    ECoG-based short-range recurrent stimulation techniques to stabilize tissue at risk of progressive damage: Theory based on clinical observations

    We introduce theoretical concepts based on chaos control to stabilize in acute stroke the tissue at risk of progressive damage by preventing adverse effects of waves of mass neuronal depolarization. Moreover, we present clinical electrocorticography (ECoG) recordings of relevant signals suggested for the feedback control. The recordings are performed in combination with novel subdural opto-electrode technology for simultaneous laser-Doppler flowmetry in patients with aneurysmal subarachnoid haemorrhage (aSAH). In aSAH patients waves of spreading depolarization (SD) have a high incidence and cause hypoxia in tissue at risk, and, importantly, the haemodynamic response is the inverse of that seen in healthy tissue. In previous clinical studies, clusters of prolonged SDs have been measured in aSAH patients in close proximity to structural brain damage as assessed by neuroimaging, and, in theoretical studies, a mechanism was presented, suggesting how a failure of internal feedback could be a putative mechanism of such SD cluster patterns in acute stroke. 

This failing internal feedback control is now suggested to be replaced by ECoG-based short-range recurrent functional stimulation that initiates the normal hyperperfusion haemodynamic response in a demand-controlled way and stabilizes the tissue at risk during the critical phase of SD passage. The suggested method has three key features: (i) it is short-range, i.e., in the order of the distance of the ECoG electrode strip, (ii) it is demand-controlled, and (iii) it uses no prior knowledge of the target state, in particular, it adapts to conditions in the healthy physiological range. On-demand type stimulation provides minimal invasive feedback as the control force is off when the target state is reached, i.e., the tissue at risk is without SD or it is back to the physiological range (out of risk). These last two features (ii-iii) are shared with classical methods of chaos control, where major progress was made in the last years with respect to extensions for spatio-temporal wave patterns. A detailed bifurcation analysis of the nonlinear model is presented, in particular, the SD cluster forming cortical state is suggested to be caused by a delay-induced saddle-node bifurcation.

    Signet-Ring Cell Carcinoma Arising in the Gastric Stump After Duodenopancreatectomy for Ductal Adenocarcinoma of the Pancreas: A Case Report

    The development of malignancy in the gastric stump following surgery for peptic ulcer disease is well recognized. There are also few reports on carcinomas occurring after surgery for malignant gastric disease. However, carcinoma of the gastric stump after duodenopancreatectomy is extremely rare. We describe what we believe to be an unusual case of signet-ring cell carcinoma of the gastric stump developing at the anastomotic site 5 years after duodenopancreatectomy for ductal adenocarcinoma of the pancreatic head. We performed remnant gastrectomy and Roux-en-Y gastrojejunostomy as a curative resection. This experience clearly underlies that g astric stump carcinoma (GSC) may mimic metastatic disease recurrence leading to diagnostic confusion after surgery for malignancy. Although an increased risk of gastric stump carcinoma after pancreatoduodenectomy for pancreatic cancer has not been established, the possibility of such a complication should be kept in mind when evaluating patients after gastric resection who present with symptoms of metastatic disease recurrence years after the primary operation. Investigations should be independent of the entity of the primary disease or its localization, since GSC may well be amenable to surgical cure as demonstrated in the presented case. Outpatient follow up results of the last four years indicated no recurrence in this case

    A Comparison of Simple Mass Estimators for Galaxy Clusters

    High-resolution N-body simulations are used to investigate systematic trends in the mass profiles and total masses of clusters as derived from 3 simple estimators: (1) the weak gravitational lensing shear field under the assumption of an isothermal cluster potential, (2) the dynamical mass obtained from the measured velocity dispersion under the assumption of an isothermal cluster potential, and (3) the classical virial estimator. The clusters consist of order 2.5e+05 particles of mass m_p \simeq 10^{10} \Msun, have triaxial mass distributions, and significant substructure exists within their virial radii. Not surprisingly, the level of agreement between the mass profiles obtained from the various estimators and the actual mass profiles is found to be scale-dependent. The virial estimator yields a good measurement of the total cluster mass, though it is systematically underestimated by of order 10%. This result suggests that, at least in the limit of ideal data, the virial estimator is quite robust to deviations from pure spherical symmetry and the presence of substructure. The dynamical mass estimate based upon a measurement of the cluster velocity dispersion and an assumption of an isothermal potential yields a poor measurement of the total mass. The weak lensing estimate yields a very good measurement of the total mass, provided the mean shear used to determine the equivalent cluster velocity dispersion is computed from an average of the lensing signal over the entire cluster (i.e. the mean shear is computed interior to the virial radius). Как показывает действительность и многочисленные исследования - естественная связь человека с природой значительно ослабевает именно в городах. Это в значительной мере оказывает влияние на психоэмоциональное и физическое состояние горожан. Изучая визуальную среду современного города, мы можем существенным образом обогатить эстетическую составляющую и несколько смягчить негативные последствия урбанизации.The rapid growth of the urban population presents a global challenge and ultimately augments the urban environment. In most cities, the natural connection of individuals with nature has significantly weakened. Their everyday environment is an artificial urban environment. In cities a special lifestyle different from the style of life in the countryside is formed, affecting to a large extent the physical and especially the mental health of the citizens. Through exploring the visual environment of the modern city, we can significantly enrich the aesthetic component and mitigate the negative effects of anthropogenic impact, creating a favorable habitat

