    The Field-Testing of a Novel Integrated Mapping Protocol for Neglected Tropical Diseases

    Neglected tropical diseases (NTDs) cause significant physical debilitation, lowered economic productivity, and social ostracism for afflicted individuals. Five NTDs with available preventive chemotherapy: lymphatic filariasis (LF), trachoma, schistosomiasis, onchocerciasis and the three soil-transmitted helminths (STH); have been targeted for control or elimination, but resource constraints in endemic countries have impeded progress toward these goals. We have developed an integrated mapping protocol, Integrated Threshold Mapping (ITM) for use by Ministries of Health to decide where public health interventions for NTDs are needed. We compared this protocol to the World Health Organizations disease-specific mapping protocols in Mali and Senegal. Results from both methodologies indicated the same public health interventions for trachoma, LF and STH, while the ITM methodology resulted in a more targeted intervention for schistosomiasis. Our study suggests that the integrated methodology, which is also less expensive and logistically more feasible to implement, could replace disease-specific mapping protocols in resource-poor NTD-endemic countries

    Evaluation of determinants of the use of health mutuals by the population of the Ziguinchor region in Senegal

    Access to health services is a concern around the world. Different strategies were developed, but Africa’s rate remains the lowest. This article aims to contribute to the population's access to healthcare, and to assess the determinants of the use of mutual health insurance by the population of the Ziguinchor region in Senegal.    Methods: The study is transversal and descriptive, carried out from July to August 2018. Through the quota method we defined the number of patients to be interviewed. Thus, by a geographic stratification according to the departments and a second-degree stratification taking into account the staff of the different hospital departments, 392 patients were selected.   Results: 73% at the Regional Hospital Center and 27% at the Regional Peace Hospital. Response rate: 97%, women 60% and men 40%. The enrollment for women (24%) is slightly higher than that for men (21%). Socio-economic factors. The rate of adherence is the highest of for patients with university level, followed by high school; income:  the highest rate for patients with a monthly income between 200,000 and 500,000 FCFA, followed by patients with an income monthly between 100,000 and 200,000 FCFA. Factors linked to the provision of care: the rate of Mutual Health Insurance adherence follows distances from patients' homes. Concerning the relation to satisfaction, education, distance and information are more determining than adherence rate.    Recommendations: 1) State: actions on education and distance; 2) Sensitizing the population on mutual health insurance; 3) Urging healthcare providers to reduce waiting times and respect schedules as well as appointments.&nbsp

    Hernie inguinale de la vessie: Ă  propos de 8 cas

    Décrire notre expérience de la prise en charge des hernies inguinales de la vessie (HIV). Il s'agit d'une étude rétrospective descriptive colligeant les dossiers des patients opérés pour une HIV entre janvier  2005 et décembre 2012. Les paramètres suivants ont été étudiés: l'âge des patients, les aspects  anatomo-cliniques de la hernie, les circonstances de découverte, l'attitude thérapeutique et les résultats de la cure. Huit HIV ont été diagnostiquées sur une période de 7 ans. Tous les patients étaient de sexe masculin. La moyenne d'âge était de 57,6 ans. La HIV siégeait à droite chez 5 patients et était associée à une HBP chez 3 patients, deux patients avaient des antécédents de herniorraphie . La découverte était per opératoire chez 6 patients, postopératoire (fistule vesicocutanée) chez un patient et  préopératoire chez un patient. Ce dernier a présenté une HIV géante diagnostiquée à l'Uroscanner. L'attitude thérapeutique était fonction des circonstances de  découverte de la HIV et de la pathologie associée. Six patients ont été opérés selon la technique de Bassini et deux selon la technique de Mac Way. La durée   moyenne de  l'hospitalisation était de 7 jours. Après un suivi régulier de 2 ans nous n'avons pas noté de récidive  herniaire. La HIV est une affection rare dont la   découverte est le plus souvent per-opératoire après une taille vésicale. Il faut l'évoquer chez tout patient aux antécédents d'herniorraphie et chez les sujets âgés de plus de 50 ans qui présentent une hernie inguinale associée à des TUBA.Key words: Vessie, hernie, découverte peroperatoire

