    A Comparative study between effects of electrical stimulation and strapping versus shoulder sling in prevention of shoulder subluxation and pain in acute hemiplegic patients.

    INTRODUCTION : Shoulder pain occurs in 34% to 85%of patients, regardless of age, gender and its onset typically takes place in the second week post stroke. The beginning of hemiplegia can compromise the normal biomechanical principles and the stability of shoulder complex due to the loss of motor control, the development of abnormal movement patterns and misalignment of the gleno humeral joint. The treatment starts with prevention of shoulder subluxation by 1.proper handling , 2.positioning, 3.straping, 4.Electrical stimulation, 5.Use of external supports like vernay brace, slings to prevent shoulder subluxation, bobath and PNF with conventional physiotherapy treatment to reduce pain and increase the range of motion of the shoulder joint. AIM OF THE STUDY : To compare the effectiveness of Electrical Stimulation with Strapping versus Shoulder Sling in the management of Hemiplegic shoulder subluxation and pain. OBJECTIVES OF THE STUDY : 1. To determine the effectiveness of Electrical Stimulation with Strapping in the management of Hemiplegic shoulder subluxation and pain. 2. To determine the effectiveness of Shoulder Sling in the management of Hemiplegic shoulder subluxation and pain. 3. To determine the effectiveness of Electrical Stimulation with Strapping versus Shoulder Sling in the management of Hemiplegic shoulder subluxation and pain. MATERIALS AND METHDOLOGY: MATERIALS - Electrical stimulator, Electrodes and pads, Pillow, Couch, Lint cloth, Leads, Adhesive tap, Cotton, Strap, Powder, Arm sling. METHODOLOGY : A total number of 30 patients having Hemiplegic Shoulder subluxation and pain, who met the inclusion criteria were recruited by convenient sampling method. After the informed consent obtained, they were partitioned into two groups as Group A and Group B, with 15 patients in each. Study Design - Quasi Experimental Study Design. Study Setting - The study was conducted at out patient department in J.K.K. Munirajah Medical Research Foundation College of Physiotherapy, Komarapalayam and District Head Quarters Hospital, Erode under the supervision of the concerned authorities Sampling Method - Convenient sampling method. Sample Size - Thirty patients with Hemiplegic Shoulder subluxation and pain, who comes under the inclusion criteria, were taken for the study. Study Duration - The study was conducted for a course of 6 weeks. Inclusion Criteria - Age group: 40-60 years, Both sexes, Both sides, Ischemic and Hemorrhagic Stroke. Exclusion Criteria - Musculo skeletal problem at shoulder (sprain and strain), Fractures at shoulder joint, Psychiatric patients, Degenerative diseases, Hemiplegia results from traumatic brain injury (TBI), space occupying lesion, Any shoulder pathology ( Recurrent shoulder subluxation). Parameters - Visual Analogue Scale, Fugl-meyer assessment of physical performance ( upper extremity). Technique - Electrical Stimulation with Strapping: Positioning, Proper handling, Preparing the Treatment area, Electrical stimulation, Strapping. Shoulder Sling - Positioning, Proper handling, Shoulder sling. CONCLUSION : The result of the study concluded that there was reduction in shoulder subluxation and pain in acute hemiplegic after the treatment with electrical stimulation and strapping than with shoulder sling alone, and Visual Analogue Scale and Fugal-Meyer assessment scale could be used as the assessment tools for pain and upper limb function