    Over-the-scope clip (OTSC (R)) closure of a recto-acetabular fistula

    A 25-year-old male Syrian refugee presented in our hospital with recurrent hip infections after having undergone hip arthroplasty abroad following destruction of his right hip joint by shell splinters in the Syrian civil war. The patient underwent hip arthroplasty revision with implantation of a cement spacer. CT-scan with rectal contrast media filling revealed a rectoacetabular fistula. Consecutively, the patient underwent ileostomy formation. The fistula was then successfully closed by endoscopic over-the-scope clipping (OTSC (R)). Fistulas between intestines and joints rarely develop and in the few cases published mostly extensive abdominal rescue surgery has been performed. Here, we present a case of a traumatic rectoacetabular fistula that was successfully closed by OTSC. This innovative method could represent a safe and suitable option to effectively close fistulas between joints and intestines thereby avoiding extensive rescue surgery with bowel resection or permanent ostomy

    EasyCloneMulti: A Set of Vectors for Simultaneous and Multiple Genomic Integrations in <i>Saccharomyces cerevisiae</i>

    Saccharomyces cerevisiae is widely used in the biotechnology industry for production of ethanol, recombinant proteins, food ingredients and other chemicals. In order to generate highly producing and stable strains, genome integration of genes encoding metabolic pathway enzymes is the preferred option. However, integration of pathway genes in single or few copies, especially those encoding rate-controlling steps, is often not sufficient to sustain high metabolic fluxes. By exploiting the sequence diversity in the long terminal repeats (LTR) of Ty retrotransposons, we developed a new set of integrative vectors, EasyCloneMulti, that enables multiple and simultaneous integration of genes in S. cerevisiae. By creating vector backbones that combine consensus sequences that aim at targeting subsets of Ty sequences and a quickly degrading selective marker, integrations at multiple genomic loci and a range of expression levels were obtained, as assessed with the green fluorescent protein (GFP) reporter system. The EasyCloneMulti vector set was applied to balance the expression of the rate-controlling step in the β-alanine pathway for biosynthesis of 3-hydroxypropionic acid (3HP). The best 3HP producing clone, with 5.45 g.L(-1) of 3HP, produced 11 times more 3HP than the lowest producing clone, which demonstrates the capability of EasyCloneMulti vectors to impact metabolic pathway enzyme activity

    Third harmonic ICRF heating of Deuterium beam ions on ASDEX Upgrade

    We report on recent experiments on the ASDEX Upgrade (AUG) tokamak (major radius R ≈1.65 m, minor radius a ≈ 0.5 m) with third harmonic ICRF heating of deuterium beam ions. Prior to this work, the scheme has been developed and applied on the JET tokamak, the largest currently operating tokamak (R ≈ 3 m, a ≈ 1 m), for fusion product studies and for testing alpha particle diagnostics in preparation of ITER [1].     Evidence of Υ(1S)J/ψ+χc1\Upsilon(1S) \to J/\psi+\chi_{c1} and search for double-charmonium production in Υ(1S)\Upsilon(1S) and Υ(2S)\Upsilon(2S) decays

    Using data samples of 102×106102\times10^6 Υ(1S)\Upsilon(1S) and 158×106158\times10^6 Υ(2S)\Upsilon(2S) events collected with the Belle detector, a first experimental search has been made for double-charmonium production in the exclusive decays Υ(1S,2S)J/ψ(ψ)+X\Upsilon(1S,2S)\rightarrow J/\psi(\psi')+X, where X=ηcX=\eta_c, χcJ(J= 0, 1, 2)\chi_{cJ} (J=~0,~1,~2), ηc(2S)\eta_c(2S), X(3940)X(3940), and X(4160)X(4160). No significant signal is observed in the spectra of the mass recoiling against the reconstructed J/ψJ/\psi or ψ\psi' except for the evidence of χc1\chi_{c1} production with a significance of 4.6σ4.6\sigma for Υ(1S)J/ψ+χc1\Upsilon(1S)\rightarrow J/\psi+\chi_{c1}. The measured branching fraction \BR(\Upsilon(1S)\rightarrow J/\psi+\chi_{c1}) is (3.90±1.21(stat.)±0.23(syst.))×106(3.90\pm1.21(\rm stat.)\pm0.23 (\rm syst.))\times10^{-6}. The 90%90\% confidence level upper limits on the branching fractions of the other modes having a significance of less than 3σ3\sigma are determined. These results are consistent with theoretical calculations using the nonrelativistic QCD factorization approach.Comment: 12 pages, 4 figures, 1 table.
