    Factors associated with late presentation of suspected tuberculosis cases to tuberculosis management facilities: The case in Dagoretti district, Nairobi, Kenya

    Background: Tuberculosis is a highly contagious disease accounting for a high number of deaths in the developing countries; its control can beeffectively achieved if individuals with the disease receive adequate and timely treatment. The objective of this study was to determine the factorsassociated with late presentation of suspects to tuberculosis management facilities in Dagoretti district in Nairobi, Kenya.Methods: A cross sectional study was conducted on patients aged 18 years and above attending TB clinics in Dagoretti District, Nairobi Kenya. A total of 426 TB suspects were interviewed. The study covered 8 clinics in Dagoretti district. Analysis was done using SPSS version 16.0 and Epi info version 6, this included Chi Square for Bivariate analysis and Binary Logistic Regression for Multivariate Analysis.Results: Out of the 426 tuberculosis suspects, 248 (58.2%) suspects had delayed in seeking medical care. In Bivariate analysis male gender (P=0.039, O.R=1.51; 95% Confidence Interval; 1.00- 2.27), level of education (Primary class 5-8) (P=0.001, O.R= 2.06; 95% C.I 1.34-3.19) and place of first medical care (drug store) (P= 0.013, O.R=1.63; 95% C.I 1.09-2.46) were all significantly associated with late presentation. After multivariate logistic regression, gender (P=0,019, OR=1.6), level of education (p=0.029, OR=1.26) and place of first medical care (P= 0.01 OR=1.27), were found to be significantly associated with late presentation. Conclusion: This study shows that age, level of education and place of first medical care are the factors associated with late presentation of suspects to tuberculosis management facilities.Key words: Tuberculosis, late presentation, management, suspects, participants, respondents, Keny

    Définition structurale des linéaments par traitement d’image satellitaire : cas du massif de Ngovayang (Sud Cameroun)

    Le massif de Ngovayang représente un domaine clé dans la compréhension géodynamique de la bordure nord du Craton du Congo au sud du Cameroun. Les travaux de télédétection, couplés aux données antérieures de terrain ont permis l’extraction et l’interprétation des linéaments obtenus par images satellitaires aux confins de Lolodorf et Bipindi. La carte des linéaments ainsi obtenue montre sur le plan structural, 821 linéaments disposés suivant trois principales directions ; la direction NW-SE avec 68,4% de fréquence est liée à la foliation générale S1/S2 lors de la deuxième phase de déformation et à un système de plis dissymétriques qualifiés de plis d’accompagnement de la foliation (drap fold) ; La direction NW-SE avec 14,5% d’information représente l’ensemble des structures cassantes (fracture, diaclase, filon, paraclase) attribuée à la phase de déformation terminale D3 ; la direction N-S avec 13,6% de fréquence, correspond à un système de grands décrochements transversaux senestres (faille et cisaillement), transition fragiloductile entre les deux dernières phases de déformation (D2 et D3).Mots-clés : craton du Congo, linéament, massif de Ngovayang

    Heavy Metal Composition Of Commercial Fertilizers From Zaria, Northern Nigeria

    The heavy metal composition of some commercial fertilizers and soil supplements used in Zaria were analysed. The fertilizers analysed include Liquid Organic fertilizer, Palette Organic fertilizer, Urea, Super phosphate, NPK, some of the soil supplements analysed are cow dung and chicken droppings. Using Atomic Absorption Spectrophotometer (AAS) the following range of heavy metal concentrations were obtained: Cu 3.92-38.14mg/l, Mn 0.057-199.17mg/l, Cr 0.19-2.207mg/l, Zn 1.35-83.12mg/l, Fe 5.00-514.18mg/l, Ni 0.063-149mg/l, Pb 9.86-15.52mg/l, Co 0.26-2.87mg/l..Super phosphate had the highest abundance of Iron (514.18mg/l). These values were compared to standards for heavy metals in fertilizers and supplements for other countries and were found to be below the limits. Like other countries, it will be important for Nigeria to have set standards for heavy metals levels for fertilizers used in the country