    Safety evaluation of Sapindus laurifolius leaf extract in Wistar rats

    Objectives:The present work was aimed to study the phytochemical composition of the Sapindus laurifolius leaves andtoxicological effect of the Sapindus laurifolius leaf extract in a systematic way using Wistar albino rats as a model animal.Materials and Methods :The identification of phytoconstituents present in the leaf extract was performed using Highperformance thin layer chromatography (HPTLC). In toxicity studies, the acute oral toxicity study was conducted as per theguidelines of Organization for Economic Co-operation and Development (OECD 423 Acute Toxic Class Method) for testingof chemicals. In repeated dose 28-day oral toxicity study (OECD 407), methanolic leaf extract administered at the dose of 50,200 and 800 mg/kg BWand limit dose of 1000 mg/kg BW.Results: Saponins, flavanoids, glycosides and bitter principles were the major phytoconstituents identified. In acute toxicitystudy, the LD cut-off values were found to be more than 2g/kg in leaf extract. In repeated dose 28-day oral toxicity, significant 50(P<0.05) increase in AST, ALT, BUN and creatinine, significant (P<0.05) increase in total protein was noticed. Thehistopathological changes confined to liver, kidney and intestine, revealed mild to moderate hepatotoxicity, severenephrotoxicity and increased goblet cell activity. The changes were found to correlate with increased dose of leaf extract.Conclusion:The phytochemical analysis of Sapindus laurifolius revealed the presence of saponins, glycosides, flavonoidsand bitter principles.The acute oral toxicity study of S. laurifolius methanolic leaf extract in rats resulted in no toxicity even atthe highest dose, but in repeated 28-day oral toxicity study revealed mild to moderate hepatotoxicity, severe nephrotoxicityand intestinal damage

    Prevalence of leukoplakia, oral submucous fibrosis, papilloma and its relation with stress among green marbles mine laborers, India

    Objectives: To determine the prevalence of leukoplakia, oral submucous fibrosis and papilloma among ?Green Marble Mines? laborer and uncover its relation with occupational stress. Methods: Mines were divided in four geographic zones, and participants were selected by stratified cluster sampling technique. A total of 513 subjects were included in final study which were alienated among the four age cohort 15-24 , 25-34 , 35-44, 45-54 respectively. The study was been conducted following the research methodology recommended by World Health Organization- Oral Health Surveys 1997. A questionnaire from ?Union of Shops, Distribution and Allied Worker (USDAW) Nationwide, Manchester? was used for stress assessment of mine workers and clinical examination for oral mucosa was conducted by one of the three examiner with the aid of an artificial light source. The kappa statistics for diagnosis of leukoplakia, oral submucous fibrosis and papilloma was determined (field teams versus expert) 0.81, 0.92 and 0.89 respectively two days prior to the examination. Data was analyzed using bivariate and multivariate analysis. Results: An overall elevated prevalence of all three oral-mucosal lesion was found among mine workers (36.7%), mainly leukoplakia affecting 171 mine workers (33.3%). The affected workers were having body problems like headache, backache and stressed due to under-payment. Individuals having papilloma have faced problem at work like noise, dust or fumes and poor maintenance of equipment. Multiple logistic regression analysis model of oral-mucosal lesion have shown highly significant relation (p<0.01) with increased stress, age, alcohol habits and malnutrition. Conclusion: The prevalence of oral mucosal lesion is higher, among marble mine laborers, and occupational stress can intensify the disease condition. Curative services along with prevention and stress reduction program, requires primary anticipation

    Evaluation of efficacy of Carica papaya leaf extracts to increase platelet count in hydroxyurea induced thrombocytopenia in Albino rats

    Background: Dengue is an infectious disease associated with high mortality and morbidity. Being a viral disease, there is no specific drug available for treatment. There are some reports that Carica papaya leaf extract may improve the clinical condition of dengue patients. However, to support this, at present, there is no systematically searched and synthesized evidence available. Hence this study was undertaken to compare the efficacy of commercial preparation of Carica papaya leaves with freshly prepared Carica papaya leaf extracted.Methods: 48 albino rats were randomly divided into eight groups of six each. Thrombocytopenia was induced by giving hydroxyurea (15mg/kg) orally. Group I and II served as saline and toxic control group respectively. Other six groups were given two different doses of either commercial extract or fresh extract orally for five days. 1ml of blood was withdrawn at baseline,3rd and 6th day of the study. Platelet, WBC, RBC count, clotting and bleeding time were determined.Results: Mean platelet count increased significantly on day 6 in both low dose (2.06 to 4.93lakh/mm3) and human equivalent dose (2.73 to 7.66lakh/mm3) of commercial extract groups compared to the toxic control group (p<0.05). Similarly, the mean platelet count increased significantly for human equivalent dose in fresh leaf extract group (3.17 to 4.69lakh/mm3) but the increase in low dose fresh extract (3.28 to 3.76lakh/mm3) was not significant. There was no significant rise in mean platelets count, mean RBC count, WBC count, decrease in mean bleeding and clotting time between commercial extract and fresh leaf extract group for both low dose and human equivalent dose.Conclusions: Efficacy of fresh leaf extract of Carica papaya was not inferior to commercial available preparation. Fresh Carica papaya leaf extract no doubt offers a potential therapeutic efficacy which is cost effective, more affordable and accessible treatment in patients with thrombocytopenia

