    A questionnaire study on the acceptability of self-sampling versus screening by clinicians for Group B Streptococcus

    BACKGROUND: Group B Streptococcus (GBS) is a leading cause of neonatal morbidity and mortality. In an effort to reduce the impact of this serious affliction, universal screening for GBS has been adopted in many countries. The objective of this study was to examine the acceptability of self-collected GBS swabs in a local population in Hong Kong. METHODS: This study is a cross-sectional questionnaire survey conducted in a tertiary teaching hospital. A total of 327 pregnant women who attended the antenatal clinic for GBS screening from April 2012 to May 2012 were included in our study. The acceptability of GBS self-screening and its associated factors were analyzed. RESULTS: Of these women, 200/320 (62.5%) participants preferred screening by healthcare workers, whereas only 18/320 (5.6%) preferred self-screening. The most common reasons why some participants preferred to be screened by clinicians were that professionals had greater knowledge, and the added worry about the accuracy of self-screening. 22/320 (69.4%) and 195/320 (60.9%) women believed that they felt comfortable enough to self-perform the vaginal swab and rectal swab respectively. Previous use of tampons was associated with higher perceived capability of self-performing the vaginal swab. Perceived capability to self-perform the rectovaginal swabs was associated with a willingness to self-perform the swabs (p < 0.001). The majority of women, 303/320 (94.7%), found the current practice of an additional clinic visit for GBS screening acceptable. However, 218/320 (68.1%) participants would like to undertake self-screening if they were given the chance to do so in the next pregnancy, and 187/320 (58.4%) would recommend others have self-screening for GBS as well. CONCLUSION: Most women in the local population still preferred physician-collected samples for GBS screening, but they welcomed the option of self-screening in future pregnancies. Improved health education about the importance of GBS screening may improve the willingness of women to perform self-screening.published_or_final_versio

    Effects of Age on Plasma Glucose Levels in Non-diabetic Hong Kong Chinese

    Aim: To analyze the relationship between age and plasma glucose levels in Hong Kong Chinese population. Methods: We performed a cross-sectional study with 15 603 non-diabetic subjects from the community of Hong Kong. Plasma glucose concentration was measured in blood specimens from the subjects. The time of blood taking varied, depending on the availability of the patients. Results: There were 11 148 (71.4%) women and 4455 (28.6%) men (mean age: 50.1 ± 16.1 years). There were 6901 (44.2%) patients who had plasma glucose measured in the fasting condition, 2999 (19.2%) who were 2-hour post-prandial, and 5703 (36.6%) who had plasma glucose measured at a random time. The correlation coefficients between age and plasma glucose levels in fasting, 2-hour post-prandial, and random group of patients were 0.159, 0.169, and 0.114, respectively (adjusted for body mass index, smoking, and gender; all P values <0.001). Fasting and random plasma glucose level increased by 0.15 mmol/L, while 2-hour post-prandial plasma glucose level increased by 0.26 mmol/L per decade-increase in age. Conclusion: Plasma glucose levels progressively increase with age in Hong Kong Chinese non-diabetic subjects

    Effects of Allium sativum and Allium cepa on semen characteristics, sperm reserves and haematology of rabbit bucks

    The effect of dietary inclusion of garlic and onion on semen characteristics, gonadal, extragonadal sperm reserves and haematology of rabbit bucks were evaluated. Twenty-four rabbit bucks of average age and weight 10 ± 2.0 months and 1.47 ± 0.01 kg respectively, were used for the study. They were randomly assigned into four groups of six bucks each. Group A served as control, while Groups B, C and D received dietary inclusion of 5% garlic, 5% onion, 2.5% garlic + 2.5% onion, respectively. The fresh bulbs of garlic and onions were peeled, air-dried, and the dried bulbs were weighed, added to the feed and grounded together to form experimental diets. Before dietary supplementation, semen samples were collected to serve as baseline values, followed by weekly collections for another 9 weeks using an artificial vagina. Haematological parameters were examined according to the standard procedure, while testosterone profile was conducted using the ELISA method. At the termination of the experiment, two bucks from each group were euthanised, and the testes were harvested to evaluate gonadal and extragonadal sperm reserves. No significant (p &gt; 0.05) difference was recorded in the live weight, sperm concentration, and sperm abnormality of the rabbit bucks. A significant (p &lt; 0.05) difference was observed in the ejaculate volume, gross motility, pH, reaction time, and percentage of live spermatozoa. The epididymal sperm reserves in group B, was significantly (p &lt; 0.05) higher in the right than left. Testosterone profile showed significant (p &lt; 0.05) difference at 9 and 10 a.m. On haematology, there was a significant increase in PCV, RBC count, haemoglobin concentration and WBC count in groups B, C and D by week 9 compared to the control. In conclusion, the dietary inclusion of garlic and onion effectively improved the spermiogram of rabbit bucks

    A novel 16-channel wireless system for electroencephalography measurements with dry spring-loaded sensors

