    Efficacy and Safety of Lamotrigine in Lennox - Gastaut Syndrome

    ObjectiveThe Lennox-Gastaut syndrome (LGS), one of the most difficult epilepsy syndromes to treat, is characterized by a triad of intractable seizures of various types, a slow (< 2.5-hertz) spike-wave pattern in EEG and mental retardation. The aim of this study was to evaluate the efficacy and safety of lamotrigine as add-on therapy in intractable epilepsy of children with LGS.Materials & MethodsIn a quasi- experimental study, 40 children with LGS referred to the pediatric neurology clinic of Shaheed Sadoughi Hospital in Yazd, between August 2007 and to November 2008, were evaluated.ResultsTwenty-two boys and 18 girls with a mean age of 4.12 ±1.8 years were evaluated. At the end of three months of treatment with lamotrigine, 12 % were seizure free, 52% had> 50% reduction in seizure frequency and 12% had increase in seizures. Means of seizure frequency/per week, before and after treatment were 70 (range 1-180) and 18.6 (range 0-60) respectively, indicating effectiveness of the drug in seizure reduction (P value = 0.003). The drug was effective in 72 % of mixed type seizures, 40 % of generalized tonic-clonic and 33% of drop attack and tonic seizures. Transient side effects were seen in 12.5 % (drowsiness in 3 and ataxia in 2 children). No serious side effects were seen.ConclusionLamotrigine should be considered as an add-on therapy in management of intractable epilepsy in LGS

    What is the right theory for Anderson localization of light?

    Anderson localization of light is traditionally described in analogy to electrons in a random potential. Within this description the disorder strength -- and hence the localization characteristics -- depends strongly on the wavelength of the incident light. In an alternative description in analogy to sound waves in a material with spatially fluctuating elastic moduli this is not the case. Here, we report on an experimentum crucis in order to investigate the validity of the two conflicting theories using transverse-localized optical devices. We do not find any dependence of the observed localization radii on the light wavelength. We conclude that the modulus-type description is the correct one and not the potential-type one. We corroborate this by showing that in the derivation of the traditional, potential-type theory a term in the wave equation has been tacititly neglected. In our new modulus-type theory the wave equation is exact. We check the consistency of the new theory with our data using a field-theoretical approach (nonlinear sigma model)

    A study of the lower gastrointestinal tract cancer with emphasis on gender and age of the patients in western Iran (Kermanshah) over 2006-2011

    Given that the incidence of cancers in the coming years will have a growing trend due to the increased average age of the world's population, the partial control of communicable diseases, and the rapid growth of the environmental risk factors. The present work was a descriptive, comparative and analytical study. The statistical population consisted of all patients residing in Kermanshah who had been suffering from the lower gastrointestinal tract cancer for five years. The results of the present study revealed that 46.10% were male, and 53.90% were female. Further, the results indicated that the age of patients and the intensity of cancer differentiation were significantly correlated.It seemed that the lack of early diagnosis would ensue from a lack of periodic screening programs at early ages and lack of forums in which specialists could get together due to the unavailability of comparative statistics.Keywords: Cancer, Lower Gastrointestinal Tract Cancer, Western Iran, Kermanshah Cit

    The Effect of the Marketing Mix and Specialized Knowledge on the Export Performance of SMEs Exporting Dry Fruits

    Export is a crucial driver of economic growth in various countries and significantly contributes to a country's entry into global markets and enhances economic success. In developing countries such as Iran, economic and social development programs prioritize the expansion of exports, particularly high value added agricultural products. The growth in non-oil exports, such as dried fruits, and the entry of domestic producers into new global markets have led to an increased demand for Iran's export products. This has also resulted in higher production levels, increased employment opportunities, and higher value added in the related activities. To enhance export performance, which is a crucial measure of a company's success in utilizing its resources and capabilities in the international arena over a specific period of time, it is important to focus on improving marketing strategies and specialized knowledge. Therefore, this research aims to examine the impact of marketing mix and specialized marketing knowledge on the export performance of small and medium-sized enterprises (SMEs) involved in exporting dried fruits in Mashhad, Iran in 2022. A total of 80 questionnaires were distributed among senior managers, board members, and business managers of dried fruits SMEs using the available sampling method. Structural equation modeling was employed for data analysis and test of research hypotheses. The statistical data and structural equation modeling revealed that the joint impact of marketing mix and specialized marketing knowledge has a positive and significant influence on export performance. In order to improve the company's profitability, it is essential for senior managers and sales managers to recognize the significance of these two factors and undergo relevant training to acquire the necessary skills. Moreover, managers should make effective use of appropriate distribution channels to expand their exports. Simultaneously, they should consider adapting product quality and packaging to align with the preferences of foreign buyers

    Predictors for the severe coronavirus disease 2019 (COVID-19) infection in patients with underlying liver disease: a retrospective analytical study in Iran

