    The effect of acupuncture on the day of embryo transfer on the in vitro fertilization outcomes: An RCT

    Background: Acupuncture is an adjunct therapy to support infertile women received in vitro fertilization (IVF) treatment; however, the efficacy of this approach needs more evaluation. Objective: This randomized clinical trial (RCT) study aimed to evaluate the influence of acupuncture on reproductive outcomes in women undergoing IVF treatment. Materials and Methods: The study was carried out on 186 participants who had undergone IVF treatment in the Mother and Child Hospital between September 2015 and February 2016. Subjects were randomly divided into three groups: Acupuncture 25 min before embryo transfer (ET) (ACU1 group, n = 62), acupuncture 25 min before and after ET (ACU2 group, n = 62), and ET without acupuncture (control group, n = 62). Pregnancy rates (biochemical, clinical, and ongoing) were evaluated and compared between groups. Results: There were significant differences between the ACU1 group and the control group regarding biochemical (p = 0.005), clinical (p = 0.006), and ongoing (p = 0.007) pregnancies. Also, our results showed that two-session acupuncture (ACU2) lead to a significant reduction in frequency of biochemical (p = 0.002), clinical (p = 0.003), and ongoing (p = 0.01) pregnancy rates when compared to the one-session acupuncture (ACU1). No significant difference was found between the ACU2 and control groups regarding the aforementioned terms (p = 0.50). Conclusion: Acupuncture 25 min before ET significantly increased the IVF outcomes in women undergoing IVF compared with no acupuncture. Repeating acupuncture 25 min after ET did not improve the IVF outcome. Key words: Acupuncture, Embryo transfer, In vitro fertilization, Pregnancy rate

    Frozen embryo transfer: Endometrial preparation by letrozole versus hormone replacement cycle: A randomized clinical trial

    Background: The endometrial preparation with stimulating natural cycles for frozen embryo transfer (FET) have benefits like lower cost and ease of use. Objective: Comparing the clinical outcome of letrozole versus hormone replacement (HR) for endometrial preparation in women with normal menstrual cycles for FET in artificial reproduction techniques. Materials and Methods: A total of 167 participants who had frozen embryos and regular ovulatory cycles were randomly divided into two groups for endometrial preparation. One group (82 women) was stimulated with letrozole 5mg/day and the other group (85 women) was hormonally stimulated by oral estradiol valerate (2 mg three times a day). All participants were followed serially by ultrasonography. Any patient who did not reach optimal endometrial thickness was excluded from the study. Implantation, biochemical and clinical pregnancy and abortion rate were reported. Results: There was no significant difference in the mean age, duration, and primary or secondary infertility, cause of the infertility, number, and quality of transferred embryos between the groups. The mean estradiol level on the day of transfer was 643 ± 217 in the HR group and 547 ± 212 in the letrozole group (P = 0.01), which was significantly different. The clinical pregnancy rate was 38.7 in the letrozole group, higher than the HR group (25.3) but not significantly different (P=0.06). Conclusion: For endometrial preparation in women with a normal cycle, letrozole yields higher pregnancy rate although it is not significant; due to its cost, ease in use, and lower side effects, letrozole is a good choice.     Background: To perform an in-vitro fertilization cycle, pretreatment with gonadotropin-releasing hormone (GnRH) agonist is widely used as a part of controlled ovarian hyper-stimulation protocols to prevent endogenous luteinizing hormone surge and spontaneous ovulation. GnRH agonist pretreatment is relatively costly and there is a risk of hypo estrogenic side effect. It would also lengthen the preparation period until pituitary desensitization occurs. Objective: Our study is aimed at evaluating the pregnancy outcome rate of frozen thawed embryo transfer with and without GnRH agonists pretreatment. Materials and Methods: Women with documented infertility who were candidate for frozen thawed embryo transfer were recruited and randomly assigned to two groups. In group A (n=100), patients received GnRH agonist, Buserelin, to induce pituitary desensitization prior to endometrial preparation and embryo transfer. Individuals in group B (n=100) received steroid manipulation without prior down-regulation of the pituitary. Chemical pregnancy, implantation rate, clinical pregnancy and ongoing pregnancy were measured and statistically compared between the two groups. Results: None of the outcome measures including clinical and chemical pregnancy rates, implantation rate, and ongoing pregnancy rate showed significant difference between the two groups. Similarly, the rate of miscarriage did not vary between the two groups. Conclusion: In this study, we found that removing the GnRH agonists pretreatment from the programmed cycles did not negatively influence the pregnancy outcome or implantation rate.     The pregnancy outcome in patients with minor β-thalassemia

