Assessment of serum cystatin C in the early detection of type 2 diabetic nephropathy in Cotonou, Benin
Introduction: Diabetic nephropathy is a frequent and dreaded complication of diabetes mellitus. The purpose of this work was to study the role of serum cystatin C in the early detection of diabetic nephropathy among type 2 diabetic patients.Methods: This was a cross-sectional study conducted in Cotonou over a period of six months. Blood samples were tested at the regional food safety testing analysis laboratory. Type 2 diabetic patients older than 15 years, who gave their informed consent, were included in the study. Patients with proven proteinuria, acute kidney injury, haematuria, a positive urine test for nitrite, or reduced glomerular filtration rate <60 mL/min/1.73 m2 were excluded from the study. All patients were subjected to serum cystatin C and microalbuminuria assays.Results: Eighty-eight patients were included in the study. Their average age was 50.7 ± 9.6 years and the male to female ratio was 1.4:1. Twenty-four-hour microalbuminuria was positive in 53 (60%) cases whereas serum cystatin C tested positive in only 2 cases. Sensitivity and specificity tests applied to cystatin C showed very low sensitivity (4%) with a positive predictive value of 100% and high specificity (100%) with a negative predictive value of 41%.Conclusions: When compared with 24-hour microalbuminuria, serum cystatin C assay was not sensitive enough to prove suitable for screening for diabetic nephropathy. Serum cystatin C would therefore not be useful for the early detection of nephropathy among type 2 diabetic patients
Transplantation rénale au Maroc: l’hémodialysé et son entourage sont-ils suffisamment informés ?
Introduction: la transplantation rénale constitue le traitement idéal de l'insuffisance rénale chronique. Cependant il existe une insuffisance de donneurs contrairement aux receveurs dont le nombre ne cesse de s'accroitre. La méconnaissance de la transplantation par les hémodialysés et leur entourage pourrait être l'une des causes. Notre but était d'évaluer les connaissances et opinions de l'hémodialysé et de son entourage sur la transplantation rénale.
Méthodes: l'enquête menée en Aout 2013 avait inclus 83 hémodialysés de notre centre et 70 membres de leur entourage. Ils ont été soumis à un questionnaire qui portait sur les thèmes suivants: statut socio-économique, volonté d'être transplanté ou d'être donneur, avantages de la transplantation rénale, point de vue de la religion sur la transplantation. Aucun des individus interrogés n'avait jamais fait l'objet d'une transplantation ou d'un don d'organe.
Résultats: parmi 83 hémodialysés on notait 49,4 % de femmes avec une moyenne d'âge de 41,4±12 ans. Le niveau socio-économique bas représentait 66,7 %. Le manque d'information chez était estimé à 62,7 %. Seuls 41% se disaient être candidat à la transplantation rénale mais 12 % seulement était inscrit sur la liste d'attente de greffe. La transplantation était estimée plus couteuse que l'hémodialyse par 50,6 % des patients et seuls 71,1 % estimaient qu'elle offrait une meilleure vie. Pour 20,5 %, l'Islam s'opposerait au don cadavérique et pour 10,9 % au don vivant. Parmi les 70 membres de l'entourage interrogé il y avait 56,8% de femmes ; la moyenne d'âge était de 44,4±10,5 ans. Le niveau économique bas représentait 52,3 %. Le manque d'information était estimé à 61,4 %. Pour 56,8 % la vie serait impossible avec un seul rein. Seuls 13,6 % étaient inscrit sur le registre de don. Pour 45,5 % l'Islam s'opposerait au don cadavérique.
Conclusion: il importe d'intensifier la sensibilisation des hémodialysées et leur famille sur la transplantation rénale
Renal Leiomyosarcoma, a Rare Presentation
Renal sarcomas are very rare malignant tumours with a very poor prognosis. Renal leiomyosarcoma, a malignant tumour of smooth muscle origin, is the most common histological type. The article reports a case of leiomyosarcoma of renal location, with a review of the literature. A 38-year-old female patient, with no previous pathological history, consulted the nephrology department of the Teaching Hospital of Lomé for abdominal pain that had been present for 4 years. Histology showed a tumour proliferation of fasciculated architecture, made of spindle cells arranged in long bundles, with cytonuclear atypia and cytoarchitectural abnormalities. Immunohistochemical examination showed positive staining for smooth muscle actin, h-caldesmone, desmin and CD34 and negative for pancytokeratin (AE1/AE3), myogenin and PS100. Renal leiomyosarcoma is an exceptional malignancy. It remains the most common renal sarcoma, the differential diagnosis of which is based on immunohistochemical findings
Hémodialyse chronique et dépression au Centre Hospitalier Universitaire Sylvanus Olympio de Lomé (Togo)
Introduction: estimer la prévalence de la dépression et rechercher les facteurs associés chez les patients insuffisants rénaux chroniques en hémodialyse. Méthodes: étude transversale descriptive allant du 1er Janvier 2014 au 31 Décembre 2014 à l’unité d’hémodialyse du service de Néphrologie du CHU Sylvanus Olympio de Lomé (Togo). L’échelle d’autoévaluation de la dépression de Beck dans sa version simplifiée a été notre outil d’évaluation. Résultats: durant la période d’étude, 88 patients ont été recrutés dont 61,4% d’hommes soit un sex-ratio de 1,6. La moyenne d’âge a été de 38,80 ± 13,24 ans avec des extrêmes de 12 et 66 ans. La majorité des patients (90,9%) étaient des travailleurs. L’hypertension artérielle a été la comorbidité somatique la plus retrouvée (45,4%). Quarante-six patients (52,3%) avaient une durée en hémodialyse comprise entre 1 et 4 ans. La dépression touchait 68,2% des patients; 47,7% des patients déprimés avaient une dépression sévère. La survenue de la dépression était significativement liée à la durée en hémodialyse (p= 0,008). Conclusion: la prise en charge du patient hémodialysé chronique doit être pluridisciplinaire incluant le néphrologue et le psychiatre.The Pan African Medical Journal 2016;2
A de novo Randall disease in a kidney transplant recipient: A case report
Introduction: Randall disease called “monoclonal immunoglobulin deposition disease (MIDD)” and is defined histologically by the presence of deposits immunoglobulin (Ig) monoclonal, diffuse and continuous linear, non-organized within the tubular basement membranes, and frequently glomerular basement and vascular, often associated with accumulation of extracellular matrix. Only 11 cases of de novo Randall disease post-renal transplantation have been reported in the literature.
Case presentation: We report a case of de novo Randall kidney and liver disease associated with myeloma diagnosed in the 20th month post-renal transplant in a woman of 49 years. The treatment had consisted of a decrease of immunosuppression and put on chemotherapy. The evolution was marked by graft loss with return to dialysis. The death occurred in a context of septic shock after the first cure of chemotherapy.
Conclusion: A de novo Randall disease post-renal transplantation is not only very rare, but also there is no consensus of it is a management
