    On the Nonlinearity of Modern Shock-Capturing Schemes

    The development is reviewed of shock capturing methods, paying special attention to the increasing nonlinearity in the design of numerical schemes. The nature is studies of this nonlinearity and its relation to upwind differencing is examined. This nonlinearity of the modern shock capturing methods is essential, in the sense that linear analysis is not justified and may lead to wrong conclusions. Examples to demonstrate this point are given

    Myofibrillar Protein Status of the Gastrocnemius in Male Rats: Effect of Mild Undernutrition

    The aim of this work was the determination of the myofibrillar protein profiles in the fed and the mildly underfed rat. Sixteen male rats were divided into 2 groups: CR (control) fed ad libitum and MR (mildly undernourished) fed 75% of energetic maintenance needs. The animals were sacrificed at day 23 and the gastrocnemius muscle was taken for myofibrillar protein characterisation. The myofibrillar protein profiles were found to be very similar in the two groups revealing the lack of preferred catabolism of myofibrillar proteins and consequently that the muscle structure is maintained even in situations of mild undernutrition

    Cost-effectiveness and resource use of implementing MRI-guided NACT in ER-positive/HER2-negative breast cancers in The Netherlands

    Background: Response-guided neoadjuvant chemotherapy (RG-NACT) with magnetic resonance imaging (MRI) is effective in treating oestrogen receptor positive/human epidermal growth factor receptor-2 negative (ER-positive/HER2-negative) breast cancer. We estimated the expected cost-effectiveness and resources required for its implementation compared to conventional-NACT. Methods: A Markov model compared costs, quality-adjusted-life-years (QALYs) and costs/QALY of RG-NACT vs. conventional-NACT, from a hospital perspective over a 5-year time horizon. Health services required for and health outcomes of implementation were estimated via resource modelling analysis, considering a current (4 %) and a full (100 %) implementation scenario. Results: RG-NACT was expected to be more effective and less costly than conventional NACT in both implementation scenarios, with 94 % (current) and 95 % (full) certainty, at a willingness to pay threshold of €20.000/QALY. Fully implementing RG-NACT in the Dutch target population of 6306 patients requires additional 5335 MRI examinations and an (absolute) increase in the number of MRI technologists, by 3.6 fte (full-time equivalent), and of breast radiologists, by 0.4 fte. On the other hand, it prevents 9 additional relapses, 143 cancer deaths, 23 congestive heart failure events and 2 myelodysplastic syndrome/acute myeloid leukaemia events. Conclusion: Considering cost-effectiveness, RG-NACT is expected to dominate conventional-NACT. While personnel capacity is likely to be sufficient for a full implementation scenario, MRI utilization needs to be intensified

    The impact of organisational external peer review on colorectal cancer treatment and survival in the Netherlands

    Background: Organisational external peer review was introduced in 1994 in the Netherlands to improve multidisciplinary cancer care. We examined the clinical impact of this programme on colorectal cancer care. Methods: Patients with primary colorectal cancer were included from 23 participating hospitals and 7 controls. Hospitals from the intervention group were dichotomised by their implementation proportion (IP) of the recommendations from each peer review (high IP vs low IP). Outcome measures were the introduction of new multidisciplinary therapies and survival. Results: In total, 45 705 patients were included (1990-2010). Patients from intervention hospitals more frequently received adjuvant chemotherapy for stage III colon cancer. T2-3/M0 rectal cancer patients from hospitals with a high IP had a higher chance of receiving preoperative radiotherapy (OR 1.31, 95% CI 1.11-1.55) compared with the controls and low IP group (OR 0.75, 95% CI 0.63-0.88). There were no differences in the use of preoperative chemoradiation for T4/M0 rectal cancer. Survival was slightly higher in colon cancer patients from intervention hospitals but unrelated to the phase of the programme in which the hospital was at the time of diagnosis. Conclusions: Some positive effects of external peer review on cancer care were found, but the results need to be interpreted cautiously due to the ambiguity of the outcomes and possible confounding factors

    Dispersive wave runup on non-uniform shores

    Historically the finite volume methods have been developed for the numerical integration of conservation laws. In this study we present some recent results on the application of such schemes to dispersive PDEs. Namely, we solve numerically a representative of Boussinesq type equations in view of important applications to the coastal hydrodynamics. Numerical results of the runup of a moderate wave onto a non-uniform beach are presented along with great lines of the employed numerical method (see D. Dutykh et al.