    Background: β-thalassemia is the most common hereditary disease in Iran and more than 2 million carriers of the β-thalassemia mutant gene are living in this country.Objective: To determine pregnancy outcome of women with β-thalassemia minor.Materials and Methods: In this retrospective, case-control study in two universities affiliated hospitals in Shiraz, all pregnancies occurred between 2006 and 2008 were included. Patients were divided in two groups regarding the presence of β-thalassemia minor. Patients in case and control groups were matched according to maternal age, gestational age and number of previous pregnancies. Cesarean delivery, hypertensive disorders, gestational diabetes mellitus, premature rupture of membranes and preterm labor were recorded in each group and were compared using the χ2 or Fisher exact tests. Results: Overall 510 β-thalassemia minor subjects and 512 healthy controls were studied. Cases with β-thalassemia minor had significantly higher prevalence of oligohydramnios (p<0.001) and cesarean section delivery (p=0.001). There was no significant difference regarding Apgar score in 1st (p=0.65) and 5th minute (p=0.25), IUGR (p=0.073), gestational diabetes mellitus (DM) (p=0.443) and preeclampsia (p=0.116) between two study groups. Conclusion: β-thalassemia minor does not significantly influence the pregnancy outcome in the negative way

    Use of endoanal ultrasound as complimentary evaluation for detection of anal sphincter injury after vaginal birth

    Purpose: Anal sphincter injury after delivery is the main factor in the pathogenesis of fecal incontinence. Clinical obvious and specific injury to anal canal sphincter is seen in 3% of vaginal deliveries. There are many women who do not have a clear and specific laceration but they are damaged by sphincter muscles of anal canal. The purpose of the present study is to investigate the frequency of occult anal sphincter injury after vaginal delivery by Endo-anal sonography. Methods: Fifty women with first pregnancy were assessed at 27–33 weeks of pregnancy, and at 6 weeks and 6 months after vaginal delivery by questionnaire, examination and Endo-anal sonography. Women age, duration of delivery, the effect of epidural anesthesia, episiotomy and birth weight were studied and Endo-anal sonography results were recorded. Anal manometry was performed for all mothers before delivery and 5 ones with sphincter injury at 6 months and 3 years after delivery. Results: Five (10%) patients, with mean age 29.4 ± 6.5 years, mean neonatal weight of 3874 ± 287, and mean duration of delivery 11.6 ± 1.51 h, had signs of sphincter injury in Endo-anal sonography. The injury was persisted at six months after delivery. Also, significant differences were seen between anal manometry before delivery and 6 months and 3 years after delivery (p = 0.006 for mean squeezing pressure) in the five mothers. Conclusion: Endo-anal sonography might be a good screening tool for early detection of postpartum anal sphincter damages. However, further prospective cost benefit studies should be performed to propose it as a standard of care. Resumo: Finalidade: A lesão de esfíncter anal após o parto é o fator principal na patogênese da incontinência fecal. Observa-se uma lesão clínica óbvia e específica ao esfíncter no canal anal em 3% dos partos vaginais. Em muitas mulheres não se percebe uma laceração nítida e específica, mas houve lesão nos músculos esfinctéricos do canal anal. A finalidade desse estudo é investigar a frequência de lesão oculta de esfíncter no canal anal em seguida ao parto vaginal por meio da ultrassonografia endoanal. Métodos: Cinquenta mulheres primíparas foram avaliadas no período de 27-33 semanas de gestação e também a 6 semanas e 6 meses após o parto vaginal por meio de questionário, exame e ultrassonografia endoanal. Foram anotados a idade das pacientes, a duração do parto, o efeito da anestesia epidural, episiotomias e peso do bebê ao nascer; também foram registrados os resultados da ultrassonografia endoanal. Antes do parto, todas as gestantes foram submetidas a um exame de manometria; e 5 mães com lesão esfinctérica também passaram por esse procedimento a 6 meses e 3 anos após o parto. Resultados: Cinco (10%) pacientes, com média de idade = 29,4 ± 6,5 anos, peso médio do bebê ao nascer = 3874 ± 287 gramas e duração média do parto = 11,6 ± 1,51 horas, apresentavam sinais de lesão esfinctérica ao exame por ultrassonografia endoanal. Seis meses após o parto, as lesões persistiam. Também foram observadas diferenças significativas entre a manometria anal antes do parto e a 6 meses e 3 anos após o parto (p = 0,006 para média de pressão de contração) nas cinco mães. Conclusão: A ultrassonografia endoanal pode ser um bom instrumento de triagem para a detecção precoce de lesões do esfíncter anal no pós-parto. Contudo, é importante que sejam realizados novos estudos prospectivos e de custo-benefício, para que essa técnica possa ser proposta como padrão terapêutico.     Metformin versus chromium picolinate in clomiphene citrate-resistant patients with PCOs: A double-blind randomized clinical trial

