    Novel imaging method to quantify stratum corneum in dermatopharmacokinetic studies:Proof-of-concept with acyclovir formulations

    Purpose: Tape stripping the stratum corneum (SC) is used in topical bioequivalence studies. Formulations are compared using drug concentration profiles as a function of relative SC position; both of these parameters require quantification of SC amount removed per tape. Here, a novel imaging method to quantify SC on tape strips is described. Comparisons are made with established SC quantification methods, specifically weighing and UV pseudo-absorption. Methods: Six stratum corneum tape strips were measured 15 times by the three methods, which were compared for precision, signal:noise ratio, sample size and speed. Furthermore, 600 tape strips were assayed by each method, and correlations examined. Results: Weighing exhibited low precision, extremely low signal:noise ratio, and was slow. UV pseudo-absorption had high precision, acceptable signal:noise ratio and was quick. However, only a fraction of the total SC removed is analysed, and inhomogeneity can affect the results. The new imaging method was precise, with high signal:noise ratio, and measured the whole SC sample, unaffected by inhomogeneity. In addition, the approach was rapid and has the potential for fast automated scanning of multiple tapes and for further image analysis. Conclusion: The novel imaging method has many advantages over established methods for quantifying SC amount per tape.</p

    An evaluation of the relative efficacy of an open airway, an oxygen reservoir and continuous positive airway pressure 5 cmH2O on the non-ventilated lung

    Publisher's copy made available with the permission of the publisher © Australian Society of AnaesthetistsThe aim of this study, during one-lung ventilation, was to evaluate if oxygenation could be improved by use of a simple oxygen reservoir or application of 5 cmH2O continuous positive airway pressure (CPAP) to the non-ventilated lung compared with an open airway. Twenty-three patients with lung malignancy, undergoing thoracotomy requiring at least 60 minutes of one-lung ventilation before lung lobe excision, were studied. After routine induction and establishment of one-lung ventilation, the three treatments were applied in turn to the same patient in a sequence selected randomly. The first treatment was repeated as a fourth treatment and these results of the repeated treatment averaged to minimize the effect of slow changes. Arterial oxygenation was measured by an arterial blood gas 15 minutes after the application of each treatment. Twenty patients completed the study. Mean PaO2 (in mmHg) was 210.3 (SD 105.5) in the 'OPEN' treatment, 186.0 (SD 109.2) in the 'RESERVOIR' treatment, and 240.5 (SD 116.0) in the 'CPAP' treatment. This overall difference was not quite significant (P=0.058, paired ANOVA), but comparison of the pairs showed that there was a significant better oxygenation only with the CPAP compared to the reservoir treatments (t=2.52, P=0.021). While the effect on the surgical field was not apparent in most patients, in one patient surgery was impeded during CPAP. Our results show that the use of a reservoir does not give oxygenation better than an open tube, and is less effective than the use of CPAP 5 cmH2O on the non-ventilated lung during one-lung ventilation.J.     Uncovering degrees of workplace bullying: a comparison of Baccalaureate nursing students' experiences during clinical placement in Australia and the UK.

    Background: Bullying in health workplaces has a negative impact on individual nurses, their families, multidisciplinary teams, patient care and the profession. Aim: This paper compares the experiences of bully and harassment of Australian and UK baccalaureate nursing students during clinical placement. Method: A secondary analysis was conducted on two primary cross-sectional studies of bullying experiences of Australian and UK nursing students. Data were collected using the SEBDCP questionnaire and analysed using descriptive and inferential statistics. The total sample consisted of 833 Australian and 561 UK students Results: Australian nursing students experienced a higher rate of bullying (50.1%) than UK students (35.5%). Across both cohorts (Aust 53%, UK 68%). students identified other nurses as the main perpetrators and few bullied students chose to report the episode/s (Aust 28.5%, UK 19.4%). The main reason given for not reporting was fear of being victimised (Aust 53.6%, UK 54.5%). Sadly, a number felt ‘it is part of the job’ (UK: 21.6%, Aust: 23.9%). Conclusions: It is clear that a culture of bullying in nursing persists internationally. Nursing students are vulnerable and often experience and/or witness bullying episodes during clinical placement, leading them to question their future in the ‘caring’ profession of nursing. Bullying behaviour requires a zero tolerance by the nursing profession and education providers need to develop clear policies and implement procedures that protect students as the future nursing workforce

    Dependence of direct detection signals on the WIMP velocity distribution

    The signals expected in WIMP direct detection experiments depend on the ultra-local dark matter distribution. Observations probe the local density, circular speed and escape speed, while simulations find velocity distributions that deviate significantly from the standard Maxwellian distribution. We calculate the energy, time and direction dependence of the event rate for a range of velocity distributions motivated by recent observations and simulations, and also investigate the uncertainty in the determination of WIMP parameters. The dominant uncertainties are the systematic error in the local circular speed and whether or not the MW has a high density dark disc. In both cases there are substantial changes in the mean differential event rate and the annual modulation signal, and hence exclusion limits and determinations of the WIMP mass. The uncertainty in the shape of the halo velocity distribution is less important, however it leads to a 5% systematic error in the WIMP mass. The detailed direction dependence of the event rate is sensitive to the velocity distribution. However the numbers of events required to detect anisotropy and confirm the median recoil direction do not change substantially.Comment: 21 pages, 7 figures, v2 version to appear in JCAP, minor change

