    Permeability evolution during progressive development of deformation bands in porous sandstones

    [1] Triaxial deformation experiments were carried out on large (0.1 m) diameter cores of a porous sandstone in order to investigate the evolution of bulk sample permeability as a function of axial strain and effective confining pressure. The log permeability of each sample evolved via three stages: (1) a linear decrease prior to sample failure associated with poroelastic compaction, (2) a transient increase associated with dynamic stress drop, and (3) a systematic quasi-static decrease associated with progressive formation of new deformation bands with increasing inelastic axial strain. A quantitative model for permeability evolution with increasing inelastic axial strain is used to analyze the permeability data in the postfailure stage. The model explicitly accounts for the observed fault zone geometry, allowing the permeability of individual deformation bands to be estimated from measured bulk parameters. In a test of the model for Clashach sandstone, the parameters vary systematically with confining pressure and define a simple constitutive rule for bulk permeability of the sample as a function of inelastic axial strain and effective confining pressure.     Audit of surgical procedures in regional and central hospitals in Limpopo Province, South Africa

    Background. There is a paucity of data on the functioning and surgical procedures performed in rural hospitals in South Africa.Objective. To determine the category of procedures performed at regional and tertiary hospitals in Limpopo Province, South Africa. Method. We conducted a retrospective analysis of surgical procedures performed in the regional and tertiary hospitals in Limpopo Province during a 1-year period from 1 March 2019 to 29 February 2020.Results. A total of 24 263 surgical procedures were performed during the study period. More than half of all cases (50.4%; n=12 252) were operated on at the tertiary hospitals while regional hospitals performed the remaining 12 011 operations.Conclusion. There is a great necessity to alleviate the central hospitals from the bulk of surgical procedures

    Behavioural and socio-ecological factors that influence access and utilisation of health services by young people living in rural KwaZulu-Natal, South Africa: Implications for intervention.

    BACKGROUND: Young people's health service utilisation (the number accessing a facility) has been the focus of guidelines and health systems strengthening policies. This is due to young people being at an increased health risk because of inequitable access and utilisation of health services, which is more pronounced in rural settings with limited service availability. This is a major concern as globally, youth constitute a considerable and increasing part of the population in Sub-Saharan Africa. OBJECTIVE: The objective of this paper is to present a comprehensive approach for the exploration of health service utilisation by young people in rural KwaZulu-Natal, South Africa. We examined barriers and facilitators conceptualised by the constructs of the Theory of Planned Behaviour, framed within a socio-ecological model. METHODS: Data were collected in January to June 2017 from two sites using in-depth interviews, spiral transect walks and community mapping with young people (aged 10 to 24 years), primary care health providers, school health professionals, community stakeholders and young people's parents. RESULTS: Socio-ecological and behavioural factors influenced young people's intention to use services. Barriers included perceived negative attitudes of health providers and perceived poor staff competencies. Facilitators included an appreciation of receiving health education and assumed improved health. At social and community levels, normative beliefs hindered young people from utilising services as they feared stigmatisation and gossip. At a public policy level, structural elements had a disempowering effect as the physical layout of the clinics hindered utilisation, limited resources influenced staffing, and facility opening times were not convenient for school goers. CONCLUSION: We suggest that to fully appreciate the complexity of health service utilisation, it is necessary to not only consider factors and processes relevant to the individual, but also acknowledge and act upon, the disjuncture between community level cultural values, norms and national policies

    Plume–lithosphere interactions and LIP-triggered climate crises constrained by the origin of Karoo lamproites

    We identified a ca. 180 Ma diamondiferous lamproite event in Zambia, establishing a link between ultrapotassic volcanism and the early Jurassic Karoo flood basalt province of sub-Saharan Africa. The cratonic lamproites erupted through the Permo–Triassic Luangwa Rift structure, but MgO-rich ultrapotassic magma formation was unrelated to rifting and triggered by plume–lithosphere interactions during the Karoo LIP event. Elevated Li–Zn–Ti concentrations in magmatic olivine (up to 18.5 ppm Li at 86–90 mol.% forsterite) and strong Sr–Nd–Hf–Pb isotopic enrichment of the host lamproites ( 87Sr/86Sr = 0.70701–0.70855, εNd = − 10.8 to − 10, εHf = − 20.3 to − 19.1, 206Pb/204Pb = 16.8–17.5) suggest partial melting of phlogopite-metasomatized lithospheric mantle domains, at approximately 180–200 km depth. The mantle-like δ7 Li values (+2.8 to +5.7‰) of the most pristine lamproite samples are compatible with source enrichment by asthenosphere-derived melts, without significant involvement of recycled sedimentary components. This geochemical fingerprint stands in sharp contrast to the negative δ7 Li compositions of primitive K-rich volcanic rocks from collision zone settings, where the shallow mantle sources contain recycled sediment. Isotope modelling demonstrates that the sub-Saharan lamproites originate from a MARID-style metasomatized peridotitic mantle source that underwent incompatible element enrichment at ca. 1 Ga, during tectonic activity associated with Rodinia supercontinent formation. Plume-sourced basaltic and picritic magmas of the 180 Ma Karoo LIP interacted with such K-rich hydrous lithospheric mantle domains, thereby attaining enriched incompatible element and radiogenic isotope compositions. Nd–Hf isotope mass balance suggests that up to 25% of MARID-sourced lamproite melt component contributed to some of the high-Ti flood volcanic units. Although large quantities of volatiles can be transferred from Earth’s mantle to the atmosphere via plume–lithosphere interactions, it is unlikely that outgassing of mantle-sourced sulphur can exceed the climatic impact caused by the release of much more abundant carbon from thick continental roots. Thus, the excess SO2 required to account for transient atmospheric cooling during the early Jurassic, coincident with the Karoo LIP event, must have had a thermogenic origin near the surface of Earth

