    A Study on Methylene Blue Chromoendoscopy for Early Diagnosis of Baretts Metaplasia, Dysplasia and Early Esophageal Adenocarcinoma in our Institute

    INTRODUCTION: Chronic GERD predisposes to Baretts Metaplasia, dysplasia & adenocarcinoma. Early detection of these lesions is important for making treatment decisions & surveillance stratergies. AIM OF THE STUDY: To evaluate and compare the effectiveness of methylene blue chromoendoscopy directed biopsies in the early diagnosis of Barrett’s metaplasia, Dysplasia & early esophageal adenocarcinoma in high risk population compared to routine random biopsy Place of study: Department of Digestive Health and Diseases, Government Peripheral Hospital, Annanagar , Chennai from April 2013 to February 2014 MATERIALS AND METHODOLOGY: A total of 50 patients who presented to the Department of Gastroenterology, KMC, Chennai, from April 2013 to February 2014 were included in the study based on inclusion criteria. Each patient underwent routine whitelight and 0.5% methylene blue chromoendoscopy with biopsy as per protocol. Results were assessed and compared for detection of barrett’s metaplasia, dysplasia and adenocarcinoma. RESULTS: A total of 50 patients (40 male & 10 female) were included in the study as per inclusion criteria. All the patients included presented with features of GERD, with heartburn (80%) being the most common presentation. Among risk factors, Chronic GERD was seen in all (100%), smoking in (56%) and alcohol intake in (24%). After methylene blue C.E Uniform staining was seen in 11 patients, patchy staining (n=35) and absent staining (n=4). On HPE, the Random biopsy arm detected barrett’s metaplasia in 6% (n=2), whereas the Methylene blue directed biopsy arm detected 10% (n=5, Z test is 0.7372, P – 0.459) with barrett’s metaplasia and 2 of these patients were also found to have low-grade dysplasia (Z is -1.4296, P – 0.153). Out of 5 patients with barrett’s in the MBDB arm, 4 had uniform staining, suggesting that patients with barrett’s metaplasia are likely to have uniform staining. CONCLUSION: In our study Methylene Blue Chromoendoscopy and biopsy diagnosed Barrett’s metaplasia to a higher percentage than white light endoscopy and routine biopsy in patients with chronic gastro-oesophageal reflux disease. Hence Chromoendoscopy with Methylene blue is a useful tool for early detection of Barrett’s oesophagus and thereby suggest appropriate treatment and surveillance for oesophageal malignancy

    Unveiling mediastinal pathology: role of EUS guided fine needle aspiration in diagnosing mediastinal lesions

    Background: Mediastinal lesion is the focus of investigation in diagnosis of infective, granulomatous or neoplastic pathology of respiratory system. Metastatic mediastinal node assessment is an integral part of oncological management. EUS provides access to sampling of mediastinal mass, sub-carinal and aorto-pulmonary nodes. This study aims to assess the clinical impact, diagnostic yield and safety of EUS guided FNA for mediastinal lesions.Methods: Retrospective analysis of prospectively collected data of 72 cases of mediastinal lesions between January 2014 and December 2017 was done. EUS-FNA was performed with a linear echoendoscope using a 22- or 25-gauge needle. Adequacy of cellularity was assessed by on site pathologist. Patient data (demographics, intervention and follow-up) were prospectively collected and introduced in a predefined computer database for later review.Results: EUS-FNA was performed from 57 lymph nodes and 15 mediastinal masses. Adequate samples were obtained in 67 of 72 patients (93.05%). All mediastinal masses were malignant and were identified in the 3rd, 5th and 6th decade of life. Of the 57 lymph nodes, 15 were malignant, 28 had granulomatous lymphadenitis of which 16 individuals became asymptomatic after anti tubercular therapy. Sample was inadequate in 5 circumstances. No major complications were encountered with the procedure in any of the individuals.Conclusions: EUS guided tissue diagnosis is a safe technique and our data supports the use of EUS-FNA in work-up of mediastinal lesions. It is minimally invasive, accurate and has easy access to mediastinum. It has significant impact on patient diagnosis, management and should be considered over other invasive techniques

    Colorectal polyps and heterotrophic esophageal polyp of colon in a paediatric cohort in South India

    Background: Clinical profile of polyps in paediatric cases are less in South India. Juvenile Polyps were the most common polyps in paediatric cases described in literature, presenting as LGI bleed. The aim of the study is to describe the clinical profile of colorectal polyps in paediatric population in a single tertiary care centre in South India.Methods: Paediatric cases between 0 and 16 years of age who underwent colonoscopy in our department from January 2002 to July 2018 were included from database. These cases were retrospectively analysed for presence of polyps, clinical presentation, indication for colonoscopy, histopathology of the resected polyps and other demographic details.  Incomplete procedures were excluded.Results: About 166 paediatric cases underwent colonoscopy in the study period. 21 cases (12.65%) had colorectal Polyps. 85.7% of the polyps were in recto sigmoid region. Most common histological type was Juvenile Polyp (51.6%). One infant had sessile polyp in descending colon which was reported as heterotrophic esophageal mucosa in histopathology. LGI bleed was the most common presentation in children with polyps (66.6%).Conclusions: The prevalence of polyps in our cohort was 12.65%. Solitary Juvenile Polyp was the most common polyp in children, with lower GI bleed as the most common presenting feature. Rare case of heterotrophic esophageal polyp was seen in descending colon

