    Study protocol for a group randomized controlled trial of a classroom-based intervention aimed at preventing early risk factors for drug abuse: integrating effectiveness and implementation research

    <p>Abstract</p> <p>Background</p> <p>While a number of preventive interventions delivered within schools have shown both short-term and long-term impact in epidemiologically based randomized field trials, programs are not often sustained with high-quality implementation over time. This study was designed to support two purposes. The first purpose was to test the effectiveness of a universal classroom-based intervention, the Whole Day First Grade Program (WD), aimed at two early antecedents to drug abuse and other problem behaviors, namely, aggressive, disruptive behavior and poor academic achievement. The second purpose--the focus of this paper--was to examine the utility of a multilevel structure to support high levels of implementation during the effectiveness trial, to sustain WD practices across additional years, and to train additional teachers in WD practices.</p> <p>Methods</p> <p>The WD intervention integrated three components, each previously tested separately: classroom behavior management; instruction, specifically reading; and family-classroom partnerships around behavior and learning. Teachers and students in 12 schools were randomly assigned to receive either the WD intervention or the standard first-grade program of the school system (SC). Three consecutive cohorts of first graders were randomized within schools to WD or SC classrooms and followed through the end of third grade to test the effectiveness of the WD intervention. Teacher practices were assessed over three years to examine the utility of the multilevel structure to support sustainability and scaling-up.</p> <p>Discussion</p> <p>The design employed in this trial appears to have considerable utility to provide data on WD effectiveness and to inform the field with regard to structures required to move evidence-based programs into practice.</p> <p>Trial Registration</p> <p><b>Clinical Trials Registration Number</b>: NCT00257088</p

    Comparative Transcriptional and Genomic Analysis of Plasmodium falciparum Field Isolates

    Mechanisms for differential regulation of gene expression may underlie much of the phenotypic variation and adaptability of malaria parasites. Here we describe transcriptional variation among culture-adapted field isolates of Plasmodium falciparum, the species responsible for most malarial disease. It was found that genes coding for parasite protein export into the red cell cytosol and onto its surface, and genes coding for sexual stage proteins involved in parasite transmission are up-regulated in field isolates compared with long-term laboratory isolates. Much of this variability was associated with the loss of small or large chromosomal segments, or other forms of gene copy number variation that are prevalent in the P. falciparum genome (copy number variants, CNVs). Expression levels of genes inside these segments were correlated to that of genes outside and adjacent to the segment boundaries, and this association declined with distance from the CNV boundary. This observation could not be explained by copy number variation in these adjacent genes. This suggests a local-acting regulatory role for CNVs in transcription of neighboring genes and helps explain the chromosomal clustering that we observed here. Transcriptional co-regulation of physical clusters of adaptive genes may provide a way for the parasite to readily adapt to its highly heterogeneous and strongly selective environment

    Potential cellular and biochemical mechanisms of exercise and physical activity on the ageing process

    Exercise in young adults has been consistently shown to improve various aspects of physiological and psychological health but we are now realising the potential benefits of exercise with advancing age. Specifically, exercise improves cardiovascular, musculoskeletal, and metabolic health through reductions in oxidative stress, chronic low-grade inflammation and modulating cellular processes within a variety of tissues. In this this chapter we will discuss the effects of acute and chronic exercise on these processes and conditions in an ageing population, and how physical activity affects our vasculature, skeletal muscle function, our immune system, and cardiometabolic risk in older adults

    Stress associated gene expression in blood cells is related to outcome in radiotherapy treated head and neck cancer patients

    <p>Abstract</p> <p>Background</p> <p>We previously observed that a radiotherapy-induced biochemical response in plasma was associated with favourable outcome in head and neck squamous carcinoma cancer (HNSCC) patients. The aim of the present study was to compare stress associated blood cell gene expression between two sub-groups of HNSCC patients with different biochemical responses to radiotherapy.</p> <p>Methods</p> <p>Out of 87 patients (histologically verified), 10 biochemical ‘responders’ having a high relative increase in plasma oxidative damage and a concomitant decrease in plasma antioxidants during radiotherapy and 10 ‘poor-responders’ were selected for gene-expression analysis and compared using gene set enrichment analysis.</p> <p>Results</p> <p>There was a significant induction of stress-relevant gene-sets in the responders following radiotherapy compared to the poor-responders. The relevance of the involvement of similar stress associated gene expression for HNSCC cancer and radioresistance was verified using two publicly available data sets of 42 HNSCC cases and 14 controls (GEO GSE6791), and radiation resistant and radiation sensitive HNSCC xenografts (E-GEOD-9716).</p> <p>Conclusions</p> <p>Radiotherapy induces a systemic stress response, as revealed by induction of stress relevant gene expression in blood cells, which is associated to favourable outcome in a cohort of 87 HNSCC patients. Whether these changes in gene expression reflects a systemic effect or are biomarkers of the tumour micro-environmental status needs further study.</p> <p>Trial registration</p> <p>Raw data are available at ArrayExpress under accession number E-MEXP-2460.</p

    Productivity differences among loblolly pine genotypes are independent of individual-tree biomass partitioning and growth efficiency

    Genetic differences in individual-tree biomass partitioning, growth efficiency, and stem relative growth rate (RGR) could confer intraspecific productivity differences and might strongly influence forest ecosystem carbon storage. We examined the relationship between genotype productivity (stem volume), whole-tree biomass partitioning, growth efficiency (stem wood production per unit leaf area), and stem RGR among nine different loblolly pine (Pinus taeda L.) genotypes from three different genetic groups of contrasting inherent genetic homogeneity: three open-pollinated (half-sib) families, three mass-control pollinated (full-sib) families, and three clonal varieties. We hypothesized that genotype productivity would be positively associated with increased partitioning to stem wood relative to other plant parts, higher stem RGR, and enhanced growth efficiency. After 3 years under plantation conditions, genotypes showed significant differences in stem volume, percent stem wood, percent branch wood, and partitioning to fine roots, yet no differences in stem RGR or growth efficiency. Furthermore, genotypic differences in stem volume were independent of genotypic differences in biomass partitioning, and overall, we found no evidence to support the hypothesized relationships. Even so, the observed variation in biomass partitioning has implications for forest C sequestration as genotypes which partition more biomass to long-lived biomass pools such as stems, may sequester more C. Moreover, the lack of a genetic relationship between stem volume and belowground partitioning suggests that highly productive genotypes may be planted without compromising belowground C storage
