    The Development of a Nutrition Screening Tool for Mental Health Settings Prone to Obesity and Cardiometabolic Complications: Study Protocol for the NutriMental Screener

    People living with serious mental illness (SMI) experience physical health complications at disproportionate rates to people without an SMI. Unhealthy dietary intake and disordered eating behaviors are key driving factors. There is a lack of valid nutrition-risk screening tools targeted to mental health services, and typically used nutrition-risk screening tools are not suitable for mental health services. This paper details the rationale and study protocol for development and validation of the NutriMental screener, a tool for use in clinical practice to identify service users who are at risk for common nutrition issues experienced by this population group and trigger referral to a specialist clinician. The development process includes five phases. Phase I is the development of nutrition-related domains of interest from screening tools used in mental health services. Phase II involves a literature review and service-user interviews to identify additional domains. Phase III consists of international workshops with relevant clinicians and persons with SMI to gain a consensus on questions to be included in the draft tool. Phase IV involves conducting multinational feasibility and preliminary validation studies. Phase V consists of performing formal validation studies. The development of a nutrition-risk screening tool for mental health services is a necessary step to help rectify the physical-health disparities and life-expectancy gap for people with SMI

    Analysis of a distributed fiber-optic temperature sensor using single-photon detectors

    We demonstrate a high-accuracy distributed fiber-optic temperature sensor using superconducting nanowire single-photon detectors and single-photon counting techniques. Our demonstration uses inexpensive single-mode fiber at standard telecommunications wavelengths as the sensing fiber, which enables extremely low-loss experiments and compatibility with existing fiber networks. We show that the uncertainty of the temperature measurement decreases with longer integration periods, but is ultimately limited by the calibration uncertainty. Temperature uncertainty on the order of 3 K is possible with spatial resolution of the order of 1 cm and integration period as small as 60 seconds. Also, we show that the measurement is subject to systematic uncertainties, such as polarization fading, which can be reduced with a polarization diversity receiver

    Im nachfolgenden Artikel beschreiben wir einige Aspekte unserer Arbeit in der Fadenspule, einem Empfangs- und Begegnungsort für kleine Kinder und ihre Familien in Zürich, der von einer Gruppe psychoanalytisch orientierter Fachpersonen seit zehn Jahren angeboten wird. Wir beziehen uns dabei auf die « Maison Verte » und die Ideen der damaligen Gründer, Françoise Dolto und ihren Mitstreitern in Paris. Im Zentrum stehen Gedanken zu den vielfältigen Übergängen, die zur Elternschaft und frühen Entwicklung der Kinder gehören. Das Angebot eines Containings und die Möglichkeit zu triangulieren scheint uns dabei bedeutsam zu sein. Die Kinder als Subjekte liegen uns besonders am Herzen. Am Beispiel ihrer frühen Äusserungen versuchen wir darzustellen, wie ihre Anliegen mehr Raum bekommen können

    Photo(geno)toxicity changes associated with hydroxylation of the aromatic chromophores during diclofenac metabolism

    [EN] Diclofenac (DCF) can cause adverse reactions such as gastrointestinal, renal and cardiovascular disorders; therefore, topical administration may be an attractive alternative to the management of local pain in order to avoid these side effects. However, previous studies have shown that DCF, in combination with sunlight, displays capability to induce photosensitivity disorders. In humans, DCF is biotransformed into hydroxylated metabolites at positions 4¿ and 5 (4¿OH-DCF and 5OH-DCF), and this chemical change produces non negligible alterations of the drug chromophore, resulting in a significant modification of its light-absorbing properties. In the present work, 5OH-DCF exhibited higher photo(geno)toxic potential than the parent drug, as shown by several in vitro assays (3T3 NRU phototoxicity, DNA ssb gel electrophoresis and COMET), whereas 4¿OH-DCF did not display significant photo(geno)toxicity. This could be associated, at least partially with their more efficient UV-light absorption by 5OH-DCF metabolite and with a higher photoreactivity.     Results of final focus test beam

    International audienceThe beam experiments of Final Focus Test Beam (FFTB) started in September 1993 at SLAC, and have produced a 1.7 μm×75 nm spot of 46 GeV electron beam. A number of new techniques involving two nanometer spot-size monitors have been developed. Several beam diagnostic/tuning schemes are applied to achieve and maintain the small spot. This experiment opens the way toward the nanometer world for future linear collider

    A systematic review of influences on implementation of peer support work for adults with mental health problems

    © 2019, The Author(s). Purpose: The evidence base for peer support work in mental health is established, yet implementation remains a challenge. The aim of this systematic review was to identify influences which facilitate or are barriers to implementation of mental health peer support work. Methods: Data sources comprised online databases (n = 11), journal table of contents (n = 2), conference proceedings (n = 18), peer support websites (n = 2), expert consultation (n = 38) and forward and backward citation tracking. Publications were included if they reported on implementation facilitators or barriers for formal face-to-face peer support work with adults with a mental health problem, and were available in English, French, German, Hebrew, Luganda, Spanish or Swahili. Data were analysed using narrative synthesis. A six-site international survey [Germany (2 sites), India, Israel, Tanzania, Uganda] using a measure based on the strongest influences was conducted. The review protocol was pre-registered (Prospero: CRD42018094838). Results: The search strategy identified 5813 publications, of which 53 were included. Fourteen implementation influences were identified, notably organisational culture (reported by 53% of papers), training (42%) and role definition (40%). Ratings on a measure using these influences demonstrated preliminary evidence for the convergent and discriminant validity of the identified influences. Conclusion: The identified influences provide a guide to implementation of peer support. For services developing a peer support service, organisational culture including role support (training, role clarity, resourcing and access to a peer network) and staff attitudes need to be considered. The identified influences provide a theory base to prepare research sites for implementing peer support worker interventions