    L'éléphantiasis vulvo-clitoridien d'origine filarienne est une affection très rare. Nous rapportons un nouveau cas chez une femme de 33 ans suivie dans un service de Maladies Infectieuses pour filariose lymphatique. Elle avait une masse vulvo-clitoridienne qui évoluait depuis plus de 10 ans. Une résection clitoridienne et une plastie vulvaire a été réalisée. Les résultats fonctionnels et esthétiques étaient satisfaisants.Key words: Eléphantiasis, filariose, vulvo-clitoridie

    Prevalence of Trachoma in Senegal: Results of Baseline Surveys in 17 Districts.

    PURPOSE: Senegal is endemic for trachoma, an infectious and potentially blinding eye disease. To complete the country's district-level baseline map of trachoma, we conducted population-based surveys in 17 health districts that were suspected-endemic but had yet to be surveyed. METHODS: We randomly selected 30 clusters (villages) per district and 30 households per village, and estimated the district-level prevalences of trachomatous inflammation-follicular (TF) in children aged 1-9 years, and trichiasis in persons aged ≥15 years. Data on household-level water, sanitation, and hygiene variables were also collected. Global Trachoma Mapping Project methods were followed in training, fieldwork, and data handling. RESULTS: 25,704 children aged 1-9 years and 30,345 adults aged 15 years and above were examined. In children aged 1-9 years, the prevalence of TF was <5% in all 17 districts, with the exception of Saint-Louis (5.1%, 95% CI 3.2-7.5). Trichiasis prevalence in participants aged 15 years and above ranged by district from 0%-1.1% (95% CI 0.7-1.5), with 9 districts having trichiasis prevalences above the elimination threshold of 0.2%. Trichiasis was seen to be significantly less frequent in males than in females (0.17% [95% CI 0.12-0.24] versus 0.49% [95% CI 0.38-0.61], p < 0.001). The prevalence of trichiasis rose steeply with age; 62% of cases were observed in people aged 55 years or above. CONCLUSIONS: Active trachoma is not a public health problem in 16 of the 17 surveyed districts, and implementation of the full Surgery (S) - Antibiotics (A) - Facial cleanliness (F) - Environmental improvement (E) strategy is not a programmatic priority. Increased provision of trichiasis surgery is warranted in nine districts

    Cancer de la prostate au Centre Hospitalier Universitaire Aristidie Le Dantec de Dakar : aspects épidemiologiques sur les cinq dernières années: Prostate cancer in Aristide Le Dantec hospital of Dakar: epidemiological aspects over the last five years