    Background: Chromium picolinate could be effective in clomiphen citrate resistant PCOS patients. Objective: To compare the effects of chromium picolinate vs. metformin in clomiphen citrate resistant PCOS patients. Materials and Methods: The present randomized clinical trial was performed on 92 women with clomiphen citrate-resistant PCOS at the clinics which were affiliated to Shiraz University of Medical Sciences, Shiraz, Iran. The subjects were randomly assigned to two groups receiving either chromium picolinate (200μg daily) or metformin (1500mg daily) for 3 months. Anthropometric and hormonal profile were measured and compared both before and after the treatment. Ovulation and pregnancy rate was measured in the two study groups, as well. Results: Chromium picolinate significantly decreased fasting blood sugar (FBS) after 3 months of treatment (p=0.042). In the same way, the serum levels of fasting insulin had significantly decreased leading to an increase in insulin sensitivity as measured by QUICKI index (p=0.014). In comparison to the patients who received chromium picolinate, those who received metformin had significantly lower levels of testosterone (p=0.001) and free testosterone (p=0.001) after 3 months of treatment. Nevertheless, no significant difference was found between the two study groups regarding ovulation (p=0.417) and pregnancy rates (p=0.500). Conclusion: Chromium picolinate decreased FBS and insulin levels and, thus, increased insulin sensitivity in clomiphene citrate-resistance PCOS women. These effects were comparable with metformin; however, metformin treatment was associated with decreased hyperandrogenism. Overall, chromium picolinate was better tolerated compared to metformin; nonetheless, the two study groups were not significantly different regarding ovulation and pregnancy rates

    Association between Angiotensinogen M235T Polymorphism and Preeclampsia in Iranian Pregnant Women.

    Objective:To determine the possible association between the M235T variant of angiotensinogen gene and preeclampsia in Iranian preeclamtic women with hypertension during pregnancy. Mand methods:During a case control study, we used polymerase chain reaction-based restriction fragment length polymorphism (PCR-RFLP) analysis to investigate the association between M235T polymorphism in preeclamtic women compared to normotensive controls. Results:The M235T polymorphism was significantly associated with increased preeclampsia risk in the studied population as supported by a p value of 0.017 and chi-square value of 8.12. The frequency of mutated allele and genotype distribution showed a significant difference between preeclamtic women and control groups. Conclusion:The result indicates that the AGT M235T polymorphism plays a significant role in preeclampsia observed in selected Iranian preeclamtic women, and it can be considered as a major risk factor for preeclampsia

    Serum HER2 Level in Epithelial Ovarian Cancer

    Introduction: The proto-oncogene HER2 plays a key role in the control ofcellular proliferation. Its overexpression has been reported to be associated with apoor prognosis in cancer, particularly in breast cancer. Materials and Methods:In the present study, serum HER2 levels wereinvestigated in patients diagnosed with epithelial ovarian cancer. Serum HER2levels were detected by an ELISAcommercial kit in 51 patients and 33 healthyindividuals. Results: The mean serum HER2 level was found to be significantly higher inpatients than healthy controls (P=0.005). In 29% of patients, serum HER2 levels werehigher than the cut-off value. HER2 serum level was not associated with tumor stageat diagnosis. Conclusion:Elevation of HER2 in a high proportion of patients with epithelialovarian cancer further strengthens the importance of this molecule in the pathogenesisof ovarian cance
