Therefore, a highly advanced 4G wireless system is required to meet a high demand for modern mobile terminals. For getting a further improvement for 4G communication systems, new paradigms of design, analysis tools and applications for 4G communication processors are necessary. In this paper, some of these new paradigms are discussed.     Association between Androgen Deprivation Therapy and Risk of Dementia in Men with Prostate Cancer

    The risk of dementia after androgen deprivation therapy (ADT) in patients with advanced prostate cancer (PCa) remains controversial. This study aimed to evaluate the association between ADT and the incidence of dementia in patients with PCa. We identified patients newly diagnosed with PCa in the National Health Insurance Database of Taiwan from 1 January 2002 to 30 June 2016 and in The Health Improvement Network of the United Kingdom (UK) from 1 January 1998 to 31 March 2018. We classified patients with PCa into ADT and ADT-naïve groups. Propensity score (PS) methods were used to minimize the differences in characteristics between the groups. We performed a Cox proportional hazard model to obtain the adjusted hazard ratio (HR) to compare the incidence of dementia between the groups. Our ADT group comprised 8743 and 73,816 patients in Taiwan and the UK, respectively, which were matched 1:1 to ADT-naïve patients by PS. The incidence rates of dementia in the ADT group were 2.74 versus 3.03 per 1000 person-years in the ADT naïve groups in Taiwan, and 2.81 versus 2.79 per 1000 person-years in the UK. There was no statistical difference between ADT and ADT-naïve groups (adjusted HR: 1.12; 95% confidence interval (CI): 0.87–1.43 in Taiwan and adjusted HR: 1.02; 95% CI: 0.85–1.23 in the UK). We found no association between the incidence of dementia and ADT in patients with advanced PCa in either database. Further studies are warranted to evaluate other possible triggers of incident dementia in patients receiving ADT for advanced PCa

    Over-expression of miR-106b promotes cell migration and metastasis in hepatocellular carcinoma by activating epithelial-mesenchymal transition process

    Hepatocellular carcinoma (HCC) is one the the most fatal cancers worldwide. The poor prognosis of HCC is mainly due to the developement of distance metastasis. To investigate the mechanism of metastasis in HCC, an orthotopic HCC metastasis animal model was established. Two sets of primary liver tumor cell lines and corresponding lung metastasis cell lines were generated. In vitro functional analysis demonstrated that the metastatic cell line had higher invasion and migration ability when compared with the primary liver tumor cell line. These cell lines were subjected to microRNA (miRNAs) microarray analysis to identify differentially expressed miRNAs which were associated with the developement of metastasis in vivo. Fifteen human miRNAs, including miR-106b, were differentially expressed in 2 metastatic cell lines compared with the primary tumor cell lines. The clinical significance of miR-106b in 99 HCC clinical samples was studied. The results demonstrated that miR-106b was over-expressed in HCC tumor tissue compared with adjacent non-tumor tissue (p = 0.0005), and overexpression of miR-106b was signficantly correlated with higher tumor grade (p = 0.018). Further functional studies demonstrated that miR-106b could promote cell migration and stress fiber formation by over-expressing RhoGTPases, RhoA and RhoC. In vivo functional studies also showed that over-expression of miR-106b promoted HCC metastasis. These effects were related to the activation of the epithelial-mesenchymal transition (EMT) process. Our results suggested that miR-106b expression contributed to HCC metastasis by activating the EMT process promoting cell migration in vitro and metastasis in vivo. © 2013 Yau et al.published_or_final_versio

    Lost therapeutic potential of monocyte-derived dendritic cells through lost tissue homing: Stable restoration of gut specificity with retinoic acid

However, in-vitro immunological effects are often not mirrored in vivo. We studied the tissue-homing potential of MoDC. Circulating monocytes and DC expressed different tissue-homing markers and, during in-vitro development of MoDC, homing marker expression was lost resulting in a 'homeless' phenotype. Retinoic acid (RA) induced gut-homing markers (β7 and CCR9) and a regulatory phenotype and function [decreased human leucocyte antigen D-related (HLA-DR) and increased ILT3 and fluorescein isothiocyanate (FITC-dextran uptake) in MoDC]. RA-MoDC were less stimulatory and primed conditioned T cells with a gut-homing profile (β7+CLA-). Unlike the normal intestinal microenvironment, that from inflamed colon of ulcerative colitis (UC) patients did not induce regulatory properties in MoDC. However, RA-MoDC maintained their regulatory gut-specific properties even in the presence of UC microenvironment. Therefore, MoDC may be ineffectual for immunotherapy because they lack tissue-homing and tissue-imprinting specificity. However, MoDC rehabilitation with gut-homing potential by RA could be useful in promoting immunotherapy in pathologies such as UC. © 2013 The Authors.     The minimum clinically important difference of the incremental shuttle walk test in bronchiectasis: a prospective cohort study.

