoVā€2 IgG was detected in seven patients, considered confirmed as cases. SARSā€CoVā€2 PCR was positive in just one patient. Although we found no increase in patient recruitment during the pandemic compared to previous years, our study indicated that SARSā€CoVā€2 is associated with poorer outcomes regarding GBS disability scale and hospital stay
