Aortic aneurysm and aortic graft infection related to Mycobacterium bovis after intravesical Bacille CalmetteâGuĂ©rin therapyâa case series
Background: So called "mycotic" aortic aneurysms account for only 0.7 to 1.3% of all aortic aneurysms and are commonly caused by Staphylococcus aureus and Salmonella species. Bacillus Calmette-Guerin (BCG), a live attenuated strain of Mycobacterium bovis, is part of the therapy of non-muscle-invasive bladder cancer (NMIBC).
Case presentation: We report a case series of three patients with a mycobacterial graft infection related to BCG after surgical treatment of a presumed mycotic aortic aneurysm as an extremely rare complication after NMIBC treatment. All three patients developed aortic aneurysm after BCG instillation and subsequent mycobacterial graft infection.
Conclusion: Diagnosis requires a high degree of suspicion because of its nonspecific symptoms and imaging. The pathogen is not detected by standard microbiological testing. Treatment includes triple antimycobacterial therapy and radical surgical interventions. Graft preservation may be considered if no anastomosis is involved

Superior virologic and treatment outcomes when viral load is measured at 3 months compared to 6 months on antiretroviral therapy.
INTRODUCTION: Routine viral load (VL) monitoring is utilized to assess antiretroviral therapy (ART) adherence and virologic failure, and it is currently scaled-up in many resource-constrained settings. The first routine VL is recommended as late as six months after ART initiation for early detection of sub-optimal adherence. We aimed to assess the optimal timing of first VL measurement after initiation of ART. METHODS: This was a retrospective, cohort analysis of routine monitoring data of adults enrolled at three primary care clinics in Khayelitsha, Cape Town, between January 2002 and March 2009. Primary outcomes were virologic failure and switch to second-line ART comparing patients in whom first VL done was at three months (VL3M) and six months (VL6M) after ART initiation. Adjusted hazard ratios (aHR) were estimated using Cox proportional hazard models. RESULTS: In total, 6264 patients were included for the time to virologic failure and 6269 for the time to switch to second-line ART analysis. Patients in the VL3M group had a 22% risk reduction of virologic failure (aHR 0.78, 95% CI 0.64-0.95; p=0.016) and a 27% risk reduction of switch to second-line ART (aHR 0.73, 95% CI 0.58-0.92; p=0.008) when compared to patients in the VL6M group. For each additional month of delay of the first VL measurement (up to nine months), the risk of virologic failure increased by 9% (aHR 1.09, 95% CI 1.02-1.15; p=0.008) and switch to second-line ART by 13% (aHR 1.13, 95% CI 1.05-1.21; p<0.001). CONCLUSIONS: A first VL at three months rather than six months with targeted adherence interventions for patients with high VL may improve long-term virologic suppression and reduce switches to costly second-line ART. ART programmes should consider the first VL measurement at three months after ART initiation
Prospective multi center trial for clinical and radiological outcome after anterior cervical fusion with PEEK-cages
Die anteriore zervikale Diskektomie und anschlieĂende Fusion (ACDF) ist eine
Standardoperation zur Behandlung degenerativer HalswirbelsÀulenerkrankungen.
