A new crystalline form of αβ-D-lactose prepared by oven drying a concentrated aqueous solution of D-lactose
A new crystalline form of αβ-D-lactose (C12H22O11, Z' = 2) has been prepared by rapid drying of an approximately 40 % w/v syrup of D-lactose. Initially identified from its novel powder X-ray diffraction pattern, the monoclinic crystal structure was solved from a microcrystal recovered from the generally polycrystalline, mixed-phase residue obtained at the end of the drying step. This is the second crystalline form of αβ-D-lactose to be identified and it has a high degree of structural 3D similarity to the previously identified triclinic form
A non-destructive method for fixing placards with masonry structures
The practise of fixing placards to masonry structures using steel nails – a method widely used in Malta – is destructive and causes irreversible damage to the building fabric, as well as having indisputable aesthetic impacts, especially problematic with culturally significant heritage buildings. With a superficial area of circa 316 km2 , Malta and its dependencies represent the smallest EU Member State and, at the same time, the nation state with the highest density of designated World Heritage sites. Its capital, Valletta, was also listed by UNESCO in its entirety as a World Heritage Site in 1980 [1]. Can adherence to dietary guidelines address excess caloric intake? An empirical assessment for the UK
The facilitation of healthier dietary choices by consumers is a key element of government strategies to combat the rising incidence of obesity in developed and developing countries. Public health campaigns to promote healthier eating often target compliance with recommended dietary guidelines for consumption of individual nutrients such as fats and added sugars. This paper examines the association between improved compliance with dietary guidelines for individual nutrients and excess calorie intake, the most proximate determinant of obesity risk. We apply quantile regressions and counterfactual decompositions to cross-sectional data from the National Diet and Nutrition Survey (2000-01) to assess how excess calorie consumption patterns in the UK are likely to change with improved compliance with dietary guidelines. We find that the effects of compliance vary significantly across different quantiles of calorie consumption. Our results show that compliance with dietary guidelines for individual nutrients, even if successfully achieved, is likely to be associated with only modest shifts in excess calorie consumption patterns. Consequently, public health campaigns that target compliance with dietary guidelines for specific nutrients in isolation are unlikely to have a significant effect on the obesity risk faced by the population
Confronting the issues of patient safety and investigator conflict of interest in an international clinical trial of myocardial reperfusion
The Global Utilization of Streptokinase and Tissue Plasminogen Activator for Occluded Coronary Arteries (GUSTO) trial is a large scale international trial of new myocardial reperfusion strategies. The primary hypothesis is that early and sustained coronary artery recanalization will be associated with a significant reduction in mortality. The four regimens that are being tested are 1) streptokinase with subcutaneous heparin; 2) streptokinase with intravenous heparin; 3) accelerated recombinant tissue-type plasminogen activator (rt-PA) with intravenous heparin; and 4) combination streptokinase, rt-PA and intravenous heparin. The planned recruitment of 41,600 patients in 1,500 sites from 15 countries is expected to be completed by December 1992 and will enable detection of a 15% reduction or 1% absolute difference in mortality compared with that associated with standard therapy (streptokinase and subcutaneous heparin). In designing the trial, two important issues were directly addressed. First, a strategy was developed to provide assurance of patient safety during large scale investigational use of an aggressive thrombolytic regimen. This includes fascimile transmission of a one-page safety summary form to the Data Coordinating Center within 24 h of death or discharge, acceptance of the concept of "net clinical benefit" and close surveillance of the trial's progress by the independent Data and Safety Monitoring Committee.
