This article describes the clinical case of acute colonic bleeding and microperforation of the sigmoid colon in a patient with bilateral pyosalpinx. At primary operative intervention, diverticulosis of the vagmoid colon was not detected through an inflammatory infiltrate, which led to repeated surgical intervention with resection of the sigmoid colon and stoma.В статье описано клинический случай острого толстокишечного кровотечения и микроперфорации сигмовидной кишки у пациентки с двухсторонним пиосальпинксом. При первичном оперативном вмешательстве не было обнаружено дивертикулеза сигмовидной кишки через воспалительный инфильтрат, что привело к повторному оперативному вмешательству с выполнением резекции сигмовидной кишки и стомы.У статті описано клінічний випадок гострої товстокишкової кровотечі та мікроперфорації сигмоподібної кишки у пацієнтки із двостороннім піосальпінксом. The aim of the work: to develop indications and contraindications for the implementation of simultaneous surgical interventions on the example of patients with combined pathology of the thyroid gland (TG) and cholelithiasis (CL) / gallbladder polyposis, using the scale of the functional state of P-POSSUM. Materials and Methods. During the period from January 2015 to September 2018, 35 patients with combined pathology of the TG and CL / gallbladder polyposis were examined and operated. At the preoperative stage, as well as on the first day of the postoperative period, all patients evaluated on the P-POSSUM scale. Results and Discussion. In 22 (62.9 %) patients, whose functional state before the intervention was assessed by physiological subscale (FS) ≤ 17 points, there were postoperative changes in health with an average difference of 0.13 points, T (Z) = 1.21, p = 0.225. In all these patients, the complexity of surgical intervention on the surgical sub-scale (SS) on the P-POSSUM scale was estimated ≤ 14 points. In 13 (37.1 %) patients, whose condition was estimated at 17 points by FS, significant changes in functional health before and after surgery were revealed, with a difference of average 1.15 points, T (Z) = 2.67; p = 0.0077. The complexity of their operations ranged from 14 to 16 points for SS.Цель работы: разработать показания и противопоказания к выполнению симультанных оперативных вмешательств на примере пациентов с сочетанной патологией щитовидной железы (ЩЖ) и желчнокаменной болезнью (ЖКБ) / полипозом желчного пузыря, с использованием шкалы функционального состояния P-POSSUM. Материалы и методы. За период с января 2015 по сентябрь 2018 года обследовано и прооперировано 35 пациентов с сочетанной патологией щитовидной железы и ЖКБ / полипозом желчного пузыря. На дооперационном этапе, а также на первые сутки послеоперационного периода, всем пациентам проводили оценку функционального состояния по шкале P-POSSUM. Результаты исследований и их обсуждение. У 22 (62,9 %) пациентов, функциональное состояние которых до вмешательства оценивалось по физиологической субшкале (ФС) ≤ 17 баллов, имели место послеоперационные изменения состояния здоровья с разницей средних 0,13 баллов, Т (Z) = 1,21, р = 0,225. В этих больных сложность оперативного вмешательства по хирургической субшкале (ХС) шкале P-POSSUM была оценена ≤ 14 баллов. У 13 (37,1 %) больных, состояние которых было оценено в 17 баллов по ФС, выявлены значимые изменения функционального состояния здоровья до и после операции, с разницей средних 1,15 балла, Т (Z) = 2,67; р = 0,0077. ИНТЕГРАТИВНЫЙ ПОДХОД К БАЗОВОЙ МАТЕМАТИЧЕСКОЙ ПОДГОТОВКЕ ЭКОНОМИСТОВ