    The incremental shuttle walk test (ISW) is an externally-paced field walking test that measures maximal exercise capacity1 and is widely used in patients with chronic obstructive pulmonary disease (COPD) undergoing pulmonary rehabilitation (PR). Its psychometric properties, including reliability, construct validity2 and responsiveness to intervention,2-5 have been demonstrated in patients with bronchiectasis, but little data exist on the minimum clinically important difference (MCID). Although two studies have investigated the MCID of ISW in patients with bronchiectasis, the generalisability of these data is limited because of the study sample characteristics,6 or did not involve an exercise-based intervention.2 The MCID enables clinicians and researchers to understand the clinical significance of change data and forms an important part of the evidence required by regulatory agencies for approval for use in clinical trials.     Inhibitory effect of Chinese green tea on cigarette smoke-induced up-regulation of airway neutrophil elastase and matrix metalloproteinase-12 via antioxidant activity

    Our recent study has indicated that Chinese green tea (Lung Chen), in which epigallocatechin-3-gallate (EGCG) accounts for 60% of catechins, protected cigarette smoke-induced lung injury. We now hypothesized that Lung Chen tea may also have potential effect on lung oxidative stress and proteases/anti-proteases in a smoking rat model. Sprague–Dawley rats were exposed to either sham air (SA) or 4% cigarette smoke (CS) plus 2% Lung Chen tea or water by oral gavage. Serine proteases, matrix metalloproteinases (MMPs) and their respective endogenous inhibitors were determined in bronchoalveolar lavage (BAL) and lung tissues by gelatin/casein zymography and biochemical assays. Green tea consumption significantly decreased CS-induced elevation of lung lipid peroxidation marker, malondialdehyde (MDA), and CS-induced up-regulation of neutrophil elastase (NE) concentration and activity along with that of α1-antitrypsin (α1-AT) and secretory leukoproteinase inhibitor (SLPI) in BAL and lung. In parallel, significant elevation of MMP-12 activity was found in BAL and lung of the CS-exposed group, which returned to the levels of SA-exposed group after green tea consumption but not CS-induced reduction of tissue inhibitor of metalloproteinase (TIMP)-1 activity, which was not reversed by green tea consumption. Taken together, our data supported the presence of local oxidative stress and protease/anti-protease imbalance in the airways after CS exposure, which might be alleviated by green tea consumption through its biological antioxidant activity.     MOTIVASI DAN DUKUNGAN KELUARGA TERKAIT KEPATUHAN PEMBATASAN ASUPAN CAIRAN PADA PASIEN GAGAL GINJAL KRONIK YANG MENJALANI HEMODIALISIS

    Latar Belakang : Kepatuhan pembatasan asupan cairan merupakan masalah penting pada pasien gagal ginjal kronik yang menjalani hemodialisis. Motivasi dan dukungan keluarga merupakan factor penting dalam menjaga kepatuhan pembatasan asupan cairan. Tujuan : Tujuan dari artikel ini adalah untuk mengkaji bagaimana motivasi dan dukungan keluarga terkait kepatuhan pembatasan asupan cairan pada pasien gagal ginjal kronik yang menjalani hemodialisis. Metode: Penelusuran literature dilakukan melalui Google scholar dan Database dari PubMed. Keywords yang digunakan dalam Bahasa Inggris adalah motivation, family support, fluid adherence, and chronic kidney disease sedangkan dalam Bahasa Indonesia adalah “ motivasi, dukungan keluarga, kepatuhan cairan dan gagal ginjal kronik”. Penelusuran dilakukan sampai 25 Januari 2021. Hasil penelusuran didapatkan 362 artikel peneliti menghilangkan 221 artikel karena tidak sesuai kriteria inklusi, tidak dapat didownload, tidak dapat dibuka, tidak mengandung kata kunci yang sesuai, dan tidak lengkap. Sehingga tersisa 141 artikel. Dari 141 artikel dikeluarkan 92 artikel, sehingga tersisa 49 artikel. Ke-49 artikel tersebut dilakukaan uji kelayakan dan artikel yang di review sebanyak 13. Hasil : ke-13 artikel menggambarkan motivasi dan dukungan keluarga merupakan factor penting dalam menjaga kepatuhan pembatasan asupan cairan. Dari 13 artikel tersebut didapatkan bahwa karakteristik responden penelitian tersebut mayoritas berusia >40 th, jenis kelamin laki-laki, pendidikan menengah dan merupakan pasien gagal ginjal kronik yang rutin melakukan hemodialisa di rumah sakit. Mayoritas responden mendapatkan dukungan keluarga serta motivasi yang baik terkait kepatuhan pembatasan asupan cairan. Kesimpulan : Motivasi dan dukungan keluarga yang baik merupakan proses terpenting dalam memberikan support kepada pasien yang mengalami penyakit gagal ginjal kronik untuk melakukan kepatuhan pembatasan asupan cairan