    Ever since the first HIV case reported, there is a dramatic increase in the prevalence of HIV infection which urged the need for an effective diagnostic method.  There are several improvements in diagnostic methods employing various technologies to screen for HIV infection using enzyme immunoassays and rapid diagnostic tests which simultaneously detect antigen and antibody with significantly reduced window period.  Further, western blot or immunofluorescence methods are used to confirm the results. Nucleic acid amplification tests, though sensitive, are expensive and hence not employed for screening but instead used to monitor the antiretroviral drug response. In spite of appreciable advancements in the sensitivity and specificity of the detection techniques, the algorithms are still the method of choice for ensuring the accuracy of test results

    Aerodynamic Design Improvement for an Intercity Bus

    Intercity buses travel about 250 to 350 km in a stretch and usually are of sleeper coach mode. The exterior styling, sleeper comfort and aerodynamically efficient design for reduced fuel consumption are the three essential factors for a successful operation in the competitive world. The bus body building companies prioritizes the exterior looks of the bus and ignore the aerodynamic aspect. Scientific design of sleepers for increased comfort of the passengers is seldom seen. The overall aim of this project was to redesign an intercity bus with enhanced exterior styling, reduced aerodynamic drag and increased comfort for the passengers. Principles of product design were used to analyze the styling and comfort. The benchmarked high floor bus was redesigned with low - floor for reduced aerodynamic drag. The exterior was redesigned with emphasis on improvised aerodynamic performance and appealing looks. The interior was modified to meet aspirations of the commuters. The results of the redesigned exterior body showed a reduction of about 45% in coefficient of drag and overall aerodynamic drag reduction by 60% due to combined effect of reduced coefficient of drag and frontal area

    Threshold Saturation for Spatially Coupled LDPC and LDGM Codes on BMS Channels

    Spatially-coupled low-density parity-check (LDPC) codes, which were first introduced as LDPC convolutional codes, have been shown to exhibit excellent performance under low-complexity belief-propagation decoding. This phenomenon is now termed threshold saturation via spatial coupling. Spatially-coupled codes have been successfully applied in numerous areas. In particular, it was proven that spatially-coupled regular LDPC codes universally achieve capacity over the class of binary memoryless symmetric (BMS) channels under belief-propagation decoding. Recently, potential functions have been used to simplify threshold saturation proofs for scalar and vector recursions. In this paper, potential functions are used to prove threshold saturation for irregular LDPC and low-density generator-matrix codes on BMS channels, extending the simplified proof technique to BMS channels. The corresponding potential functions are closely related to the average Bethe free entropy of the ensembles in the large-system limit. These functions also appear in statistical physics when the replica method is used to analyze optimal decoding

    Effect of Surface Roughness on the Squeeze Film Characteristics of Circular Plates in the Presence of Conducting Couplestress Fluid and Transverse Magnetic Field

    The combined effect of surface roughness and magnetic field on the performance characteristic of the circular plates lubricated with conducting couplestress fluid (CCSF) has been studied. On the basis of the Christensen Stochastic model, the generalized stochastic Reynold’s equation is derived. Modified equations for the nondimensional pressure, load load-carrying capacity, and squeeze film time are derived. The results are presented both numerically and graphically and compared with conducting smooth surface case. It is observed that the surface roughness effects are more pronounced for couplestresses as compared to nonconducting Newtonian fluid (NCNF) in the presence of magnetic field