    Context and objective. Prostate cancer represents a major public health issue, but data from sub-Saharan Africa are scarce. This study aimed to describe the epidemiological aspects of prostate cancer during the last five years in Aristide Le Decantec hospital of Dakar.&nbsp;Methods. it’s a retrospective and descriptive study involving 5 last years including all patients with histologically confirmed prostate cancer. The studied parameters were: prevalence, incidence, age, clinical stage, lethality and death rate.&nbsp;Results. Two hundred and thirty-three patients were enrolled. The prevalence of prostate cancer during the study period was 0.8%. Depending on the stage, metastatic cancer was the most common form with 45.9% of cases. The new cases were 199 with an average of 39.8 per year. The total incidence of prostate cancer over the study period was 0.7%. The mean age of the patients at the diagnosis time was 68.6 ± 9.2 years. The lethality was 0.5%. The global death rate was 0.9 ‰. The specific death rate was 0.9‰. The annual mortality rate was higher in 2017 (36.4%) compared to other years. Depending on the stage, the death rate was higher in metastatic stages patients.&nbsp;Conclusion. The incidence of prostate cancer is increasing in our medical center. Metastatic forms remain more common with higher death rate. Early detection campaigns for prostate cancer should be considered. Contexte et objectif. Le cancer de la prostate représente un enjeu majeur de santé publique et mais il reste très peu documenté en Afrique subsaharienne. L’objectif de cette étude était d’évaluer les aspects épidémiologiques du cancer de la prostate sur les 5 dernières années dans notre centre.&nbsp;Méthodes. Il s’agissait d’une étude documentaire et descriptive sur 5 ans ayant colligé les dossiers de tous les patients avec cancer de la prostate histologiquement confirmé. Les paramètres étudiés étaient : la prévalence, l’incidence, l’âge, le stade clinique, la létalité et la mortalité.&nbsp;Résultats. Deux cent trente-trois patients ont été retenus. La prévalence du cancer de la prostate durant la période étudiée était de 0,8%. En fonction du stade, le stade de cancer métastatique était prépondérant (45,9%). Les nouveaux cas étaient de 199, soit une moyenne de 39,8 nouveaux cas par an. L’incidence totale du cancer de la prostate sur la période étudiée était de 0,7%. L’âge moyen des patients au moment du diagnostic était de 68,6 ± 9,2 ans. Le taux létalité était de 0,5%. La mortalité globale était de 0,9‰. Le taux de mortalité annuelle était plus important en 2017 (36,4%) en comparaison aux autres années étudiées. En fonction du stade, le taux de mortalité était plus important pour les stades métastatiques.&nbsp;Conclusion. L’incidence du cancer de la prostate est en augmentation dans notre centre. Les formes métastatiques restent prédominantes assombrissant le pronostic vital. Des campagnes de dépistage précoce du cancer de la prostate sont à envisager

    Facial cleanliness indicators by time of day: results of a cross-sectional trachoma prevalence survey in Senegal.

    BACKGROUND: The World Health Organization-recommended strategy for trachoma elimination as a public health problem is known by the acronym "SAFE", where "F" stands for facial cleanliness to reduce transmission of ocular Chlamydia trachomatis infection. Accurately and reliably measuring facial cleanliness is problematic. Various indicators for measuring an unclean face exist, however, the accuracy and reliability of these indicators is questionable and their relationship to face washing practices is poorly described. METHODS: Clean face indicator (ocular or nasal discharge, flies on the face, and dirt on the face), trachoma clinical sign, and ocular C. trachomatis infection data were collected for 1613 children aged 0-9 years in 12 Senegalese villages as part of a cross-sectional trachoma prevalence study. Time of examination was recorded to the nearest half hour. A risk factor questionnaire containing Water, Sanitation and Hygiene (WASH) questions was administered to heads of compounds (households that shared a common doorway) and households (those who shared a common cooking pot). RESULTS: WASH access and use were high, with 1457/1613 (90.3%) children living in households with access to a primary water source within 30 min. Despite it being reported that 1610/1613 (99.8%) children had their face washed at awakening, > 75% (37/47) of children had at least one unclean face indicator at the first examination time-slot of the day. The proportion of children with facial cleanliness indicators differed depending on the time the child was examined. Dirt on the face was more common, and ocular discharge less common, in children examined after 11:00 h than in children examined at 10:30 h and 11:00 h. CONCLUSIONS: Given the high reported WASH access and use, the proportion of children with an unclean face indicator should have been low at the beginning of the day. This was not observed, explained either by: the facial indicators not being reliable measures of face washing; eye discharge, nose discharge or dirt rapidly re-accumulated after face washing in children in this population at the time of fieldwork; and/or responder bias to the risk factor questionnaire. A high proportion of children had unclean face indicators throughout the day, with certain indicators varying by time of day. A reliable, standardised, practical measure of face washing is needed, that reflects hygiene behaviour rather than environmental or cultural factors

    Diagnostic Accuracy of a Prototype Point-of-Care Test for Ocular Chlamydia trachomatis under Field Conditions in The Gambia and Senegal

