    Eine modifizierte viskosimetrische Methode wurde zur Bewertung des Reifegrades vom Delikatessenmais entwickelt. Die optimale Deformationsgeschwindigkeit des Testes, у = 48,6 s - 1 wurde bestimmt. Ein einfaches Flussmodell gemäss dem Potenzgesetzes wurde dabei angewendet. Die scheinbare Viskosität wies einen starken Zusammenhang mit den Parametern der Reife auf. Die Viskosität von Maisbreien wurde durch Gefrierenlassen erhöht. Gefrorener Mais kann durch seine scheinbare Viskosität und durch seine höchste Zurückströmungskraft vom Standpunkt seiner Qualität genügend gekennzeichnet werden. A modified viscometric method was developed for the evaluation of sweet corn maturity. The optimal deformation rate of the test, у = 48.6 s - 1 was determined. A simple power law flow model was applied. The apparent viscosity highly correlated with the maturity parameters. Freezing increased the viscosity of the corn slurries. For frozen corn the apparent viscosity and the Back Extrusion maximum force were satisfactory indexes of quality

    Comparison of the efficiency of Na+/Ca2+ exchanger or Na+/H+ exchanger inhibition and their combination in reducing coronary reperfusion-induced arrhythmias

    During ischaemia/reperfusion, the rise in [Na+]i, induced by simultaneous depression of the Na+/K+-ATPase and activation of the Na+/H+ exchanger (NHE), shifts the Na+/Ca2+ exchanger (NCX) into reverse transport mode, resulting in Ca2+ i overload, which is a critical factor in enhancing the liability to cardiac arrhythmias. The inhibition of NHE, and recently NCX has been suggested to effectively protect the heart from reperfusion-induced arrhythmias. In this study, we investigated and compared the efficacy of individual or the simultaneous inhibition of the NHE and NCX against reperfusion-induced arrhythmias in Langendorff-perfused rat hearts by applying a commonly used regional ischaemiareperfusion protocol. The NHE and NCX were inhibited by cariporide and SEA0400 or the novel, more selective ORM10103, respectively. Arrhythmia diagrams calculated for the reperfusion period were analysed for the incidence and duration of extrasystoles (ESs), ventricular tachycardia (VT) and ventricular fibrillation (VF). NHE inhibition by cariporide was highly efficient in reducing the recorded reperfusion-induced arrhythmias. Following the application of SEA0400 or ORM-10103, the number and duration of arrhythmic periods were efficiently or moderately decreased. While both NCX inhibitors effectively reduced ESs, the most frequently triggered arrhythmias, they exerted limited or no effect on VTs and VFs. Of the NCX inhibitors, ORM-10103 was more effective. Surprisingly, the simultaneous inhibition of the NCX and NHE failed to significantly improve the antiarrhythmic efficacy reached by NCX blockade alone. In conclusion, although principal simultaneous NHE+NCX inhibition should be highly effective against all types of the recorded reperfusion-induced arrhythmias, NCX inhibitors, alone or in combination with cariporide, seem to be moderately suitable to provide satisfactory cardioprotection - at least in the present arrhythmia model. Since ORM10103 and SEA0400 are known to effectively inhibit after-depolarisations, it is suggested that their efficacy and that of other NCX inhibitors may be higher and more pronounced in the predominantly Ca2+ i-dependent triggered arrhythmias. © 2015, Polish Physiological Society.     Introduction to the Pécs Model: Innovation in Teaching Medical History Taking

    [EN] Among clinical skills, doctor-patient communication is considered a high priority in establishing the proper diagnosis. Therefore, our team at the Department of Languages for Specific Purposes at the University of Pécs, Medical School (UPMS) has designed an educational model to improve these fundamental skills. Our primary objective is to share these innovative practices with others. Peer-tutoring history taking courses are offered to international students assisted byHungarian senior students, as well as digitally available educational materials. The latter is based on authentic doctor-patient conversations (video-recordings in Hungarian) while acquiring the patient’s history. Following video recording, EU standardized texts from A1 to B2 levels are implemented including task-based case studies and check-lists to ensure the use of authentic communication. The scene for pilot experimentation is a university course with blended-learning methods; the video-recorded conversations are used in class, followed by hospital-based practice to promote motivation and increase cultural awareness.     3D shape estimation in video sequences provides high precision evaluation of facial expressions