    Qualitative study to understand the barriers to recruiting young people with cancer to BRIGHTLIGHT: a national cohort study in England

    Objectives: BRIGHTLIGHT is a national evaluation of cancer services for teenagers and young adults in England. Following challenges with recruitment, our aim was to understand more fully healthcare professionals’ perspectives of the challenges of recruiting young people to a low-risk observational study, and to provide guidance for future recruitment processes. / Design: Qualitative. / Setting: National Health Service (NHS) hospitals in England. / Methods: Semistructured telephone interviews with a convenience sample of 23 healthcare professionals. Participants included principal investigators/other staff recruiting into the BRIGHTLIGHT study. Data were analysed using framework analysis. / Results: The emergent themes were linked to levels of research organisational management, described using the levels of social network analysis: micro-level (the individual; in this case the target population to be recruited—young people with cancer); meso-level (the organisation; refers to place of recruitment and people responsible for recruitment); and macro-level (the large-scale or global structure; refers to the wider research function of the NHS and associated policies). Study-related issues occurred across all three levels, which were influenced by the context of the study. At the meso-level, professionals’ perceptions of young people and communication between professionals generated age/cancer type silos, resulting in recruitment of either children or adults, but not both by the same team, and only in the cancer type the recruiting professional was aligned to. At the macro-level the main barrier was discordant configuration of a research service with a clinical service. / Conclusions: This study has identified significant barriers to recruitment mainly at the meso-level and macro-level, which are more challenging for research teams to influence. We suggest that interconnected whole-system changes are required to facilitate the success of interventions designed to improve recruitment. Interventions targeted at study design/management and the micro-level only may be less successful. We offer solutions to be considered by those involved at all levels of research for this population

    'This is what is going to help me': Developing a co-designed and theoretically informed harm reduction intervention for mobile youth in South Africa and Uganda

    Young migrants in sub-Saharan Africa are particularly vulnerable to HIV-acquisition. Despite this, they are consistently under-served by services, with low uptake and engagement. We adopted a community-based participatory research approach to conduct longitudinal qualitative research among 78 young migrants in South Africa and Uganda. Using repeat in-depth interviews and participatory workshops we sought to identify their specific support needs, and to collaboratively design an intervention appropriate for delivery in their local contexts. Applying a protection-risk conceptual framework, we developed a harm reduction intervention which aims to foster protective factors, and thereby nurture resilience, for youth ‘on the move’ within high-risk settings. Specifically, by establishing peer supporter networks, offering a ‘drop-in’ resource centre, and by identifying local adult champions to enable a supportive local environment. Creating this supportive edifice, through an accessible and cohesive peer support network underpinned by effective training, supervision and remuneration, was considered pivotal to nurture solidarity and potentially resilience. This practical example offers insights into how researchers may facilitate the co-design of acceptable, sustainable interventions

    Virtual obstacles for sensors incapacitation in robot navigation : a systematic review of 2D path planning

    The field of mobile robot (MR) navigation with obstacle avoidance has largely focused on real, physical obstacles as the sole external causative agent for navigation impediment. This paper has explored the possible option of virtual obstacles (VOs) dominance in robot navigation impediment in certain navigation environments as a MR move from one point in the workspace to a desired target point. The systematically explored literature presented reviews mostly between the years 2000 and 2021; however, some outlier reviews from earlier years were also covered. An exploratory review approach was deployed to itemise and discuss different navigation environments and how VOs can impact the efficacy of both algorithms and sensors on a robotic vehicle. The associated limitations and the specific problem types addressed in the different literature sources were highlighted including whether or not a VO was considered in the path planning simulation or experiment.     Keratocystic odontogenic tumour masquerading as a mucosal antral cyst

    Keratocystic odontogenic tumour (KCOT) is of clinical importance because of its pronounced tendency to recur. Among the factors that complicate KCOT management are its protean radiological presentations. KCOT showing involvement of the maxillary antrum in the absence of an associated impacted tooth is exceptionally rare. In these instances an odontogenic source of the lesion is often not suspected and the clinico-radiological features may be misinterpreted.