    Trachoma, caused by infection of the eye with the bacterium Chlamydia trachomatis, is the leading infectious cause of blindness and is associated with poverty. Antibiotic treatment of all community members is one of the recommended control strategies for trachoma. However, in places where the prevalence of clinical signs is low, C. trachomatis eye infection is often absent. Laboratory testing for C. trachomatis infection by polymerase chain reaction (PCR) is highly sensitive but expensive and requires well-trained staff. A simple point-of-care (POC) test that can be used in trachoma-affected communities could help trachoma control efforts. We evaluated a POC test for C. trachomatis eye infection. Children under 10 years of age were screened for clinical signs of trachoma and C. trachomatis eye infection. The POC test result was compared with laboratory PCR test results. The POC test detected just over half of PCR test positives correctly. However, the POC test tended to give false-positive results in hot and dry conditions, which is the typical environment of trachoma. The POC test requires high specificity since it would be used to make treatment decisions at the community level. Therefore, its present format requires improvement before it can be utilized in trachoma control

    Population-based prevalence survey of follicular trachoma and trachomatous trichiasis in the Casamance region of Senegal.

    BACKGROUND: Trachoma, caused by ocular infection with Chlamydia trachomatis, is the leading infectious cause of blindness worldwide. We conducted the first population-based trachoma prevalence survey in the Casamance region of Senegal to enable the Senegalese National Eye Care Programme (NECP) to plan its trachoma control activities. The World Health Organization (WHO) guidelines state that any individual with trachomatous trichiasis (TT) should be offered surgery, but that surgery should be prioritised where the prevalence is >0.1%, and that districts and communities with a trachomatous inflammation, follicular (TF) prevalence of ≥10% in 1-9 year-olds should receive mass antibiotic treatment annually for a minimum of three years, along with hygiene promotion and environmental improvement, before re-assessing the prevalence to determine whether treatment can be discontinued (when TF prevalence in 1-9 year-olds falls 1% in all districts. CONCLUSION: With a prevalence <5%, TF does not appear to be a significant public health problem in this region. However, TF monitoring and surveillance at sub-district level will be required to ensure that elimination targets are sustained and that TF does not re-emerge as a public health problem. TT surgery remains the priority for trachoma elimination efforts in the region, with an estimated 1819 TT surgeries to conduct

    The global burden of trichiasis in 2016.

    BACKGROUND: Trichiasis is present when one or more eyelashes touches the eye. Uncorrected, it can cause blindness. Accurate estimates of numbers affected, and their geographical distribution, help guide resource allocation. METHODS: We obtained district-level trichiasis prevalence estimates in adults for 44 endemic and previously-endemic countries. We used (1) the most recent data for a district, if more than one estimate was available; (2) age- and sex-standardized corrections of historic estimates, where raw data were available; (3) historic estimates adjusted using a mean adjustment factor for districts where raw data were unavailable; and (4) expert assessment of available data for districts for which no prevalence estimates were available. FINDINGS: Internally age- and sex-standardized data represented 1,355 districts and contributed 662 thousand cases (95% confidence interval [CI] 324 thousand-1.1 million) to the global total. Age- and sex-standardized district-level prevalence estimates differed from raw estimates by a mean factor of 0.45 (range 0.03-2.28). Previously non- stratified estimates for 398 districts, adjusted by Ă—0.45, contributed a further 411 thousand cases (95% CI 283-557 thousand). Eight countries retained previous estimates, contributing 848 thousand cases (95% CI 225 thousand-1.7 million). New expert assessments in 14 countries contributed 862 thousand cases (95% CI 228 thousand-1.7 million). The global trichiasis burden in 2016 was 2.8 million cases (95% CI 1.1-5.2 million). INTERPRETATION: The 2016 estimate is lower than previous estimates, probably due to more and better data; scale-up of trichiasis management services; and reductions in incidence due to lower active trachoma prevalence