    Risk factors for clinical outcomes of COVID-19 pneumonia have not yet been well established in patients with underlying liver diseases. Our study aimed to describe the clinical characteristics and outcomes of COVID-19 infection among patients with underlying liver diseases and determine the risk factors for severe COVID-19 among them. In a retrospective analytical study, 1002 patients with confirmed COVID-19 pneumonia were divided into two groups: patients with and without underlying liver diseases. The admission period was from 5 March to 14 May 2020. The prevalence of underlying conditions, Demographic data, clinical parameters, laboratory data, and participants' outcomes were evaluated. Logistic regression was used to estimate the predictive factors. Eighty-one (8) of patients had underlying liver diseases. The frequencies of gastrointestinal symptoms such as diarrhea and vomiting were significantly higher among patients with liver diseases (48 vs. 25 and 46.1 vs. 30 respectively, both P < 0.05). Moreover, ALT and AST were significantly higher among patients with liver diseases (54.5 ± 45.6 vs. 37.1 ± 28.4, P = 0.013 and 41.4 ± 27.2 vs. 29.2 ± 24.3, P = 0.028, respectively). Additionally, the mortality rate was significantly high in patients with liver disease (12.4 vs. 7, P = 0.018). We also observed that the parameters such as neutrophil to leukocyte ratio Odds Ratio Adjusted (ORAdj) 1.81, 95% CI 1.21�3.11, P = 0.011 and blood group A (ORAdj 1.59, 95% CI 1.15�2.11, P = 0.001) were associated with progression of symptoms of COVID-19. The presence of underlying liver diseases should be considered one of the poor prognostic factors for worse outcomes in patients with COVID-19. © 2021, The Author(s)

    Hydrology and hydrobiology and environmental pollutions in lower than 10 meters depths of Caspian Sea

    The present project in related to survey of factors and hydrology and hydrochemical features (water temperature, dissolve oxygen saturation, pH, clearance, salinity, nitrogen, phosphorus and silicon) hydrobiology (zooplankton, phytoplankton, macrobenthos) and survey of bio environment pollution (oil, heavy metal, detergent) executed in lower 10m in different water larger in southern Caspian Sea in 2002-2003. For sampling 8 lines number were vertical on coast that selected from Astra in west to Gomishan in east in southern Caspian Sea basin. The result indicated the average physical factors such as pH were 8.11 and salinity12.12 ppt ,and disolve oxygen6.7 mg/l. Average chemical factors such as NO_2 , NO_3 and NH_4 were 1.2 µg/l, 25.7 µg/l, 13 µg/l respectively.Total nitogen and organic nitrogen and inorganic nitrogen were 690.2 µg/l , 667.6 µg/l , 41.6 µg/l. Average silicat were recrded 266.35 µg/l . Total Phosphorus was observed 37.35 µg/l and average of organic Phosphorus concentration was and 20.25 µg/l .Average of Total organic matter (T.O.M) was 4.98% maximum amount were observed in Lisar and minimum in Nooshahr . Concentration of heavy metal during sampling were respectively ,Fe>Mn>Zn>Cr>Ph>Co>Cd>Cu. Maximum concentration of Fe were determined in winter in Nooshahr and Babolsar respectively 13/3 µg/l 17/1 µg/l. In many stations and different Season, the amount of heavy meta were lower standard of in marine water. The concentration of oil hydrocarborate (PAHs) in autumn was 0/13 ppb and in winter 0/12 ppb. The amount of (PAHs) in Southern Caspian Sea were Lower than other parts of Caspian Sea. The average of detergent concentration (LAS) was 0/036 µg/l that was two fold higher than determined in 2001. Total 107 species of phytoplankton belong to 5 phylum were identified. The numbers of species of phytoplankton groups were respectively, chrysophyta (42 species), cyanophyta (17 species), pyruphyta (17 species), chlorophyta (21 species) and euglenophyta (9 species). The maximum diversity of phytoplankton observed in summer and minimum in autumn. High diversity of chrysophta and cyanophyta observed in summer and phyrophyta and chrlophyta in spring. The composition of phytoplanhkton groups were respectively, chrysophyta (70%), phyrophyta (9%) and chlorophyta (7%) and euglonophyta (1%). Maximum density of phytoplankton was observed in autumn and minimum in winter. Total 19 species of Zooplankton were identified. Maximum diversity was observed in summer and minimum in winter. Zooplankton changes during sampling, showed amount of density of zooplankton in 5m were more than 10 m depths. Total (17 species macrobenthos were identified. The composition of macrobenthos groups were respectively , Annalida (92/7% ) , Bivalvia (2/7%) gumarida (108%) cumacea ( 1/5%) , Balanidae 103% . max . density were observed in Astara and min . in Sefied roud Average of density were 1218 0/851 ind /m^2 and biomass 14 15 g/m^2 High density were recorded in autumn and low density in winter . Correlation of phytoplankton and zooplankton with physicochemical parameter and also relation between total organic matter and sediment grain size were calculated.Ecological indicies (simpson diversity evenns diversity and shanoon-wiever diversity) were calculated for macrobenthos.