The demands of modern society are setting up the prospects of the dynamic development of the innovative environment that combines vocational training with advanced science and competitive businesses. In turn, these trends necessitate the use of an integrative approach to the vocational training in higher education. The structure of the integrative approach implemented in the framework of a certain basic learning discipline produces multiple links at four levels of integration: interdisciplinary, intra-disciplinary, interpersonal, intrapersonal. The paper discusses the experience gained with the integrative approach applied to the basic mathematical program for first-year students trained for the BA degree in Economics at Financial University. Three components of the integrative approach are described. The first is a “socio-cultural” aspect related to the review-and-entertaining mathematics within the propedeutics of the future research. The second component of the integrative approach represented by the design activity is concerned with solution of practice-oriented tasks using the mathematical apparatus studied by first-year students. Finally, the third component of the approach referred to as the case-method aiming at mathematical solution of situational problems develops the ability to formulate a problem independently, structurize the information, find an optimal solution and prove its optimality.Потребности современного общества задают перспективу динамичного развития инновационной среды, объединяющей профессиональное образование с передовой наукой и конкурентоспособным бизнесом. В свою очередь, эти тенденции вызывают необходимость интегративного подхода к профессиональной подготовке в системе высшего образования. Структура интегративного подхода, осуществляемого в рамках некоторой базовой образовательной дисциплины, реализует многообразие связей на четырех уровнях интеграции: междисциплинарном, внутридисциплинарном, межличностном, внутриличностном. В статье обсуждается опыт интегративного подхода к базовой математической подготовке на 1-м курсе бакалавриата экономики Финансового университета при Правительстве РФ. Авторы характеризуют три элемента интегративного подхода. Первый из них, «социокультурный» аспект, связан с выполнением обзорно-занимательных работ математической тематики в рамках пропедевтики будущих научных исследований. Второй элемент интегративного подхода в форме проектной деятельности затрагивает решение практико-ориентированных задач с использованием изучаемого первокурсниками математического аппарата. Наконец, третий элемент подхода, кейс-метод решения ситуационных задач математическими средствами, развивает умение самостоятельно формулировать проблему, структурировать информацию, находить оптимальное решение и обосновывать его оптимальность

Observations of Non-radial Pulsations in Radio Pulsars

We introduce a model for pulsars in which non-radial oscillations of high spherical degree (l) aligned to the magnetic axis of a spinning neutron star reproduce the morphological features of pulsar beams. In our model, rotation of the pulsar carries a pattern of pulsation nodes underneath our sightline, reproducing the longitude stationary structure seen in average pulse profiles, while the associated time-like oscillations reproduce "drifting subpulses"--features that change their longitude between successive pulsar spins. We will show that the presence of nodal lines can account for observed 180 degree phase jumps in drifting subpulses and their otherwise poor phase stability, even if the time-like oscillations are strictly periodic. Our model can also account for the "mode changes" and "nulls" observed in some pulsars as quasiperiodic changes between pulsation modes of different l or radial overtone n, analogous to pulsation mode changes observed in oscillating white dwarf stars. We will discuss other definitive and testable requirements of our model and show that they are qualitatively supported by existing data. While reserving judgment until the completion of quantitative tests, we are inspired enough by the existing observational support for our model to speculate about the excitation mechanism of the non-radial pulsations, the physics we can learn from them, and their relationship to the period evolution of pulsars.Comment: 28 pages, 9 figures (as separate png files), Astrophysical Journal, in pres

On the kinematics of a corotating relativistic plasma stream in the perpendicular rotator model of a pulsar magnetosphere

An investigation of the kinematics of a rotating relativistic plasma stream in the perpendicular rotator model of the pulsar magnetosphere is presented. It is assumed that the plasma (ejected from the pulsar) moves along the pulsar magnetic field lines and also corotates with them. The field lines are considered to be radial straight lines, located in the plane which is perpendicular to the pulsar rotation axis. The necessity of taking particle inertia into account is discussed. It is argued that the "massless" ("force-free") approximation cannot be used for the description of this problem. The frame selection is discussed and it is shown that it is convenient to discuss the problem in the noninertial frame of ZAMOs (Zero Angular Momentum Observers). The equation of motion and the exact set of equations describing the behaviour of a relativistic plasma stream in the pulsar magnetosphere is obtained. The possible relevance of this investigation for the understanding of the formation process of a pulsar magnetosphere is discussed.Comment: Plain LaTe
