    Residual mother-to-child transmission of HIV in Burkina Faso

    Background: Burkina Faso is one of the countries in West Africa most affected by the HIV/AIDS pandemic, despite the implementation of a mother-to-child HIV transmission prevention program as a strategy to reduce the risk of vertical transmission of the disease.Objective: To assess the current risk of mother-to-child transmission of HIV in Burkina Faso.Materials and methods: A prospective study was conducted between December 2014 and July 2016, in the 13 health regions of Burkina Faso. Women who were screened HIV-positive during a prenatal consultation were followed until delivery. Their babies received dry blood spot (DBS) at birth, at week 6 and at 1year, to screen for HIV.Results: Overall, 186 pregnant women were included in the study, with a mean age of 29.17±6.13 years. Of their children, 430 DBS actually received a PCR test, giving a 91.1% PCR implementation rate. After analyses, 6 (1.3%) babies were identified as carriers of HIV1. The newborn’s serological status was associated with delivery pattern (p=0.000), the administration of antiretroviral drugs to the mother after delivery (p=0.0064), the administration of Nevirapine to the newborn at birth (p=0.022), the use of contraceptive methods after delivery (p=0.028) and the presence of breast affections/infections since delivery (p=0.013).Conclusion: The results of our study are encouraging and demonstrate the effectiveness of interventions in the mother-tochild prevention program (PMTCT) for HIV-positive pregnant women can be improved through early initiation of triple therapy in early pregnancy and improved adherence to antiretroviral (ARV) therapy.Keywords: Burkina Faso, HIV/AIDS, mother-to-child transmission, antiretroviral drugs, pregnant wome

    Amélioration de la valeur nutritionnelle des gousses de Piliostigma reticulatum (D. C.) Hochst dans l’alimentation du bétail en période de soudure

    Dans la zone nord soudanienne du Burkina Faso, les gousses de Piliostigma reticulatum sont intensément utilisées pendant la saison sèche comme aliment d’appoint pour le bétail. L’objectif de cette étudeest d’améliorer la qualité nutritionnelle de ces gousses pour optimiser leur apport. Neuf rations comprenant chacune des gousses de P. reticulatum (70 p.100) et du foin de Pennisetum pedicellatum (30 p.100) distribuéesà raison de 50 g MS/kg p0,75 ont été testées sur des ovins de race Djallonké. Les gousses ont subi différents traitements physiques (concassage, mouture) suivis ou non de traitement à l’urée auxquelles est associé dans certains cas du charbon de bambou à des doses de 0,25 ou 0,50 g/kg PV dans le but d’influencer l’action des tanins. L’utilisation de l’urée a entraîné, quelle que soit la forme de distribution, une amélioration de la digestibilité, surtout celle des matières azotées en raison de l’augmentation significative de leur teneur. L’adjonction du charbon de bambou n’a amélioré la digestibilité des gousses non traitées à l’urée qu’à la dose de 0,50 g/kg PV.     Using K-MADe for learning task modeling: interests and difficulties

    International audienceTeaching user-centred software design covers many aspects. One of the most important ones is task modeling. Since task modeling contributes largely to the ergonomic quality and acceptance of the resulting software, it is essential for task modeling concepts to be well understood by future software designers. To this end, this study aims at evaluating a task model (K-MAD) and its associated tool (K-MADe) as regards the task modeling training. This article describes the rationale of the study, the model and the software tool used, the teaching steps, and the experience feedback on the practical use of the software. The latter identifies the benefits and disadvantages of this practical use from the teaching point of view, but also from a practical standpoint, including in terms of evolution of the models obtained and of their edition.L'enseignement de la conception de logiciel centrée utilisateur concerne de nombreux aspects. Un de ces aspects les plus importants est la modélisation des tâches. La modélisation des tâches contribuant fortement à la qualité ergonomique et à l'acceptation du logiciel résultant, il est indispensable qu'elle soit la mieux comprise possible par les futurs concepteurs de logiciels. Dans ce but, cette étude cherche à évaluer un modèle de tâche particulier (K-MAD) et son outil associé (K-MADe) dans une démarche d'enseignement de la modélisation des tâches. Cet article décrit la problématique de l'étude, le modèle et le logiciel enseigné, les différentes phases de l'enseignement, et le retour d'expérience d'utilisation concrète du logiciel.     Prevalence of occult hepatitis B virus infection among blood donors in Ouagadougou, Burkina Faso