Im Lauf der Zeit wurden verschiedene Materialien (z.B. autologer Knochenspan,
PMMA, Carbon und Titan) als Fusionsmittel untersucht. Neuere Studie zeigten
sehr gute klinische Resultate fĂŒr die Verwendung von Polyetheretherketon
(PEEK)-Cages. Diese scheinen aufgrund der Materialeigenschaften von PEEK
besonders gut fĂŒr die interkorporale Fusion geeignet zu sein. In dieser
prospektiven multizentrischen Studie wurden daher sowohl das Fusionsverhalten
und die Sinterungsraten, als auch das klinische Outcome nach zervikaler
Diskektomie und Fusion mittels PEEK-Cage untersucht. Von 2006 bis 2010 wurden
insgesamt 292 Patienten mit einer degenerativen HalswirbelsÀulenerkrankung
mittels ACDF behandelt. Alle Patienten erhielten einen ungefĂŒllten PEEK-Cage
und wurden im Rahmen des Follow-up nach 6, 12 und 18 Monaten untersucht. Dabei
wurde das radiologische Ergebnis anhand des Fusionsgrades und der Sinterung
ermittelt. Das klinische Ergebnis wurde mit Hilfe der Visuellen Analogskala
(VAS), der Denis Pain Scale (DPS), des Neck Disability Index (NDI) und der
Odoms Kriterien analysiert. Nach 18 Monaten fand sich bei 82,5% der Patienten
eine funktionelle Fusion. Die mittlere Sinterung nach 18 Monaten betrug 1,70
mm. Hohe Fusionsgrade korrelierte dabei mit niedrigen Sinterungsraten (rs=
-0,336, p<0,05). Die mittleren VAS-Werte konnten von prÀoperativ 5,89 Punkten
auf 1,45 Punkte nach 18 Monaten gesenkt werden. Analog dazu wurden die
mittleren DPS-Werte von prÀoperativ 4,5 Punkten auf 1,79 Punkte nach 18
Monaten gesenkt. Die FunktionseinschrÀnkung gemessen mittels NDI wurde von
prÀoperativ 18,75 auf 7,02 Punkte nach 18 Monaten reduziert. Anhand der Odoms
Kriterien hatten 89,4 % der Patienten ein gutes bzw. exzellentes
Behandlungsergebnis. Es fanden sich keine signifikanten ZusammenhÀnge zwischen
Alter, Geschlecht und operiertem WirbelsÀulensegment und dem klinischen und
radiologischen Outcome. Polyetheretherketon eignet sich aufgrund seiner hohen
BiokompatibilÀt bei niedrigen Sinterungsraten als interkorporales
Fusionsmittel. Im Vergleich zu autologem Knochen oder Metallimplantaten
scheint jedoch eine verlangsamte Fusionsgeschwindigkeit bzw. ein potentiell
anderes Einwachsverhalten vorzuliegen. Bei allen Patienten konnten die
prÀoperativen Schmerzen und die FunktionseinschrÀnkungen signifikant gesenkt
werden. Ein deutlicher Vorteil gegenĂŒber anderen Fusionsmitteln konnte jedoch
nicht gefunden werden. Der Einfluss des Materials auf die klinischen und
radiologischen Ergebnisse ist weiterhin unklar. Um den Einfluss des Materials
besser bewerten zu können wĂ€ren nun Studien mit Cages gleicher GröĂe und
Struktur aus verschiedenen Materialien, ggf. auch im Tiermodell unter
histopathologischer Kotrolle wĂŒnschenswert.Anterior cervical discectomy and fusion (ACDF) is a standard surgical
procedure for degenerative spine disease. Various materials (autologous bone,
PMMA,carbon, titan) for the intercorporal fusion have been reviewed. Current
studies showed good clinical results fort he use of Polyetheretherketone
(PEEK)-Cages because oft he excellent material properties. In this prospective
multicenter trial fusion and subsidence rates as well as clinical outcome
after ACDF with PEEK-Cages have been investigated. The wanted to show high
fusion rates with low subsidence rates by using PEEK-Cages. Between 2006 and
2010 292 patients underwent ACDF for degenerative spine disease. All patients
got a non-filled PEEK-Cage. The follow-up after 6, 12 and 18 months included
X-ray and clinical evaluation with visual analog scale (VAS), Denis pain scale
(DPS) and Neck Disability Index (NDI). After 18 month the treatment result was
examined with odomâs criteria. After 18 months in 82,5% oft the patients a
solid fusion was found. The middle subsidence after 18 months was 1.70 mm.
Higher fusion scores correlated with lower sibsidence rates (rs= -0,336,
p<0,05). Postoperative pain measured by VAS could be reduced from 5.89 before
operation to 1.45 after 18 months. The points for DPS could be also reduced
from 4.5 before operation to 1.79 after 18 months. The functional limitation
measured by NDI decreased from 18.75 before operation to 7.02 after 18 months.
Based on odomâs criteria 89,4 % oft he patients had a good or excellent
clinical outcome. There were no significant correlations between age, sex or
operated spine segment and clinical or radiological outcome.