    Abstract Person independent and pose invariant estimation of facial expressions and action unit (AU) intensity estimation is important for situation analysis and for automated video annotation. We evaluated raw 2D shape data of the CK+ database, used Procrustes transformation and the multi-class SVM leave-one-out method for classification. We found close to 100% performance demonstrating the relevance and the strength of details of the shape. Precise 3D shape information was computed by means of Constrained Local Models (CLM) on video sequences. Such sequences offer the opportunity to compute a time-averaged '3D Personal Mean Shape' (PMS) from the estimated CLM shapes, which -upon subtraction -gives rise to person independent emotion estimation. On CK+ data PMS showed significant improvements over AU0 normalization; performance reached and sometimes surpassed state-ofthe-art results on emotion classification and on AU intensity estimation. 3D PMS from 3D CLM offers pose invariant emotion estimation that we studied by rendering a 3D emotional database for different poses and different subjects from the BU 4DFE database. Frontal shapes derived from CLM fits of the 3D shape were evaluated. Results demonstrate that shape estimation alone can be used for robust, high quality pose invariant emotion classification and AU intensity estimation

    Absztrakt: Bevezetés: A XXI. századi orvosképzés kihívásaira adott egyik válasz a szimulációs oktatás, amely hozzájárul az orvostanhallgatók klinikai készségeinek hatékony fejlesztéséhez, csökkentve a klinikusok és a kórházi betegek terheit. Célkitűzés: A Pécsi Tudományegyetem Általános Orvostudományi Karán létrehoztunk egy szimulációs oktatási programot a MediSkillsLab intézményén belül, ahol célul tűztük ki az orvostanhallgatók anamnézisfelvételi és szakmai kommunikációs készségeinek interdiszciplináris formában történő hatékony fejlesztését. Módszer: A kurzust a korábbi évek magyar orvosi szaknyelvi oktatása során szerzett tapasztalataink mellett nemzetközi példák felhasználásával hívtuk életre. Módszerül az Amerikában már az 1960-as években elterjedt Standardized Patient Programot választottuk. Eredmények: Az újdonság egyrészt a program hazai, elsőkénti bevezetésében, másrészt interdiszciplinárissá tételében rejlik. Az órákon szakorvosok, nyelvészek, színész-páciensek és orvostanhallgatók együttműködésével fejlődik a hallgatók szakmai, szaknyelvi és kommunikációs kompetenciája. Következtetés: A kurzust teljesítő diákokkal készített interjúk megerősítik a módszer létjogosultságát, amelynek eredményeként a betegekkel eredményesebben kommunikáló orvosok kezdhetik meg klinikai munkájukat. Orv Hetil. 2017; 158(26): 1022–1027. | Abstract: Introduction: Medical training in the 21st century faces simulation-based education as one of the challenges that efficiently contributes to clinical skills development while moderating the burden on the clinicians and patients alike. Aim: The University of Pécs, Medical School has launched a simulation program in the MediSkillsLab based on history taking with actors to improve patient interviewing communication skills. Method: This new program was inspired by experiences gathered in previous medical language teaching and integrates the method of the “Standardized Patient Program”. The method has been applied in America since the 1960s. Results: This is the first time the program has been introduced in Hungary and implemented in an interdisciplinary design, where medical specialists, linguists, actor-patients and medical students collaborate to improve professional, language and communicative competence of the students. Conclusion: A course like this has its pivotal role in the medical training, and as a result more efficient and patient-oriented communication may take place at the clinical setting.     Hemizygous nonsense variant in the moesin gene (MSN) leads to a new autoimmune phenotype of Immunodeficiency 50