    Detecting mycobacteraemia for diagnosing tuberculosis

    Background & objectives: In human immunodeficiency virus (HIV) infected persons with pulmonary tuberculosis (TB), sputum may not always show acid fast bacilli (AFB). Moreover, in most cases of suspected extrapulmonary TB (irrespective of HIV status) mycobacteria-containing material is not readily available for investigation. This study evaluated whether blood culture for Mycobacterium tuberculosis bacteraemia (mycobacteraemia) help in diagnosing TB in such cases. Methods: A total of 93 consecutive subjects with a clinical diagnosis of tuberculosis with or without laboratory confirmation, 42 with and 38 without coexisting HIV infection, and 13 patients with HIV infection without clinical evidence of TB were enrolled. Mycobacterial blood cultures were done using lysis centrifugation technique followed by subculturing onto the modified Lowenstein-Jenson medium (LJ-1) and Selective Kirchner's medium followed by subculturing onto the modified Lowenstein- Jenson medium (LJ-2, LJ-3). Results: Of the 15 (16.2%) subjects with evidence of mycobacteremia in 4 (26.7%) blood was the first/ only source of diagnosing TB. Among 80 patients with clinical diagnosis of TB whether supported by laboratory tests or not, 14 (17.5%) had mycobacteraemia. Among the 21 HIV infected patients with laboratory proven TB, 9 (43%) had mycobacteraemia. Interpretation & conclusion: Blood culture appears to be a useful additional test to diagnose TB in personss with HIV infection. In patients without HIV infection, but with clinical picture compatible with TB, blood culture for mycobacteraemia may occasionally help in the diagnosis. We recommend the use of the lysis centrifugation technique followed by direct smear of the sediment along with inoculation of the sediment into both modified Lowenstein-Jenson medium and the Selective Kirchner's medium with subsequent subculturing onto the modified Lowenstein-Jenson medium for mycobacterial blood culture for detecting mycobacteraemia

    Peroxisome proliferator-activated receptors (PPARs; PPAR-alpha, PPAR-delta, and PPAR-gamma) comprise a family of nuclear receptors that sense fatty acid levels and translate this information into altered gene transcription. Previously, it was reported that treatment of mice with a synthetic ligand activator of PPAR-delta, GW0742, ameliorates experimental autoimmune encephalomyelitis (EAE), indicating a possible role for this nuclear receptor in the control of central nervous system (CNS) autoimmune inflammation. We show that mice deficient in PPAR-delta (PPAR-delta(-/-)) develop a severe inflammatory response during EAE characterized by a striking accumulation of IFN-gamma(+)IL-17A(-) and IFN-gamma(+)IL-17A(+) CD4(+) cells in the spinal cord. The preferential expansion of these T helper subsets in the CNS of PPAR-delta(-/-) mice occurred as a result of a constellation of immune system aberrations that included higher CD4(+) cell proliferation, cytokine production, and T-bet expression and enhanced expression of IL-12 family cytokines by myeloid cells. We also show that the effect of PPAR-delta in inhibiting the production of IFN-gamma and IL-12 family cytokines is ligand dependent and is observed in both mouse and human immune cells. Collectively, these findings suggest that PPAR-delta serves as an important molecular brake for the control of autoimmune inflammation

    Synthesis and Characterisation of Bis-azido Methyl Oxetane and its Polymer and Copolymer with Tetrahydrofuran

    Bis-azido methyl oxetane (BAMO) was synthesised from pentaerythritol in two steps. Pentaerythritol was chlorinated to yield a mixture of mono, di, tri and tetra chloro compounds. The trichloro compound on ring closure gives bis-chloro methyl oxetane (BCMO). It was reacted with sodium azide in aqueous medium to obtain BAMO. The latter was polymerised using BF3 etherate catalyst and 1,4-butanediol initiator. Similarly, the BAMO- THF copolymer was also synthesised. All the monomers and polymers were characterised by IR, 1H-NMR, 13C-NMR, and refractive index. The polymers were also characterised for molecular weight, hydroxyl value, etc. Thermal analysis showed that both polymers degrade exothermically with T max of 237 °C for poly BAMO and 241°C for BAMO- THF copolymer with activation energy of 39 kcal/mol and 40 kcal/mol, respectively.     TEX: The New Insensitive High Explosive

    Insensitive high explosive 4, 10-dinitro-2,6,8, 12-tetraoxa-4, 10-diazatetracyclo (5.5.0.0.5,9 03,11) dodecane (TEX) has been synthesised by an improved laboratory-scale process using 93 per cent to 96 per cent nitric acid as nitrating agent. Characterisation of the product was done based on its physical constants, infrared, differential thermal analysis, and mass spectral studies. Explosive and ballistic parameters of TEX containing formulations were computed using Becker-Kistiakowsky-Wilson (BKW) code and NASA Chemical Equilibrium Composition- 71 programme, respectively. Semi-empirical quantum mechanical calculations using the parametric model 3 (PM3) method have been carried out for the TEX molecule.