    WIMP abundance and lepton (flavour) asymmetry

    We investigate how large lepton asymmetries affect the evolution of the early universe at times before big bang nucleosynthesis and in particular how they influence the relic density of WIMP dark matter. In comparison to the standard calculation of the relic WIMP abundance we find a decrease, depending on the lepton flavour asymmetry. We find an effect of up to 20 per cent for lepton flavour asymmetries lf=O(0.1)l_f= {\cal O}(0.1).Comment: 16 pages, 4 figures; v2:minor changes to some wording

    Identification of women with an increased risk of developing radiation-induced breast cancer: A case only study

    Introduction: Radiation exposure at a young age is one of the strongest risk factors for breast cancer. Germline mutations in genes involved in the DNA-damage repair pathway (DDRP) may render women more susceptible to radiation-induced breast cancer. Methods: We evaluated the contribution of germline mutations in the DDRP genes BRCA1, BRCA2, CHEK2 and ATM to the risk of radiation-induced contralateral breast cancer (CBC). The germline mutation frequency was assessed, in a case-only study, in women who developed a CBC after they had a first breast cancer diagnosed before the age of 50 years, and who were (n = 169) or were not (n = 78) treated with radiotherapy for their first breast tumour. Results: We identified 27 BRCA1, 5 BRCA2, 15 CHEK2 and 4 truncating ATM germline mutation carriers among all CBC patients tested (21%). The mutation frequency was 24.3% among CBC patients with a history of radiotherapy, and 12.8% among patients not irradiated for the first breast tumour (odds ratio 2.18 (95% confidence interval 1.03 to 4.62); p = 0.043). The association between DDRP germline mutation carriers and risk of radiation-induced CBC seemed to be strongest in women who developed their second primary breast tumour at least 5 years after radiotherapy.     A multi-disciplinary perspective on climate model evaluation for Antarctica

    A workshop was organized by Antarctic Climate 21 (AntClim21), with the topic 'evaluation of climate models' representation of Antarctic climate from the perspective of long-term twenty-first-century climate change.' The suggested approach for evaluating whether climate models over- or underestimate the effects of ozone depletion is to diagnose simulated historical trends in lower-stratospheric temperature and compare these to observational estimates. With regard to more regional changes over Antarctica, such as West Antarctic warming, the simulation of teleconnection patterns to the tropical Pacific was highlighted. To improve the evaluation of low-frequency variability and trends in climate models, the use and development of approaches to emulate ice-core proxies in models was recommended. It is recommended that effort be put into improving datasets of ice thickness, motion, and composition to allow for a more complete evaluation of sea ice in climate models. One process that was highlighted in particular is the representation of Antarctic clouds and resulting precipitation. It is recommended that increased effort be put into observations of clouds over Antarctica, such as the use of instruments that can detect cloud-base height or the use of remote sensing resources

    Trends in hospitalizations and survival of acute decompensated heart failure in four US communities (2005–2014) ARIC study community surveillance

    BACKGROUND: Community trends of acute decompensated heart failure (ADHF) in diverse populations may differ by race and sex. METHODS: The ARIC study (Atherosclerosis Risk in Communities) sampled heart failure-related hospitalizations (≥55 years of age) in 4 US communities from 2005 to 2014 using International Classification of Diseases, Ninth Revision, Clinical Modification codes. ADHF hospitalizations were validated by standardized physician review and computer algorithm, yielding 40173 events after accounting for sampling design (unweighted n=8746). RESULTS: Of the ADHF hospitalizations, 50% had reduced ejection fraction, and 39% had preserved EF (HFpEF). HF with reduced ejection fraction was more common in black men and white men, whereas HFpEF was most common in white women. Average age-adjusted rates of ADHF were highest in blacks (38.1 per 1000 black men, 30.5 per 1000 black women), with rates differing by HF type and sex. ADHF rates increased over the 10 years (average annual percentage change: black women +4.3%, black men +3.7%, white women +1.9%, white men +2.6%), mostly reflecting more acute HFpEF. Age-adjusted 28-day and 1-year case fatality proportions were ≈10% and 30%, respectively, similar across race-sex groups and HF types. Only blacks showed decreased 1-year mortality over time (average annual percentage change: black women –5.4%, black men –4.6%), with rates differing by HF type (average annual percentage change: black women HFpEF –7.1%, black men HF with reduced ejection fraction –4.7%). CONCLUSIONS: Between 2005 and 2014, trends in ADHF hospitalizations increased in 4 US communities, primarily driven by acute HFpEF. Survival at 1 year was poor regardless of EF but improved over time for black women and black men