    Here we describe the investigation of two male siblings with juvenile total loss of teeth, early onset chronic leg ulcers and autoimmune thyroiditis in both patients, as well as focal segmental glomerulosclerosis with associated pulmonal emphysema in one and diabetes mellitus in the other patient. The clinical picture and lupus anticoagulant, cryoglobulin and cold agglutinin positivity suggested the diagnosis of antiphospholipid syndrome. Flow cytometry analysis showed immunophenotypes consistent with immune dysregulation: low number of naive T cells, elevated CD4+ T cell counts and decreased CD8+ T cell counts were detected, and more than half of the T helper population was activated. Because of the siblings’ almost identical clinical phenotype genetic alteration was suspected in the background of the immunodeficiency. Whole exome sequencing identified a previously not described hemizygous nonsense variant (c.650G>A, p.W217X) within exon 6 of the moesin gene (MSN) localized on chromosome X, resulting in significantly decreased MSN mRNA expression compared to healthy controls. We present a putative new autoimmune phenotype of Immunodeficiency 50 (MIM300988) characterized by antiphospholipid syndrome, Hashimoto’s thyroiditis, leg ulcer and juvenile loss of teeth, associated with W217X mutation of the MSN gene

    We report the case of a 46-year-old female patient with recurrent rhabdomyolysis. In the background of her metabolic myopathy an inherited metabolic disorder of the fatty acid oxidation, very long-chain acyl-coenzyme A-dehydrogenase deficiency was diagnosed. The diagnosis was based on abnormal acyl-carnitine- and urine organic-acid profile in addition to low residual enzyme activity, and was confirmed by genetic testing. After introduction of dietotherapy metabolic crisis necessitating hospital admission has not occurred neither have fixed myopathic changes developed. We present here the differential diagnosis of rhabdomyolysis and exertional muscle complaints, with the metabolic myopathies in focus. The main features of fatty acid oxidation disorders are highlighted, acute and chronic managements of very long-chain acyl-coenzyme A-dehydrogenase deficiency are discussed. Metabolic myopathies respond well to treatment, so good quality of life can be achieved. However, especially in fatty acid oxidation disorders, a metabolic crisis may develop quickly and can be fatal, albeit rarely. Some of these disorders can be identified by newborn screening, but occasionally the symptoms may manifest only in adulthood. With the presentation of this case we would like to point out that in the differential diagnosis of recurrent rhabdomyolysis inherited metabolic disorders should be considered regardless of the patient's age.     Heat therapy shows benefit in patients with type 2 diabetes mellitus : a systematic review and meta-analysis

    Type-2 diabetes mellitus (T2DM) is a common health condition which prevalence increases with age. Besides lifestyle modifications, passive heating could be a promising intervention to improve glycemic control. This study aimed to assess the efficacy of passive heat therapy on glycemic and cardiovascular parameters, and body weight among patients with T2DM.A systematic review and meta-analysis were reported according to PRISMA Statement. We conducted a systematic search in three databases (MEDLINE, Embase, CENTRAL) from inception to 19 August 2021. We included interventional studies reporting on T2DM patients treated with heat therapy. The main outcomes were the changes in pre-and post-treatment cardiometabolic parameters (fasting plasma glucose, glycated plasma hemoglobin, and triglyceride). For these continuous variables, weighted mean differences (WMD) with 95% confidence intervals (CIs) were calculated. Study protocol number: CRD42020221500.Five studies were included in the qualitative and quantitative synthesis, respectively. The results showed a not significant difference in the hemoglobin A1c [WMD -0.549%, 95% CI (-1.262, 0.164), p = 0.131], fasting glucose [WMD -0.290 mmol/l, 95% CI (-0.903, 0.324), p = 0.355]. Triglyceride [WMD 0.035 mmol/l, 95% CI (-0.130, 0.200), p = 0.677] levels were comparable regarding the pre-, and post intervention values.Passive heating can be beneficial for patients with T2DM since the slight improvement in certain cardiometabolic parameters support that. However, further randomized controlled trials with longer intervention and follow-up periods are needed to confirm the beneficial effect of passive heat therapy
