Evolution of Orchestration Towards 5G
Service orchestration is an essential activity in 5G networks. It performs optimal resource allocation and provisions services in an effective sequence based on demands across a collection of physical or virtual network functions (P/VNF). This paper summarizes several orchestration environments and components along with their evolution towards 5G. A brief operational comparison of platforms such as Open Source Management and Orchestration (OSM MANO), Open Platform for NFV (OPNFV) and Open Network Automation Platform (ONAP) have been presented, along with different deployment models and architectural alternatives
Synthetic Cannabinoids (SCs) in metaphors: A metaphorical analysis of user experiences of synthetic cannabinoids in two countries.
New psychoactive substances (NPS) remain a global public health and clinical challenge. Popular NPS include synthetic cannabinoids (SCs). A conceptual metaphorical analysis of user experiences of SCs was conducted in two European countries (Western and Central European). Metaphors are increasingly used to better understand drug user experiences and as medium to inform and guide clinical responses. Semi-structured interviews with 12 SC users were conducted in Hungary (n = 6) and Ireland (n = 6). Thematically segmented texts of SC usage (1) motivation, (2) effects, (3) consequences and (4) the setting were selected. A systematic analysis of conceptual metaphors was conducted on the selected texts. The conceptual target and source domains of the experiences of SC usage (motivation, effects, consequences) were analysed. Four conceptual source domains were found in all of the segmented and analysed narratives of psychological factors (motivations, effects, consequences): change in the vertical perception, perceiving that SC is everywhere, feeling disconnected, recognition of lack of control and identifying the SC as a destroying entity. The conceptual metaphors reflect how users perceive experiences of SC use, the discrepancy between the experience of embodiment and disembodiment. This study provides clinicians with an insight into the experiences of SC motivations, effects and consequences and can be used to inform and guide clinical and therapeutic responses in the support of those recovering from SC dependence

Voices of temporary sobriety - a diary study of an alcohol-free month in Hungary
Background: Temporary abstinence from alcohol as a challenge could support self-knowledge, self-care, and health consciousness in several ways.
Objectives: The present study explored Dry November participants’ personal experiences and coping strategies during a one-month abstinence period. The research is embedded in the Hungarian context of drinking habits, culture, society, and alcohol policy.
Methods: This qualitative study comprised the thematic analysis of 23 participants’ diaries, reported twice a week for 30 days (in November 2017), to identify and understand the common experiences of temporary sobriety.
Results: Three main themes emerged from the analysis: challenge, community, and relationship toward alcohol and abstinence. Results showed that there are no categorical differences between successful and non-successful participants.
Conclusions: The present research demonstrated that during the challenge, rather than simply saying ‘no’ to alcohol, participants utilized other refusal strategies to avoid social confrontation

Usefulness of electroanatomical mapping during transseptal endocardial left ventricular lead implantation
AimFailure rate to implant left ventricular (LV) lead
transvenously is 4-8% in cardiac resynchronization therapy (CRT)
patients. Epicardial lead placement is an alternative method and
if not applicable case reports and small series showed the
feasibility of endocardial LV lead implantation.
Electroanatomical mapping might be a useful tool to guide this
procedure.Methods and resultsFour patients had undergone
endocardial LV lead implantation after unsuccessful transvenous
implantation or epicardial LV lead dysfunction using the
transseptal approach. Electroanatomical mapping was used to mark
the location of the transseptal puncture. This location point
guided the mapping catheter from the subclavian access and
facilitated positioning of the LV lead at the adjacent latest
activation area of the left ventricle detected by activation
mapping. Endocardial active fixation LV leads were successfully
implanted in all patients with stable electrical parameters
immediately after implantation and over a mean follow-up of 18.3
months (lead impedance 520 +/- 177 vs. 439 +/- 119 Omega and
pacing threshold 0.8 +/- 0.2 V, 0.5 ms vs. 0.6 +/- 0.1 V, 0.5
ms, respectively). Patients were maintained on anticoagulation
therapy with a target international normalized ratio of 3.5-4.5
and did not show any thromboembolic, haemorrhagic events, or
infection. Echocardiography showed significant improvement of LV
systolic function with marked improvement of the functional
status.ConclusionsElectroanatomical mapping is a useful
technical tool to guide endocardial LV lead implantation. It
helps to identify the location of the transseptal puncture and
the use of activation mapping might facilitate location of the
optimal lead positions during CRT
