    Development of a dual optical fiber probe for the hydrodynamic investigation of a horizontal annular drive gas/liquid ejector

    A dual-channel optical fiber probe was developed to quantify the bubble characteristics (void fraction, velocity, and bubble size) in a gas–liquid annular ejector system. Water is pumped upstream of the ejector contraction. Since a low pressure region exists downstream in the ejector diffuser, this permits air to be sucked into the flowing liquid by jet pump action and the inlet air volumetric flow rate is measured by a flow meter. Verification of the void fraction (range 0.15–0.5) measured by the optical fiber probe was then possible and deviations were generally around ± 5%. Also, bubble velocity was measured using the optical probe by cross-correlating signals from the two fibers whose tips are separated by a known distance. Alternatively measuring bubble velocity using a particle image velocimetry method provided validation for the optical fiber probe system where a high speed camera was used to capture instantaneous bubble images at time intervals of 0.125 ms. Excellent agreement between the velocities using both methods is reported. For bubble size measurements, analyzing the temporal signals from a single probe enabled estimation of the size of a bubble. Bubble sizes measured ranged between 1.5 and 6.0 mm and size distributions were constructed for different ejector water volumetric flow rates ranging from 0.0022 to 0.0063 m3/s. LabVIEW provided a convenient platform for coding the algorithms for estimating the void fraction, bubble velocity and bubble size. For further comparison, a CFD study of the ejector system was done, and the vertical radial profiles of the void fraction were compared with those obtained by the optical fiber system and these showed good agreement

    High-Spatial Resolution Monitoring of Phycocyanin and Chlorophyll-a Using Airborne Hyperspectral Imagery

    Hyperspectral imagery (HSI) provides substantial information on optical features of water bodies that is usually applicable to water quality monitoring. However, it generates considerable uncertainties in assessments of spatial and temporal variation in water quality. Thus, this study explored the influence of different optical methods on the spatial distribution and concentration of phycocyanin (PC), chlorophyll-a (Chl-a), and total suspended solids (TSSs) and evaluated the dependence of algal distribution on flow velocity. Four ground-based and airborne monitoring campaigns were conducted to measure water surface reflectance. The actual concentrations of PC, Chl-a, and TSSs were also determined, while four bio-optical algorithms were calibrated to estimate the PC and Chl-a concentrations. Artificial neural network atmospheric correction achieved Nash-Sutcliffe Efficiency (NSE) values of 0.80 and 0.76 for the training and validation steps, respectively. Moderate resolution atmospheric transmission 6 (MODTRAN 6) showed an NSE value >0.8; whereas, atmospheric and topographic correction 4 (ATCOR 4) yielded a negative NSE value. The MODTRAN 6 correction led to the highest R-2 values and lowest root mean square error values for all algorithms in terms of PC and Chl-a. The PC:Chl-a distribution generated using HSI proved to be negatively dependent on flow velocity (p-value = 0.003) and successfully indicated cyanobacteria risk regions in the study area

    Rescue Endovascular Treatment to Prevent Neurological Deterioration in Acute Symptomatic Bilateral Vertebral Artery Occlusion

    Bilateral vertebral artery occlusive disease has been considered as a favorable condition with good collaterals. However, the prognosis of acute ischemic stroke secondary to symptomatic bilateral vertebral artery occlusion (BVAO) and endovascular treatment (EVT) has rarely been reported. We retrospectively selected patients with acute ischemic stroke admitted for symptomatic BVAO between January 2020 and February 2023. All patients with ischemic stroke were evaluated for ischemic lesion and arterial status using brain imaging and angiography. The prognosis of acute stroke with symptomatic BVAO was compared between EVT and conventional treatment. Outcomes were evaluated using modified Rankin Scale (mRS) score at 3 months follow-up. Within the study period, 17 of 2,655 acute ischemic stroke patients were diagnosed with ischemic stroke with symptomatic BVAO. The median age of these patients was 70 (interquartile range 44–89) years, and 13 (76%) were male. Seven patients received emergent EVT with stenting and 10 patients received conventional medical treatment only. Nine of 10 patients with conventional treatment had in-hospital stroke progression and developed new ischemic lesions in the pons and midbrain. Five patients with fetal and hypoplastic posterior communicating artery presented bilateral cerebral peduncular lesions. At 3 months follow-up, 6 patients (35%) had favorable outcomes (mRS 0–2), of which 5 were treated with vertebral artery stenting and 1 received conventional treatment. Ischemic stroke in patients with acute symptomatic BVAO is uncommon. However, stroke progression is common, and the prognosis of most patients is poor. Rescue management such as EVT might be considered for symptomatic BVAO

    Safety and optimal neuroprotection of neu2000 in acute ischemic stroke with reCanalization: study protocol for a randomized, double-blinded, placebo-controlled, phase-II trial

    BACKGROUND: The potential of neuroprotective agents should be revisited in the era of endovascular thrombectomy (EVT) for acute large-artery occlusion because their preclinical effects have been optimized for ischemia and reperfusion injury. Neu2000, a derivative of sulfasalazine, is a multi-target neuroprotectant. It selectively blocks N-methyl-D-aspartate receptors and scavenges for free radicals. This trial aimed to determine whether neuroprotectant administration before EVT is safe and leads to a more favorable outcome. METHODS: This trial is a phase-II, multicenter, three-arm, randomized, double-blinded, placebo-controlled, blinded-endpoint drug trial that enrolled participants aged ≥ 19 years undergoing an EVT attempt less than 8 h from symptom onset, with baseline National Institutes of Health Stroke Scale (NIHSS) score ≥ 8, Alberta Stroke Program Early CT score ≥ 6, evidence of large-artery occlusion, and at least moderate collaterals on computed tomography angiography. EVT-attempted patients are randomized into control, low-dose (2.75 g), and high-dose (5.25 g) Neu2000KWL over 5 days. Seventy participants per group are enrolled for 90% power, assuming that the treatment group has a 28.4% higher proportion of participants with functional independence than the placebo group. The primary outcome, based on intention-to-treat criteria is the improvement of modified Rankin Scale (mRS) scores at 3 months using a dichotomized model. Safety outcomes include symptomatic intracranial hemorrhage within 5 days. Secondary outcomes are distributional change of mRS, mean differences in NIHSS score, proportion of NIHSS score 0-2, and Barthel Index > 90 at 1 and 4 weeks, and 3 months. DISCUSSION: The trial results may provide information on new therapeutic options as multi-target neuroprotection might mitigate reperfusion injury in patients with acute ischemic stroke before EVT