    Background: In Burkina Faso, the polymerase chain reaction (PCR) assay is not routinely used in the biological qualification of blood donations and this constitutes a risk factor for the transmission of occult hepatitis B virus (HBV) infection during blood transfusion. The objective of this study is to determine the prevalence of occult B infection (OBI) among blood donors for the purposes of improved blood safety in Burkina Faso. Methodology: A descriptive cross-sectional study of 300 HBsAg negative blood donors was conducted in the city of Ouagadougou, Burkina Faso from April to October 2020. Anti-HBc antibody was determined using the BOSON® brand rapid tests. HBV DNA was detected in 75 selected donors by real-time PCR (rt PCR) using the 7500 Fast Real Time PCR assay technique. Results: Of the 300 HBsAg negative donors, 208 (69.3%) were males while 92 (30.7%) were females, with average age of 30.18 years. Anti-HBc antibody was detected in 39 cases (13%). Of the 75 donor samples tested by rt PCR, 3 (4%) were positive for HBV DNA (occult B infection); 2 of which were anti-HBc antibody positive (seropositive OBI) while 1 was anti-HBc antibody negative (seronegative OBI). Conclusion: Given the prevalence of OBI of 4% in this study and its consequences in blood recipients, it appears necessary that in addition to the classic serological markers of hepatitis B, to test for the presence of HBV DNA among blood donors in order to improve transfusion safety.     Importance of the confirmatory assay for the detection of the HBsAg in the epidemiological studies and in the diagnosis of the viral Hepatitis B

    Several epidemiological studies have reported high prevalence of HBsAg among pregnant women in Burkina Faso. They used various algorithms, as it is also done for the routine diagnostic. Knowing this antigen carriage rate in such a population or in other clinic attendees is important for the implementation of a national immunisation programme and the monitoring of patients with hepatitis B. Often, the screening tests were not confirmed in spite of the existence of known false positive and false negative results. The aim of this study was to determine a more accurate prevalence of HBsAg, among the pregnant women in Burkina Faso. From October 2006 to January 2007, blood samples were collected from 1139 pregnant women. Each sample was analyzed for HBsAg, using two assays and according to manufacturers’ instructions vis, Hepanostika®HBsAg Uniform II B9 (Bio-Mérieux; France) and HBsAg (V2) Abbott AxSYM® system (Abbott Diagnostics). All the positive samples were tested with a confirmatory neutralization assay- Hepanostika®HBsAg Uniform II B9 Confirmatory (Bio-Merieux). The mean age of the pregnant women was 24.85years [range: 15-45years] and the age range of 20-24 (37%) and 25-29 (25.4%) years were the most represented. The overall rate of HBsAg-positive pregnant women with the two screening assays was 20.9%.     Resistance to a wide variety of common antimicrobials is observed among clinical strains designed as extended spectrum β-lactmase (ESBL)  producers. They produce enzymatic protein which inactivates efficiently oxyimino cephalosporin and constitutes a serious global health concern that has complicated treatment strategies. Many studies report high prevalence of ESBL producers among Gram negative bacilli. The aim of this work was to identify the presence of TEM, SHV and CTX-M families in thesestrains which were initially screened by phenotypic method. Gram negative bacilli resisting third or four generation cephalosporin were isolated during anti-biogram study. The presence of ESBL positivity was detected using the double disk synergy test. Minimal inhibitory concentrations (MICs) of ceftriazon for any strain were determined using E-test manufacturing protocol. Polymerase chain reaction (PCR) analysis for β-lactamase (bla) genes of TEM, SHV and CTX-M family was carried out using designed primers in 171 ESBL isolates producers. Among 259 Gram negative bacilli collected, 171 (66, 02%) exhibited ESBL producers’ profile. Urine samples constitute major source of ESBL producers. The highest prevalence of ESBL was observed in Escherichia coli (75, 50%). Among ESBL isolates producers, gene prevalence of bla-CTX-M (65, 49%) was highest, followed by bla-TEM (25, 73%) and bla-SHV (18, 71%) in the present study. The frequency of ESBL producing strains among clinical isolates has been steadily increased. Continual drug resistance surveillance and molecular characteristics of ESBL isolates are necessary to guide the appropriate and judicious antibiotic use.Key words: Extended spectrum β-lactamase (ESBL), double disk synergy test, blaTEM, blaSHV, blaCTX-M, PCR

    Microbiological and kinetic detection of gram negative bacilli producing extended-spectrum- β-lactamases (esbl) in emergencies and reanimation units of university hospital center, Yalgadoouedraogo, Burkina Faso