Polyetheretherketone is a good fusion medium because of high biocompatibility,
good fusion score and low subsidence rates. Compared to autologeous bone or
metal implants PEEK seems to have a lower fusion speed. In all patients the
preoperativ pain and functional limitation could be reduced significantly. A
noticeable advantage over other fusion mediums couldnât be found. The
influence oft the material on clinical and radiological outcome is unclear. To
investigate the influence oft the material further studies with cages oft he
same size and structure but different materials for example in an animal
experiment with histopathological control are necessary
Identifizierung und Verifizierung eines Damage Tolerance Kriteriums fĂŒr Faserverbunde
Die vorliegende Arbeit befasst sich mit der Entwicklung einer Damage Tolerance-Methode fĂŒr Faser-Kunststoff-Verbunde im Rahmen des Gesamtentwurfs eines Flugzeugs. Damage Tolerance Methoden berĂŒcksichtigen, dass SchĂ€den wĂ€hrend des Betriebs eines Flugzeugs kaum zu vermeiden sind und gewĂ€hrleisten den erforderlichen sicheren Betrieb einer Flugzeugstruktur, indem SchĂ€den und ihr Ausbreitungsverhalten im Auslegungsprozess berĂŒcksichtigt und im Betrieb ĂŒberwacht werden. Solche Damage Tolerance-Methoden haben sich fĂŒr metallische Strukturen etabliert, können jedoch nicht genauso ĂŒbernommen werden, da Faser-Kunststoff-Verbunde ein anderes SchĂ€digungsverhalten mit unterschiedlichen SchĂ€digungsmodi aufweisen.
Im Rahmen dieser Arbeit wird das Damage Tolerance-Kriterium fĂŒr eine Delamination unter zyklischer Drucklast fĂŒr zwei verschiedene Bauteilkonfigurationen entwickelt. Dabei handelt es sich zum einen um an CAI-Versuchsproben (Compression After Impact) angelehnte Laminate und zum anderen um ein mit Stringern versteiftes Paneel.
Es erfolgt die Entwicklung einer analytischen Methode, die das Schadenswachstum der Delamination beschreibt. Dazu werden energetische Betrachtungen durchgefĂŒhrt, die zu richtungsabhĂ€ngigen Energiefreisetzungs- und Rissfortschrittsraten fĂŒhren. AnschlieĂend wird versucht, die analytische Methode mittels eines Vergleichs der analytischen Ergebnisse mit den Ergebnissen einer FE-Simulation zu verifizieren. AbschlieĂend erfolgt die Definition einer Analysekette nach der Luftfahrtrichtlinie AC 20-107B, in der alle erforderlichen Schritte zur Damage Tolerance-Auslegung der betrachteten delaminationsbehafteten Faserverbundstruktur unter Druckbelastung festgelegt werden
Empty polyetheretherketone (PEEK) cages in anterior cervical diskectomy and fusion (ACDF) show slow radiographic fusion that reduces clinical improvement: results from the prospective multicenter âPIERCE-PEEKâ study
Abstract Background Anterior cervical diskectomy and fusion (ACDF) is a well-established surgical treatment for radiculopathy and myelopathy. Previous studies showed that empty PEEK cages have lower radiographic fusion rates, but the clinical relevance remains unclear. This paperâs aim is to provide high-quality evidence on the outcomes of ACDF with empty PEEK cages and on the relevance of radiographic fusion for clinical outcomes. Methods This large prospective multicenter clinical trial performed single-level ACDF with empty PEEK cages on patients with cervical radiculopathy or myelopathy. The main clinical outcomes were VAS (0â10) for pain and NDI (0â100) for functioning. Radiographic fusion was evaluated by two investigators for three different aspects. Results The median (range) improvement of the VAS pain score was: 3 (1â6) at 6Â months, 3 (2â8) at 12Â months, and 4 (2â8) at 18Â months. The median (range) improvement of the NDI score was: 12 (2â34) at 6Â months, 18 (4â46) at 12Â months, and 22 (2â44) at 18Â months. Complete radiographic fusion was reached by 126 patients (43%) at 6Â months, 214 patients (73%) at 12Â months, and 241 patients (83%) at 18Â months. Radiographic fusion was a highly significant (pâ<â0.001) predictor of the improvement of VAS and NDI scores. Conclusion This study provides strong evidence that ACDF is effective treatment, but the overall rate of radiographic fusion with empty PEEK cages is slow and insufficient. Lack of complete radiographic fusion leads to less improvement of pain and disability. We recommend against using empty uncoated pure PEEK cages in ACDF. Trial registration ISRCTN42774128 .