    Understanding brain function using electroencephalography (EEG) is an important issue for cerebral nervous system diseases, especially for epilepsy and Alzheimer's disease. Many EEG measurement systems are used reliably to study these diseases, but their bulky size and the use of wet sensors make them uncomfortable and inconvenient for users. To overcome the limitations of conventional EEG measurement systems, a wireless and wearable multichannel EEG measurement system is proposed in this paper. This system includes a wireless data acquisition device, dry spring-loaded sensors, and a sizeadjustable soft cap. We compared the performance of the proposed system using dry versus conventional wet sensors. A significant positive correlation between readings from wet and dry sensors was achieved, thus demonstrating the performance of the system. Moreover, four different features of EEG signals (i.e., normal, eye-blinking, closed-eyes, and teeth-clenching signals) were measured by 16 dry sensors to ensure that they could be detected in real-life cognitive neuroscience applications. Thus, we have shown that it is possible to reliably measure EEG signals using the proposed system. This paper presents novel insights into the field of cognitive neuroscience, showing the possibility of studying brain function under real-life conditions. © 2014 IEEE

    Large CP Violation in B_s Meson Mixing with EDM constraint in Supersymmetry

    Motivated by the recent measurement of the like-sign dimuon charge asymmetry, we investigate anomalous CP violation in the B_s- bar{B}_s mixing within the supersymmetry. We show that when gluino diagrams dominate supersymmetry contributions, it is very difficult to realize a large B_s- bar{B}_s mixing phase under the constraint from electric dipole moments barring cancellations. This constraint can be ameliorated by supposing superparticles decoupled. In this limit, we find that it is possible to achieve the large CP asymmetry, and the branching ratio of B_s -> mu^+ mu^- tends to become sizable.Comment: 20 pages, 5 figure

    Cholangiocarcinoma: Epidemiology and risk factors

    Cholangiocarcinoma (CCA) is a heterogeneous disease arising from a complex interaction between host-specific genetic background and multiple risk factors. Globally, CCA incidence rates exhibit geographical variation, with much higher incidence in parts of the Eastern world compared to the West. These differences are likely to reflect differences in geographical risk factors as well as genetic determinants. Of note, over the past few decades, the incidence rates of CCA appear to change and subtypes of CCA appear to show distinct epidemiological trends. These trends need to be interpreted with caution given the issues of diagnosis, recording and coding of subtypes of CCA. Epidemiological evidences suggest that in general population some risk factors are less frequent but associated with a higher CCA risk, while others are more common but associated with a lower risk. Moreover, while some risk factors are shared by intrahepatic and both extrahepatic forms, others seem more specific for one of the two forms. Currently some pathological conditions have been clearly associated with CCA development, and other conditions are emerging; however, while their impact in increasing CCA risk as single etiological factors has been provided in many studies, less is known when two or more risk factors co-occur in the same patient. Moreover, despite the advancements in the knowledge of CCA aetiology, in Western countries about 50% of cases are still diagnosed without any identifiable risk factor. It is therefore conceivable that other still undefined etiologic factors are responsible for the recent increase of CCA (especially iCCA) incidence worldwide

    Sensory Electrical Stimulation Improves Foot Placement during Targeted Stepping Post-Stroke

    Proper foot placement is vital for maintaining balance during walking, requiring the integration of multiple sensory signals with motor commands. Disruption of brain structures post-stroke likely alters the processing of sensory information by motor centers, interfering with precision control of foot placement and walking function for stroke survivors. In this study, we examined whether somatosensory stimulation, which improves functional movements of the paretic hand, could be used to improve foot placement of the paretic limb. Foot placement was evaluated before, during, and after application of somatosensory electrical stimulation to the paretic foot during a targeted stepping task. Starting from standing, twelve chronic stroke participants initiated movement with the non-paretic limb and stepped to one of five target locations projected onto the floor with distances normalized to the paretic stride length. Targeting error and lower extremity kinematics were used to assess changes in foot placement and limb control due to somatosensory stimulation. Significant reductions in placement error in the medial–lateral direction (p = 0.008) were observed during the stimulation and post-stimulation blocks. Seven participants, presenting with a hip circumduction walking pattern, had reductions (p = 0.008) in the magnitude and duration of hip abduction during swing with somatosensory stimulation. Reductions in circumduction correlated with both functional and clinical measures, with larger improvements observed in participants with greater impairment. The results of this study suggest that somatosensory stimulation of the paretic foot applied during movement can improve the precision control of foot placement

    Recommendations for a core outcome set for measuring standing balance in adult populations: a consensus-based approach

    Standing balance is imperative for mobility and avoiding falls. Use of an excessive number of standing balance measures has limited the synthesis of balance intervention data and hampered consistent clinical practice.To develop recommendations for a core outcome set (COS) of standing balance measures for research and practice among adults.A combination of scoping reviews, literature appraisal, anonymous voting and face-to-face meetings with fourteen invited experts from a range of disciplines with international recognition in balance measurement and falls prevention. Consensus was sought over three rounds using pre-established criteria.The scoping review identified 56 existing standing balance measures validated in adult populations with evidence of use in the past five years, and these were considered for inclusion in the COS.Fifteen measures were excluded after the first round of scoring and a further 36 after round two. Five measures were considered in round three. Two measures reached consensus for recommendation, and the expert panel recommended that at a minimum, either the Berg Balance Scale or Mini Balance Evaluation Systems Test be used when measuring standing balance in adult populations.Inclusion of two measures in the COS may increase the feasibility of potential uptake, but poses challenges for data synthesis. Adoption of the standing balance COS does not constitute a comprehensive balance assessment for any population, and users should include additional validated measures as appropriate.The absence of a gold standard for measuring standing balance has contributed to the proliferation of outcome measures. These recommendations represent an important first step towards greater standardization in the assessment and measurement of this critical skill and will inform clinical research and practice internationally