    Background: Epidemiology of extended-Spectrum- β-lactamases has become worldwide, and our aim was to establish the prevalence of isolates producer in university hospital center Yalgado OUEDRAOGO particularly in reanimation and emergencies units.Material and methods: Prospective study was drive during July 2009 to march 2012 in order to collect strains resisting to third generation of cephalosporin during diagnosis analysis of biological specimens. Susceptibility of bacteria to antimicrobial agents was evaluated by disc diffusion method. Production of extended-spectrum β-lactamases has been investigated by double disc diffusion and kinetic methods.Results: 259 isolates which resisted at least to one of third generation of cephalosporins were collected. Among them 188 (72, 58 %) were positive to synergy test by a double disc diffusion method. The MICs of ceftriaxone determined by E-test were under than 50kg/ml, 100kg/ml et 256kg/ml for respect 81,57°/° ; 55,26°/° et 39,74°/° of isolates. Hydrolyze of β-lactam ring by bacterial extract followed at spectrophotometer showed speeds running at 0 to 0,090UAb.mn-1 for both isolates. Extract of 171 bacterial strains positives to synergy test had hydrolyzed at least one of oxy-iminocephalosporins and were identified as producing extended- spectrum β-lactamases. Spices reported by this study were 99 Escherichia coli (57,89%) ; 28 Klebsisella pneumonia (16,37%) ; 15 Enterobactersp (8,77%) ; 19 Pseudomonas aeruginosa (11,11%) ; 4 Citrobactersp (2,33%) 2 Acinetobactersp (1,16%), 3 Proteus mirabilis (1,75%) and 1 Salmonella typhi (0,05%).Conclusion: This study showed that bacterial resistances by extended- spectrum β-lactamases are a reality in University Hospital center YalgadoOuedraogo.     Campylobacter Spp. Epidemiology and Antimicrobial Susceptibility in a Developing Country, Burkina Faso (West Africa)

    Data on campylobacteriosis are almost nonexistent in Burkina Faso. In this study conducted from 2006 to 2008 in Ouagadougou, stool specimens and sociodemographic data were collected from 1 246 patients attending the university teaching hospital for enteritis. Stool samples were analyzed for the presence of Campylobacter by the direct culture method on selective mCCDA agar followed by antibiotic susceptibility testing on the isolated strains. The isolation rate of Campylobacter was 2.3%,comprising of the following species C. jejuni (51.8%), C. coli (13.8%), and C. upsaliensis (3.5%). However, 30.9% of the isolates were unidentified. No resistant strain was found to gentamicin. The resistance to amoxicillin+clavulanic acid (3.4%) was lower than those (10.3-34.5%) to the other antibiotics: erythromycin (10.3%), tetracycline (10.3%), ciprofloxacin (13.8%), amoxicillin (24.1%) and ceftriaxone (34.5%), nalidixic acid (34.5%). Significant associations were found between Campylobacter enteritis and contact with animals (P=0.03), and HIV infection (P<0.0001), in contrast to other sociodemographic and seasonal factors. From the data obtained Amoxicillin+clavulanic acid appear to be the first choice for treatment. The implementation of a national program may be helpful in controlling the spread of the disease and the increase of resistance to antibiotics.Keywords: Campylobacter, epidemiology, HIV, drug resistance, Burkina Fas

    Relationships between soil seed banks and above-ground vegetation along a disturbance gradient in the W National Park trans-boundary biosphere reserve, West Africa

    Aims Vegetation succession depends on the availability of suitable propagules in the soils, thus knowledge of soil seed banks is essential for formulating effective strategies for restoring the vegetation of degraded sites. The W National Park, the only trans-boundary biosphere reserve in West Africa, is being extensively fragmented and degraded in recent decades. The aims of this study were to assess the reserve’s soil seed banks, their relationships with standing vegetation and bundle of disturbances and their potential significance for vegetation restoration. Methods The size and composition of the above-ground species vegetation were assessed in nine plots of 1 ha each representing a range of habitats with differing disturbance severity (low, intermediate and high). A total of 702 soil samples were taken from three layers (0–3, 3–6 and 6–9cm) and soil seed bank was analyzed using the seedling emergence technique. Important Findings Generally, seeds of non-woody taxa dominated in samples from all soil depths and habitats of all disturbance severities. The mean soil seed density was 17.8, 24.4 and 26.3 seeds/dm3 in samples from the least, intermediate and most disturbed sites, respectively, and highest in the upper soil layers in all cases. The results indicate that there is limited potential for restoring woody vegetation solely from soil seed banks, and that woody species in the region rely more on recently shed seeds trapped in the standing dead biomass and litter on the ground than soil seed banks for regeneration. Thus, human intervention is needed to accelerate forest recovery, mainly through alleviating anthropogenic impacts on the ecosystem (for instance, avoiding destruction of new seeds by intense fire), and site manipulation to improve environmental conditions for seedling establishment and growth. Other ways of restoring forests than through the soil seed bank (e.g. sowing seeds collected elsewhere, and planting tree seedlings) could also be relevant
