    Patient and Provider Perspectives on Medication Non-adherence Among Patients with Depression and/or Diabetes in Diverse Community Settings - A Qualitative Analysis.

    BACKGROUND: Diabetes and depression affect a significant percentage of the world\u27s total population, and the management of these conditions is critical for reducing the global burden of disease. Medication adherence is crucial for improving diabetes and depression outcomes, and research is needed to elucidate barriers to medication adherence, including the intentionality of non-adherence, to intervene effectively. The purpose of this study was to explore the perspectives of patients and health care providers on intentional and unintentional medication adherence among patients with depression and diabetes through a series of focus groups conducted across clinical settings in a large urban area. METHODS: This qualitative study utilized a grounded theory approach to thematically analyze qualitative data using the framework method. Four focus groups in total were conducted, two with patients and two with providers, over a one-year period using a semi-structured facilitation instrument containing open-ended questions about experiences, perceptions and beliefs about medication adherence. RESULTS: Across the focus groups, communication difficulties between patients and providers resulting in medication non-adherence was a primary theme that emerged. Concerns about medication side effects and beliefs about medication effectiveness were identified as perceptual barriers related to intentional medication non-adherence. Practical barriers to medication adherence, including medication costs, forgetting to take medications and polypharmacy, emerged as themes related to unintentional medication non-adherence. CONCLUSION: The study findings contribute to a growing body of research suggesting health system changes are needed to improve provider education and implement multicomponent interventions to improve medication adherence among patients with depression and/or diabetes, both chronic illnesses accounting for significant disease burden globally

    Influences of various variables on body image satisfaction among highly fertile women and infertile women

    Objective: Involuntarily not being able to conceive a child has always been a social stigma which has resulted in emotional trauma and relationship strain. Body image is affected by infertility. Although several studies have revealed emotional stress in infertile individuals in Turkey and compared western and eastern societies’ view of body image in infertile women, a comparison between highly fertile and infertile women has never been made before. Herein, we aimed to compare body image perception between infertile women and highly fertile women in the southeast part of Turkey. Method: In this study, 67 infertile and 59 highly fertile (women having children more than 5) women were enrolled. Body Image Scale was applied to both groups. Some sociodemographic information such as age, smoking status, and difficulties experienced in infertility processes were evaluated. Results: We showed that Body Image Scale scores including sexual power were significantly higher in highly fertile women. Body image was not associated with the gender of the child/children. The infertile group was found to have less smoking rates than the highly fertile group. Discussion: Therefore, the conclusion drawn deduces that body image is negatively affected by infertility and positively affected by high fertility, especially, in societies where having many children is expected and appreciated

    Second Line Palliative Endobronchial Radiotherapy with HDR Ir 192 in Recurrent Lung Carcinoma

    Purpose To observe the efficiency of reirradiation with high dose rate intraluminal brachytherapy in symptomatic palliation of recurrent endobronchial tumors. Materials and Methods Between January 1994 and June 1998, 21 patients diagnosed with recurrent endobronchial tumors following external beam radiotherapy were treated palliatively with high dose rate intraluminal irradiation at Hacettepe University Oncology Institute. A single fraction of 10 Gy was prescribed to the specified area in 9 patients and 15 Gy to 12. Results Endobronchial treatment improved the performance and reduced symptomatology in 17 (81%) patients. Ten dyspneic patients (10/14, 71%) recovered clinically with an accompanying radiological downstaging. The median symptomatic palliation was 45 days (range, 0 - 9 months), and the overall median survival was 5.5 months (range, 4 - 12 months). The palliative intrabronchial brachytherapy was well tolerated, with the exception of in one patient with a fatal hemorrhage, and another with medically salvaged bronchospasm and intrabronchial edema. Conclusion Recurrent patients with a history of previous thoracic external beam irradiation can be effectively palliated with high dose rate endobronchial reirradiation if the symptoms are directly related to the endobronchial tumor

    Su darbesini idare eden süreklilik ve hareket denklemleri değişken zaman aralıklı karakteristikler yöntemi ile çözülmüştür. Sayısal model Çatalan Hidroelektrik Santralı basınçlı borusuna uygulanmış ve borunun mansap ucunda hesaplanan basınç yüksekliğinin zamana göre değişimleri Francis türbininin test sonuçlan ile karşılaştınlmtşttr. Boru hattında, memba sınır şartı için sabit seviyeli hazne, ve mansap sınır şartı olarak da türbin karakteristikleri kullanılmıştır. Ayrıca yaklaşık bir uygulama olarak türbin ayar kanatlarının bir vana gibi çalıştığı varsayımı sınır şartı olarak alınarak basınç değişimleri hesaplanmıştır. Her iki tüf mansap sınır şartına göre elde edilen bulguların test sonuçları ile uyumlu olduğu görülmüştür.The governing equations of water hammer, continuity and equation of motion, are solved using the variable-grid method of characteristics. The numerical scheme is applied to the penstock of the Catalan Hydroelectric Plant and the computed transient pressure head histories at the end of the pipeline are compared with the test results of the Francis turbine. For the upstream boundary condition of the penstock a constant head reservoir is considered and the turbine characteristics are used for the downstream boundary condition. In addition, as an approximate application, by setting the boundary condition of assuming lhat turbine wicket gates operate like a downstream control valve, the pressure variations are computed. Findings based on both of the downstream boundary conditions are found to agree well with the test results
