    Penetrating tracheal injuries – A Kashmir experience

    Background: The increased incidence of tracheal injuries encountered during war like situation in the valley of Kashmir has prompted us to conduct this study.Method: The study was conducted retrospectively on 29 patients managed in the past in our hospital and 7 patients were managed after the study was undertaken. After recording the observations made in managing the patients of penetrating tracheal trauma, the results were analyzed and entered in the individual patients’ protocol.Results: There was a predominance of males with a male to female sex ratio of 5: 1. The majority (69% had fire arms trauma. Hissing wounds, respiratory distress and haemoptysis were the commonest presenting complaints. There were 4 deaths. Complications were observed in 16 (50%) of the 36 patients who survived and included hoarseness in 6 and tracheal stenosis in 5 cases.Conclusion: It was concluded that earliest possible diagnosis followed by securing ofairway and prompt repair is the mainstay of management of penetrating tracheal injuries

    Tratamiento antifúngico tópico en pacientes con candidosis crónica bucal. Estudio comparativo.

    El objetivo del presente trabajo fue evaluar el efecto del fenticonazol en el tratamiento tópico de las candidosis crónicas bucales y compararlo con el del ketoconazol y la nistatina. Se incluyeron ochenta pacientes con candidosis crónica eritematosa, de los cuales cincuenta y uno finalizaron la prueba. Fueron divididos en cuatro grupos a los cuales se les administró: fenticonazol al 3%, fenticonazol al 2%, nistatina 100000 UI y ketoconazol al 2%, en orabase respectivamente. Se los controló a los 7, 15, 30 y 45 días. Los datos obtenidos se analizaron estadísticamente con el ANAVA y los test de Kruskall Wallis y Wilcoxon. Se encontró una disminución de las lesiones altamente significativa en todos los grupos de pacientes (p≤ 0,0001). Se analizó el grado de remisión según la localización de las lesiones; se encontró que en todos los pacientes las localizadas en mucosa yugal y comisura alcanzaron la remisión total, mientras que las lesiones de lengua y paladar mostraron una disminución significativa de la intensidad de las mismas (p≤ 0,00001) con todos los tratamientos. El fenticonazol demostró ser tan efectivo como la nistatina y el ketoconazol en el tratamiento tópico de las candidosis orales.publishedVersio

    First Measurement of Z/gamma* Production in Compton Scattering of Quasi-real Photons

    We report the first observation of Z/gamma* production in Compton scattering of quasi-real photons. This is a subprocess of the reaction e+e- to e+e-Z/gamma*, where one of the final state electrons is undetected. Approximately 55 pb-1 of data collected in the year 1997 at an e+e- centre-of-mass energy of 183 GeV with the OPAL detector at LEP have been analysed. The Z/gamma* from Compton scattering has been detected in the hadronic decay channel. Within well defined kinematic bounds, we measure the product of cross-section and Z/gamma* branching ratio to hadrons to be (0.9+-0.3+-0.1) pb for events with a hadronic mass larger than 60 GeV, dominated by (e)eZ production. In the hadronic mass region between 5 GeV and 60 GeV, dominated by (e)egamma* production, this product is found to be (4.1+-1.6+-0.6) pb. Our results agree with the predictions of two Monte Carlo event generators, grc4f and PYTHIA.Comment: 18 pages, LaTeX, 5 eps figures included, submitted to Physics Letters

    Background: Biliary tract disease is very common in Indian side of Jammu and Kashmir and because of this we got prompted to undertake a study to find out the association between presenting features and bactibilia. Methods: The study was conducted prospectively on 121 patients of various biliary tract disorders in the Department of Surgery, Government Medical College Jammu, India prospectively over a period of one year from March 2003-February 2004. Details of age, history, radiology, operation and postoperative course were noted and information obtained recorded on special forms. All the patients included in this study were given single shot of preoperative antibiotic at the induction of anaesthesia and antibiotics modified postoperatively on the culture report. Results; After recording the observations made while managing these patients of biliary tract disease the results were analysed and entered into the individual patient’s protocol. The age range of our series of 121 patients was 15-74 years with a mean age of 42.5 years. There were 100 females and 21 males with male to female ratio of 4.7:1. Abdominal pain was the commonest symptom (98.3%) followed by dyspepsia (47.1%), abdominal lump (16.5%), jaundice (9.9%) and cholangitis (3.3%).Most of the patients had more than one presentation. Out of 121 cases 42(34.7%) turned out to be culture positive and rest of the cases turned out to be culture negative. In culture positive cases aerobes were identified in 38 (34.1%) and anaerobes were identified in 10 (8.2%) patients. The results of gram stain of bile were compared with bile cultures. Out of 121 patients gram staining was positive in 51(42.1%) patients. Among these patients bile cultures were positive in 42 (82.3%) patients. Conclusion: It was concluded that the presence or absence of bactibilia can predict possibility to stratify patients in low and high risk group so that necessary pre-emptive measures be taken
