Paths reunited: initiation of the classical and lectin pathways of complement activation
Understanding the structural organisation and mode of action of the initiating complex of the classical pathway of complement activation (C1) has been a central goal in complement biology since its isolation almost 50 years ago. Nevertheless, knowledge is still incomplete, especially with regard to the interactions between its subcomponents C1q, C1r and C1s that trigger activation upon binding to a microbial target. Recent studies have provided new insights into these interactions, and have revealed unexpected parallels with initiating complexes of the lectin pathway of complement: MBL–MASP and ficolin–MASP. Here, we develop and expand these concepts and delineate their implications towards the key aspects of complement activation via the classical and lectin pathways
Secure Cloud Controlled UAS Operations
Integrating a small unmanned aircraft system (sUAS) with cloud capabilities for military or enterprise use has not usually been feasible due to cybersecurity concerns. With recent advancements in blockchain networks the possibility of large cloud connected UAS networks has emerged. Our team investigates how to integrate data collected from a sUAS with a cloud-based service for data collection, storage, and processing implemented to ensure data privacy and data integrity. Our proposed network architecture implements a blockchain network to maintain decentralized security for the network. The research’s objectives include running security tests against a blockchain network & host/client networks and then comparing their performance and abilities to support the cloud based UAS. Specifically, we are using an open-source project called AirSim to support a virtual UAS that is connected to the UAS flight controller, the Pixhawk, to test a hardware-in the loop solution. This test is a preliminary proof of concept, and after it proves successful we are moving to a test involving a physical UAS. Data is transmitted from the UAS to a client server in Amazon Web Services (AWS) where it is placed into a blockchain and sent to the host server for processing. Overall, we believe a cloud supported communication network with a blockchain to secure data is an efficient and wise method of UAS control with information processing
Reliability and Validity of Self-Reported Questionnaires Related to Adolescent Violence and Consequences, Thailand

Research in this area has been hampered by a lack of psychometrically sound tools that have been validated to assess problem behaviours in Asian youth. The purpose of this paper is to provide validity and reliability data on an instrument to measure violence in Thai youth. In this study, reliability and validity data for a sample of adolescent Thai youth are reported for the Communities That Care Youth Survey (CTC-YS), a measure of risk and protective factors for violent behaviour, and the STAXI-II, a measure of angry experience and expression. The findings showed overall high internal consistency for both questionnaires, and there was evidence of construct validity. It is concluded that these measures are appropriate for use in research that seeks to investigate youth violence among adolescents in Thailand
The importance of resilience to primary care practitioners: an interactive psycho-social model
In this paper, it is argued that an understanding of the factors that make up resilience can
enhance communication and concordance between practitioner and patient. A model is
presented demonstrating that resilience is an interaction between factors in the internal
domain, comprising psychological characteristics and resources, and the external domain,
comprising the social environment surrounding the individual. As resilience manifests itself
in different ways across the life-cycle, and according to individual circumstances, time is also
an important part of the model presented in this paper. Understanding this model of resilience
can lead to an insight that there are factors that can be influenced whereby the primary care
practitioner can treat the patient, or refer them after a process of concordance through a
deeper understanding of the factors that surround a patient’s current health status. Underlying
the model is the view that resilience is linked to the assets model of health, seeking to
promote and maintain health and prevent illness. Therefore, primary care practitioners,
through a deeper understanding of the circumstances of the patient, and through
understanding the factors that promote resilience, may be better able to take action in health
promotion and maintenance

Consolidation whole abdomen irradiation following adjuvant carboplatin-paclitaxel based chemotherapy for advanced uterine epithelial cancer: feasibility, toxicity and outcomes
Background: To evaluate feasibility and preliminary outcomes associated with sequential whole abdomen irradiation (WAI) as consolidative treatment following comprehensive surgery and systemic chemotherapy for advanced endometrial cancer. Methods: We conducted a retrospective analysis of patients treated at our institution from 2000 to 2011. Inclusion criteria were stage III-IV endometrial cancer patients with histological proof of one or more sites of extra-uterine abdomen-confined disease, treated with WAI as part of multimodal therapy. Endpoints were feasibility, acute toxicity, late effects, recurrence-free survival (RFS) and overall survival (OS). Twenty patients were identified. Chemotherapy consisted of 3 to 6 cycles of a platinum-paclitaxel regimen in 18 patients. WAI was delivered using conventional technique to a median total dose of 27.5 Gy. Results: No grade 4 toxicities occurred during chemotherapy or radiotherapy. No radiation dose reduction was necessary. Three patients developed small bowel obstruction, all in the context of recurrent intraperitoneal disease. Kaplan-Meier estimates and 95% confidence intervals for RFS and OS at one year were 63% (38–80%) and 83% (56-94%) and at 3 years 57% (33-76%) and 62% (34-81%), respectively. On univariate Cox analysis, stage IVB and serous papillary (SP) histology were found to be statistically significantly (at the p = 0.05 level) associated with worse RFS and OS. The peritoneal cavity was the most frequent site of initial failure. Conclusions: Consolidative WAI following chemotherapy is feasible and can be performed without interruption with manageable acute and late toxicity. Patients with endometrioid adenocarcinoma, especially stage FIGO III, had favorable outcomes possibly meriting prospective evaluation of the addition of WAI following chemotherapy in selected patients. Patients with SP do poorly and do not routinely benefit from this approach
Identification of structural protein–protein interactions of herpes simplex virus type 1
AbstractIn this study we have defined protein–protein interactions between the structural proteins of herpes simplex virus type 1 (HSV-1) using a LexA yeast two-hybrid system. The majority of the capsid, tegument and envelope proteins of HSV-1 were screened in a matrix approach. A total of 40 binary interactions were detected including 9 out of 10 previously identified tegument–tegument interactions (Vittone, V., Diefenbach, E., Triffett, D., Douglas, M.W., Cunningham, A.L., and Diefenbach, R.J., 2005. Determination of interactions between tegument proteins of herpes simplex virus type 1. J. Virol. 79, 9566–9571). A total of 12 interactions involving the capsid protein pUL35 (VP26) and 11 interactions involving the tegument protein pUL46 (VP11/12) were identified. The most significant novel interactions detected in this study, which are likely to play a role in viral assembly, include pUL35–pUL37 (capsid–tegument), pUL46–pUL37 (tegument–tegument) and pUL49 (VP22)–pUS9 (tegument–envelope). This information will provide further insights into the pathways of HSV-1 assembly and the identified interactions are potential targets for new antiviral drugs
The environment under the knife: A review of current eco-surgical strategies and recommendations for Pakistan
The healthcare sector at its core is based on the fundamentals belief to do no harm and bring about betterment in the lives of the people. Paradoxically, hospitals are one of the leading contributors to pollution, greenhouse gas (GHG) emissions and toxic waste material worldwide. Surgical care delivery is quite resource intensive, consuming significant amount of energy and equipment as well as producing large quantities of waste. With climate change being a global priority, it is crucial that hospitals re-evaluate the environmental impact of such practices. The current review was planned to identify areas of improvement in surgical care in terms of sustainability, as well as describe efficient and innovative strategies for hospitals in Pakistan to lessen their impact on the environment. The implementation of the 5 R\u27s strategy for surgical care (Reduce, Reuse, Recycle, Rethink and Research) as well as general measures to improve energy efficiency, waste management and inter-sectoral collaboration will provide significant benefits to the environment and advance efforts to creating a more sustainable future for surgical healthcare in Pakistan