    Analysis of radiation-induced cell death in head and neck squamous cell carcinoma and rat liver maintained in microfluidic devices

    Objective The aim of this study was to investigate how head and neck squamous cell carcinoma (HNSCC) tissue biopsies maintained in a pseudo in vivo environment within a bespoke microfluidic device respond to radiation treatment. Study Design Feasibility study. Setting Tertiary referral center. Subjects and Methods Thirty-five patients with HNSCC were recruited, and liver tissue from 5 Wistar rats was obtained. A microfluidic device was used to maintain the tissue biopsy samples in a viable state. Rat liver was used to optimize the methodology. HNSCC was obtained from patients with T1-T3 laryngeal or oropharyngeal SCC; N1-N2 metastatic cervical lymph nodes were also obtained. Irradiation consisted of single doses of between 2 Gy and 40 Gy and a fractionated course of 5×2 Gy. Cell death was assessed in the tissue effluent using the soluble markers lactate dehydrogenase (LDH) and cytochrome c and in the tissue by immunohistochemical detection of cleaved cytokeratin18 (M30 antibody). Results A significant surge in LDH release was demonstrated in the rat liver after a single dose of 20 Gy; in HNSCC, it was seen after 40 Gy compared with the control. There was no significant difference in cytochrome c release after 5 Gy or 10 Gy. M30 demonstrated a dose-dependent increase in apoptotic index for a given increase in single-dose radiotherapy. There was a significant increase in apoptotic index between 1×2 Gy and 5×2 Gy. Conclusion M30 is a superior method compared with soluble markers in detecting low-dose radiation-induced cell death. This microfluidic technique can be used to assess radiation-induced cell death in HNSCC and therefore has the potential to be used to predict radiation response

    V<sub>H</sub> replacement in rearranged immunoglobulin genes

    Examples suggesting that all or part of the V&lt;sub&gt;H&lt;/sub&gt; segment of a rearranged V&lt;sub&gt;H&lt;/sub&gt;DJ&lt;sub&gt;H&lt;/sub&gt; may be replaced by all or part of another V&lt;sub&gt;H&lt;/sub&gt; have been appearing since the 1980s. Evidence has been presented of two rather different types of replacement. One of these has gained acceptance and has now been clearly demonstrated to occur. The other, proposed more recently, has not yet gained general acceptance because the same effect can be produced by polymerase chain reaction artefact. We review both types of replacement including a critical examination of evidence for the latter. The first type involves RAG proteins and recombination signal sequences (RSS) and occurs in immature B cells. The second was also thought to be brought about by RAG proteins and RSS. However, it has been reported in hypermutating cells which are not thought to express RAG proteins but in which activation-induced cytidine deaminase (AID) has recently been shown to initiate homologous recombination. Re-examination of the published sequences reveals AID target sites in V&lt;sub&gt;H&lt;/sub&gt;-V&lt;sub&gt;H&lt;/sub&gt; junction regions and examples that resemble gene conversion

    Indirect interaction between two native thistles mediated by an invasive exotic floral herbivore

    Spatial and temporal variation in insect floral herbivory is common and often important. Yet, the determinants of such variation remain incompletely understood. Using 12 years of flowering data and 4 years of biweekly insect counts, we evaluated four hypotheses to explain variation in damage by the Eurasian flower head weevil, Rhinocyllus conicus, to the native North American wavyleaf thistle, Cirsium undulatum. The four factors hypothesized to influence weevil impact were variations in climate, weevil abundance, phenological synchrony, and number of flower heads available, either on wavyleaf thistle or on the other co-occurring, acquired native host plant (Platte thistle, Cirsium canescens), or on both. Climate did not contribute significantly to an explanation of variation in R. conicus damage to wavyleaf thistle. However, climate did influence weevil synchrony with wavyleaf flower head initiation, and phenological synchrony was important in determining R. conicus oviposition levels on wavyleaf thistle. The earlier R. conicus was active, the less it oviposited on wavyleaf thistle, even when weevils were abundant. Neither weevil abundance nor availability of wavyleaf flower heads predicted R. conicus egg load. Instead, the strongest predictor of R. conicus egg load on wavyleaf thistle was the availability of flower heads on Platte thistle, the more common, earlier flowering native thistle in the sand prairie. Egg load on wavyleaf thistle decreased as the number of Platte thistle flower heads at a site increased. Thus, wavyleaf thistle experienced associational defense in the presence of flowering by its now declining native congener, Platte thistle. These results demonstrate that prediction of damage to a native plant by an exotic insect may require knowledge of both likely phenological synchrony and total resource availability to the herbivore, including resources provided by other nontarget native species

