    Idiopathic hypoparathyroidism

    Get PDF
Idiopathic hypoparathyroidism is a rare disease. Drake, Albright, Baur and Castleman 3 fully described it in 1939. Only 50 or 60 proven cases (de Mowbray,2 1953) have been reported in the literature. As far as we know the following is the first reported case in an African

Variant (prinzmetal's) form of angina pectoris manifesting in complicating ventricular extrasystoles

    Get PDF
A case of the variant (Prinzmetal's) atypical form of angina pectoris is presented. Secondary and primary changes affecting the S-T segment and T wave are discussed. The features were also present, and indeed more marked, in complicating ventricular extrasystoles, one of which reflected the infarction pattern.S. Afr. Med. J., 48. 1146 (1974)

Right bundle-branch block associated with transient left posterior hemiblock in a case of acute myocardial infarction

    Get PDF
The intraventricular conduction defect of left posterior hemiblock is very uncommon. Its occurrence as a transient phenomenon during the course of acute myocardial infarction, as described in this report, is exceptionally rare. The presentation revealed a planar reorientation of QRS forces, and demonstrated the masking effect of left posterior hemiblock on the parameters of anterolateral myocardial ischaemia.S. Afr. Med. J., 48, 1237 (1974)

Is cryptogenic stroke a tenable diagnosis? The Milpark Hospital patent foramen ovale closure experience

    Get PDF
Patent foramen ovale (PFO) is increasingly being documented in patients presenting with “cryptogenic” neurological and peripheral ischemic events. Therapeutic options include the use of anti-platelet agents and in some cases non-surgical closure of the defect. In this article we present the Milpark Hospital experience of percutaneous closure of PFO to date. The importance of looking for this defect is highlighted, particularly in patients presenting with recurrent events despite antiplatelet therapy. Whilst routine closure of PFO is not advocated, it should be considered in certain select cases

Energy Transition on St Helena Island:A system dynamics approach

    Get PDF
This paper uses a system dynamics approach to explore low carbon energy transition on St Helena (SH) Island, identifying dominant system behaviors and opportunities for sustainable development. The British overseas territory is geographically remote and electrically isolated, making it an interesting, well-bounded case study. SH government wants to transition to renewable energy but financing investment in new systems will be challenging as import costs are high and their economy is stagnant, not yet recovered from the effects of the pandemic. Affordable energy would have a big impact on local businesses and increase consumer spending, boosting the economy. Qualitative data is collected through semi-structured interviews with energy stakeholders that explore how the current energy provision impacts the island and possible effects of future development plans. Transcripts from these interviews are coded to extract causal relationships and synthesized to develop a causal loop diagram (CLD) of island energy behaviors. This model undergoes methodical simplification to identify dominant behaviors and explore how system behaviors can support energy transition in the interest of the whole island. In this case, it is observed that public planned energy systems are an opportunity for more positive impact on the whole island than private systems. Analysis also highlights how strongly consumer energy costs affect the rest of the system and thereby indicates which stakeholder groups need to be included in decision-making processes. In some areas where the polarity of behavioral links was disputed, a basic systems archetypes study demonstrates the importance of effective communication and transparent decision-making to prevent initiation of unfavorable dynamics. Authors hope the clearly demonstrated methods offer a useful example of how this approach can offer useful insights into complex system problems.<br/

Adolescents\u27 Digital Literacies in Flux: Intersections of Voice, Empowerment, and Practices

    Get PDF
This article features a collaborative autoethnographic examination of three adolescent-researchers’ digital literacies. The participatory design punctuates the role of the adolescent-researchers as they explored their meaning-making practices. Such collaborative research, which included three adolescents and their parents, not only resurfaces parent-inquiry, but also brings the adolescent-researcher voice to the forefront of literacy research. Two research questions guided the investigation: (a) What do adolescent-researchers tell us about their digital and nondigital literacy practices? and (b) In what ways do adolescent-researchers’ retrospective examinations of their own practices reveal their perspectives of these practices and the power (and power struggles) that underlie them? The research team engaged in two rounds of coding, embracing first dramaturgical coding and then versus coding. Results suggested that Perspective/Attitude was the most prevalent attribute in the adolescent-researchers’ discourse. Moreover, versus coding revealed strong relationships between “then versus now.” Overall, the voices of the adolescent-researchers offer ongoing authenticity to discussions of their practices, creating continued opportunities to rethink the implications and applications of digital and nondigital practices in adolescents’ lives