    Antibacterial, Anti-HIV-1 Protease and Cytotoxic Activities of Aqueous Ethanolic Extracts from Combretum Adenogonium Steud. Ex A. Rich (Combretaceae).

    \ud \ud Records have shown that Combretum adenogonium Steud. Ex A. Rich (Combretaceae) is used in traditional medicine systems of several tribes in Tanzania. This study focused on the investigation of antibacterial activity, anti-HIV-1 protease activity, toxicity properties and classes of phytochemicals in extracts from C. adenogonium Steud. Ex A. Rich (Combretaceae) to evaluate potential of these extracts for development as herbal remedies. Dried plant material were ground to fine powder and extracted using 80% aqueous ethanol to afford root, leaf and stem bark extracts. The extracts were assayed for anti-HIV-1 protease activities, antibacterial activities using microdilution methods and cytotoxicity using brine shrimps lethality assay. Screening for major phytochemical classes was carried out using standard chemical tests. All extracts exhibited antibacterial activity to at least one of the test bacteria with MIC-values ranging from 0.31-5.0 mg/ml. Two extracts, namely, root and stem bark exhibited anti-HIV-1 PR activity with IC50 values of 24.7 and 26.5 μg/ml, respectively. Stem bark and leaf extracts showed mild toxicity with LC50 values of 65.768 μg/ml and 76.965 μg/ml, respectively, whereas roots were relatively non-toxic (LC50 = 110.042 μg/ml). Phytochemical screening of the extracts indicated presence of flavonoids, terpenoids, alkaloids, tannins, glycosides and saponins. These results provide promising baseline information for the potential development of C. adenogonium extracts in treatment of bacterial and HIV/AIDS-related opportunistic infections

    A randomised controlled trial and cost-effectiveness evaluation of "booster" interventions to sustain increases in physical activity in middle-aged adults in deprived urban neighbourhoods

    Background: Systematic reviews have identified a range of brief interventions which increase physical activity in previously sedentary people. There is an absence of evidence about whether follow up beyond three months can maintain long term physical activity. This study assesses whether it is worth providing motivational interviews, three months after giving initial advice, to those who have become more active. Methods/Design: Study candidates (n = 1500) will initially be given an interactive DVD and receive two telephone follow ups at monthly intervals checking on receipt and use of the DVD. Only those that have increased their physical activity after three months (n = 600) will be randomised into the study. These participants will receive either a "mini booster" (n = 200), "full booster" (n = 200) or no booster (n = 200). The "mini booster" consists of two telephone calls one month apart to discuss physical activity and maintenance strategies. The "full booster" consists of a face-to-face meeting with the facilitator at the same intervals. The purpose of these booster sessions is to help the individual maintain their increase in physical activity. Differences in physical activity, quality of life and costs associated with the booster interventions, will be measured three and nine months from randomisation. The research will be conducted in 20 of the most deprived neighbourhoods in Sheffield, which have large, ethnically diverse populations, high levels of economic deprivation, low levels of physical activity, poorer health and shorter life expectancy. Participants will be recruited through general practices and community groups, as well as by postal invitation, to ensure the participation of minority ethnic groups and those with lower levels of literacy. Sheffield City Council and Primary Care Trust fund a range of facilities and activities to promote physical activity and variations in access to these between neighbourhoods will make it possible to examine whether the effectiveness of the intervention is modified by access to community facilities. A one-year integrated feasibility study will confirm that recruitment targets are achievable based on a 10% sample.Discussion: The choice of study population, study interventions, brief intervention preceding the study, and outcome measure are discussed
